Background: The prevalence of mental illness in young people is the highest of any age group, with the onset of depression, anxiety and substance use peaking between 18 and 24 years. Effective treatments that target sub-threshold or mild to moderate levels of disorder in young people are required to reduce the risk of persistence and recurrence. The aims of this study are to evaluate whether treatments that are less intensive than cognitive-behaviour therapy, such as problem solving therapy and exercise treatments, are acceptable and effective in managing depression and anxiety symptoms in young people and to identify possible attributes in those who are likely to respond to these treatments., Methods/design: This is a factorial randomised controlled trial conducted at a large, metropolitan youth mental health service. Participants are young help-seekers aged 15-25 years with sub-threshold or mild to moderate levels of depression and anxiety (with or without comorbid substance use). The interventions comprise 4 treatment combinations delivered by psychologists over 6 sessions on a weekly basis: a psychological intervention (problem solving therapy versus supportive counselling) and an exercise intervention (behavioural exercise versus psychoeducation). Structured assessments occur at baseline, mid-point, end-point (6 weeks) and at a 6- and 12-month follow-up. The primary outcomes are depression and anxiety symptoms as measured by the Beck Depression and Anxiety Inventories. Secondary outcomes include remission (defined as no longer meeting the diagnostic criteria for a disorder if threshold level was reached at baseline, or no longer scoring in the clinical range on scale scores if sub-threshold at baseline), substance use, and functioning., Discussion: The effectiveness of less complex psychological and exercise interventions in young help-seekers with sub-threshold or mild to moderate presentations of high prevalence disorders is yet to be explored. This study has been designed to examine the effectiveness of these interventions delivered alone, or in combination, in a youth-specific service. If effective, the interventions have the potential to prevent the progression of early symptoms and distress to later and potentially more serious stages of mental disorder and reduce the likelihood of ongoing problems associated with the risk of persistence and recurrence., Trial Registration: Australian New Zealand Clinical Trials Registry ACTRN12608000550303.

While the prevalence, correlates and mental health impacts of intimate partner violence are well documented in adolescents and young adults, fewer studies have considered physical dating violence among clinical samples of help-seeking young people. In a sample of 98 young people aged 15-24 years (54% females) referred to a specialist public youth mental health service, we examined the 12-month prevalence of physical violence inflicted by an intimate partner and its relationship with psychiatric disorders and psychosocial functioning. The reported prevalence of dating violence in the 12 months prior to referral was 13%. Physical dating violence reported at referral was associated with poorer psychosocial functioning, substance dependence and comorbid Axis I diagnoses at 6-month follow-up. These findings suggest that youth mental health services are well positioned not only to screen for dating violence but to intervene to ameliorate the mental health consequences of abuse and to prevent further violence.

Objective: The purpose of the present study was to determine the frequency at which assault and attempted assault occur as outcomes among stalking victims, and to explore the common characteristics of those cases in which the victims report these harmful outcomes as a result of the stalking behaviour., Method: A sample of 3700 men and women randomly selected from the electoral roll were sent a postal questionnaire to determine the prevalence and nature of their experiences of stalking behaviours. Those who self-reported that they had been stalked were divided according to whether their pursuer had, or had not, attacked them., Results: Of the 432 who reported having been stalked, 75 (17.4%) had been attacked. A number of victim and perpetrator factors differentiated those who reported attacks. A combination of the victim being threatened, being an ex-intimate, and being younger at the time of the stalking incident were predictive of attack. A multivariate model containing these factors correctly classified 82% of the sample and achieved an AUC of 0.87., Conclusions: Stalking is an all too common problem behaviour that can result in a range of harmful outcomes for victims. Threats made by ex-intimates are a particular cause for concern. The generalizability of these findings should be tested using robust prospective methodologies in diverse populations.

Background: No studies have assessed psychopathology among victims of stalking who have not sought specialist help., Aims: To examine the associations between stalking victimisation and psychiatric morbidity in a representative community sample., Method: A random community sample (n=1844) completed surveys examining the experience of harassment and current mental health. The 28-item General Health Questionnaire (GHQ-28) and the Impact of Event Scale were used to assess symptomatology in those reporting brief harassment (n=196) or protracted stalking (n=236) and a matched control group reporting no harassment (n=432)., Results: Rates of caseness on the GHQ-28 were higher among stalking victims (36.4%) than among controls (19.3%) and victims of brief harassment (21.9%). Psychiatric morbidity did not differ according to the recency of victimisation, with 34.1% of victims meeting caseness criteria 1 year after stalking had ended., Conclusions: In a significant minority of victims, stalking victimisation is associated with psychiatric morbidity that may persist long after it has ceased. Recognition of the immediate and long-term impacts of stalking is necessary to assist victims and help alleviate distress and long-term disability.

Objective: This study examines the extent and nature of stalking victimisation in a random community sample., Method: A postal survey was distributed to 3700 adult men and women selected from the electoral roll in the State of Victoria. Outcome measures included the lifetime and annual cumulative incidence of stalking, the duration and methods of harassment, rates of associated violence and responses to victimisation., Results: Almost one in four respondents (23.4%;432) had been stalked, the unwanted behaviour they were subjected to being both repeated and fear-provoking. One in 10 (197) had experienced a protracted course of stalking involving multiple intrusions spanning a period of at least one month. Women were twice as likely as men to report having been stalked at some time in their lives, though the rates of victimisation in the 12 months prior to the study did not differ significantly according to gender. Younger people were significantly more likely than older respondents to report having been stalked. Victims were pursued by strangers in 42% of cases. The most common methods of harassment involved unwanted telephone calls, intrusive approaches and following. Associated threats (29%) and physical assaults (18%) frequently arose out of the stalking. Significant social and economic disruption was created by the stalking for 63% of victims. Most sought assistance to manage their predicament (69%)., Conclusions: The experience of being stalked is common and appears to be increasing. Ten percent of people have been subjected at some time to an episode of protracted harassment. Assaults by stalkers are disturbingly frequent. Most victims report significant disruption to their daily functioning irrespective of exposure to associated violence.

Objective: The authors examined whether female stalkers differ from their male counterparts in psychopathology, motivation, behavior, and propensity for violence., Method: Female (N=40) and male (N=150) stalkers referred to a forensic mental health clinic were compared., Results: In this cohort, female stalkers were outnumbered by male stalkers by approximately four to one. The demographic characteristics of the groups did not differ, although more male stalkers reported a history of criminal offenses. Higher rates of substance abuse were also noted among the male stalkers, but the psychiatric status of the groups did not otherwise differ. The duration of stalking and the frequency of associated violence were equivalent between groups. The nature of the prior relationship with the victim differed, with female stalkers more likely to target professional contacts and less likely to harass strangers. Female stalkers were also more likely than male stalkers to pursue victims of the same gender. The majority of female stalkers were motivated by the desire to establish intimacy with their victim, whereas men showed a broader range of motivations., Conclusions: Female and male stalkers vary according to the motivation for their pursuit and their choice of victim. A female stalker typically seeks to attain a close intimacy with her victim, who usually is someone previously known and frequently is a person cast in the professional role of helper. While the contexts for stalking may differ by gender, the intrusiveness of the behaviors and potential for harm does not.
