Objective: In this study, researchers used two different adhesives to determine which ones had the strongest push-out connection between resin composite posts and primary teeth's intracanal dentin. Methods: Fifty primary lateral incisors were split into two groups (n = 25) and treated with either Adper Single Bond 2 (ASB group) or Clearfil SE Bond 2 (CSE). Resin composite was used to fill the canal in the root's coronal one-third. A universal testing equipment was used to conduct the push-out evaluation. The data was analyzed using t-test. Results: Strengths of push-out bonds were found to be significantly different amongst the two groups using t-test (p=0.000). The mean±S.D push-out bond strength of the samples in ASB group was 8.41±4.83 Mpa, while mean±S.D push-out bond strength of the samples in CSE group was 14.21±4.03 Mpa. Most failures were adhesive, and there was no difference (p=0.327) in the fracture mode distribution between bonding agents. Conclusion: In terms of push-out bond strength, CSE group was clearly superior to ASB group. When placing resin composite posts in primary anterior teeth, it is advised to utilize a universal adhesive system and self-etch adhesives from the sixth generation. Due to their simplicity of use, reduced technical sensitivity, and reduced number of required clinical processes, bonding agents may be a desirable choice for repairing primary teeth with short resin composite posts. The Coronavirus pandemic has significantly disrupted various sectors across the world. The education sector is one of them. Due to Covid-19 the government across the world temporarily started shutting down schools and colleges. As the days pass by with no immediate solution to stop the outbreak of Covid-19, school and university closures will not only have a short-term impact on the continuity of learning for more than 285 million young learners in India but also engender far-reaching economic and societal consequences. This paper provides an overview of the impact of Covid-19 on education across the world, examining how it has affected different stages of education, from pre-school to tertiary level. The paper highlights the challenges faced by educators, students, and parents, including school closures, remote learning, and increased inequality. Additionally, the paper discusses the measures implemented by different countries to mitigate the impact of Covid-19 on education, including the use of technology, revised curriculum, and blended learning. The paper concludes that Covid-19 has brought significant changes to the education system, which will have lasting effects on the future of education worldwide.

Tungsten Sulfide (WS2) has been appeared as visible range semi-conductors with having substantial direct band gap. In two-dimensional (2D) transition metal dichalcogenides (TMDC) i.e.WS2 has been growing research attention over more than the past twenty years in low cost, energy-efficient, adaptable or environment-friendly material, which is crucial part of optoelectronic emission devices. WS2 semiconductor thin films were grown on glass and Si substrates by using DC sputtering method. During deposition the substrate were kept at temperature of 200°C. The thin films were studied by X-ray diffraction, UV-Vis spectrometry and Spectrofluorometer (FS5). XRD analysis revealed the nature of grown film effected by temperature of substrate results in to be amorphous. Photoluminescence analysis at 532nm and optical study proved the direct to indirect band transition in WS2.

Women empowerment is crucial for a society to have a knowledge-based society wherein the personal growth of women will decisively contribute to individual families, thereby contributing to the society and the country as a whole. Social Marketing is a rising trend and a powerful channel for discussing women's empowerment. This study attempts to examine the impact of the Benazir Income Support Program (BISP) on women's empowerment in Pakistan. BISP is one of the rigorous social welfare programs initiated by the government of Pakistan. The program was initiated to empower economically deprived women. It provides social security, improves living standards, and reduces poverty. To gather the data, a sample of 300 respondents was utilized from Khairpur, Pakistan. The study carried out quantitative techniques to measure the impact of BISP on women's empowerment, thus, for this, regression analysis was conducted, and the association between the variables was explained through Pearson correlation. Furthermore, the questionnaire was designed and distributed among BISP beneficiaries to understand its effect. Hypotheses were tested and validated through statistical analysis. The findings of the study recommend that there is a positive and significant impact of BISP on women's empowerment and it has a strong correlation with women's empowerment. In this study, the prediction of living standards through this program contributed 35%, poverty alleviation strategy 29% and social security contributed 24%. In this regard, it is ensured that the government of Pakistan may take more similarly effective initiatives to empower not only women but without gender discrimination to empower communities at large.

This article pertains to a six-volume work titled “Kitāb al-Khilāf fῑ Al-Aḥkām” which roughly translates to "Book of Differences Regarding Juristic Rulings." This book is part of a wider genre of Shīʿī legal literature in which the writer poses a legal issue, states the Shīʿī position, describes the position of the other prominent legal schools (including any partisans who may have different or nuanced perspectives), and then provides reasoning (dalīl) for why the Shīʿī perspective is correct. The book contains all of the chapters one would expect in a work of fiqh (both acts of ritual worship as well as societal dealings), such as purity, almsgiving, business transactions, etc.

Breast cancer (BCa) is the most prevalent cancer in females and has a high rate of mortality, especially due to increased metastasis to skeletal and non-skeletal sites. Despite the marked clinical accomplishment of immune checkpoint inhibitor (CPI) therapy in patients with several cancers, it has had limited success in luminal subtypes of BCa. Accordingly, recent efforts have focused on combination therapy with CPI, including epigenetic modulators, to increase response rates of CPI in luminal BCa. We have previously shown that S-adenosylmethionine (SAM), the ubiquitous methyl donor, has strong anti-cancer effects in various cancers, including all subtypes of BCa. In the current study, we took a novel approach and examined the effect of CPI alone and in combination with SAM on tumor growth and metastasis in a syngeneic mouse model of luminal B BCa. We showed that SAM decreases cell proliferation, colony-formation (survival), and invasion of luminal B BCa cell lines (Eo771, R221A) in vitro. In in vivo studies, in Eo771 tumor-bearing mice, either SAM or anti-PD-1 antibody treatment alone significantly reduced tumor growth and progression, while the SAM+anti-PD-1 combination treatment had the highest anti-cancer efficacy of all groups. The SAM+anti-PD-1 combination reduced the percentage of animals with lung metastasis, as well as total metastatic lesion area, compared to control. Additionally, the SAM+anti-PD-1 combination significantly reduced the skeletal lesion area and protected tibial integrity to a greater extent than the monotherapies in an Eo771 bone metastasis model. Transcriptome analysis of Eo771 primary tumors revealed significant downregulation of pro-metastatic genes, including Matrix metalloproteinases (MMPs) and related pathways. On the other hand, CD8+ T cell infiltration, CD8+ T cell cytotoxicity (elevated granzymes), and immunostimulatory genes and pathways were significantly upregulated by the combination treatment. The results presented point to a combination of SAM with CPI as a possible treatment for luminal B BCa that should be tested in clinical studies.

BACKGROUND: The coronavirus disease 2019 (COVID-19) pandemic's effects on nephrology fellows' educational experiences, preparedness for practice, and emotional wellbeing are unknown. METHODS: We recruited current adult and pediatric fellows and 2020 graduates of nephrology training programs in the United States to participate in a survey measuring COVID-19's effects on their training experiences and wellbeing. RESULTS: Of 1005 nephrology fellows-in-training and recent graduates, 425 participated (response rate 42%). Telehealth was widely adopted (90% for some or all outpatient nephrology consults), as was remote learning (76% of conferences were exclusively online). Most respondents (64%) did not have in-person consults on COVID-19 inpatients; these patients were managed by telehealth visits (27%), by in-person visits with the attending faculty without fellows (29%), or by another approach (9%). A majority of fellows (84%) and graduates (82%) said their training programs successfully sustained their education during the pandemic, and most fellows (86%) and graduates (90%) perceived themselves as prepared for unsupervised practice. Although 42% indicated the pandemic had negatively affected their overall quality of life and 33% reported a poorer work-life balance, only 15% of 412 respondents who completed the Resident Well-Being Index met its distress threshold. Risk for distress was increased among respondents who perceived the pandemic had impaired their knowledge base (odds ratio [OR], 3.04; 95% confidence interval [CI], 2.00 to 4.77) or negatively affected their quality of life (OR, 3.47; 95% CI, 2.29 to 5.46) or work-life balance (OR, 3.16; 95% CI, 2.18 to 4.71). CONCLUSIONS: Despite major shifts in education modalities and patient care protocols precipitated by the COVID-19 pandemic, participants perceived their education and preparation for practice to be minimally affected.

Current screening and confirmatory testing lack sensitivity and carry a poor negative predictive value.

The enumeration of biological entities is a critical part of experimental assays and usually requires large lengths of time. The standard method is to count the entities by hand or with OpenCV-based software, which can lead to inaccurate results. Here, we propose an online platform for biologists consisting of a system with multiple trained machine learning weights to detect various biological entities such as yeast colonies, bacterial colonies, and melanoma clusters. The Pytri model achieved a median relative error rate of 7.56% for bacterial and yeast colonies on Petri dishes, 6.58% for colonies on 96-well plates and 10.28% for melanoma cluster microscopy images. We showcase the application of state-of-the-art deep learning tools in bacterial entity detection, achieving significantly higher accuracy than traditional methods when compared to our base standard manual count.

Concern for contrast-induced acute kidney injury (CI-AKI) or nephrogenic systemic fibrosis may lead to withholding important studies from patients with kidney disease. However, the actual risk or even the existence of these conditions has recently been called into question. The truth probably lies somewhere in the middle.

This prospective study was conducted over the period of one year at M.Y. Hospital & Chacha Nehru Bal Chikitsalaya, Indore, in the Department of Pediatrics, M.G.M. Medical College, Indore.Detailed history of present illness with duration of convulsion were enquired. A detailed history of mother during antenatal, natal and post natal period was taken. Type of delivery (vaginal/ LSCS ; full term / preterm), enquired about the indication of LSCS. History of labour with particular ref to birth injury, asphyxia (delayed cry), neonatal history regarding jaundice & feeding history, detailed family history in relation to epilepsy convulsion and mental diseases were recorded. Out of 100 cases 64% were males and 36% were females. Male female ratio was 1.77:1. Male cases were predominantly present in all 3 age groups. Majority of seizures occurred within 24 hrs. P value is not significant. Out of 100 cases 43 had tonic convulsions, 38 had subtle, and 19 had focal convulsions.43 neonates had seizures in 1st 24 hrs. Subtle seizures (60%) were most common followed by tonic seizures (32.5%) and then focal seizures (7.5%). In 2 – 7 day age group tonic seizures (42.8%) were most common followed by focal seizures (38%). In > 7 day age group also tonic seizures (73.6%) were most common. P value was < 0.001. Detailed clinical examination was done & investigations were carried out in all patients. Cranial USG, EEG, & CT scan were done as per requirement. Out of total 100 cases 74 were full term & 26 were preterm. In our study maximum no. of cases developed seizure within 1st 24 hrs (43%) of life followed by 42% in 2–7 day age group & 15% after 7 days. Convulsions were more common in males (64%) in all age group.

Out of 74 term babies most common cause of seizure was birth asphyxia (58%) followed by meningitis (16.2%), hypocalcaemia (13.5%), hypoglycemia (8.1%). Out of 26 preterm babies 30% had meningitis, 30% had hypoglycemia, 19.2% had hypocalcaemia, and 15.3% had birth asphyxia. 1 case of ICH was reported. P value was 0.004. In present study out of total 100 cases most common etiology was birth asphyxia (47%), followed by meningitis (20%), hypocalcaemia (15%), and hypoglycemia (14%). During 1st 24 hrs. most common cause was birth asphyxia (88%). In 2 – 7 day age group metabolic abnormalities (hypocalcaemia & hypoglycemia) was most common (54.5%). And after 7 days meningitis (80%) was most common etiology. In term babies most common etiology was birth asphyxia (58%) followed by metabolic (21%) & meningitis (16.2%). In preterm babies most common cause was metabolic (50%) followed by meningitis (30%).

The utilization of Fiber Optic (FO) in 5G communication systems has achieved several advantages such as increasing the capacity and the bit rate with a reduction in the total implementation cost. Radio over Fiber (RoF) systems could form the basis for 5G communication networks, due to several reasons related to its ability to provide the required bandwidth for the broadband data transmission to the end-users, other advantages are related with the lower loss in attenuation and higher immunity to RF (Radio Frequency) interference. Particularly, the utilization of Wavelength – Division - Multiplexing (WDM) and Dense - Wavelength - Division - Multiplexing (DWDM) techniques with the RoF system has gained all the attention of researchers in the last few years. As a result, this paper demonstrates a review of the proposed systems, schemes, and methods that contribute in enhancing the WDM and DWDM based RoF systems.

Anemia is a well-known complication of chronic kidney disease, and its treatment remains a challenge. Although erythropoiesis-stimulating agents (ESAs) raise hemoglobin levels, their benefits appear to be limited to decreasing the number of blood transfusions needed and perhaps improving quality of life. The newly developed prolyl hydroxylase inhibitors (PHIs)-agents that increase endogenous erythropoietin production-promise to improve outcomes for patients with anemia of chronic kidney disease. Randomized controlled trials have found these drugs to be at least as effective as ESAs, and the drugs are used in other countries. However, PHIs have yet to be approved in the United States.

Diabetic kidney disease (DKD) is the most common cause of end-stage kidney disease (ESKD) in the United States. Risk factor modification, such as tight control of blood glucose, management of hypertension and hyperlipidemia, and the use of renin–angiotensin–aldosterone system (RAAS) blockade have been proven to help delay the progression of DKD. In recent years, new therapeutics including sodium-glucose transport protein 2 (SGLT2) inhibitors, endothelin antagonists, glucagon like peptide-1 (GLP-1) agonists, and mineralocorticoid receptor antagonists (MRA), have provided additional treatment options for patients with DKD. This review discusses the various treatment options available to treat patients with diabetic kidney disease.

Background: Fracture-related infections (FRI) remain a difficult consequence for orthopedic trauma patients, their relatives, the treating physicians, and the healthcare systems. Delayed fracture-related infection is an important step in the infection process that can be controlled by diagnosing and preventing it from moving to the next level. Neutrophils CD64 and CD66b were identified as sensitive indicators in the event of infection. Normal sequential changes, on the other hand, occur after surgery and are extremely high. They are back to normal on the 10th day after the operation. The aim of this study was, therefore, to examine the risk factors associated with fracture-related infection by comparing cluster of differentiation (CD) indicators with conventional markers and comparing them with gold standards culture reports. As a result, it could be an early sign of a closed fracture infection. Material & Methods: Between February 2020 and March 2021, 510 patients from the Department of Orthopedics at King George Medical University in Lucknow agreed to participate in the study. The study included patients who had a closed fracture and had undergone elective or emergency surgery. Blood was withdrawn before the surgery (baseline) on day one and again on the third, seventh, and 10th day after the operation to measure the quantitative measurements of the biomarkers (total leucocyte count [TLC], erythrocyte sedimentation rate [ESR], C reactive protein [CRP], CD64, and CD66b) in all follow-up examinations. Patients were monitored for delayed signs of the infection for 2 to 10 weeks. The biomarkers were evaluated and linked to the culture reports. Results: Of the 510 patients included, 272 were men (53.3%) and 238 women (46.7%), the mean age was 40 (20-78), the mean age for fracture related infection with positive culture (FRI POS) was 48.0 (SD: 19.47), for fracture related infection with negative culture (FRI NEG) was 46.20 (SD: 17.18), and for patient with no signs of infections (NON-FRI) was 45.13 (SD: 17.62) (p

Introduction: Hypernatremia and hypokalemia have been reported with the use of Tropomyosin Receptor Kinase Inhibitors (TRKI) but the pathophysiology is not clear. We hereby describe a 72-year-old male with metastatic papillary thyroid carcinoma presenting with hypernatremia and hypokalemia after starting Entrectinib therapy. Case description: A 72-year-old male with metastatic papillary thyroid carcinoma was admitted to the hospital for failure to thrive and worsening mental status 6 weeks after starting Entrectinib therapy. He was found to have hypernatremia at 154 mmol/L and hypokalemia at 2.9 mmol/L. Serum and urine osmolalities were 320 and 293 mOsm/kg, respectively with an elevated 24 h urinary potassium of 132 mmol. Polydipsia and polyuria were reported raising the suspicion for underlying diabetes insipidus (DI). Twenty-four hour urine output was around 4.5 L. A DDAVP challenge was performed with no change in hourly urine output and a trivial change in urine osmolality toward 339 mOsm/kg indicating a possible partial nephrogenic DI. Potassium was supplemented and patient was treated with free water enterally and parenterally while stopping Entrectinib therapy. One week later, a water deprivation test failed to elicit a hypernatremic response. Unfortunately, urine osmolality variations with water deprivation were not checked but urine volumes dropped significantly suggesting resolution of presumed nephrogenic DI. Conclusion: Hypernatremia has been reported with Tyrosine Kinase Inhibitors but no pathophysiology has been proposed. Hypokalemia however has not been reported. This case reports hypokalemia and hypernatremia in association with Entrectinib therapy with renal potassium wasting and partial nephrogenic as possible pathophysiologic bases respectively. These pathologies seem to be reversible with drug cessation. With the growing use of targeted therapies in oncology and beyond, it seems prudent to better understand the unique electrolyte and water balance abnormalities that might occur so that patients can be counseled and managed appropriately.

Current maritime transportation and shipping is characterized by rapid technological developments effecting the basic concepts of operating ships and even changing traditional paradigms of controlling ships. The e-Navigation concept of the International Maritime Organization (IMO) specifically aims at more comprehensive and reliable support of the human operators on-board and ashore. However, autonomous unmanned ships remote controlled or even autonomously navigating are expected to come soon. In this paper, selected operational aspects of maritime traffic merging conventional and unmanned remote controlled ships in coastal areas are discussed. Furthermore, some preliminary results of experimental simulation studies into a future scenario of maritime traffic are presented and preliminary conclusions in respect to job profiling and training requirements are discussed.

This work focuses on the development of a deterministic dynamic risk method that can be used by operational end-users such as seafarers, when navigating in restricted waters where there is spatial conflict due to multi-use of marine areas by the shipping and offshore renewable energy industries. The developed method can also be used during the planning stages of offshore renewable energy installations. A case study of vessel operations near an offshore wind farm is also presented. The article also briefly discusses the potential application of manoeuvring-based dynamic risk methods to resolve spatial conflicts in the maritime, as well as other transport domains.

The incidence of hematologic malignancies is on the rise worldwide. Kidney disease is ubiquitous in patients with hematologic malignancies, encompassing a wide spectrum of disorders involving each kidney compartment, including the vasculature, tubules, interstitium, and glomerulus, and there is significant overlap of kidney involvement with each hematologic malignancy. Vascular disorders include both microvascular and macrovascular damage, via thrombotic microangiopathy, hyperleukocytosis, hyperviscosity, and cryoglobulinemia. The tubulointerstitial compartment may be affected by prerenal azotemia and acute tubular injury, but malignant infiltration, tumor lysis syndrome, extramedullary hematopoiesis, cast nephropathy, granulomatous interstitial nephritis, and lysozymuria should be considered in certain populations. Obstructive uropathy may occur due to nephrolithiasis or retroperitoneal fibrosis. Glomerular disorders, including membranoproliferative, membranous, minimal change, and focal segmental glomerulosclerosis, can rarely occur. By understanding how each compartment may be affected, care can best be optimized for these patients. In this review, we summarize the widely varied etiologies of kidney diseases stratified by kidney compartment and hematologic malignancy, focusing on demographics, pathology, pathophysiology, mechanism, and outcomes. We conclude with common electrolyte abnormalities associated with hematologic malignancies.
