Intimate partner violence (IPV) is prevalent in the United States, and many women who experience IPV have children in their care. Substance use is common among this population and affects parenting behaviors such as maternal acceptance and child outcomes. Maternal experience of IPV affects a mother's ability to parent and interact with her child. Little is known about the combined influence of both maternal substance use and IPV on parenting behaviors and child problem behaviors. The current study examined differences in maternal acceptance and child internalizing and externalizing behaviors among mothers who reported experiencing IPV to mothers who never reported experiencing IPV. Results showed that mothers with a history of IPV reported lower rates of maternal acceptance and higher rates of child problem behaviors compared with those with no history of IPV. In addition, frequency of substance use moderated this relationship. This is the first study, to date, to examine the relationship between maternal acceptance and child problem behaviors among substance using mothers with a history of IPV and is a first step to understanding the parenting practices of this population.

A strong emphasis on self-management for health maintenance in a variety of chronic diseases has been shown to benefit patients' outcomes and quality of life. However, little has been published on such programs in patients with chronic kidney disease. We studied the feasibility and effectiveness of the Chronic Disease Self-Management Program (CDSMP) in 14 patients with ESRD undergoing conventional hemodialysis. This program is designed to enhance skills in the areas of medical, emotional, and role management. Outcome measures in health status, self-management behaviors, self-efficacy, and health care utilization were evaluated through use of questionnaires at baseline and after six months.

This article documents the experiences of providing housing and supportive services, or ecologically based treatment, to shelter-recruited, substance-abusing homeless women with young children in their care. Among clients, observed experiences related to housing, substance abuse, and health and mental health care are discussed. Among therapists, experiences related to managing the chaotic nature of the client's lives, wanting to manage the client's lives, and frustration with client's life trajectories are reviewed. Observations related to the therapeutic process include the client's relationship to the therapist, balancing the client's independence and need for assistance, and unrealistic expectations among the clients. Recommendations for successfully approaching these clinical situations and experiences are offered. The purpose of this article is to document these therapy experiences to facilitate the work of future teams seeking to intervene in the lives of homeless families through homeless shelters or other settings.

Background: No single study has delineated the prevalence of intimate partner violence (IPV) over multiple time periods for Latina versus non-Latina women and compared the health of abused Latina women to the health of abused non-Latina women using multiple health indicators. The objective of this study was to describe the prevalence of IPV over multiple time periods and the association between lifetime IPV exposure and current health in Latina and non-Latina women., Methods: A total of 3429 women (mean age=47 years) were randomly sampled from a large U.S. healthcare system; 139 (4%) were Latina. During a telephone survey, past-5-year and past-year IPV prevalence was assessed using five questions on physical and psychological abuse from the Behavioral Risk Factor Surveillance System (BRFSS), and ten questions from the Women's Experience with Battering Scale; lifetime IPV prevalence was assessed using the BRFSS questions. Current physical, social, and mental health was assessed using well-validated questionnaires., Results: Prevalence of IPV for Latina versus non-Latina women was, respectively: 44.6% vs 44% lifetime; 20.1% vs 14.5% for the past 5 years; and 11.5% vs 7.8% for the past year. In models adjusted for age and income, women with a lifetime IPV history had significantly worse health compared to non-abused women across many health indicators; for example, Latina women with a lifetime IPV history had Short Form-36 Health Survey (SF-36) subscale scores that were 5.62 (mental health) to 7.77 (vitality) points lower than those for non-abused Latina women; depression prevalence more than two times higher; and more physical symptoms. Adverse IPV-related health was significantly worse for Latina than non-Latina women for overall mental health functioning (p<0.02), vitality (p<0.01), and emotional functioning (p<0.01) according to SF-36., Conclusions: This exploratory study showed that IPV is common in Latina and non-Latina women, and adverse IPV-related mental health was pronounced in Latina women.

Ethical considerations for working with runaway and homeless youth in research and treatment settings are presented. Issues of parents' and adolescents' consent for research and treatment are discussed, with particular attention given to the lack of explicit guidelines for working with abused and neglected youth. The principles of beneficence and justice are discussed as they apply to intervening with a high-risk, multiproblem population. The authors offer a rationale for allowing adolescents to self-consent to research and treatment. They argue that in many circumstances, requiring parental consent may not be in the youth's best interest and may preclude his or her participation in treatment and research programs.
