Abstract

Sexuality is an integral part of the human experience--a complex phenomenon with physical emotional intellectual social and spiritual dimensions. While deeply personal sexuality occurs within specific social economic cultural political and religious contexts. Those contexts in turn profoundly shape the possibilities and limitations of an individuals sexual experience and with that many other aspects of the individuals life. At the same time evolving expressions of sexuality by individuals and groups influence culture politics and religion. Sexuality is both central to some of the greatest problems of our time and at the core of human well-being for people of all ages. A cultures treatment of sexuality can influence whether an individual is safe or unsafe; ill or healthy; rich or poor; educated or not; employed or enslaved; treated with justice or with contempt. Sexuality plays a crucial role in determining womens social status which in turn affects the number of women children and families living in poverty. It influences a womans ability to protect herself against violence disease and unwanted pregnancy; to survive pregnancy and childbirth; and to manage her fertility. Sex can be an arena for contested power relations and result in debilitating discrimination abuse and violence based on gender and sexual orientation. Sex can also be a means for transmitting devastating diseases as evidenced by extraordinary rates of morbidity and mortality from sexually transmitted infections (STIs) and HIV/AIDS. Young people today as in past generations are particularly vulnerable to the risks of unintended pregnancy unsafe abortion maternal mortality violence and sexual exploitation. (excerpt)

Abstract

Summary: Background: Necrotizing soft‐tissue infections (NSTI) are rare, life‐threatening conditions. Objectives: To assess whether admitting hospital characteristics were associated with NSTI mortality. Methods: We studied the French nationwide hospital discharge database (retrospective national cohort). All patients admitted in the period 2007–12 with an International Classification of Diseases 10 code of necrotizing fasciitis were eligible. We extracted data on the patients (age, sex, intensive care unit admission, comorbidities) and hospitals (public vs. private proprietary; for public hospitals, teaching, yes/no; and number of NSTI admissions, ≥ 3 NSTI cases/year, yes/no). Multivariable analyses were performed to identify independent predictors of 28‐day mortality and in‐hospital mortality using mixed logistic regression and Cox proportional hazards models, respectively. Results: We identified 1537 patients (915 males) with a median age of 60 years (interquartile range 48–75), admitted to 326 hospitals, public (82%) and admitting < 3 NSTI cases/year (93%). Overall, 364 patients died [23·7%; 95% confidence interval (CI) 21·6–25·9]. Patients treated in public teaching centres with ≥ 3 NSTI cases annually had lower 28‐day mortality (adjusted odds ratio 0·68; 95% CI 0·46–0·99; P = 0·045) and in‐hospital mortality rates than patients treated in local hospitals, even after adjusting for potentially relevant individual risk factors. No significant association was found between mortality and interhospital transfer. Conclusions: Our findings highlight an increased survival in teaching centres with high‐volume NSTI procedures. If confirmed in other settings, these findings reinforce the importance of expertise in early diagnosis and management of this condition. [ABSTRACT FROM AUTHOR]

1 februari 2012, Contains fulltext : 124361.pdf (publisher's version ) (Open Access), Aspergillus fumigatus is the most frequent fungus found in the sputum of cystic fibrosis (CF) subjects. Itraconazole is prescribed for allergic bronchopulmonary aspergillosis (ABPA) or Aspergillus bronchitis in CF subjects. We hypothesized that A. fumigatus isolates in the sputum of CF subjects with previous exposure to itraconazole was associated with higher prevalence of azole resistance. From June 2010 to April 2011, sputum samples from adult CF subjects at Cochin University Hospital (France) were examined systematically for the detection of A. fumigatus. MICs of A. fumigatus isolates against azoles were screened using Etest, and reduced susceptibility to azoles was confirmed using the CLSI broth microdilution method. A. fumigatus was isolated from the sputum of 131/249 (52.6%) adult CF subjects, and 47/131 (35.9%) subjects had received previous treatment with itraconazole. Reduced A. fumigatus susceptibility to itraconazole (MIC, >/=2 mg/liter) was confirmed in 6/131 (4.6%) subjects. All 6 isolates also had reduced susceptibility to posaconazole (MIC, >/=0.5 mg/liter), and 3/6 isolates had reduced susceptibility to voriconazole (MIC, >/=2 mg/liter). Mutations in the cyp51A gene were detected at positions previously implicated to cause resistance in 5 isolates. Azole-resistant A. fumigatus isolates were found in 5/25 (20%) subjects exposed to itraconazole within the previous 3 years. High rates of azole-resistant A. fumigatus isolates were present in adult CF subjects and were associated with recent itraconazole exposure. Although the clinical implications of these findings will require further studies, the cautious use of itraconazole in adult CF subjects can be recommended.
