- Abstract
High sensitivity troponin T (hsTnT) is a strong predictor of adverse outcome during SARS-CoV-2 infection. However, its determinants remain partially unknown. We aimed to assess the relationship between severity of inflammatory response/coagulation abnormalities and hsTnT in Coronavirus Disease 2019 (COVID-19). We then explored the relevance of these pathways in defining mortality and complications risk and the potential effects of the treatments to attenuate such risk. In this single-center, prospective, observational study we enrolled 266 consecutive patients hospitalized for SARS-CoV-2 pneumonia. Primary endpoint was in-hospital COVID-19 mortality. hsTnT, even after adjustment for confounders, was associated with mortality. D-dimer and CRP presented stronger associations with hsTnT than PaO2. Changes of hsTnT, D-dimer and CRP were related; but only D-dimer was associated with mortality. Moreover, low molecular weight heparin showed attenuation of the mortality in the whole population, particularly in subjects with higher hsTnT. D-dimer possessed a strong relationship with hsTnT and mortality. Anticoagulation treatment showed greater benefits with regard to mortality. These findings suggest a major role of SARS-CoV-2 coagulopathy in hsTnT elevation and its related mortality in COVID-19. A better understanding of the mechanisms related to COVID-19 might pave the way to therapy tailoring in these high-risk individuals. © 2021, The Author(s).

In the near future, all the economic/political decisions, industrial strategies and scientific efforts about the hydrogen production, separation, and utilization will pass through the "Hydrogen Strategy in the Green-Deal", which will constitute the biggest challenge that the World is facing, but also the greatest opportunity for making reliable a sustainable World's perspective [1]. After the meeting of G20, in Japan on 2019, a general agreement has been noticed about the common interest to deplete the fossil fuels dependence and reduce the greenhouse gases emissions. Hence, the combination of renewable sources and alternative technologies seems to be the future perspective in several sectors, from energy and petrochemical industry to gas separation, environment and transportation [2]. Under the vision of the Hydrogen Economy, the membrane engineering represents today a valuable and emerging option to generate, separate and purify hydrogen, adopting the principles of the Process Intensification Strategy (Figure 1) [3]. The Institute on Membrane Technology dedicates a relevant part of its research activities to hydrogen, from the application of the membrane reactor technology to generate COx-free hydrogen to the separation and purification of syngas or hydrogen-rich streams via inorganic, polymeric and composite membranes. The aim of this work is to illustrate the past, present and future perspectives of membrane technology applied to the hydrogen area at CNR-ITM, showing the most relevant results achieved under the development of international and national projects, research collaborations and ongoing research activities.

Accurate risk stratification in COVID-19 patients consists a major clinical need to guide therapeutic strategies. We sought to evaluate the prognostic role of estimated pulse wave velocity (ePWV), a marker of arterial stiffness which reflects overall arterial integrity and aging, in risk stratification of hospitalized patients with COVID-19. This retrospective, longitudinal cohort study, analyzed a total population of 1671 subjects consisting of 737 hospitalized COVID-19 patients consecutively recruited from two tertiary centers (Newcastle cohort: n = 471 and Pisa cohort: n = 266) and a non-COVID control cohort (n = 934). - Abstract
The efficacy of hydroxychloroquine (HCQ) in treating SARS-CoV-2 infection is harshly debated, with observational and experimental studies reporting contrasting results. To clarify the role of HCQ in Covid-19 patients, we carried out a retrospective observational study of 4,396 unselected patients hospitalized for Covid-19 in Italy (February–May 2020). Patients’ characteristics were collected at entry, including age, sex, obesity, smoking status, blood parameters, history of diabetes, cancer, cardiovascular and chronic pulmonary diseases, and medications in use. These were used to identify subtypes of patients with similar characteristics through hierarchical clustering based on Gower distance. Using multivariable Cox regressions, these clusters were then tested for association with mortality and modification of effect by treatment with HCQ. We identified two clusters, one of 3,913 younger patients with lower circulating inflammation levels and better renal function, and one of 483 generally older and more comorbid subjects, more prevalently men and smokers. The latter group was at increased death risk adjusted by HCQ (HR[CI95%] = 3.80[3.08-4.67]), while HCQ showed an independent inverse association (0.51[0.43-0.61]), as well as a significant influence of cluster∗HCQ interaction (). This was driven by a differential association of HCQ with mortality between the high (0.89[0.65-1.22]) and the low risk cluster (0.46[0.39-0.54]). These effects survived adjustments for additional medications in use and were concordant with associations with disease severity and outcome. These findings suggest a particularly beneficial effect of HCQ within low risk Covid-19 patients and may contribute to clarifying the current controversy on HCQ efficacy in Covid-19 treatment.

63. Le ultrastrutture della parete cellulare in Lithothamnion corallioides: un segnale del controllo biologico della mineralizzazione?

- Abstract
Le alghe calcaree sono importanti organismi calcificanti biocostruttori di fondi mobili (rodoliti) e duri diffuse in tutto il Cenozoico. Tra le rodoliti, il morfotipo a rami liberi (maerl) è presente sia in Atlantico che in Mediterraneo. Lithothamnion corallioides è una delle specie più abbondanti e diffuse nel maerl, tra 28°N e 58°N (Irvine and Chamberlain, 1994). Essa forma strutture ramificate, di colore da marrone a viola, spesso sterili, con diametro dei rami compreso tra 1-2 mm, e con apice a forma di pomello (Irvine and Chamberlain, 1994). In sezione longitudinale, i rami di L. corallioides mostrano un’alternanza di bande più chiare, legate allo sviluppo di cellule più allungate, e bande più scure, caratterizzate da cellule più tozze. Il banding è stato interpretato come la risposta della crescita variabile stagionale (Halfar et al., 2000), ma anche come l’alternanza tra cellule con ultrastrutture mineralizzate della parete differenti (Nash et al. 2019). Le pareti cellulari mostrano solitamente uno strato esterno con calcite aciculare tangenziale al plasmalemma, e uno più interno con calcite aciculare radiale (Giraud and Cabioch, 1976). La forma dei cristalli è stata interpretata come espressione fenotipica del genotipo, con risultati incoraggianti a livello di famiglia (Auer and Piller, 2020). Per valutare se le ultrastrutture subiscono modificazioni morfologiche nei diversi ambienti di vita della specie e a tassi di crescita diversi, sono stati studiati sei campioni di L. corallioides provenienti dall’Oceano Atlantico e dal Mar Mediterraneo da profondità variabili (12-66 m). A livello macroscopico, i campioni sono tutti morfotipi a ramo di colore bianco-rosa-lilla, lunghezza massima di 3 cm e diametro inferiore a 2,5 mm. - Abstract
Background: Protease inhibitors have been considered as possible therapeutic agents for COVID-19 patients. Objectives: To describe the association between lopinavir/ritonavir (LPV/r) or darunavir/cobicistat (DRV/c) use and in-hospital mortality in COVID-19 patients. Study Design: Multicenter observational study of COVID-19 patients admitted in 33 Italian hospitals. Medications, preexisting conditions, clinical measures, and outcomes were extracted from medical records. Patients were retrospectively divided in three groups, according to use of LPV/r, DRV/c or none of them. Primary outcome in a time-to event analysis was death. We used Cox proportional-hazards models with inverse probability of treatment weighting by multinomial propensity scores. Results: Out of 3,451 patients, 33.3% LPV/r and 13.9% received DRV/c. Patients receiving LPV/r or DRV/c were more likely younger, men, had higher C-reactive protein levels while less likely had hypertension, cardiovascular, pulmonary or kidney disease. After adjustment for propensity scores, LPV/r use was not associated with mortality (HR = 0.94, 95% CI 0.78 to 1.13), whereas treatment with DRV/c was associated with a higher death risk (HR = 1.89, 1.53 to 2.34, E-value = 2.43). This increased risk was more marked in women, in elderly, in patients with higher severity of COVID-19 and in patients receiving other COVID-19 drugs. Conclusions: In a large cohort of Italian patients hospitalized for COVID-19 in a real-life setting, the use of LPV/r treatment did not change death rate, while DRV/c was associated with increased mortality. Within the limits of an observational study, these data do not support the use of LPV/r or DRV/c in COVID-19 patients.

65. An empirical study of the B/Ca proxy in calcareous red algae

- Abstract
Calcareous red algae have a long and rich fossil record (Basso et al., 2007; Bracchi et al., 2014), and a widely recognized importance as paleoclimate archives (Halfar et al., 2008; McCoy and Kamenos, 2015). Their geochemical composition is used to infer past seawater temperature (Fietzke et al., 2015; Ragazzola et al., 2020), salinity (Kamenos et al., 2012), and pH (Anagnostou et al., 2019). The B/Ca ratio in benthic foraminifera has been used as a proxy for past CO32- concentrations (Yu and Elderfield, 2007; Rae et al., 2011), but scarce data exist on B/Ca in coralline algae, and its driving factors. Recent experiments on cultured Neogoniolithon sp. showed an inverse relationship between seawater dissolved inorganic carbon (DIC) and B/Ca (Donald et al., 2017). B and Ca contents were measured by laser ablation inductively coupled plasma mass spectrometry (LA-ICP-MS) in the maerl species Lithothamnion corallioides (P. Crouan & H. Crouan) P. Crouan & H. Crouan 1867, which has a wide geographic distribution in European waters. Samples were collected in the Mediterranean Sea (Pontian Islands, 66 m depth; Elba, 45 m depth and Aegadian Islands, 40 m depth), and in the NE Atlantic Ocean (Morlaix Bay, 12 m depth). DIC data from the sampling sites have been extracted by CMEMS (E.U. Copernicus Marine Service Information) products, and the Kruskal-Wallis test was run in R 3.6.3 software to evaluate B/Ca differences among samples. The B/Ca ratio showed significant differences among samples (p<0.01). The mean DIC was lower in the Atlantic Ocean, where the alga registered high B/Ca values. In Pontian Isl., the deepest site, the B/Ca was significantly lower than the other Mediterranean sites (p<0.01), despite very similar DIC. The measurements on growth rates resulted in a lower rate in Pontian Isl. (0.10 mm/yr), compared to the other samples (~0.13 mm/yr), likely related to the variations in light availability with depth (Halfar et al., 2011). In general, the diffe

- Abstract
INTRODUCTION: A hypercoagulable condition was described in patients with coronavirus disease 2019 (COVID-19) and proposed as a possible pathogenic mechanism contributing to disease progression and lethality.AIM: We evaluated if in-hospital administration of heparin improved survival in a large cohort of Italian COVID-19 patients.METHODS: In a retrospective observational study, 2,574 unselected patients hospitalized in 30 clinical centers in Italy from February 19, 2020 to June 5, 2020 with laboratory-confirmed severe acute respiratory syndrome coronavirus-2 infection were analyzed. The primary endpoint in a time-to event analysis was in-hospital death, comparing patients who received heparin (low-molecular-weight heparin [LMWH] or unfractionated heparin [UFH]) with patients who did not. We used multivariable Cox proportional-hazards regression models with inverse probability for treatment weighting by propensity scores.RESULTS: Out of 2,574 COVID-19 patients, 70.1% received heparin. LMWH was largely the most used formulation (99.5%). Death rates for patients receiving heparin or not were 7.4 and 14.0 per 1,000 person-days, respectively. After adjustment for propensity scores, we found a 40% lower risk of death in patients receiving heparin (hazard ratio=0.60; 95% confidence interval: 0.49-0.74; E-value=2.04). This association was particularly evident in patients with a higher severity of disease or strong coagulation activation.CONCLUSION: In-hospital heparin treatment was associated with a lower mortality, particularly in severely ill COVID-19 patients and in those with strong coagulation activation. The results from randomized clinical trials are eagerly awaited to provide clear-cut recommendations.

The increasing CO 2 concentration in the atmosphere exerts a significant influence on global warming and climate change. The capture and utilization of CO 2 by conversion to useful products is an area of active research. In this work, the photodriven reduction of CO 2 was investigated using graphitic carbon nitride (g-C 3 N 4 ) as a potential photocatalyst. The photocatalytic reduction of CO 2 was investigated with g-C 3 N 4 powder immobilized on a glass support in a batch gas-phase photoreactor. The experiments were carried out under UV-vis irradiation at 70 °C and an initial pressure of 2.5 bar. The only gas-phase product detected during the irradiation of the g-C 3 N 4 in the presence of CO 2 was CO, and the rate of production was observed to decrease over time. Oxygen-doped g-C 3 N 4 was also tested for CO 2 reduction but had efficiency lower than that of the parent g-C 3 N 4 . Repeated cycles of photocatalytic CO 2 reduction showed a decline in the activity of the g-C 3 N 4 . In the absence of CO 2 some CO generation was also observed. Characterization of used and unused materials, using FTIR and XPS, showed an increase in the oxygen functional groups following UV-vis irradiation or thermal treatment. While others report the use of g-C 3 N 4 as a photocatalyst, this work highlights the important need for replicates and control testing to determine material stability.

Introduction: The care at the end of children's lives must be sensitive to the needs of the child and their family. An understanding of the illness is required from the perspective of parents faced with the death of their child, in order to improve quality and guide the development of end-of-life care in Paediatrics. Method: A retrospective observational study was conducted between June 2014 and June 2017 using a questionnaire, to assess the needs, experiences, and satisfaction with the care received, from a sample of parents who lost a child due to a foreseeable cause. Three different study groups were formed based on the team responsible for end-of-life care, and an analysis was carried out on the differences between the group treated by the paediatric palliative care team, the group attended by non-palliative paediatricians, and the neonatal group. Results: Of the 80 eligible families, 64 could be contacted, and 28 (43.8%) finally completed the questionnaire. Our study shows positive experiences and high satisfaction of parents with the care received at the end of their child's life. The highest scores in experiences and satisfaction were given by the parents of the children served by the paediatric palliative care team, with statistically significant differences in family support, communication, shared decision making, and bereavement support (P < .05). Conclusions: Parents are satisfied with the care received at the end of their children's lives, but the intervention of a specific paediatric palliative care team improves the quality of care at the end of life in paediatrics. OBJECTIVE To explore whether at-admission hyperglycemia is associated with worse outcomes in patients hospitalized for coronavirus disease 2019 (COVID-19). RESEARCH DESIGN AND METHODS Hospitalized COVID-19 patients (N = 271) were subdivided based on at-admission glycemic status: 1) glucose levels RESULTS Neutrophils were higher and lymphocytes and PaO2/FiO2 lower in HG than in DM and NG patients. DM and HG patients had higher D-dimer and worse inflammatory profile. Mortality was greater in HG (39.4% vs. 16.8%; unadjusted hazard ratio [HR] 2.20, 95% CI 1.27–3.81, P = 0.005) than in NG (16.8%) and marginally so in DM (28.6%; 1.73, 0.92–3.25, P = 0.086) patients. Upon multiple adjustments, only HG remained an independent predictor (HR 1.80, 95% CI 1.03–3.15, P = 0.04). After stratification by quintile of glucose levels, mortality was higher in quintile 4 (Q4) (3.57, 1.46–8.76, P = 0.005) and marginally in Q5 (29.6%) (2.32, 0.91–5.96, P = 0.079) vs. Q1. CONCLUSIONS Hyperglycemia is an independent factor associated with severe prognosis in people hospitalized for COVID-19.

- Abstract
Objective: The hypothesis that been set forward that use of Renin Angiotensin Aldosterone System (RAAS) inhibitors is associated with COVID-19 severity. We set-up a multicenter Italian collaboration (CORIST Project, ClinicalTrials.gov ID: NCT04318418) to retrospectively investigate the relationship between RAAS inhibitors and COVID-19 in-hospital mortality. We also carried out an updated meta-analysis on the relevant studies. Methods: We analyzed 4,069 unselected patients with laboratory-confirmed SARS-CoV-2 infection and hospitalized in 34 clinical centers in Italy from February 19, 2020 to May 23, 2020. The primary end-point in a time-to event analysis was in-hospital death, comparing patients who received angiotensin-converting-enzyme inhibitors (ACE-I) or angiotensin-receptor blockers (ARB) with patients who did not. Articles for the meta-analysis were retrieved until July 13th, 2020 by searching in web-based libraries, and data were combined using the general variance-based method. Results: Out of 4,069 COVID-19 patients, 13.5% and 13.3% received ACE-I or ARB, respectively. Use of neither ACE-I nor ARB was associated with mortality (multivariable hazard ratio (HR) adjusted also for COVID-19 treatments: 0.96, 95% confidence interval 0.77-1.20 and HR=0.89, 0.67-1.19 for ACE-I and ARB, respectively). Findings were similar restricting the analysis to hypertensive (N=2,057) patients (HR=1.00, 0.78-1.26 and HR=0.88, 0.65-1.20) or when ACE-I or ARB were considered as a single group. Results from the meta-analysis (19 studies, 29,057 COVID-19 adult patients, 9,700 with hypertension) confirmed the absence of association. Conclusions: In this observational study and meta-analysis of the literature, ACE-I or ARB use was not associated with severity or in-hospital mortality in COVID-19 patients.

- Abstract
Background and aims: There is poor knowledge on characteristics, comorbidities and laboratory measures associated with risk for adverse outcomes and in-hospital mortality in European Countries. We aimed at identifying baseline characteristics predisposing COVID-19 patients to in-hospital death. Methods and results: Retrospective observational study on 3894 patients with SARS-CoV-2 infection hospitalized from February 19th to May 23rd, 2020 and recruited in 30 clinical centres distributed throughout Italy. Machine learning (random forest)-based and Cox survival analysis. 61.7% of participants were men (median age 67 years), followed up for a median of 13 days. In-hospital mortality exhibited a geographical gradient, Northern Italian regions featuring more than twofold higher death rates as compared to Central/Southern areas (15.6% vs 6.4%, respectively). Machine learning analysis revealed that the most important features in death classification were impaired renal function, elevated C reactive protein and advanced age. These findings were confirmed by multivariable Cox survival analysis (hazard ratio (HR): 8.2; 95% confidence interval (CI) 4.6–14.7 for age ≥85 vs 18–44 y); HR = 4.7; 2.9–7.7 for estimated glomerular filtration rate levels <15 vs ≥ 90 mL/min/1.73 m2; HR = 2.3; 1.5–3.6 for C-reactive protein levels ≥10 vs ≤ 3 mg/L). No relation was found with obesity, tobacco use, cardiovascular disease and related-comorbidities. The associations between these variables and mortality were substantially homogenous across all sub-groups analyses. Conclusions: Impaired renal function, elevated C-reactive protein and advanced age were major predictors of in-hospital death in a large cohort of unselected patients with COVID-19, admitted to 30 different clinical centres all over Italy.
