This study was conducted to assess the patient characteristics, types of treatment, and outcomes of patients who are surgically treated for vertebral osteomyelitis (VO) in the United States. VO can be treated with or without surgical intervention. Surgically treated cases of VO are associated with significant morbidity and mortality, and incur major healthcare costs. There are few studies assessing the characteristics and outcomes of patients with VO who are treated surgically, as well as the overall impact of surgically managed VO on the healthcare system of the United States. Utilizing the Nationwide Inpatient Sample (NIS) database, 44,401 patients were identified who underwent surgical treatment for VO over a fifteen year period. Severity of comorbidity burden was calculated using the Deyo Index (DI). Surgical approach and comorbidities were analyzed in regard to their impact on complications, mortality rate, LOS, and hospitalization charges. The incidence of surgical intervention for patients who had VO increased from 0.6 to 1.1 per U.S. persons over the study period. Surgically treated patients had a mean age of 56 years, were 75.8% white, were 54.5% male, 37.9% carried Medicare insurance, and they had a mean DI of 0.88. Anterior/posterior approach (OR: 3.53), thoracolumbar fusion (OR: 2.69), thoracolumbar fusion (OR: 19.94), and anterior/posterior approach (OR: 64.73) were the surgical factors that most significantly predicted any complication, mortality, increased LOS, and increased hospital charges, respectively (P < 0.001). The mean inflation-adjusted total hospital cost increased from $20,355 to $39,991 per patient over the study period. VO has been steadily increasing in the United States. Incidence and inflation-adjusted costs nearly doubled. Anterior/posterior approach and thoracolumbar fusion most significantly predicted negative outcomes. VO is associated with lengthy and expensive hospital stays resulting in a significant burden to patients and the healthcare system.

Background: Rheumatoid arthritis (RA) is a chronic inflammatory condition. Chronic inflammation is associated with atherosclerosis, hypertension, diabetes, chronic obstructive pulmonary disease (COPD), chronic kidney disease. But sparse data are available regarding the trends of cardiovascular diseases and complications in RA. We conducted a National Inpatient Sample database analysis to demonstrate the trends of cardiac complications in patients with RA., Methods: We used National Inpatient Sample data from 2005 to 2014 to identify admissions with the diagnosis of RA and identified who had associated cardiovascular complications also. The International Classification of Diseases-9th Revision-Clinical Modification codes were used for the diagnoses of RA; congestive heart failure (CHF), acute myocardial infarction (AMI), and atrial fibrillation (AF)., Results: A statistically significant increasing trend of AMI, CHF, and AF was found. Independent predictors of mortality in RA patients with AMI were age (OR 1.03, CI 1.02-1.04; P < 0.001), COPD (OR 1.67, CI 1.40-2.00; P < 0.001), cerebrovascular disease (OR 2.207, CI 1.71-2.86; P < 0.001), renal disease (OR 1.42, CI 1.16-1.75; P = 0.001), and alcohol abuse (OR 2.73, CI 1.73-4.32; P < 0.001). Independent predictors of mortality in RA patients with CHF were age (odds ratio [OR] 1.02, confidence interval [CI] 1.017-1.024; P < 0.001]), COPD (OR 1.09, CI 1.01-1.18; P = 0.023), cerebrovascular disease (OR 1.67, CI 1.44-1.95; P < 0.001), renal disease (OR 1.16, CI 1.07-1.27; P = 0.001). Independent predictors of mortality in RA patients with AF were age (OR 1.02, CI 1.02-1.03; P < 0.001), race (OR 1.16, CI 1.02-1.31; P = 0.022), COPD (OR 1.56, CI 1.42-1.71; P < 0.001), peripheral arterial disease (OR 1.34, CI 1.16-1.53; P < 0.001), cerebrovascular disease (OR 2.27, CI 1.0-2.58; P < 0.001), renal disease (OR 1.60, CI 1.44-1.80; P < 0.001). The mortality trend has increased significantly in the CHF (P = 0.025) and AF (P = 0.042) groups during this study period., Conclusions: We have found a significant increase in trend of cardiovascular complications in RA patients. Pemphigus vulgaris (PV) is a rare, potentially life threatening, autoimmune blistering skin disease. The International Pemphigus and Pemphigoid Foundation (IPPF) has recently developed a disease registry with the aim to enhance our understanding of autoimmune bullous diseases with the long-term goal of acquiring information to improve patient care. Patients were recruited to the IPPF disease registry through direct mail, e-mail, advertisements, and articles in the IPPF-quarterly, -website, -Facebook webpage, and IPPF Peer Health Coaches to complete a 38-question survey. We present here the initial analysis of detailed clinical information collected on 393 PV patients. We report previously unrecognized gender differences in terms of lesion location, autoimmune comorbidity, and delay in diagnosis. The IPPF disease registry serves as a useful resource and guide for future clinical investigation.

Introduction: Smoking by health care professionals poses a barrier to interventions with patients. This study reports smoking status changes among health care professionals using the Tobacco Use Supplement-Current Population Surveys (TUS-CPS)., Methods: TUS-CPS self-reported smoking status (current, former, and never) identified by occupation (physicians, physician assistants, registered nurses [RNs], licensed practical nurses [LPNs], respiratory therapists, dentists, and dental hygienists), were analyzed for the 2003 (N = 4,095) and 2006/2007 (N = 3,976) cohorts. Quit ratios among U.S. health care professionals were calculated by dividing the number of former smokers by the number of ever-smokers using weighted estimates., Results: In 2006/2007, LPNs (20.55%) and respiratory therapists (19.28%) had the highest smoking prevalence. Physicians (2.31%), dentists (3.01%), pharmacists (3.25%), and RNs (10.73%) had the lowest prevalence. Data from 2006/2007 indicate that physicians, pharmacists, dentists, and physician assistants had the highest quit ratios; all groups had quit ratios higher than the general public, except LPNs (.52 vs. .46, respectively). Current smoking varied by group but did not significantly decline from 2003 to 2006/2007. The majority of health care professionals were never-smokers., Conclusions: These data indicate that only 4 health care professional groups met the Healthy People 2010 goal of 12% smoking prevalence. LPNs were the only group with quit ratios lower than the general population. The lack of significant decline in smoking rates among health professionals was similar to the "flat" rate seen among adults in the United States. This is of concern as smoking among health care professionals limits their interventions with smokers and their involvement in tobacco control.

This study examines agreement between parental reports of children's ADHD outpatient services ascertained with the Child and Adolescents Services Assessment (CASA) and provider records among a sample of elementary school students who participated in an epidemiologically based health services study. Parental reports of any outpatient mental health treatment, services intensity, and medication regimens were compared to records of the specified mental health and primary care providers using intraclass correlations and kappa estimates. Results indicated that parental reports using the CASA produced valid data on whether any outpatient ADHD services had been received in the past 12 months (83% agreement), and on details of the child's medication regimens (kappas above 0.90), but that agreement was poor on how many times the child had been seen (intraclass correlation 0.29), without agreement differences by provider type. These findings support the validity of using parental report for ADHD services in primary care settings and for study of medication regimens.
