Abstract

Background There is no information regarding the outcome of Crohn's disease (CD) patients treated with endoscopic balloon dilation (EBD) in non-referral hospitals, nor on the efficacy of EBD in ulcerative colitis (UC). We report herein the results of the largest series published to date. Aim To assess the efficacy and safety of EBD for inflammatory bowel disease (IBD) stenosis performed in 19 hospitals with different levels of complexity and to determine factors related to therapeutic success. Methods We identified IBD patients undergoing EBD in the ENEIDA database. Efficacy of EBD was compared between CD and UC and between secondary and tertiary hospitals. Predictive factors of therapeutic success were assessed with multivariate analysis. Results Four-hundred dilations (41.2% anastomotic) were performed in 187 IBD patients (13 UC/Indeterminate colitis). Technical and therapeutic success per dilation was achieved in 79.5% and 55.3%, respectively. Therapeutic success per patient was achieved in 78.1% of cases (median follow-up: 40 months) with 49.7% requiring more than one dilation. No differences related to either diagnosis or hospital complexity was found. Abstract

Background Thiopurines are the most widely used immunosuppressants in IBD although drug-related adverse events (AE) occur in 20%-30% of cases. Aim To evaluate the safety of thiopurines in elderly IBD patients Methods Cohort study including all adult patients in the ENEIDA registry who received thiopurines. Patients were grouped in terms of age at the beginning of thiopurine treatment, specifically in those who started thiopurines over 60 years or between 18 and 50 years of age. Thiopurine-related AEs registered in the ENEIDA database were compared. Results Out of 48 752 patients, 1888 thiopurines when over 60 years of age and 15 477 under 50 years of age. Median treatment duration was significantly shorter for those who started thiopurines >60 years (13 [IQR 2-55] vs 32 [IQR 5-82] months; P < .001). Patients starting >60 years had higher rates of all types of myelotoxicity, digestive intolerance and hepatotoxicity. Thiopurines were discontinued due to AEs (excluding malignancies and infections) in more patients starting >60 years (67.2% vs 63.1%; P < .001). Elderly age and female sex were independent risk factors for most AEs. Conclusion In elderly IBD patients, thiopurines are associated with an increased risk of non-infectious, non-neoplastic, AEs.

Abstract

Background There are limited data of ustekinumab administered according to the doses recommended in the UNITI studies. Aim To assess the real-world, short-term effectiveness of ustekinumab in refractory Crohn's disease (CD) Methods Multicentre study of CD patients starting ustekinumab after June 2017 at the recommend dose (260, 390 or 520 mg based on weight ~6 mg/kg IV week 0 and 90 mg subcutaneously week 8). Values for Harvey-Bradshaw Index (HBI), C-reactive protein (CRP) and faecal calprotectin (FC) were recorded at baseline and at weeks 8 and 14. Demographic and clinical data, previous treatments, AEs and hospitalisations were documented. Possible predictors of clinical remission were examined. Results Three hundred and five patients were analysed (>= 2 previous anti-TNF alpha therapies 64% and vedolizumab 29%). At baseline, 217 (72%) had an HBI >4 points. Abstract

ObjectiveTo evaluate the safety, tolerability and efficacy of a probiotic supplementation for Helicobacter pylori (H.pylori) eradication therapy. DesignConsecutive adult naive patients with a diagnosis of H.pylori infection who were prescribed eradication therapy according to clinical practice (10-day triple or nonbismuth quadruple concomitant therapy) randomly received probiotics (1x10(9) colony-forming units each strain, Lactobacillus plantarum and Pediococcus acidilactici) or matching placebo. Side effects at the end of the treatment, measured through a modified De Boer Scale, were the primary outcome. Secondary outcomes were compliance with therapy and eradication rates. ResultsA total of 209 patients (33% triple therapy, 66% non-bismuth quadruple therapy) were included [placebo (n=106) or probiotic (n=103)]. No differences were observed regarding side effects at the end of the treatment between groups ( -0.023, P 0.738). Female gender (P90% in all cases) between triple and quadruple concomitant therapy. ConclusionProbiotic supplementation containing Lactobacillus Plantarum and Pediococcus acidilactici to H.pylori treatment neither decreased side effects nor improved compliance with therapy or eradication rates.

Abstract

Objective: The main purpose of this study was to describe a group of Colombian physical therapists' beliefs and attitudes towards Evidence-Based Practice (EBP), their education, knowledge and skills for implementing EBP, the use of relevant literature in clinical practice, access to and availability of scientific information and perceived barriers to including EBP in practice. Methods: This was a cross-sectional study which involved 1,064 Colombian physical therapists. The study used a 50-item screening questionnaire EBP developed to estimate attitudes, beliefs, knowledge and skills regarding. This instrument has been adapted and was validated previously in Colombia by Flórez-López et al. Results: The population mostly consisted of young females (77.2%) aged 22 to 29 years old (79.4%). Most respondents had an undergraduate degree (87.7%). The physical therapists stated that they had positive attitudes and beliefs regarding EBP, most of them answering that they agreed or strongly agreed that EBP is necessary (71.6%), the relevant literature is useful for practice (61.3%), EBP improves the quality of patient care (64.1%) and evidence helps in decision-making (44.5%). Forty-one percent of the respondents indicated that a lack of research skills was the most important barrier to the use of evidence in practice. Conclusion: The physical therapists reported that they had a positive attitude to EBP and were interested in learning about or improving the skills necessary to adopt EBP in their clinical practice. Objetivo: Describir en un grupo colombiano de fisioterapeutas las i) creencias y actitudes hacia la practica basada en la evidencia (PBE), ii) la educación, el conocimiento y las habilidades para implementar la PBE; iii) el uso de la literatura relevante en la práctica clínica; iv) el acceso y la disponibilidad de información científica; y v) la percepción de las barreras para la inclusión de la PBE. Métodos: Se realizó un estudio transversal en 1,064 fisioterapeutas colombianos. El estudio usó el cuestionario que consta de 50-ítems para estimar las actitudes, creencias, conocimientos y habilidades hacia la PBE. Dicho instrumento fue adaptado y validado en Colombia por Flórez-López et al. Resultados: La mayoría de la población participante fueron mujeres (77.2%) en edades comprendidas entre 22 y 29 años (79.4%). El 87.7% de los encuestados eran titulados en fisioterapia. Los fisioterapeutas manifestaron tener actitudes y creencias positivas hacia la PBE. Una mayoría respondió que estaba de acuerdo o muy de acuerdo en que es necesaria la PBE (71.6%), en que la literatura es útil para la práctica clínica (61.3%), que la PBE mejora la calidad de la atención a los pacientes (64.1%), y en que la evidencia ayuda en la toma de decisiones clínicas (44.5%). El 41.0% de los encuestados indicaron que la falta de habilidades de investigación era la barrera más importante para el uso de la evidencia científica en la práctica clínica. Conclusiones: Los fisioterapeutas manifestaron una actitud positiva acerca la PBE y estaban interesados en aprender o mejorar las habilidades necesarias para adoptar la PBE en la práctica clínica.

Abstract

The cohesive crack model and the equivalent crack are summarized. These models allow quantitative predictions in concrete structures of the increase in brittleness as size increases; the well known size effect. In the second part, two applications of the cohesive crack model are briefly described; one for plain concrete and the other related to beams with minimum reinforcement. En este artículo se comentan dos métodos utilizados para modelizar el proceso de fractura del hormigón; el modelo de la fisura cohesiva y el de la fisura equivalente. A partir de estos modelos se puede predecir cuantitativamente el aumento de la fragilidad de un elemento estructural al aumentar su tamaño, fenómeno conocido como efecto del tamaño. En la segunda parte se muestran dos aplicaciones de la Mecánica de la Fractura al hormigón; una para el hormigón en masa y otra para vigas débilmente armadas.

Abstract

Background: Evidence-based practice (EBP) has been widely implemented in differing areas of physiotherapy. Nevertheless, few studies have investigated EBP-related barriers amongst Latin-American physical therapists working in primary care. The primary objective of this study was to describe the current state concerning perceived barriers engagement in EBP among physical therapy in Colombia. A secondary objective was to identify factors associated with barriers to implementation EBP. Methods: A cross-sectional online survey was conducted. The study involved physical therapists working in public and private hospital who were contacted through professional networks (Email, Facebook®, ResearchGate® and Linked-In®) and invited to participate. Multiple logistic regression (MLR) and multiple correspondence analysis (MCA) were used for examining factors associated with perceived barriers to including EBP in their work. Results: The final sample size was 1064 (77.2% female). Forty-one percent of the respondents indicated that a "lack of research skills" was the most important barrier to evidence being used in practice. MLR analysis suggested that several variables were associated with perceived barriers to including EBP: i.e. hours of work per week, current main role in therapy center and undergraduate degree. The MCA model established two groups of similarities regarding the different barriers; the "lack of understanding of statistical analysis", "insufficient time" and "understanding of the English in which articles are written" barriers were weighted more heavily regarding in the first group (the second factor on MCA) and the rest barriers on the second group (first factor on the MCA). Conclusions: Although most physiotherapists had a positive opinion regarding EBP, they considered that they needed to improve their knowledge, skills and attitudes towards EBP. Initiatives to advance EBP in Colombia with no academic or research tradition should primarily target practitioner-level factors. [ABSTRACT FROM AUTHOR]
