Epidemiological studies continue to question the recommendations to the general public on physical activity, particularly whether vigorous, intense activity is required or desirable to reduce the risk of premature cardiovascular disease. Here the authors review the evidence before and after the US Surgeon General's Report of 1996 in a wide range of occupational and population groups, noting that the necessity for physical activity at work or in our leisure time has declined steeply since the early epidemiological studies in the 1950s and 1960s. The authors suggest that further meta-analyses should be conducted among selected population groups to ensure that preventive advice is appropriate for specific age and sex groups in the population.
