In September 2023, CDC's Advisory Committee on Immunization Practices recommended updated 2023-2024 (monovalent XBB.1.5) COVID-19 vaccination for all persons aged =6 months to prevent COVID-19, including severe disease. As with past COVID-19 vaccines, additional doses may be considered for persons with immunocompromising conditions, who are at higher risk for severe COVID-19 and might have decreased response to vaccination. In this analysis, vaccine effectiveness (VE) of an updated COVID-19 vaccine dose against COVID-19-associated hospitalization was evaluated during September 2023-February 2024 using data from the VISION VE network. Among adults aged =18 years with immunocompromising conditions, VE against COVID-19-associated hospitalization was 38% in the 7-59 days after receipt of an updated vaccine dose and 34% in the 60-119 days after receipt of an updated dose. Few persons (18%) in this high-risk study population had received updated COVID-19 vaccine. All persons aged =6 months should receive updated 2023-2024 COVID-19 vaccination; persons with immunocompromising conditions may get additional updated COVID-19 vaccine doses =2 months after the last recommended COVID-19 vaccine. We report 2 cases of pharyngeal monkeypox virus and group A Streptococcus co-infection in the United States. No rash was observed when pharyngitis symptoms began. One patient required intubation before mpox was diagnosed. Healthcare providers should be aware of oropharyngeal mpox manifestations and possible co-infections; early treatment might prevent serious complications. [ABSTRACT FROM AUTHOR]

Numerous studies indicated that patients with tobacco use disorder (TUD) are inversely associated with mortality in what is known as the smoker's paradox. However, limited studies have been conducted on the impact of TUD on the in-hospital mortality rates of patients with secondary pulmonary hypertension (PH, Non-Group 1 PH). Using the 2019 National Inpatient Sample, we identified PH and divided it into TUD and non-TUD to compare the comorbidities and in-hospital mortality between the two after 1:1 propensity-score matching. Of 1,129,440 PH hospitalizations, 12.1 % had TUD. After matching (n=133545, each group), TUD had lower median age (62 vs. 63), higher females (49 vs. 46.6 %), blacks (25.9 vs. 25.3 %), lower household income (40.8 vs. 32.7 %), Medicaid (22.4 vs. 14.8 %), non-elective (93.5 vs. 89.8 %), rural (9.3 vs. 6.7 %), urban non-teaching (17.2 vs 15.8 %) admissions. All CV comorbidities and other substance use were higher in TUD except CHF and valvular heart disease, TUD+ cohort and lower mortality (3.3 vs. 4.2 %, OR 0.78, p<0.001), higher routine discharges (53.8 vs. 51.3 %, p<0.001) and lower total charges ($47155 vs. 51909, p<0.001) than non-TUD. Although PH patients with TUD had a higher comorbidity burden, they had lower in-hospital mortality rates along with lower total charges of hospitalization, mandating real-world data to validate these results. Objectives: Childhood-onset systemic lupus erythematosus (cSLE), representing 15%-20% of individuals with SLE, has been difficult to study globally due to differences between registries. This initiative, supported by Childhood Arthritis Rheumatology Research Alliance (CARRA) and Paediatric Rheumatology European Society (PReS), aims to create Core and Expanded cSLE Datasets to standardise and enhance research worldwide., Methods: 21 international cSLE experts and 4 patients participated in a Delphi process (questionnaires, 2 topic-specific focus groups and 3 virtual consensus meetings) to create 2 standardised cSLE datasets. The Core cSLE Dataset was designed to include data essential to meaningful clinical research across many settings. The Expanded cSLE Dataset was designed for centres able to consistently collect data to address broader research questions. Final data items for the Core and Expanded datasets were determined by consensus defined as >80% agreement) using an adapted nominal group technique and voting., Results: The resulting Core cSLE Dataset contains 46 items, including demographics, clinical features, laboratory results, medications and significant adverse events. The Expanded cSLE Dataset adds 26 additional items and includes patient-reported outcomes. Consensus was also achieved regarding the frequency and time points for data collection: baseline, quarterly follow-up visits, annually and flare visits., Conclusion: Standardised Core and Expanded cSLE Datasets for registry-based international cSLE research were defined through the consensus of global experts and patient/caregiver representatives, endorsed by CARRA and PReS. Objectives: High levels of caregiver burden (CB) are experienced by informal caregivers of pediatric patients with cancer. There is increasing evidence highlighting the extent of CB across sub-Saharan African countries, although there remains lack of interventions that target improvements in their experience. This study aimed to determine the impact of a structured psychoeducation program on caregivers' outcomes relating to preparedness to provide care, burden of caregiving, and quality of life (QoL)., Methods: This quasi-experimental (pre-and-posttest) design, involved family caregivers of children on admission for cancer treatment in 4 Nigerian tertiary hospitals. Eligible participants received 2 structured, psychoeducational training sessions delivered by a multidisciplinary oncology team, focusing on the management of patients' condition, spiritual care, self-care, and support., Results: Subjects were mainly female (79.5%) and mostly mothers to children undergoing cancer treatment (74.7%). Commonest cancer type was acute lymphoblastic leukemia (23.9%) with evidence of metastatic disease found in 9.6% of children. Significant improvements were observed between pre- and posttest for unmet needs ( z = -9.3; p < 0.001), preparedness for caregiving in palliative care (PCPC) ( z = -7.0; p < 0.001), and overall QoL ( z = -7.3; p < 0.001). A significant reduction in CB was also reported ( z = -8.7; p < 0.001)., Significance of Results: This psychoeducational intervention (PEI) resulted in significant improvements in unmet needs, CB and significant improvements in PCPC. However, a reduction in QoL of the family caregivers was also observed. Findings from this study should encourage the use of well-crafted PEIs, delivered within hospital settings to promote improvements in outcomes for informal caregivers of hospitalized children suffering from cancer, in an African context. Further intervention development is required to better understand intervention components influencing changes in outcomes, while exploring feasibility testing and adaptation to similar settings in Nigeria and within Africa.

Childhood cancer is a significant cause of morbidity and mortality worldwide. It is the second-leading cause of non-communicable deaths among children worldwide with more than 90% of childhood cancer-associated mortality occurring in low-income and middle-income countries. Disparity in mortality rates has been linked to late presentation, inaccurate diagnosis, treatment abandonment and poor access to appropriate therapy. Access to enhanced diagnostics such as immunohistochemistry (IHC) is vital for proper diagnosis and prompt institution of appropriate treatment for children with cancers. However, despite the ever-increasing insight into the use of these techniques for timely and accurate diagnosis of pediatric cancers; simple histological diagnosis remains the mainstay in most of the LMIC owing to financial constraints, unavailability of facilities and skilled manpower. Limited access results in imprecise diagnosis including missed diagnosis, misdiagnosis, and wrong diagnosis, subsequently resulting in increased cost of care and poor treatment outcomes. Childhood cancer patients are a vulnerable population who are adversely affected by any disaster that disrupts the healthcare ecosystem. The objective of this study was to describe the impact of flooding on access to care for childhood cancer patients in Bayelsa state, southern Nigeria. We review the effect of the 2022 flooding on childhood cancer care at the paediatric oncology unit of the Niger Delta University Teaching Hospital, southern Nigeria. The devastating floods caused closure of the health facility for four weeks. The challenges faced by the oncology patients included inability to access the facility due to destruction of roads and telecommunication networks, inaccessibility to chemotherapy drugs, postponement of surgeries, parental financial constraints due to income loss occasioned by the flood and worsened by inadequate health insurance. Two children, who were undergoing chemotherapy for rhabdomyosarcoma and retinoblastoma had their care transferred to an unaffected secondary care facility 24km away. Through teamwork and determination, the oncology team was able to overcome various obstacles to provide uninterrupted care for the patients and improve on future patient care during disasters. Care for childhood cancer patients should be prioritized by healthcare facilities especially in predictable flood prone areas like Bayelsa state. Emphasis should be on disaster preparedness training, development of outstations equipped with patient information, chemotherapy drugs and other requirements for continued care to prevent adverse childhood cancer care outcomes., Les patients atteints de cancer pédiatrique constituent une population vulnérable fortement touchée par toute catastrophe perturbant l'écosystème de soins de santé. L'objectif de cette étude était de décrire l'impact des inondations sur l'accès aux soins des patients atteints de cancer pédiatrique dans l'État de Bayelsa, dans le sud du Nigeria. Nous examinons l'effet des inondations de 2022 sur les soins du cancer pédiatrique à l'unité d'oncologie pédiatrique de l'Hôpital universitaire de la Niger Delta, dans le sud du Nigeria. L'accent devrait être mis sur la formation à la préparation aux catastrophes, le développement de postes extérieurs équipés d'informations sur les patients, de médicaments de chimiothérapie et d'autres éléments nécessaires pour assurer des soins continus, afin de prévenir des résultats défavorables dans les soins del'enfance atteinte de cancer. Purpose: Volumetric modulated arc therapy (VMAT) is a relatively new treatment technique in sub-Saharan Africa. Although craniospinal irradiation (CSI) in the pediatric population has been practiced in Nigeria for many years, the use of VMAT to deliver this treatment is previously undocumented. We reviewed the first set of patients to undergo CSI at a cancer center in Nigeria, detailing the treatment technique, the progress experienced, dose statistics achieved, treatment toxicities, and cancer outcomes to date., Methods and Materials: This was a prospective case series of 5 children with histologically diagnosed cancers requiring CSI whose parents consented to the study. They were recruited at evaluation and followed through the process of their therapy. Toxicity was monitored at weekly review appointments using the Common Terminology Criteria for Adverse Events version 5.0. Follow-up of the children will continue in the long-term effects clinic., Results: Five patients with a median age of 6 were recruited. Diagnoses were intracranial germ cell tumor (n = 2), medulloblastoma (n = 1), pineoblastoma (n = 1), and ependymoma (n = 1). For all patients, a dose of 36.0 Gy in 1.8 Gy daily fractions was prescribed to the entire neuraxis. A subsequent boost of 18 Gy (n = 4) to 19.8 Gy (n = 1) in 10 daily fractions to the primary tumor bed (n = 2) and posterior fossa (n = 2) was delivered. Four patients had chemotherapy before, during, or after radiation therapy. No patient experienced grade 3 or greater toxicity., Conclusions: Our results indicate great progress has been made in the delivery of CSI in Nigeria, demonstrating tolerable acute side effects using VMAT. This series suggests the feasibility of implementing VMAT technology in low- or middle-income countries. Nasopharyngeal carcinoma (NPC) refers to malignancy arising from the epithelium of the nasopharynx. It is a rare tumor globally but seen in higher prevalence in certain populations associated with the endemicity of Epstein- Barr Virus. It is usually seen in clinical settings in developing countries in late stages majorly due to poor health-seeking behavior, healthcare costs, and misdiagnosis arising from its vague and ambiguous symptoms. The outcome of NPC depends strongly on the stage at diagnosis and access to the right treatment which may prove challenging in low-resource settings where care is paid for out of pocket. Background: Narrative medicine allows patients to reconstruct medical experiences through written portrayals of perspectives, building a mutual depiction of illness while creating a sense of belonging. This modality has not been previously studied in youth with rheumatologic illnesses, a population with high mental health burden and worse health-related quality of life. We aimed to assess the feasibility of a storytelling intervention in this patient population. Methods: This is a mixed-methods study of 14−21-year-olds with rheumatologic diseases followed in the Bronx, NY. Participants completed an hour-long creative writing session focused on patient experience with chronic disease. Pre- and post-questionnaires assessed patient-reported outcomes, and post-participation video interviews assessed personal experiences through the storytelling session. Results: Thirteen female patients were divided amongst four creative writing sessions. Twelve patients completed pre-study questionnaires and 10 completed post-study questionnaires, with 100% completion of the post-participation interviews. PedsQL surveys showed statistically significant improvement in physical health (p < 0.02), and there was no significant difference between pre- and post-scores for any other questionnaires. Interview thematic domains included writing motivation, prior writing experience, illness experience, relating to others, relationship with providers, and support. Conclusion: Creative writing is a feasible and acceptable intervention for youth with rheumatologic illnesses.

Introduction: Hepatobiliary abnormalities occur commonly in sickle cell anaemia and these have been extensively reported in the adult patients. However, complications have been sparsely reported in children especially in the sub-Saharan African continent. This study aimed to highlight the hepatobiliary complications in this group of children using clinical examination, laboratory testing and abdominal ultrasonography. The challenges in a resource limited country are also highlighted., Subjects, Materials and Methods: One hundred and thirty- four (134) children aged 1-18years with sickle cell anaemia in steady state were recruited into this crossectional study. Clinical history and physical examination obtained were documented. Relevant basic haematologic and biochemical indices (Full blood count, liver enzymes and viral markers for hepatitis B and C) and abdominal ultrasonographic parameters were documented for all the children. The relationship between the complications and possible risk factors (age, frequency of crisis and blood transfusions) were also documented., Result: Fifty-three (39.6%) of the children had hepatobiliary abnormalities. Chronic hepatitis B infection was the most prevalent complication (14.9%) seen followed by cholelithiasis (12.7%) and Hepatitis C infection (4.5%). Other complications identified were cholecystitis (3.0%), biliary sludge (1.5%), liver cirrhosis (0.7%). Age was significantly associated with viral hepatitis (p=0.003) and cholelithiasis (p=0.0007) and the conditions were more prevalent in the older age group. The hepatobilary complications were also more prevalent in the males. Frequent blood transfusions was significantly related to viral hepatitis (p=0.03). The use of hydroxyurea was not significantly related to any of the complications., Conclusion: Hepatobiliary abnormalities are prevalent in children with sickle cell anaemia. Introduction: Sorethroat is a common reason for presentation in primary paediatric care. Because only a minority of cases of pharyngitis is caused by bacteria, physicians have been guided by various recommendations on the judicious use of antibiotics to avoid overprescription. In the absence of guidelines, the treatment approaches between physicians may differ. The management of children with sorethroat by physicians in Benin City, Mid-Western Nigeria was evaluated., Subjects and Methods: The experience and practice of 25 paediatricians and 30 non paediatricians who routinely attend to children with sorethroat in Benin metropolis, Nigeria were evaluated with the aid of a self administered questionnaire. Information sought for included biodata, empirical antibiotic prescription. The choice of antibiotic and complications ofpharyngitis encountered in practice., Results: Majority of respondents 31(56.4 %) considered viruses as the commonest cause of pharyngitis. Despite this an equal proportion 31(56.4 %) treated children with sorethroat empirically with antibiotics. Of these, significantly more paediatricians 19(61.3 %) than non paediatricians 12(38.7 %) considered viruses the commonest cause of pharyngitis, P = 0.013. CI (0.10 0.63). Almost three quarter (72.7 %) of respondents examine the throat of the children while only 18.2 % obtained throat swab for microbiological analysis. The 24 doctors who did not treat empirically would prescribe antibiotic if the patient has purulent pharyngeal exudates, fever and adenitis. Augmentin and cefuroxime were the most prescribed antibiotics., Conclusion: A lot of children served by these doctors receive antibiotic needlessly from empirical antibiotic treatment ofpharyngitis. National guidelines on appropriate antibiotic use is needed to promote rational use of antibiotics and reduce antibiotic overuse.

The HIV/AIDS epidemic appears to have stabilized in the world's developed countries while it is still worsening in many areas of the resource-poor world. HIV/AIDS patients in countries such as the United States are living longer primarily due to the availability of potent antiretroviral drug combinations and tests to monitor the risk of progression of disease. Management of HIV is very complicated. General guidelines for treatment are available. Published guidelines, however, are often too simplistic and do not reflect new or controversial information. Most patients will develop resistant virus during their treatment. Several antiretroviral medications are on the horizon that will afford better options for treatment, and studies are underway to evaluate treatment strategies that would be potent, less toxic, and convenient for patients. Because the field of HIV care continues to evolve rapidly and there is increasing diversity among infected patients, treatment and prevention are optimized when provided by a dedicated care team supervised by an HIV specialist.

Zonocerus variegatus is a common grasshopper in parts of west and equtorial Africa. The distribution in Nigeria extends from the lowland rainforest zone to the savannah in the north. The influence of lure on the behaviour of grasshopper inside cages (120 insect per cage) was investigated. Nymphs and adults of Zonocerus variegatus responded positively to intact leaves, crushed leaves and inflorescence of the common compositae weed Chromolaena odorata inside muslin bags, and intact plants. There were significant differences in the attraction recorded for starved nymphs, fed nymphs and starved adults. Attraction was more to intact leaves and is by olfaction. The increase in the attraction of starved nymphs is time dependent. Attraction to plant parts ceased after the plants were dried for 24 and 48 hours at room temperature and when plants were placed in transparent polythene bags. Gut motility and gut activity were higher during the day than at night. Nymphs, adults and egg pods placed separately inside muslin bags were not attractive to adults or nymphs.
