B cell activating factor (BAFF), belonging to the TNF (tumor necrosis factor) family, is critical for B cell survival and maturation. In the present study, a quail BAFF cDNA, named qBAFF, was amplified from quail spleen by RT-PCR and RACE (rapid amplification of cDNA ends) strategies. The open reading frame (ORF) of qBAFF cDNA encodes a protein consisting of 288-amino acid. The deduced amino acid sequence contains a predicted transmembrane domain and a putative furin protease cleavage site like other identified BAFF homologues. The qBAFF shows 96, 93, 93, 53 and 51% amino acid sequence identity with chicken (cBAFF), goose (gBAFF), duck (dBAFF), human (hBAFF) and mouse BAFF (mBAFF), respectively, with the functional soluble parts of qBAFF is 98, 99, 98, 78 and 71%, respectively. RT-PCR showed that BAFF is expressed in many tissues in the quail, including bursa, spleen, liver, brain, heart, intestine, kidney, thymus and muscle. Recombinant soluble qBAFF (qsBAFF) fused with His(6) tag was efficiently expressed in Escherichia coli BL21 (DE3) and its molecular weight of approximately 19kDa was identified by sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE) and Western blotting. In vitro, purified qsBAFF was able to promote the survival of quail bursa B cells. Our results suggest that qBAFF plays an important role in survival of quail B cells cultured in vitro.

Background and Purpose: The occurrence of preeclampsia-eclampsia during pregnancy has been reported to increase the risk of stroke in mainly Western populations. However, few studies have evaluated stroke risk in Asian populations and followed women beyond the early postpartum period. Thus, the present study determined the risk of stroke in women in Taiwan during pregnancy and the first postpartum year., Methods: A population-based cohort study was performed on 1,132,019 parturients during 1999 to 2003 using a dataset linking birth certificates and National Health Insurance hospital discharge data. Stroke-free survival rates were estimated using the Kaplan-Meier method, and the log-rank test was used to examine the effect of preelampsia-eclampsia on the prevalence of stroke. Sociodemographic factors and obstetric complications were used in multivariate logistic regression models to determine the adjusted odds ratios of preeclampsia-eclampsia on the risk of hemorrhagic and ischemic stroke during pregnancy and within the first postpartum year., Results: The incidence of stroke was 21.47 cases per 100,000 deliveries. There were 139 cases of hemorrhagic stroke and 107 cases of ischemic stroke. The respective adjusted relative risk of preeclampsia-eclampsia for hemorrhagic and ischemic stroke were 10.68 (95% CI, 3.40 to 33.59) and 40.86 (95% CI, 12.14 to 137.47) within 3 months antepartum; 6.45 (95% CI, 1.42 to 29.29) and 34.71 (95% CI, 11.08 to 108.68) in the first 3 days postpartum; 5.61 (95% CI, 0.71 to 44.10) and 11.23 (95% CI, 2.45 to 51.59) from 3 days to 6 weeks postpartum; 11.76 (95% CI, 4.05 to 34.11) and 11.60 (95% CI, 3.30 to 40.82) from 6 weeks to 6 months pospartum; and 19.90 (95% CI, 7.75 to 51.11) and 4.35 (95% CI, 0.58 to 32.92) from 6 months to 12 months postpartum., Conclusions: Women with preeclampsia-eclampsia have a significantly higher risk of stroke during pregnancy and in the first postpartum year. These results suggest that women with preeclampsia-eclampsia should be closely monitored even after pregnancy.

The treatment of sudden sensorineural hearing loss (SSNHL) has remained one of the most controversial issues in otolaryngology, and there are wide disparities between clinicians' treatment methods for SSNHL. Using five-year population-based data, we explored the relationship between physician and hospital characteristics and the length of stay (LOS) for SSNHL patients in Taiwan. The dataset was derived from the National Health Insurance Research Database. The patients in the study sample were identified by a principal diagnosis of sudden hearing loss. A total of 8712 hospitalization cases between 1998 and 2002 inclusive were included. Multiple regression analyses were performed in order to explore the relationship between physician and hospital characteristics and LOS, adjusting for patients' age and gender, as well as complications or comorbidities. The mean LOS for the duration of the study period was 5.62 days, while the regression analysis showed that, as compared to district hospitals, the LOS in medical centers and regional hospitals was 0.42 and 0.37 days longer, respectively (all p < 0.001). The analysis also revealed that the LOS was 0.71 days shorter for patients who were attended by physicians specializing in otolaryngology than for those attended by physicians specializing in other areas (p < 0.001). This study concludes that there is indeed a relationship between physician and hospital characteristics and the LOS for SSNHL patients. The results of this study serve to highlight the importance of developing treatment protocols for SSNHL so as to reduce the variation in physician or hospital behavior.

With field culture experiments and model calculations, the natural-light-dependent growth and the optimal light layers in sea water for growth of red tide dinoflagellate Prorocentrum donghaiense Lu were studied in order to analyze the role of light on algal blooms in the coastal area in East China Sea in spring. The results show that the relationship of growth and light can be well described by Steele's equation, and the optimal light intensity (Iopt) of P. donghaiense is (38.2 +/- 3.8) W x m(-2), which is lower than Iopt for several other red tide algae (40-133 W x m(-2)), meaning that P. donghaiense may have an advantage when developing blooms in turbid environments where solar irradiance is easily attenuated. The optimal light layers for P. donghaiense growth are thicker offshore than inshore, and the thickness of optimal light layers in the subsurface water usually in 3-15 m in depth is about 5-10 m in the red tide area. The trade-off of light and nutrient fitness results in blooms in the so-called red tide area, and the light-optimum characteristic of the subsurface water is an important factor for the subsurface bloom development in spring.

Based on the data from four cruises that carried out in the Changjiang River estuary and its adjacent areas from 2002-04 to 2003-02, the seasonal variation and spatial distribution of nutrients were analyzed, and the relationship between nutrients condition and the harmful algal blooms (HABs) was also discussed. Results showed that the annual average concentration of nutrient was (17.93 +/- 2.46) micromol x L(-1) for DIN, (0.59 +/- 0.11) micromol x L(-1) for PO4(3-) -P, (15.34 +/- 3.23) micromol x L(-1) for SiO3(2-) -Si, and the study area was in the state of eutrophication. The average concentration of nutrient showed a remarkable seasonal fluctuation with the higher value in autumn and winter and lower value in spring and summer. The spatial distribution of the nutrients was typically such that the concentrations of DIN, PO4(3-) -P and SiO3(2-) -Si decreased from inshore to offshore area, and the contours generally decreased rapidly in down-coast directions. DIN and SiO3(2-) -Si of the study area were largely contributed by Changjiang River diluted water and other terrigenous inputs, while PO4(3-) -P mainly by Changjiang River diluted water and the Taiwan Warm Current. The following HABs showed that its occurrences were usually laid in the areas with higher nutrients and lower total suspended particles (TSP).

Exchange rates of dissolved nutrients at the sediment-water interface were studied before and after diatom bloom from March to May in the East China Sea in 2005. Intact sediment cores which were sampled at 10 sites were incubated on board, and exchange rates were calculated in the function method. It was shown that sediment absorbed NO3(-) -N before bloom with the exchange rate ranging from - 1.33 mmol/(m2 d) to - 0.68 mmol/(m2 d). The sediment released NO3(-) -N after bloom with the exchange rate ranging from - 0.69 mmol/( m2 d) to 0.82 mmol/( m2 d). NH4(+) -N was almost released from sediment with the exchange rate ranging from -0.65 mmol/(m d) to 1.46 mmol/( m2 d), and the rate was higher before bloom than that after bloom. NO2(-) -N moved into sediment all along except at Zc17 with the exchange rate ranging from - 0.09 mmol/(m2 d) to 0.05 mmol/(m2 d), and the rate was a little higher after bloom than that before bloom. SiO3(2-) -Si was released from sediment all the time with the exchange rate ranging from 0.85 mmol/(m d) to 9.23 mmol/( m2 d), and the rate was higher after bloom than that before bloom. The sediment absorbed PO4(3-) -P before bloom with the exchange rate ranging from -0.06 mmol/(m2 d) to -0.01 mmol/(m2 d), but released PO4(3-) -P after bloom with the exchange rate ranging from 0 mmol/(m2 d) to 1.26 mmol/(m2 d). PO4(3-) -P and NO3(-) -N which were absorbed by sediment in spring restricted the diatom bloom, but their release from sediment after diatom bloom may make a great contribution to subsequent dinoflagellate bloom.

Fluorescence discrimination technology for 11 species of the Red Tide algae at genus level was constructed by principle component analysis and non-negative least squares. Rayleigh and Raman scattering peaks of 3D fluorescence spectra were eliminated by Delaunay triangulation method. According to the results of Fisher linear discrimination, the first principle component score and the second component score of 3D fluorescence spectra were chosen as discriminant feature and the feature base was established. The 11 algae species were tested, and more than 85% samples were accurately determinated, especially for Prorocentrum donghaiense, Skeletonema costatum, Gymnodinium sp., which have frequently brought Red tide in the East China Sea. More than 95% samples were right discriminated. The results showed that the genus discriminant feature of 3D fluorescence spectra of Red Tide algae given by principle component analysis could work well.

Objective: To explore the fact that the east border of Heilongjiang had been a lyme disease natural focus,we investigated the species and distribution of ticks and isolated bacteria from ticks and identified genomic species of Borrelia burdorferi sensu lato. This study provided evidence for prevention and control of lyme disease., Methods: Ticks were caught by flagging method and Direct immunofluorescence method was used to detect the rate of bacteria borne by the tick. BSK UI culture medium was used to isolate the agent and Specific McAbs were used to identify the bacteria. SDS-PAGE protein profile and PCR-RFLP method were also used to identify the species of Spirochetes., Results: Ticks, collected from China-Russia border of east Heilongiiang province were classified including Ixodes persulcatus Schulze, Dermacentor sivarum Olener, Haemaphysalis concinna Kock,and Haemaphysalis japonica Kock. We found that the distributon of ticks was different under different circumstances and the predominant species were also different in different ports. The rate of bacteria borne by Iodes persulaatus Schulze was 31.4% ,by Dermacentor sivarum Olener and Haemaphysalis concinna Kock were 2.2% and 3.8%, respectively. However,it was negative for Haenaphysalis japonica Kock. Spirochetes isolated from Ixodes persulcatus Schulze were collected from Dongning and Tongjiang while Genomic species of Spirochetes, isolated from ticks of the border belonged to B. garinii., Conclusion: All the results showed that the east border of Heilongjiang province was the natural focus of lyme disease.

Background: This study sets out to determine whether adjustments for specific patient caesarean delivery risk factors have an affect on the assessment of performance rates among the municipal hospitals of Taipei City., Methods: Analysis of National Health Insurance (NHI) claims data, linked with birth certificate data, was undertaken on a cohort of 27,693 live births in the six general hospitals of the Taipei Medical Hospital System (TMHS) between 1999 and 2001. Using multivariable logistic regression modeling of the risk factors independently associated with caesarean deliveries, an expected caesarean delivery rate was constructed for each of the hospitals. By contrasting observed rates with expected rates to quantify the magnitude of the deviation from average practice, a measurement similar to relative risk (RR) was also constructed for each hospital., Results: The observed rates for two of the six hospitals examined fell within the expected 95% confidence interval (CI), two were above the expected upper limit, and two were below the expected lower limit. The RR ranking of Hospitals A (RR=1.08, CI=1.01-1.15) and C (RR=1.01, CI=1.00-1.03) improved from first to second, and third to fourth, whilst the RR of Hospitals B (RR=1.09, CI=1.05-1.14) and D (RR=1.02, CI=0.99-1.06) worsened from second to first, and fourth to third, respectively. The RR rankings of Hospitals E (RR=0.92, CI=0.88-0.96) and F (RR=0.80, CI=0.77-0.84) were the same as the observed rates., Conclusions: Caesarean delivery rate profiles, or hospital comparisons without risk adjustment, may be methodologically biased and may lead to unfair judgments by healthcare purchasers.

Objective: This study uses 5-year population data to determine the incidence rate of sudden sensorineural hearing loss (SSNHL) in Taiwan, taking into consideration the age and gender of patients, as well as seasonal variations., Study Design: A retrospective cross-sectional study., Method: The data used is from the Taiwan National Health Insurance Research Database covering the period from 1998 to 2002. A total of 9,267 sample patients were identified from the database by a principal diagnosis of unspecified sudden hearing loss (ICD-9-CM code 3882). The autoregressive integrated moving average (ARIMA) method was also carried out in order to identify any variations in the monthly incidence rates of SSNHL., Results: We find that for the period under examination, gender-specific incidence rates per 100,000 of the population were 8.85 for males, and 7.79 for females, and that there was an increase in age-specific SSNHL incidence with age. The ARIMA test for seasonality was found to be significant for the whole sample. Of all the seasons in Taiwan, the highest SSNHL incidence rates were found in autumn., Conclusions: Our finding of significant variations in the monthly incidence of SSNHL provides valuable information for otolaryngologists and public health officials, creating an awareness of periods of a potential increased risk of SSNHL., (Copyright (c) 2006 S. Objective: This study utilizes 5-year population data to examine the association between weather conditions and the incidence of sudden sensorineural hearing loss (SSNHL) in Taiwan with a specific focus on ambient temperature, relative humidity, atmospheric pressure, rainfall and total hours of sunshine., Method: The data, covering the period from 1998 to 2002, is sourced from the Taiwan National Health Insurance Research Database (NHIRD), with a total of 8712 first-time admissions being identified from the database by a principal diagnosis of unspecified sudden hearing loss (ICD-9-CM code 3882). After controlling for time trend effects, this study adopted the autoregressive integrated moving average regression method as a means of evaluating the effects of climatic and monthly factors on SSNHL incidence rates., Results: Although significant associations were found between ambient temperature, relative humidity and the SSNHL incidence rates for the total population, after adjusting for seasonality, months and trends, the significant relationship between SSNHL incidence rates and the climatic parameters disappeared., Conclusions: This study has demonstrated that after adjusting for seasonality, months and trends, there is no significant relationship between monthly SSNHL incidence rates and weather conditions. Therefore, the theory that weather is a triggering factor in SSNHL pathogenesis is not supported by this study., (Copyright (c) 2006 S. Karger AG, Basel.)

This study sets out to explore the relationship between hospital characteristics, asthma length of stay (LOS), and costs per discharge. The study adopts hospitalization data from the Taiwan National Health Insurance Research Database covering the period from 1997 to 2001. Study subjects were identified from the database by principal diagnosis of asthma or asthmatic bronchitis, with a total of 139,630 cases being included in the study. Multiple-regression analyses were performed to explore the relationship between LOS, costs per discharge and hospital characteristics, adjusting for age, gender, and discharge status of patients, as well as complications or comorbidities. The regression analyses showed that, compared with district hospitals, medical centers and regional hospitals have longer and more statistically significant LOS, as well as higher costs. Hospitals operating on a for-profit basis have shorter LOS and lower costs than public and not-for-profit hospitals. This study shows the existence of wide variations in LOS and costs per discharge for asthma hospitalizations, between the various types of hospitals in Taiwan.

Aim: To obtain the information of ligand-receptor binding between the S protein of SARS-CoV and CD13, identify the possible interacting domains or motifs related to binding sites, and provide clues for studying the functions of SARS proteins and designing anti-SARS drugs and vaccines., Methods: On the basis of comparative genomics, the homology search, phylogenetic analyses, and multi-sequence alignment were used to predict CD13 related interacting domains and binding sites in the S protein of SARS-CoV. Molecular modeling and docking simulation methods were employed to address the interaction feature between CD13 and S protein of SARS-CoV in validating the bioinformatics predictions., Results: Possible binding sites in the SARS-CoV S protein to CD13 have been mapped out by using bioinformatics analysis tools. The binding for one protein-protein interaction pair (D757-R761 motif of the SARS-CoV S protein to P585-A653 domain of CD13) has been simulated by molecular modeling and docking simulation methods., Conclusion: CD13 may be a possible receptor of the SARS-CoV S protein, which may be associated with the SARS infection. This study also provides a possible strategy for mapping the possible binding receptors of the proteins in a genome.

Primary non-Hodgkin's lymphoma (NHL) of the sinonasal tract is uncommon. Morphologically and radiographically, sinonasal lymphomas are difficult to distinguish from other malignant neoplasms or non-neoplastic processes. However, there is no general consensus about the need of routine histopathological examination for nasal polyps among otolaryngologists. We present a case of primary sinonasal NHL mimicking chronic rhinosinusitis and debate the issue of routine histopathological examination. We suggest that all tissues removed from the sinonasal tract during surgery must be submitted for histopathological examination. Failure to do so may miss diagnosis and delay appropriate treatment.
