Hepatitis B immune globulin (HBIG) and immune serum globulin (ISG) were examined in a randomized, double-blind trial to assess their relative efficacies in preventing type B hepatitis after needle-stick exposure to hepatitis B surface antigen (HBsAG)-positive donors. Clinical hepatitis developed in 1.4% of HBIG and in 5.9% of ISG recipients (P = 0.016), and seroconversion (anti-HBs) occurred in 5.6% and 20.7% of them respectively (P less than 0.001). Mild and transient side-effects were noted in 3.0% of ISG and in 3.2% of HBIG recipients. Available donor sera were examined for DNA polymerase (DNAP) and e antigen and antibody (HBeAg; anti-HBE). Both DNAP and HBeAg showed a highly statistically significant correlation with the infectivity of HBsAg-positive donors. Hepatitis B immune globulin remained significantly superior to ISG in preventing type B hepatitis even when the analysis was confined to these two high-risk subgroups. The efficacy of ISG in preventing type B hepatitis cannot be ascertained because a true placebo group was not included. [ABSTRACT FROM AUTHOR]

Mammalian orthoreovirus (MRV) infects multiple mammalian species including humans. A United States Midwest swine farm with approximately one thousand 3-month-old pigs experienced an event, in which more than 300 pigs showed neurological signs, like "down and peddling", with approximately 40% mortality. A novel MRV was isolated from the diseased pigs. Sequence and phylogenetic analysis revealed that the isolate was a reassortant virus containing viral gene segments from three MRV serotypes that infect human, bovine and swine. The M2 and S1 segment of the isolate showed 94% and 92% nucleotide similarity to the M2 of the MRV2 D5/Jones and the S1 of the MRV1 C/bovine/Indiana/MRV00304/2014, respectively; the remaining eight segments displayed 93%-95% nucleotide similarity to those of the MRV3 FS-03/Porcine/USA/2014. Pig studies showed that both MRV-infected and native contact pigs displayed fever, diarrhoea and nasal discharge. MRV RNA was detected in different intestinal locations of both infected and contact pigs, indicating that the MRV isolate is pathogenic and transmissible in pigs. Seroconversion was also observed in experimentally infected pigs. A prevalence study on more than 180 swine serum samples collected from two states without disease revealed 40%-52% positive to MRV. All results warrant the necessity to monitor MRV epidemiology and reassortment as the MRV could be an important pathogen for the swine industry and a novel MRV might emerge to threaten animal and public health.

Diabetic devices including glucose monitoring systems and insulin pumps are used worldwide and are increasingly recognized as a source of irritant contact dermatitis and allergic contact dermatitis (ACD). Isobornyl acrylate is a chemical used in the glue of some of these devices and has been implicated as a frequent culprit allergen in patients who become sensitized to their device. Patch testing with isobornyl acrylate 0.1% in petrolatum is not necessary in standard screening panels but should be considered in patients with suspected ACD to glucose sensors or insulin pumps.

We have used a death-record search to define the frequency of lethal outcomes of hepatitis B virus infection among a population of more than 15,000 overtly healthy blood donors found positive in routine HBsAg testing. We have compared the study population with a control group of some 18,000 donors selected on the basis of a negative test result. The index and control groups were observed for periods reflecting a total of 55 and 59 thousand person-years, respectively. Twenty percent of the 134 deaths identified among HBsAg positive donors were in some way liver related, including seven deaths due to hepatitis, seven to cirrhosis and six to hepatoma. In contrast, only one of the 95 deaths in the control population was liver related, and was due to fatty degeneration of the liver. The majority (four) of the hepatoma deaths occurred among blacks, three of whom were less than 35 at the time of death. In contrast, deaths from cirrhosis were all among whites. We conclude that there is significant mortality associated with the HBsAg positive state, even though the affected individuals may be asymptomatic and well enough to give blood at some stage. We estimate the standardised mortality ratio for hepatoma among HBsAg-positive persons in the United States is at least 27, confirming the association observed in other populations. The risk for hepatoma among young, HBsAg positive black males appears to approach that reported for HBsAg positive males in Taiwan. Data on the feasibility of AFP testing for early detection of hepatoma are included and discussed.

From January 1975 to December 1978, 19,140,169 units of blood were collected by the American Red Cross at 57 regional locations. Each unit was tested for hepatitis B surface antigen (HBsAg) by a single commercial radioimmunoassay test system. A total of 15,954 donations were reported reactive, representing a rate of HBsAg reactivity of 0.83/1,000 units tested. The rate during 1975 was 0.94/1,000 units tested, decreasing to 0.71/1,000 units tested in 1978 (p < 0.005). The prevalence of HBsAg among first-time donors was 2.08/1,000 during 1977 and 1978, 2.7 times higher than that calculated for repeat donors (0.77/1,000 donors). Substantial geographic variation in the prevalence and rate of detection of HBsAg was observed.

The present study was undertaken to determine whether there were any differences in the distribution of hepatitis B virus-(HBV) associated serologic markers among hepatitis B surface antigen-(HBsAg) positive first-time and repeat blood donors. The markers examined in samples from 412 newly identified HBsAg-positive donors (254 first-time and 158 repeat) included HBsAg titer and subtype, HBeAg/anti-HBe, and anti-HBc. Repeat donors were more frequently HBeAg-positive (25.9%) than were first-time donors (17.7%). Anti-HBc and anti-HBe were observed more often among first-time (99.6 and 76%) than repeat (91.8 and 60.8%) donors. No differences were found in the mean HBsAg titer nor in the subtype distribution in the two populations. The frequency of HBeAg positivity and the mean HBsAg titer in blood were significantly lower among first-time donors aged 30 or older as compared to those younger than 30. Such age-related tendencies did not occur among the repeat donors. The profiles of HBV makers suggest that the HBsAg-positive first-time donor group consists predominantly of long-term HBsAg carriers who may have acquired HBV at an early age, while the HBsAg-positive repeat donors have newly acquired infections.

A total of 20,643 samples of blood donated to three American Red Cross Blood Service regions were tested for antibodies to hepatitis B core antigen (anti-HBc) in order to evaluate and understand its significance and implications. All 20,643 samples were non-reactive for hepatitis B surface antigen (HBsAg) when screened at the regions. Overall, 2.2% of all samples were found to have anti-HBc, and a total of 16 had anti-HBc in the absence of antibodies to surface antigen (anti-HBs) activity. 1 of the 16 samples with anti-HBc in the absence of anti-HBs was found to have HBsAg when tested by a sensitive procedural modification of the Austria II test. Samples with low levels of anti-HBc activity were significantly more likely to be negative for anti-HBs.

During the year 1978 this laboratory evaluated the specificity of all samples found reactive for hepatitis B surface antigen (HBsAg) by 44 of 57 regions of the American Red Cross Blood Services. Radioimmunoassay detected a total of 1.921 HBsAg-reactive sampled among more than three million donor units tested. A vast majority (96%) of the samples had high level of HBsAg (greater than or equal to 20 ng/ml). Only about 50% of the samples with low level of HBsAg (less than 20 ng/ml) were reactive in reversed passive hemagglutination. There were 13 samples that were repeatable for HBsAg but were considered nonspecific as they were nonneutralizable in radioimmunoassay, 2 donors who showed nonspecific reactivity were further tested and it was found that the reactivity in radioimmunoassay persisted for more than 9 months, and this reactivity was also detectable by a second commercial kit for HBsAg. Antibodies to core and surface antigen were not found in any of the nine samples that were tested. The explanation of this nonspecific reactivity is unclear, but the data suggest that the nonspecific factor(s) may be an inherent property of the sample rather than a deficiency of the test reagents.

Eight hepatitis B surface antigen (HBsAg) proficiency panels were tested using a radioimmunoassay (RIA) test kit during 1978 and 1979 by routine processing laboratories within the 57 American Red Cross Blood Services Regions. The test results were analyzed and it was found that lot-to-lot variability in the reagents may result in a measurable loss of sensitivity of the test leading to an increased error rate. On the basis of these studies, it is recommended that the positive to negative (P/N) ratio for validation of the test be raised to at least 25.

A total of 1,915 sera collected in 1979 from asymptomatic hepatitis B surface antigen (HBsAg) carriers were tested for delta antigen, antibody to delta antigen (anti-delta), hepatitis B e antigen (HBeAg) and antibody to hepatitis B e antigen (anti-HBe) in addition to HBsAg and its subtypes. These sera represented blood donated by volunteers to 49 of 57 regions of the American Red Cross located in nine geographic regions of the United States and Puerto Rico. A total of 72 (3.8%) sera had anti-delta activity while none had a detectable level of delta antigen. A significantly higher (p less than 0.01) prevalence of anti-delta (12.1%) was found in San Jose, California (Pacific Region); on the other hand, the East South Central region covering Alabama, Kentucky, Mississippi and Tennessee had a significantly lower (p less than 0.05) prevalence (1.4%) of anti-delta when compared with all other regions combined. Anti-delta was, however, detected in all regions of the United States and in Puerto Rico. The cause of significant differences in the prevalence of anti-delta was not clear. The distribution of anti-delta was not associated with age, sex or blood type of the donor. Sixty-nine of 70 samples with anti-delta were found among the 1,527 samples that had either HBeAg or anti-HBe. And among 149 that lacked both HBeAg and anti-HBe, only one sample had anti-delta. The difference is statistically significant (p less than 0.05). The presence of anti-delta was not associated with HBsAg/ad (2.7%) or HBsAg/ay (4.6%).

A new antigenic specificity for HBsAg is described, and its usefulness as an epidemiological marker is discussed. This specificity, l, was found in specimens from all geographic areas studied (United States, Latin America, South Africa, the Solomon Islands, and New Guinea). In these areas, l was strongly associated with HBsAg/ay; y-positive, l-negative samples were observed only among specimens from United States volunteer blood donors. This determinant allowed a distinction of HBsAg/ad classes. Thus, the l determinant was detected in 51.4% of HBsAg/adw4, 44.1% of HBsAg/adw(non-w4), and 50% of HBsAg/adr. The association of l and other HBsAg determinants with HBeAg or anti-HBe was also investigated. HBeAg was found to be associated with the presence of the l determinant and with the r determinant. Anti-HBe was associated with the presence of the d and w determinants and with the absence of the l specificity.

The American Red Cross collects blood from a number of defined subsets of the donor population and teh proportion of blood collected from each subset varies widely from center to center. A large part of the variation in prevalence of HBsAg may be related to variations in the proportion of blood collected from plants and factories, military units and schools or colleges. We have derived a regression equation, significant at the p less than 0.001 level, which links HBsAg prevalence with these collection parameters. Using this equation, we were able to predict the prevelance of HBsAg among first-time donors in 6 of the 9 geographic divisions of the United States with an accuracy exceeding 10%. The predictions for the remaining division were within 35% of the actual value. Correlation studies were supported by measurements of true donor prevalence in three blood centers.
