The article discusses the use of the evidence-based 4Ms model to improve outpatient geriatric dermatology care. Purpose: The purpose of this study was to analyze the quality and readability of the information generated by an online artificial intelligence (AI) platform regarding 4 common hand surgeries and to compare AI-generated responses to those provided in the informational articles published by the American Society for Surgery of the Hand (ASSH) HandCare website., Methods: An open AI model (ChatGPT) was used to answer questions commonly asked by patients on 4 common hand surgeries (carpal tunnel release, cubital tunnel release, trigger finger release, and distal radius fracture fixation). These answers were evaluated for medical accuracy, quality and readability and compared to answers derived from the ASSH HandCare materials., Results: For the AI model, the Journal of the American Medical Association benchmark criteria score was 0/4, and the DISCERN score was 58 (considered good). The areas in which the AI model lost points were primarily related to the lack of attribution, reliability and currency of the source material. For AI responses, the mean Flesch Kinkaid Reading Ease score was 15, and the Flesch Kinkaid Grade Level was 34, which is considered to be college level. For comparison, ASSH HandCare materials scored 3/4 on the Journal of the American Medical Association Benchmark, 71 on DISCERN (excellent), 9 on Flesch Kinkaid Grade Level, and 60 on Flesch Kinkaid Reading Ease score (eighth/ninth grade level)., Conclusion: An AI language model (ChatGPT) provided generally high-quality answers to frequently asked questions relating to the common hand procedures queried, but it is unclear when or where these answers came from without citations to source material. Furthermore, a high reading level was required to comprehend the information presented. The AI software repeatedly referenced the need to discuss these questions with a surgeon, the importance of shared decision-making and individualized care, and compliance with surgeon treatment recommendations., Clinical Relevance: As novel AI applications become increasingly mainstream, hand surgeons must understand the limitations and ramifications these technologies have for patient care., (Copyright © 2023 American Society for Surgery of the Hand. In April 2020, in light of COVID-19-related blood shortages, the US Food and Drug Administration (FDA) reduced the deferral period for men who have sex with men (MSM) from its previous duration of 1 year to 3 months.Although originally born out of necessity, the decades-old restrictions on MSM donors have been mitigated by significant advancements in HIV screening, treatment, and public education. The severity of the ongoing COVID-19 pandemic-and the urgent need for safe blood products to respond to such crises-demands an immediate reconsideration of the 3-month deferral policy for MSM.We review historical HIV testing and transmission evidence, discuss the ethical ramifications of the current deferral period, and examine the issue of noncompliance with donor deferral rules. We also propose an eligibility screening format that involves an individual risk-based screening protocol and, unlike current FDA guidelines, does not effectively exclude donors on the basis of gender identity or sexual orientation. Our policy proposal would allow historically marginalized community members to participate with dignity in the blood donation process without compromising blood donation and transfusion safety outcomes.

Background: Recent studies show that early operative intervention in patients who fail nonoperative management of adhesive small bowel obstruction (ASBO) is associated with improved outcomes. The purpose of this study was to determine the trend in practice pattern and outcomes of patients with ASBO in the United States., Methods: Data from the National Inpatient Sample data (2003-2013) were extracted for analysis and included patients (age ≥18 years) who were discharged with primary diagnosis codes consistent with ASBO. We analyzed the data to examine changes in mortality and hospital length of stay in addition to any trends in rate and timing of operative interventions., Results: During the study period, 1,930,289 patients were identified with the diagnosis of ASBO. Over the course of the study period, the rate of operative intervention declined (46.10-42.07%, p = 0.003), and the timing between admission and operative intervention was significantly shortened (3.09-2.49 days, p < 0.001). In addition, in-hospital mortality rate decreased significantly (5.29-3.77%, p < 0.001). In the multiple logistic regression analysis, the relative risk of mortality decreased by 5.6% per year (odds ratio, 0.944; 95% confidence interval, 0.937-0.951; p < 0.001). Hospital length of stay decreased from 10.39 to 9.06 days (p < 0.001)., Conclusion: Over the last decade, fewer patients with ASBO were managed operatively, whereas those requiring an operation underwent one earlier in their hospitalization. Although further studies are warranted, our results suggest that recent changes in practice pattern may have contributed to improved outcomes., Level of Evidence: Therapeutic study, level IV.
