A feedback method is used to obtain real-time information on the mechanical disruption of tissue through ultrasound cavitation. The analysis has shown that the substantial backscatter reduction might be an efficient feedback mechanism for assessing treatment efficacy in ultrasound surgery by using pulsed ultrasound to create cavitation.

Spatial variability in acoustic backscatter is examined as a potential feedback metric in order to assess the lesion morphology during cavitation-mediated mechanical tissue disruption. The results have shown that interrogating backscatter radio frequency (RF) signals for the presence of spatial variability have provided information regarding the efficacy of the treatment, especially the extent of homogenate production.

A strategy is proposed to enhance the cavitation-induced tissue erosion, wherein tissue erosion is initiated by a short, high-intensity sequence of pulses and sustained by lower intensity pulses. Results show that the initiating sequence increases the probability of erosion occurrence and the erosion rate with only slight overall increases in propagated energy, and if extinguished and reinitiated, the sustained cavitation period becomes shorter after each initiation.

The effects of various parameters namely, pulse repetition frequency (PRF), pulse duration (PD), and gas content of liquid on the erosion rate and energy efficiency are explored. The results show that very short pulses delivered at certain PRFs could maximize the erosion rate and energy efficiency.

Purpose: Our goal was to describe changes in bladder health, defined as "a complete state of physical, mental, and social well-being related to bladder function that permits daily activities, adapts to short-term stressors, and allows optimal well-being," in women over time. Materials and Methods: We used data on 15 lower urinary tract symptoms (LUTS) and interference from urinary experiences assessed at the baseline and 5-year followup interviews of the BACH (Boston Area Community Health) Survey to estimate changes in bladder health over time in women. Associations between baseline and followup bladder health (defined as the maximum frequency of LUTS or interference at each time point) were calculated by ordinal logistic regression and generalized linear models. Results: A total of 2,526 women provided complete information on bladder health at baseline and followup. Over the 5-year followup, 6.5% of women maintained optimal bladder health (no LUTS or interference), 33.6% developed worse bladder health (including 10.4% who transitioned from optimal to less than optimal health), 31.4% maintained their less than optimal bladder health status and 28.7% improved. Despite these changes, women with poorer bladder health at baseline were still more likely to have poorer bladder health 5 years later (eg multivariable-adjusted relative risk[3.27, 95% confidence interval: 2.49e4.29 for severe LUTS/interference at followup among those with severe LUTS/interference at baseline). Conclusions: Findings from our large secondary analysis of BACH Survey data suggest considerable variability in bladder health over time, and underscore the importance of bladder health promotion to prevent the initial onset and progression of poor bladder health in women. Researchers have investigated some practical issues associated with planar linear ultrasound phased arrays for transrectal thermotherapy of prostate diseases. Particular attention was given to the significance of the effects of acoustical and electrical interactions between array elements on the ultrasound fields produced. It was established that the difference between theoretically simulated and experimentally measured acoustical field distributions produced by the array are probably linked with the interaction between array elements.

Noninvasive multipoint temperature control that operates in real-time for ultrasound phased array treatments has been developed. The method estimates spatio-temporal temperature variations in samples, and uses this data as the input for a multipoint ultrasound phased array temperature controller. A linear relationship between temperature and the apparent echo displacement, as seen on A-lines derived by pulse-echo ultrasound of the heated region, forms the basis of the technique. This method can replace the invasive thermocouple measurements used in controlling hyperthermia treatment.

Computer simulations of phased array systems, reveal that the method of simultaneous multiple focusing without scanning is the best suited technique for ultrasound surgery. It requires the smallest maximum intensity and dose, and gives the most even dose distribution, as compared to other techniques. However, single-focus scanning of individual lesions gives the worst results. This indicates that multiple-focus patterns reduce the maximum intensity and dose, and provide a more even dose distribution.

The treatment includes geometric and thermal optimization methods, which provide the necessary information required for the optimization of individual patient treatments.

Tissue inhomogenecities, driving signal digitization and imperfect transducers produce phase errors that degrade phased array focal pattern generation under ultrasound hyperthermia. The amount of phase aberrations determines the extent of degradation. The focal degradation exhibits scaling with phase error circular variance. Phase aberration correction improves degraded power deposition patterns. Stabilizing the foci positions after motion compensation helps obtain sharply focused patterns.

This paper presents a new method which obtains ultrasound hyperthermia applicator phased-array element driving signals from a desired temperature distribution. The approach combines a technique which computes array element driving signals from focal point locations and intensities with a new technique which calculates focal point locations and power deposition values from temperature requirements. Temperature specifications appear here as upper and lower bounds within the tumor volume, and a focal point placement algorithm chooses focal patterns capable of achieving the temperature range objective. The linear algebraic structure of the method allows rapid calculation of both the phased-array driving signals and an approximate temperature field response. Computer simulations verify the method with a spherical section array (SSA) for a variety of temperature specifications and blood perfusion values. This scheme, which applies to any phased-array geometry, completes an essential step in both treatment planning and feedback for hyperthermia with ultrasound phased-array applicators.

The development, construction and subsequent testing of a sector-vortex phased-array applicator for ultrasound hyperthermia are discussed. Designed as a 32-element, two-track system for 750 kHz operation, the applicator yielded annular focal fields without any undesired foci. This is obtained by the Mth order Bessel function approximation during driven array phase distributiom. Its focal acoustic power compared to computer simulation forecasts is evaluated.

Monocarboxylate transporters (MCTs) represent a diverse group of transmembrane proteins encoded by the SLC16 gene family found ubiquitously across mammalian species. Two members of this family, MCT1 and MCT4, have been linked to key roles in the metabolic activity of tissues through the proton-coupled transport of monocarboxylates, most notably L-lactate, ketone bodies, and pyruvate. This review aims to provide an overview of MCT1 and MCT4, followed by the implications of their expression in a multitude of cancers and in glioblastoma (GBM) specifically. Further, the possible mechanisms underlying these effects will be discussed. Given the relationships between MCT1 and MCT4 and cancer, they offer a unique opportunity for novel treatment strategies. We aim to explore current therapies focused on MCT1 and MCT4 and propose future studies to better understand their role in GBM to optimize future treatment regimens.
