Abstract: With the increasing number of patients with diabetes, and with the increasing complexity of care, managing the care of patients in a co-ordinated effective and efficient way is an increasing challenge. Data linkage of health-care databases using a single patient identifier has enabled us to establish a comprehensive patient management system. Originally developed as a research programme (DARTS) looking at epidemiology and patient outcomes, it is now a patient management system acting as a common repository of clinical information for all health-care professionals involved ion the care of patients with diabetes. This has aided the sharing of clinical information, improving communication and enhancing the efficiency of patient care. This system is now being rolled out across Scotland as a National diabetes clinical system. [Copyright &y& Elsevier]
