Present correspondence on the lung cancer mortality of the Mayo Lung Cancer Project in the United States. Impact of overdiagnosis; Excess of lung cancer incidence; Allegation of underplaying the excess mortality.

Background: Japanese society is more egalitarian than the United States as is reflected by the lower degree of prevalence of social inequalities in health. We examined whether subjective socioeconomic status is associated with different trajectories of self-rated health (SRH), and whether this relationship differs between the United States and Japan., Methods: We analyzed the responses of 3968 Americans from the survey Midlife in the United States, 2004-06, and the responses of 989 Japanese from the survey Midlife in Japan, 2008. We conducted a multilevel analysis with three self-ratings of health (10 years ago, current and 10 years in the future) nested within individuals and nested within 10 levels of subjective social status. Age, sex, educational level and subjective financial situation were adjusted., Results: After making statistical adjustments for confounding variables, respondents in Japan continued to report lower average levels of health. However, the rate of expected decline in SRH over the next decade was strongly socially patterned in the United States, whereas it was not in Japan., Conclusion: The Japanese showed no disparity in the anticipated trajectory of SRH over time, whereas the Americans showed a strong social class gradient in future trajectories of SRH.

Background: The use of a structured abstract has been recommended in reporting medical literature to quickly convey necessary information to editors and readers. The use of structured abstracts increased during the mid-1990s; however, recent practice has yet to be analyzed., Objectives: This article explored actual reporting patterns of abstracts recently published in selected medical journals and examined what these journals required of abstracts (structured or otherwise and, if structured, which format)., Methods: The top thirty journals according to impact factors noted in the "Medicine, General and Internal" category of the ISI Journal Citation Reports (2000) were sampled. Articles of original contributions published by each journal in January 2001 were examined. Cluster analysis was performed to classify the patterns of structured abstracts objectively. Journals' instructions to authors for writing an article abstract were also examined., Results: Among 304 original articles that included abstracts, 188 (61.8%) had structured and 116 (38.2%) had unstructured abstracts. One hundred twenty-five (66.5%) of the abstracts used the introduction, methods, results, and discussion (IMRAD) format, and 63 (33.5%) used the 8-heading format proposed by Haynes et al. Twenty-one journals requested structured abstracts in their instructions to authors; 8 journals requested the 8-heading format; and 1 journal requested it only for intervention studies., Conclusions: Even in recent years, not all abstracts of original articles are structured. The eight-heading format was neither commonly used in actual reporting patterns nor noted in journal instructions to authors.

Cigarette smoking is an established risk factor for lung cancer. However, the magnitude of the relative risk (RR) on lung cancer mortality in relation to cigarette smoking is reported to be lower in Japan than in Western countries. We investigated whether this discrepancy could be explained by differences in the exposure to cigarettes smoked, by differences in sensitivity to smoking, or by differences in lung cancer mortality among non-smokers. We examined the 10-year follow-up data on 88,153 participants in a Japanese population-based prospective study conducted in three prefectures. Data used as a Western counterpart was retrieved from a published report of the US Cancer Prevention Study (CPS)-II. Although there was a significant increased risk of lung cancer death among current smokers compared with non-smokers, the observed RR in the Three-Prefecture Study were much lower than RR reported in the CPS-II. Lung cancer mortality of our Japanese sample was lower among current smokers and higher among non-smokers regardless of age and sex. Current smokers in our sample had initiated smoking at an older age and smoked fewer cigarettes per day for shorter durations than those in the CPS-II sample. The Poisson regression model (controlling for age, number of cigarettes smoked per day and duration of smoking) showed that male current smokers in our sample had a lower risk of lung cancer compared with those in the CPS-II sample (rate ratio 0.34 [95%CI 0.27-0.43]). These findings might explain why Japanese risks of lung cancer are lower than those observed in Western countries.

A monoclonal antibody (mAb) with a novel human B-cell allospecificity was produced by immunizing a C3H/He mouse with the human B lymphoblastoid cell line EBV-Wa (HLA-DR4/Dw15/DQblank homozygous). The mAb, termed HU-46, reacted with B cells from not only DR4/Dw15-positive individuals but also certain DRw8/Dw8-positive ones whose DQ phenotypes had not yet been defined. Two-dimensional gel analyses indicated that the mAb recognized class II antigens which were encoded by the HLA-DQ locus. Furthermore, in genetic analysis, the gene encoding the class II antigen detected by HU-46 met the Hardy-Weinberg condition as a fourth allele of the DQ locus. We provisionally labeled this novel DQ specificity DQWa.
