Abstract

The literature on chronic diseases recognizes the role of illness cognition as a mediator between stress and illness. Few conceptualizations and instruments, however, give an indication of both unfavorable and favorable ways of adjusting to an uncontrollable long-term stressor, such as a chronic disease. The authors propose 3 generic illness cognitions that reflect different ways of reevaluating the inherently aversive character of a chronic condition: helplessness as a way of emphasizing the aversive meaning of the disease, acceptance as a way to diminish the aversive meaning, and perceived benefits as a way of adding a positive meaning to the disease. A self-report instrument, the Illness Cognition Questionnaire, was developed to assess these cognitions across different chronic diseases. The results support the reliable and valid assessment of these illness cognitions in patients with rheumatoid arthritis and multiple sclerosis and indicate the maladaptive function of helplessness and the adaptive function of acceptance and perceived benefits for the long-term physical and psychological health of patients with a chronic disease.

Abstract

In order to examine the influence of active and passive pain-coping strategies and social support characteristics on the change in functional status in the first stage of the disease in rheumatoid arthritis patients, self-report data and clinical and laboratory measures were collected from 91 patients (70% female, mean age 57 yr) shortly after diagnosis and 1 yr later. Multiple regression analyses indicated that, after taking the influence of demographic variables, disease activity and pain into account, a decrease in functional status (mobility, self-care, grip strength) after 1 yr could be predicted by an initially more frequent use of the passive pain-coping strategies of worrying and resting. A decrease in mobility could be additionally predicted by an initially smaller social network. Results indicate the impact of passive pain-coping strategies and social network characteristics for the prognosis of functional outcome in the first stage of the disease and suggest the early manifestation of avoidance mechanisms, including behavioral, cognitive-emotional and social components, in face of a chronic stressor. Abstract

Objectives: To provide an update of the European League Against Rheumatism (EULAR) rheumatoid arthritis (RA) management recommendations to account for the most recent developments in the field. Methods: An international task force considered new evidence supporting or contradicting previous recommendations and novel therapies and strategic insights based on two systematic literature searches on efficacy and safety of disease-modifying antirheumatic drugs (DMARDs) since the last update (2016) until 2019. A predefined voting process was applied, current levels of evidence and strengths of recommendation were assigned and participants ultimately voted independently on their level of agreement with each of the items. Results: The task force agreed on 5 overarching principles and 12 recommendations concerning use of conventional synthetic (cs) DMARDs (methotrexate (MTX), leflunomide, sulfasalazine); glucocorticoids (GCs); biological (b) DMARDs (tumour necrosis factor inhibitors (adalimumab, certolizumab pegol, etanercept, golimumab, infliximab), abatacept, rituximab, tocilizumab, sarilumab and biosimilar (bs) DMARDs) and targeted synthetic (ts) DMARDs (the Janus kinase (JAK) inhibitors tofacitinib, baricitinib, filgotinib, upadacitinib). Guidance on monotherapy, combination therapy, treatment strategies (treat-to-target) and tapering on sustained clinical remission is provided. Cost and sequencing of b/tsDMARDs are addressed. Initially, MTX plus GCs and upon insufficient response to this therapy within 3 to 6 months, stratification according to risk factors is recommended. With poor prognostic factors (presence of autoantibodies, high disease activity, early erosions or failure of two csDMARDs), any bDMARD or JAK inhibitor should be added to the csDMARD. Abstract

Background: Oral glucocorticoids combined with disease-modifying antirheumatic drugs are beneficial and retard radiologic joint damage in rheumatoid arthritis. Objective: To investigate the clinical efficacy, disease-modifying properties, and side effects of low-dose glucocorticoids as monotherapy for previously untreated patients with early active rheumatoid arthritis. Design: 2-year randomized, double-blind, placebo-controlled clinical trial. Patients: 81 patients with early active rheumatoid arthritis who had not been treated with disease-modifying antirheumatic drugs. Intervention: 41 patients were assigned to 10 mg of oral prednisone per day, and 40 were assigned to placebo. Nonsteroidal anti-inflammatory drugs were allowed in both groups. After 6 months, sulfasalazine (2 g/d) could be prescribed as rescue medication. Measurements: Clinical variables were assessed at baseline and every 3 months; radiologic studies were performed every 6 months. Adverse effects were documented every 3 months. Results: In the first 6 months, the prednisone group showed more clinical improvement than the placebo group. This effect was not seen after 6 months except in grip strength and the 28-joint score for tenderness. Use of additional therapies was significantly less common in the prednisone group, particularly in the first 6 months. More than 65% of those who completed the study were not taking sulfasalazine. After month 6, radiologic scores showed significantly less progression in the prednisone group than in the placebo group. No clinically relevant adverse effects were observed, except for a higher incidence of osteoporotic fractures in the prednisone group. Conclusions: Prednisone, 10 mg/d, provides clinical benefit, particularly in the first 6 months, and substantially inhibits progression of radiologic joint damage in patients with early active rheumatoid arthritis and no previous treatment with disease-modifying antirheumatic drugs. Because of their limited disease-modifying effects, glucocorticoids should be combined with disease-modifying antirheumatic drugs in patients with rheumatoid arthritis.

Rheumatoid arthritis (RA) and hay fever may represent opposite extremes of T- cell activity, and modulation of immune responses may have a place in RA treatment strategies. Researchers performed immunological tests on 643 clinic patients, 12 of whom had hay fever. The rate of hay fever was 4% among RA patients and 8% among those without RA. RA patients with hay fever had less severe arthritis symptoms and increased T2 cell activity, indicating a potentially protective immunological state.

An imbalance in type 1 (T1) and type 2 (T2) T cell activities in the blood of patients with rheumatoid arthritis (RA) may be related to the severity of joint destruction resulting from the disease. Blood samples from 66 patients with RA, 19 patients with osteoarthritis, and 15 healthy individuals were assessed for T cell products associated with the two types of cells. Significantly higher amounts of T2 cell products and less T1 cell products were found in the patients with RA. The abnormal levels were connected to the amount of joint destruction observed.

The effect of sodium fluoride (NaF) on the bone mineral density (BMD) of the lumbar spine when added to etidronate seems superior to etidronate alone in some patients with osteoporosis. Forty patients with osteoporosis who were being treated with corticosteroids were given etidronate with NaF added or etidronate alone. After 2 years, the 20 patients taking the etidronate with NaF had a 9.3% increase in the BMD of the lumbar spine while the other 20 had only a 0.3% increase, a difference of 8.9%.

Abstract

Objective: To compare two therapeutic strategies for patients with recent-onset rheumatoid arthritis. Design: Open, randomized clinical trial. Setting: Outpatient clinics of six clinical centers. Patients: 238 consecutive, patients with recently diagnosed rheumatoid arthritis. interventions: Delayed or immediate introduction of treatment with slow-acting antirheumatic drugs (SAARDs). Measurements: Primary end points were functional disability, pain, joint score, and erythrocyte sedimentation rate at 6 and 12 months and progression of radiologic abnormalities at 12 months. Results: Statistically significant advantages at 12 months for patients receiving the SAARD strategy (immediate treatment with SAARDs) with regard to all primary end points that may be clinically important are indicated by the differences in improvements from baseline and their 95% CIs. These differences were 0.3 (95% CI, 0.2 to 0.6) for disability (range, 0 to 3), 10 mm (CI, 1 to 19 mm) for pain (range, 0 to 100 mm), 39 (CI, 4to 74) for joint score (range, (0 to 534), and 1 1 mm/h (CI, 3 to 19 mm/h) for erythrocyte sedimentation rate (range, 1 to 140 mm/h), all in favor of SAARD treatment. The SAARD strategy also appears to be advantage6us at 6 months. Radiologic abnormalities progressed at an equal rate in the SAARD and the NON-SAARD groups; the difference in progression (range, 0 to 448) was 1 (CI, -3 to 5). Analyses were based on the intention-to-treat principle and thus included 29% of patients in the NON-SAARD group who discontinued the NON-SAARD treatment strategy; treatment was usually discontinued because of insufficient effectiveness. The SAARD strategy including two alternative SAARDs could not be continued by 8% of patients, usually because of adverse reactions. Conclusions: Early introduction of SAARDs may be more beneficial than delayed introduction for patients with recently diagnosed rheumatoid arthritis., It may be advisable to introduce stronger drugs earlier in the disease cycle in patients with rheumatoid arthritis. Researchers compared the effectiveness of using disease treating drugs such as intramuscular gold, hydroxychloroquine, or methotrexate against using the traditional initial treatment of nonsteroidal anti-inflammatory drugs (NSAIDS) in 238 new patients with rheumatoid arthritis. The 181 patients taking the disease treating drugs scored 19% to 26% better than the 57 patients taking only NSAIDS on tests of joint pain, joint function, and blood tests that may indicate inflammation status. They saw this beneficial effect at both 6-months and 12-months following the start of treatment. Joint damage as measured on X-rays, however, was similar for both treatment groups as treatment progressed. The most common side effect reported with the disease treating drugs was stomach and intestinal symptoms (28%).
