Obesity and other metabolic variables are associated with abnormal brain structural volumes and cognitive dysfunction in HIV-uninfected populations. Since individuals with HIV infection on combined antiretroviral therapy (CART) often have systemic metabolic abnormalities and changes in brain morphology and function, we examined associations among brain volumes and metabolic factors in the multisite CNS HIV AntiRetroviral Therapy Effects Research (CHARTER) cohort, cross-sectional study of 222 HIV-infected individuals. Metabolic variables included body mass index (BMI), total blood cholesterol (C), low- and high-density lipoprotein C (LDL-C and HDL-C), blood pressure, random blood glucose, and diabetes. MRI measured volumes of cerebral white matter, abnormal white matter, cortical and subcortical gray matter, and ventricular and sulcal CSF. Multiple linear regression models allowed us to examine metabolic variables separately and in combination to predict each regional volume. Greater BMI was associated with smaller cortical gray and larger white matter volumes. Higher total cholesterol (C) levels were associated with smaller cortex volumes; higher LDL-C was associated with larger cerebral white matter volumes, while higher HDL-C levels were associated with larger sulci. Higher blood glucose levels and diabetes were associated with more abnormal white matter. Multiple atherogenic metabolic factors contribute to regional brain volumes in HIV-infected, CART-treated patients, reflecting associations similar to those found in HIV-uninfected individuals. Sternal malunion, or loss, developed after a median sternotomy cannot only be difficult to manage and treat, but also may diminish one’s quality-of-life drastically. The technique presented here represents a multispecialty approach in one stage for the reconstruction of an unstable thoracic cage. The procedure utilized a donated sternum and ribs. The sternum with ribs harvested from a single donor included adipose derived stromal vascular fraction (ADSVF) cells with marrow also from the same donor. Autologous muscle flaps, stabilized with acellular dermal matrix were utilized to provide a robust blood supply to the ADSVF cells and bone grafts. Acellular dermal matrix was used to construct the ribs and stabilize the plugs of stem cells and bone. These procedures, in the hands of multispecialty physicians, have led to several successful reconstructions involving complex chest wall deformities. This surgical intervention was performed in a one stage operation. This represents the first successful complete sternal transplant in a patient with return to normal activities and increased quality-of-life.

Abstract: Frost disturbance is often mentioned in southern African savanna literature, but it is seldom discussed or investigated further. However, it can represent an above-ground disturbance as effective as fire or browsing, depending on the resistance capacity of the effected plants. A severely freeze-damaged stand of Colophospermum mopane along a slope in the Venetia Limpopo Nature Reserve provided an opportunity to investigate the nature of freeze-damage impacts on C. mopane. Is this disturbance a possible demographic limitation of C. mopane preventing its southwards spread? Freeze-damage of individual trees was assessed according to tree height and landscape position — with lower elevations representing the most severe freeze zones and higher elevations representing the least severe. Lower elevation trees were relatively small (2.24m) and coppicing, whilst higher elevation trees were taller (3.65m) with no coppice present. No freeze-damage was observed on tree canopies above 4m in height. Trees<4m in height that had experienced 100% freeze-damage, failed to regrow to their original heights of the previous season. This is a possible driver of the pre-freeze height differences seen across the slope; with trees at low elevations having to recover from freeze events and subsequent topkill more frequently, resulting in a net decrease in tree height for that growing season. It appears that C. mopane has limited resistance to freeze events, and this may be linked to the absence of this species at colder latitudes in the Southern Hemisphere. [Copyright &y& Elsevier]

The Early Eocene Okanagan Highlands series of lacustrine shale and coal deposits, in far western North America, constitutes a significant group of fossil sites with exceptional preservation of a diverse suite of organisms (Lagerstätten). With contemporaneous basins arrayed across about 1000 kilometres of southern British Columbia and northern Washington, these sites offer a unique opportunity to examine the paleoecology of terrestrial communities spanning a temperate, low-seasonality landscape in a montane setting during a time of generally warm temperatures across the globe. The Okanagan Highlands sites provide an unparalleled comparative framework within which to examine this major turning point in terrestrial community development during the emergence of their broad modern character. In southern African savannas, grass production, and therefore the annual extent of fire, is highly dependent on rainfall. This response has repeatedly been noted in the literature but authors used different input variables and modelling approaches and the results are not comparable. Using long-term fire occurrence data from six protected areas in southern Africa we tested various methods for determining the relationship between antecedent rainfall and burned area. The types of regression model, the most appropriate index of accumulated rainfall, and the period over which to calculate annual burned area were all investigated. The importance of accumulating rainfall over more than one growing season was verified in all parks - improving the accuracy of the models by up to 30% compared with indices that only used the previous year's rainfall. Up to 56% of the variance in burned area between years could be explained by an 18-month accumulated rainfall index. Linear models and probit models performed equally well. The method suggested in this paper can be applied across southern Africa. Most premenopausal women diagnosed with breast cancer receive adjuvant chemotherapy or tamoxifen. These treatments help to prevent the recurrence of breast cancer but have potentially permanent menopausal side-effects, termed chemically-induced menopause (CIM). Following a diagnosis of breast cancer and subsequent treatment, CIM and its associated side-effects may represent additional stressors for women. Thirty women participated in interviews regarding the impact of CIM on their sexual functioning. Participants were also asked about the emotional impact of these sexual changes. Interviews were transcribed and analyzed using a thematic analysis technique that was informed by the constant comparison method of grounded theory. Themes emerged regarding women's experiences of: (a) the sexual changes following treatment; (b) the emotional impact of sexual changes; and, (c) moderators of the negative impact of these sexual changes. Six new species of the extinct family Dinopanorpidae (Mecoptera) are described in the new genus Dinokanaga gen. nov. from five Early (and early Middle?) Eocene Okanagan Highlands localities in British Columbia and Washington State. These are Dinokanaga hillsi sp. nov., D. wilsoni sp. nov., D. dowsonae sp. nov., D. andersoni sp. nov., D. sternbergi sp. nov., and D. webbi sp. nov. The family Dinopanorpidae is previously known only from a hind wing of Dinopanorpa megarche Cockerell, and an undescribed species of Dinopanorpa, both from the Paleogene of Primorye (Maritime Province) in Pacific coastal far-eastern Russia. The family includes mostly large species, with forewings up to 43 mm in length. Dinopanorpidae is particularly distinguished by a long R1 vein in both the fore- and hind wings, extending almost to the wing apex and bending posteriad distally. Their wings are mostly dark with light bands and spots; have many to extremely dense crossveins; and an expanded costal space in the basal quarter of the forewing, which is more developed in larger species. The late Early to early Middle Eocene Okanagan Highlands fossil sites, spanning ~1000 km north–south (northeastern Washington State, southern British Columbia) provide an opportunity to reconstruct biotic communities across a broad upland landscape during the warmest part of the Cenozoic. Plant taxa from these fossil sites are characteristic of the modern eastern North American deciduous forest zone, principally the mixed mesophytic forest, but also include extinct taxa, taxa known only from eastern Asian mesothermal forests, and a small number of taxa restricted to the present-day North American west coast coniferous biome. In this preliminary report, paleoclimates and forest types are reconstructed using collections from Republic in Washington State, USA., and Princeton, Quilchena, Falkland, McAbee, Hat Creek, Horsefly, and Driftwood Canyon in British Columbia, Canada. Both leaf margin analysis (LMA) and quantitative bioclimatic analysis of identified nearest living relatives of megaflora indicated upper microthermal to lower mesothermal moist environments (MAT ~10–15 °C, CMMT > 0 °C, MAP > 100 cm/year). Some taxa common to most sites suggest cool conditions (e.g., Abies, other Pinaceae; Alnus, other Betulaceae). However, all floras contain a substantive broadleaf deciduous element (e.g., Fagaceae, Juglandaceae) and conifers (e.g., Metasequoia) with the bioclimatic analysis yielding slightly higher MAT than LMA. Thermophilic (principally mesothermal) taxa include various insects, the aquatic fern Azolla, palms, the banana relative Ensete, taxodiaceous conifers, Eucommia and Gordonia, taxa which may have occurred near their climatic limits. The mixture of thermophilic and temperate insect and plant taxa indicates low-temperature seasonality (i.e., highly equable climate). [ABSTRACT FROM AUTHOR]
