Abstract

مقدمه: با توجه به افزایش جمعیت سالمندان در ایران و جهان و بالطبع افزایش درخواست اورژانس از جانب این گروه سنی، بررسی نگرش به سالمندی درکارکنان فوریتهای­ پزشکی به عنوان یک ضرورت مطرح می ­گردد. هدف از این پژوهش تعیین نگرش به سالمندی و عوامل مرتبط با آن در کارکنان فوریت های پزشکی بوده است. روش کار: در این مطالعه توصیفی – تحلیلی، مقطعی، کارکنان فوریت­های پزشکی استان گلستان با روش تمام شماری مورد مطالعه قرار گرفتند. جهت جمع­آوری داده­ها از پرسشنامه نگرش نسبت به سالمندان کوگان (Kogan) و برای تحلیل داده­ها از نرم افزار SPSS نسخه 18 و آزمون­های تی-دو نمونه‌ای، من-ویتنی، تحلیل واریانس و آزمون کروسکال والیس در سطح معنی­داری 05/0>P استفاده شد. برای بررسی نیکویی برازش مدل از شاخص­های CFI، TLI،RMSEA و WRMR و از نرم‌افزار Mplus7.2 استفاده گردید. یافته­ها: میانگین نمره نگرش کارکنان فوریت­های پزشکی نسبت به سالمندی مثبت بوده است. بین نگرش مثبت به سالمندی با وضعیت تاهل (007/0=P)، قومیت (018/0=P) و مدرک تحصیلی (026/0=P) و شهر محل خدمت (001/0=P) ارتباط معنی­دار وجود داشت. نگرش منفی نسبت به سالمندی در کارکنان فوریت­های پزشکی شهری به طور معنی­داری بیشتر از کارکنان فوریت­های پزشکی جاده­ای بوده است (046/0=P). با افزایش سن (006/0=P) و تعداد سال­های اشتغال در فوریت­های پزشکی (043/0=P)، نمره نگرش مثبت به سالمندی افزایش یافت. پرسنل با قومیت ترکمن از نظر نمره تعصب (نگرش منفی) دارای امتیاز کمتری نسبت به پرسنل با قومیت فارس بودند (046/0=P). میانگین نمره تعصب نسبت به سالمندان در پرسنل فوریت­های پزشکی مجرد به طور معنی­داری بیشتر از پرسنل متاهل بود (01/0=P). نتیجه گیری: نگرش کلی کارکنان فوریت­های پزشکی استان گلستان نسبت به سالمندی مثبت بوده است. افزایش سن و تعداد سال­های اشتغال فرد در فوریت­های پزشکی نیز در نگرش مثبت و منفی به سالمندی موثر بوده است.

Abstract

Objective(s): When the nerve is injured near its entrance to the muscle belly, we cannot perform conventional methods. One useful method in such a situation is neurotization surgery. In this study, Bone marrow mesenchymal stem cells (BMSCs) implanted into the paralyzed muscle after neurotization surgery. These cells can stimulate axon growth and motor endplate formation, also prevent muscle atrophy. Materials and Methods: Thirty-six adult male Sprague-Dawley rats were randomized into six groups: intact group, sham surgery group, control group, DMEM group, cell+DMEM group, denervated group. The motor nerve of the lateral head of gastrocnemius muscle was cut, and the proximal portion of the severed nerve was transplanted to the proximal third of the muscle paralysis. BMSCs with/or DMEM was injected into the site of injury. All animals were evaluated by withdrawal reflex latency (WRL), electromyography, muscle weight, histology and immunohistochemistry. Abstract

Background and purpose: Spinal cord injury (SCI) is one of the most disabling diseases with various physical and psychological consequences. One of the treatments of SCI is using agents that have neuroprotective effects such as Vitamin C and ubiquinone. This research aimed at investigating the effect of co-administration of these agents on rat experimental model of SCI. Materials and methods: Adult male Wistar rats (n=40) were divided into sham, lesion, AA, CO10, and AA+COQ10 groups. In sham group only laminectomy was performed and in other groups contusion model of SCI was done. After 24 hr, the animals in AA, COQ10, and AA+COQ10 groups received intraperitoneal injection of AA, COQ10 and AA+COQ10, respectively. Then, behavioral assessment and biochemical study were performed. Eight weeks after treatment, the brains were removed and 5µ sections were prepared. Finally, cresyl violet staining was done. 60. Multi-Modal Emotion Recognition by Text, Speech and Video Using Pretrained Transformers

Abstract

Due to the complex nature of human emotions and the diversity of emotion representation methods in humans, emotion recognition is a challenging field. In this research, three input modalities, namely text, audio (speech), and video, are employed to generate multimodal feature vectors. For generating features for each of these modalities, pre-trained Transformer models with fine-tuning are utilized. In each modality, a Transformer model is used with transfer learning to extract feature and emotional structure. These features are then fused together, and emotion recognition is performed using a classifier. To select an appropriate fusion method and classifier, various feature-level and decision-level fusion techniques have been experimented with, and ultimately, the best model, which combines feature-level fusion by concatenating feature vectors and classification using a Support Vector Machine on the IEMOCAP multimodal dataset, achieves an accuracy of 75.42%. Abstract

Given the significance of speech emotion recognition, numerous methods have been developed in recent years to create effective and efficient systems in this domain. One of these methods involves the use of pretrained transformers, fine-tuned to address this specific problem, resulting in high accuracy. Despite extensive discussions and global-scale efforts to enhance these systems, the application of this innovative and effective approach has received less attention in the context of Persian speech emotion recognition. In this article, we review the field of speech emotion recognition and its background, with an emphasis on the importance of employing transformers in this context. We present two models, one based on spectrograms and the other on the audio itself, fine-tuned using the shEMO dataset. These models significantly enhance the accuracy of previous systems, increasing it from approximately 65% to 80% on the mentioned dataset. Subsequently, to investigate the effect of multilinguality on the fine-tuning process, these same models are fine-tuned twice. First, they are fine-tuned using the English IEMOCAP dataset, and then they are fine-tuned with the Persian shEMO dataset. 62. Iterative Prompt Refinement for Radiation Oncology Symptom Extraction Using Teacher-Student Large Language Models

Abstract

This study introduces a novel teacher-student architecture utilizing Large Language Models (LLMs) to improve prostate cancer radiotherapy symptom extraction from clinical notes. Mixtral, the student model, initially extracts symptoms, followed by GPT-4, the teacher model, which refines prompts based on Mixtral's performance. This iterative process involved 294 single symptom clinical notes across 12 symptoms, with up to 16 rounds of refinement per epoch. Results showed significant improvements in extracting symptoms from both single and multi-symptom notes. For 59 single symptom notes, accuracy increased from 0.51 to 0.71, precision from 0.52 to 0.82, recall from 0.52 to 0.72, and F1 score from 0.49 to 0.73. In 375 multi-symptom notes, accuracy rose from 0.24 to 0.43, precision from 0.6 to 0.76, recall from 0.24 to 0.43, and F1 score from 0.20 to 0.44. These results demonstrate the effectiveness of advanced prompt engineering in LLMs for radiation oncology use.

Introduction: This study aimed to investigate the decisional balance for exercise and its relationship with stages of change in exercise behavior among employees in city of Gorgan, north of Iran. Objective: Human amniotic membrane (HAM) used as a wound coverage for more than a century. The aim of this study is to evaluate the efficacy of amniotic membrane on wound healing and reduce post-operative complication. Study design: Randomized clinical trial study.
