Objectives: Hypertension is the leading cause of HF in sub-Saharan Africa. Electrocardiography (ECG) is a cheap and easily available stratification tool for the diagnosis and prognostication of individuals with hypertension. The aim of this study was to develop an ECG-based HF diagnostic score among patients with hypertension attending a specialist cardiology clinic. Methods: One hundred and one (101) case-control age- and sex-matched pairs were recruited. The study population were adults with a clinical diagnosis of hypertensive HF failure (cases) and systemic hypertension without HF (controls). Participants underwent clinical assessment and ECG. Associations between ECG variables and HF risk were tested with chi square test. Logistic regression modelling (age- and sex adjusted) was trained on a random subset of participants and tested on the remaining participants to determine the ECG abnormalities that are diagnostic of HF and develop a HF diagnostic score. The HF diagnostic score was then validated in an independent dataset of the ECG-Hypertension Audit. Goodness of fit and c-statistics of the HF summed diagnostic score in the training, testing and validation datasets are presented. A two-sided p value of <0.05 was considered statistically significant. Results: The independent ECG diagnostic markers of HF among hypertensive patients in this study in decreasing order of effect size were sinus tachycardia (aOR: 7.72, 95% CI: 2.31-25.85). arrhythmia (aOR: 7.14, 95% CI: 2.57-19.86), left ventricular hypertrophy (aOR: 4.47; 1.85-10.77) and conduction abnormality (aOR: 3.41, 95% CI: 1.21-9.65). The HF summed diagnostic score showed excellent calibration and discrimination in the training (Hosmer Lemeshow p=0.90; c-statistic 0.82; 95% CI 0.76e0.89) and test samples (Hosmer Lemeshow p=0.31; c-statistic 0.73 95% CI 0.60 to 0.87) of the derivation cohort and an independent validation audit cohort (Hosmer Lemeshow p=0.17; c-statistic 0.79 95% CI 0.74 to 0.84) respectively. The model showed high diagnostic accuracy in individuals with different intermediate pre-test probabilities of HF. Conclusions: A ECG based HF score consisting of sinus tachycardia, arrhythmia, conduction abnormality and left ventricular hypertrophy is diagnostic of HF especially in those with intermediate pre-test probability of HF. Objective: To identify predominant dietary patterns in four African populations and examine their association with obesity.Design: Cross-sectional study.Setting/SubjectsWe used data from the Africa/Harvard School of Public Health Partnership for Cohort Research and Training (PaCT) pilot study established to investigate the feasibility of a multi-country longitudinal study of non-communicable chronic disease in sub-Saharan Africa. We applied principal component analysis to dietary intake data collected from an FFQ developed for PaCT to ascertain dietary patterns in Tanzania, South Africa, and peri-urban and rural Uganda. The sample consisted of 444 women and 294 men.Results: We identified two dietary patterns: the Mixed Diet pattern characterized by high intakes of unprocessed foods such as vegetables and fresh fish, but also cold cuts and refined grains; and the Processed Diet pattern characterized by high intakes of salad dressing, cold cuts and sweets. Women in the highest tertile of the Processed Diet pattern score were 3·00 times more likely to be overweight (95 % CI 1·66, 5·45; prevalence=74 %) and 4·24 times more likely to be obese (95 % CI 2·23, 8·05; prevalence=44 %) than women in this pattern's lowest tertile (both P<0·0001; prevalence=47 and 14 %, respectively). We found similarly strong associations in men. There was no association between the Mixed Diet pattern and overweight or obesity.Conclusions: We identified two major dietary patterns in several African populations, a Mixed Diet pattern and a Processed Diet pattern. While outbreaks of infectious diseases have long presented a public health challenge, especially in developing countries like Nigeria; within recent years, the frequency of such outbreaks has risen tremendously. Furthermore, with the recent outbreaks of emerging and re-emerging infectious diseases such as Ebola virus disease and other epidemic prone diseases in Nigeria demanding immediate public health action, there is a need to strengthen the existing notifiable disease surveillance and notification system with increased clinicians' involvement in timely reporting of notifiable diseases to designated public health authorities for prompt public health action. Hence, this paper provides the opportunity to increase awareness among clinicians on the importance of immediate reporting of notifiable diseases and intensify engagement of clinicians in disease notification activities by describing various notifiable diseases in Nigeria using their surveillance case definition, outlines the reporting channel for notifying these diseases and highlights the roles of clinicians in the current disease surveillance and notification network for early disease outbreak detection and public health response in Nigeria. Objective: Home management of malaria (HMM) strategy was based on presumptive diagnosis of malaria and use of correct dose of chloroquine. However, the development of chloroquine resistant parasites in most endemic areas necessitated the recommendation of artemisinin combination therapy (ACT); and this has been demonstrated to be effective in HMM. However, the recommendation that ACT should be dispensed only to laboratory confirmed cases underscores the need to review the accuracy of mothers' presumptive diagnosis and provide evidence for a switch to parasite based diagnostic test. Methods: This was a follow-up study nested to a larger controlled intervention study carried out to assess the effect a malaria treatment guideline on mothers' adherence to correct treatment of malaria in children. In a subset, thick blood smears were prepared from finger prick blood samples of febrile children who have been presumptively diagnosed to have malaria by mothers on Days 0, 1, 2, 3, 7 and 14 to detect parasitaemia. The presumed diagnosis was compared with the presence of parasitaemia. Results: A total of 162 children with febrile illness (88 and 74 in intervention and control groups respectively) were studied. Sixty-four (72.7%) and 62 (83.8%) of the febrile cases presumed to have malaria in the intervention and control groups respectively had parasitaemia on Day 0. The sensitivity and specificity of mothers' diagnoses was 78.1% and 29.2%; 82.3% and 8.3%; in the intervention and control groups respectively. Conclusion: The low specificity of presumptive diagnosis has implication for the deployment of ACT at the community level in Nigeria. This supports current views on the need for a more sensitive and specific parasite-based diagnosis of malaria before ACT treatment. Further study to compare presumptive diagnosis of malaria with malaria diagnostic test carried out at the community level is needed to inform policy on the adoption of parasite based malaria diagnosis at community level. Purpose: To examine the association between benign breast disease (BBD) and breast cancer (BC) in a heterogeneous population of African women., Methods: BC cases and matched controls were enrolled in three sub-Saharan African countries, Nigeria Cameroon, and Uganda, between 1998-2018. Multivariable logistic regression was used to test the association between BBD and BC. Risk factors dually associated with BBD and BC were selected. Using a parametric mediation analysis model, we assessed if selected BC risk factors were mediated by BBD., Results: Of 6418 participants, 55.7% (3572) were breast cancer cases. 360 (5.7%) self-reported BBD. Fibroadenoma (46.8%) was the most reported BBD. Women with a self-reported history of BBD had greater odds of developing BC than those without (adjusted odds ratio [aOR] = 1.47, 95% CI: 1.13-1.91). Biopsy-confirmed BBD was associated with BC (aOR = 3.11, 95% CI: 1.78-5.44). Introduction: Injury-related morbidity is a neglected health concern in many low-income and middle-income countries. Most injury data in Africa have been collected from hospital-based studies, and few studies have occurred across multiple countries. Using data from a novel cohort, we examined the prevalence and incidence of serious injuries and associated risk factors across five sites in sub-Saharan Africa (SSA)., Methods: A common baseline and follow-up survey was administered to participants. The study population included 1316 persons at baseline and 904 persons at follow-up. Frequencies were calculated, and logistic regression models were used to assess risk factors for injury., Results: A total of 233 (17.7%) persons reported a serious injury at baseline and 60 (6.6%) reported a serious injury 6 months later at follow-up. Sixty-nine per cent of participants responded to the follow-up questionnaire. At baseline and follow-up, the most common cause of serious injury at urban sites was transport related, followed by poison/overdose. In rural Uganda, sharp instruments injuries were most common, followed by transport-related injuries. Living at an urban site was associated with an increased odds for serious injury compared with those at the rural site (OR: 1.83, 95% CI 1.15 to 2.90). Participants who consumed above a moderate amount of alcohol were at a higher risk of serious injury compared with those who did not consume alcohol (OR: 1.86, 95% CI 1.02 to 3.41). High level of education was an important risk factor for injury., Conclusion: At baseline and follow-up, common causes of serious injury were transport related, sharp instrument and poison/overdose. Alcohol consumption, urban location and education are important risk factors for injury. It is feasible to collect longitudinal injury data using a standardised questionnaire across multiples sites in SSA. Background: The Roll Back Malaria (RBM) initiative recommended that all pregnant women receive Inter mittent Preventive Treatment (IPTp) and that by 2010 at least 80% of people at risk of malaria (including pregnant women) use insecticide-treated bednets (ITN) in areas with stable transmission. We evaluated ITN/IPTp coverage, explored its associated factors, and estimated the number of pregnancies protected from malaria., Materials and Methods: This analysis was based on data from the 2012 National HIV/AIDS and Reproductive Health Survey (NARHS Plus). To assess ITN coverage, we used the population of women that was pregnant (n=22,438) at the time of the survey. For IPTp coverage, we used women that had a live birth in the 5 years preceding the survey (n= 118,187) and extracted the population of pregnant women that, during their last pregnancy, received drugs for protection against malaria. We estimated the number of live births using the projected population of females in each state, population of women of child -bearing age and the total fertility rate. The estimated number of pregnancies covered/protected by ITN and IPTp was obtained from a product of the estimated live births and the reported coverage. Multivariate logistic regression was used to determine factors associated with ITN and IPTp use., Results: We estimated that there were 5,798,897 live births in Nigeria in 2012, of which 3,537,327 and 2,302,162 pregnancies were protected by ITN and IPTp, respectively. Four of 36 states achieved the 80% RBM target for ITN coverage. No state achieved the 100% target for IPTp. Education and socio-economic status were associated with IPTp use., Conclusion: ITN cover age was higher than in previous estimates even though it is still below the RBM targets. Background: In or der to combat the bur den of malaria, different strategies, including Insecticide Treated Nets (ITNs), have been put in place. ITNs have been distributed with support from international donors and this necessitates an increase in monitoring and evaluation efforts in order to determine ITN impact as well as prioritise future programmes. The current standard for estimating impact indicators of ITNs are household surveys. These, however, are expensive and not conducted frequently. Collecting information from school children has been found to be a cheap and fast means for routine monitoring and evaluation of malaria control programmes in sub-Saharan Africa. This study was conducted to explore school children's report of household ownership and use of ITNs in Oyo State, Nigeria., Materials and Methods: A cross-sectional survey was conducted. A three-stage sampling technique was used to select 611 pupils from 15 out of 88 primary schools. Information on pupils' socio-demographics, report of household ownership and use of ITNs were obtained using a semi-structured interviewer-administered questionnaire. Data was analysed using descriptive statistics and Chi-square tests at 5% level of significance., Results: Respondents' mean age was 10.5±1.7 yrs; 52.7% were females, 84.6% were Yoruba and 65.3% lived with children below 5 yrs of age in their households. Most of the respondents (81.7%) reported household ownership of at least one ITN. The majority (76.4%) obtained nets through mass distribution campaigns. Most of the respondents (89%) reported use of ITNs by a household member the night preceding the survey. More than half of the respondents (51.6%) reported ITN use by children below 5 yrs of age. Class was significantly associated with reported household ownership of ITNs (χ2= 9.217, p <0.010)., Conclusion: The majority of the pupils reported household ownership and use of ITNs. Background: Febrile illnesses are common causes of morbidity and mortality among under-five children in sub-Saharan Africa. The recommended strategy for effective case management of uncomplicated malaria is parasitological confirmation prior to use of artemisinin-based combination therapy (ACT). There is a lack of qualitative information explaining factors, which influence malaria case management practices among health workers. This study explores the perceptions of health managers and health care providers on the case management of uncomplicated malaria among under-fives in selected primary health care (PHC) facilities of two Local Government Areas (LGAs), Katcha and Gbako, as part of baselines for capacity-building interventions planned in Niger State, Nigeria., Methods: Interviewees included state- and LGA-level health programme managers, and frontline health workers purposively selected to cover a range of cadres involved in case management of sick children. Issues explored were history taking, diagnosis, appropriate diagnosis of malaria, prescription for malaria, referrals and adherence to referral. Data coding was carried out with Nvivo qualitative software (version 8) and content analysed., Results: History taking was often not carried out appropriately by the health workers. Treatment of malaria was not based on parasite-based diagnosis. Most of the health workers reported that they prescribed ACTs for treating presumed uncomplicated malaria. Care givers' preferences, poor transportation systems and lack of financial resources led to poor adherence to referral advice. Absence of health workers from their duty post hindered effective service delivery. Prescription of ACTs as a first line of treatment for uncomplicated malaria without a parasite-based diagnosis was the standard case management practice., Conclusion: Parasite-based diagnosis for malaria will invariably lead to better treatment for non-malaria fever cases among the studied age group. Background: Malaria vaccine is a potentially effective addition to the armamentarium for malaria control. The candidate RTS,S malaria vaccine has undergone phase III clinical trials and WHO has indicated that a policy recommendation is possible in 2015. Given the delays with adoption of other novel interventions including vaccines, there is a need to ensure that all elements that will inform the decision to adopt a malaria vaccine, including community willingness, will be available on time. This study was carried out to assess the willingness of caregivers of under-5 children to accept a malaria vaccine once available and recommended for use., Materials and Methods: 427 consenting caregivers, selected using a cluster sampling technique, from five communities in Ibadan, Nigeria, participated in a questionnaire. In-depth interviews (IDIs) were conducted among 47 key community members. Data were analysed using descriptive statistics, Chi-square and logistic regression at p ≤0.05. Thematic content analysis was used to analyse the transcribed IDI data., Results: The mean age of survey respondents was 29.8±5.8 years. Only 20.1% of the respondents had ever heard of malaria vaccine; 87.0% showed willingness to accept a malaria vaccine. Reasons stated for not willing included 'husband did not want immunisation' (73.6%), 'felt it might be expensive' (47.2%) and 'felt it might paralyse children' (24.5%). Nearly half (48.7%) of the respondents said that if vaccine is not given orally like polio vaccine it might not be accepted. Influence of community health workers was found to predict willingness to accept a malaria vaccine (OR: 0.316, 95% CI: 0.142-0.705). IDI participants were favourably disposed to introduction of a vaccine against malaria, although they had concerns about the formulation of the vaccine and possible adverse events., Conclusion: Well-designed communication strategies implemented prior to the introduction of a malaria vaccine would be essential to foster a supportive environment for eventual adoption and acceptance thereof., Competing Interests: Competing interests: No competing interests declared., (Copyright © 2015: B.I. Malaria control efforts currently lay emphasis on reducing transmission by limiting human-vector contact. More studies have been carried out on mosquito avoidance practices in the rural areas, leaving the urban areas understudied. This study was conducted to identify knowledge of malaria transmission and to investigate geographical/ecological differentials in the use of insecticide-treated nets (ITNs) among caregivers of under-fives in Somolu Local Government Area, Lagos State. A household survey was conducted by interviewing 394 female caregivers of under-fives selected using the WHO Lot Quality Technique from communities stratified based on level of planning and drainage. The mean age of the respondents was 33.6 ± 7.7 years. Malaria transmission was attributed mostly to mosquito bites in all strata: S1 (58.3%), S2 (56.1%) and S3 (61.4%). Mosquito net was mentioned as a preventive measure by: 59.3% (S1), 80.7% (S2) and 64.3% (S3). Ownership of long-lasting insecticidal nets was: 76.0% (S1), 75.4% (S2) and 68.6% (S3), and of these, 73.1% (S1), 70.7% (S2) and 72.4% (S3) reported that their child slept under the net the night before the survey. There is a need to reinforce education on transmission and ownership of ITNs especially among caregivers in unplanned, poorly drained communities.

Objective: This study determined the level of knowledge of malaria and preventive measures among pregnant women and its influence on the uptake of preventive measures., Methods: A cross-sectional survey was carried out among 209 participants selected from pregnant women attending antenatal clinics in primary healthcare centres in Irepodun/Ifelodun, a local government area in Ekiti state, Nigeria., Results: Knowledge of malaria was found to be very good, average and poor among two (1.0%), 165 (78.9%) and 42 (20.1%) respondents, respectively. Of the 109 (52.2%) respondents who had heard about intermittent preventive treatment, eight (7.3%) scored "very good" on knowledge, while 53 (48.6%) and 48 (44.1%) scored "average" and "poor," respectively. Of the 144 (68.9%) respondents who had heard about insecticide-treated nets, 95 (66.0%) scored "good" on knowledge, while 49 (34.0%) scored "poor." Factors that significantly influenced knowledge about malaria were occupation, level of education, months at first appearance at antenatal clinic and transportation cost. Knowledge significantly influenced uptake of insecticide-treated nets and intermittent preventive treatment in pregnancy ( p < .05)., Conclusion: There is a need to intensify efforts to provide health education on malaria and preventive measures as well as to encourage preventive practices among pregnant women.

Studies of care seeking in Nigeria show that a tremendous amount of treatment for malaria takes place at home and, in most instances, such treatments are incorrect. This deficiency is attributed to caregivers' poor knowledge of treatment. This study was designed to empower households to treat malaria correctly in partnership with community members. Selected mothers from study communities were trained as "mother trainers" and were expected to train other members of their communities using a treatment protocol. "Mother trainers" were acceptable to most communities and judged to be effective. They were enthusiastic and their participation in the study boosted their ego and status in the community. Drop-out-rate of "mother trainers" was 24.2%. A few limitations to the use of mothers as trainers that were identified are discussed. It is concluded that mothers have good potential to effectively carry out health education activities in the community if appropriately selected, trained, and supervised.
