Locomotor capability ranges from no ambulatory limitation to the need for a powered wheelchair. Limited evidence suggests an association between motor performance and specific chest deformities. The objectives of this study were: (1) to characterize chest deformity in children with types III and IV OI and (2) to evaluate relationships between chest deformity, motor performance, and joint mobility. SUBJECTS: Forty-one consecutive children (27 girls, 14 boys; ages 10 months-18 yrs), with type III (21) or IV (20) OI were evaluated at 3, 4, 6 or 12 month intervals during a 14 month period as part of an OI research program. Included were 1 Asian-American, 2 African-American, 1 Pakistani-American, 2 Hispanic-American, 1 East Indian-American, 1 Samoan-American, 1 Chilean, and 32 Euro/ Scandinavian-American children. METHODS AND MATERiALS: Children were categorized as having no deformity, pectus carinatum (PC) or excavatum (PE). Chest AP diameter and circumference were measured at the xiphoid process. Motor performance was assessed using the 10-point Brief Assessment of Motor Function (BAMF, Cintas et al 1998). Goniometric measurements were obtained for knee extension, elbow extension, and ankle dorsiflexion. ANALYSES: Spearman correlation, Kruskal-Wallis, and multiple regression procedures were used to identify significant relationships and predictors of motor performance. RESULTS: 31 children (76%) had chest deformity. 8 changed from no deformity or to a different chest deformity during the observation interval. Based on n=96 evaluation events, children with PE had significantly higher motor performance levels compared to children with no deformity or those with PC (Kruskal-Wallis P=.021). PC was associated with increased joint hypomobility (Kruskal-Wallis P=.015), PE with increased joint hypermobility (Kruskal-Wallis P=.02). Lower BAMF performance was predicted by larger AP chest diameter indexed to body length for girls (Re=.6294. P=.0001) and larger chest circumference indexed to body length for boys ([R.sup.2]=.4738, P=.0006). CONCLUSIONS: (1) Chest deformity in children with types III and IV OI is common and may change with maturation. (2) PE is significantly associated with higher motor performance and increased incidence of joint hypermobility, PC with lower motor performance and increased hypomobility. (3) Early identification of PC should promote efforts to increase joint mobility and accelerate motor performance.
