Abstract

Purpose: Dynamic biomechanical markers are known to be associated with knee osteoarthritis (OA) progression, pain, and functional limitations. While exercise in general is known to improve knee OA patients’ clinical outcomes, it remains unclear if specific rehab exercises can modify these biomechanical markers. Previous studies suggest that neuromuscular exercises, which emphasize on the control of weight-bearing movements, appear to be more suitable for this purpose. A recent randomized controlled trial (RCT) showed that a personalized conservative care approach including a tailored home-based exercise program, derived from a knee kinesiography exam, can impact these markers and improve clinical outcomes. The present study aimed at assessing, in patients from this RCT, the impact of adding neuromuscular exercises to the home-based program to address patient’ 3D biomechanical markers.

71. Predictors of long-term opioid effectiveness in chronic non-cancer pain patients attending multidisciplinary pain treatment clinics : a Quebec pain registry study

Abstract

Objective: This study aimed at identifying characteristics of individuals who are most likely to benefit from long-term opioid therapy in terms of reduction in pain severity and improved mental health-related quality of life (mQoL) without considering potential risks. Methods: This was a retrospective cohort study of 116 patients (age=51.3 ±12.5 years, male=42.2%) enrolled in the Quebec Pain Registry between 2008 and 2011 and who initiated opioid therapy after their first appointment in a multidisciplinary pain clinic and persisted with this treatment for at least 12 months. Clinically significant improvement was defined as a 2-point decrease on the PEG 0-10 Scale of pain severity at 12-month follow-up and a 10-point increase on the SF-12v2 Mental Health-Related Quality of Life Summary Scale which corresponds to one standard deviation of the mean in the general population (Mean = 50, SD = 10). Results: Clinically significant reduction in pain severity was observed in 26.7% of patients while improvement in mQoL was reported by 20.2% of patients on long-term opioid therapy. Older age (OR=1.04 (95% CI: 1.0 – 1.08), p=0.032) and alcohol or drug problems (OR=0.26 (95% CI: 0.07 – 0.96), p=0.044) were weakly associated with pain severity at 12-month follow-up. Baseline higher pain severity (OR=0.62 (95% CI: 0.43 – 0.91), p=0.014) and baseline higher mQoL (OR=0.89 (95% CI: 0.83 – 0.95), p=0.001) were associated with non-improvement in mQoL. Conclusion: The analysis failed to identify clinically meaningful predictors of opioid therapy effectiveness making it difficult to inform clinicians about which CNCP patients are most likely to benefit from long-term opioid therapy.

Abstract

Purpose: The diagnosis of knee osteoarthritis (OA) is typically well established with a clinical evaluation and confirmed with an X-Ray assessing the joint’ structural changes and disease progression. Guidelines also recommend taking into account mechanical factors (static and dynamic) to better understand knee function, since they may influence treatment outcomes. However, the relationship between clinical condition of the knee and biomechanical characteristics is not well known, including how such information stands compared to those from other conventional assessments, such as X-ray and physical assessment. The aim of this study is to evaluate the associations between the knee clinical condition assessed by patient-reported outcome measures and parameters from three different types of assessments, namely radiographic, musculoskeletal, and biomechanical assessment in OA patients. Methods: This cross-sectional study was conducted on patients with 1) knee pain ≥ 4/10 on a numeric rating scale in the past 7 days, 2) Kellgren-Lawrence (KL) radiographic OA severity grade higher than KL2, and 3) who were not on a waiting list for knee arthroplasty. Patients’ knee clinical condition was assessed using the Knee Injury and Osteoarthritis Outcome Score (KOOS) questionnaire that consists of five subscales: pain, symptoms, function in daily living (ADL), function in sport and recreation (Sport/Rec) and knee-related quality of life (QOL). Twenty musculoskeletal tests were performed by a therapist, including passive flexion and extension ranges of motion (ROM), muscle strength (10 tests assessing hip, knee, and ankle), flexibility (4 tests), swelling measured by the circumference difference between knees, effusion, balance, and functional 30-second chair stand tests (30s_CST). 99. Impact of a personalized care approach on 3D gait impairments in knee osteoarthritis patients (a cluster randomized controlled trial)

Abstract

Purpose: Knee osteoarthritis (OA) often leads to gait kinematic impairments. The knee kinesiography exam, measuring three-dimensional (3D) knee kinematics during gait on a treadmill, allows to objectively identify gait impairments (GIs) in order to provide recommendations for a personalized care approach (targeted home-based exercises, bracing, etc.) to correct these impairments. A clinical trial showed that this approach can lead to significant improvement in function and pain reduction after 6 months compared to a control group. The aim of this study was to assess the impact of this personalized care approach (PCA) on 3D mechanical GIs in knee OA patients compared to a control group. Methods: Primary care physicians in this cluster randomized controlled trial in the Province of Quebec (Canada) were asked to recruit patients with a clinical diagnosis of knee OA. Patients were included if 1) knee OA was the main cause of their knee pain, 2) they rated their worst pain in the past 7 days ≥ 4 on a 0-10 pain intensity scale, 3) they had a Kellgren-Lawrence grade ≥ 2 on radiographs. Eligible patients from a same primary care clinic were randomized to the same group: 1- a control group (usual care), 2- a group with the PCA, and 3- a group with the PCA + an educational program. In all of the three groups, primary care physicians managed their patients according to their individual needs, but only physicians from groups 2- and 3- had access to the recommendations for the PCA. These were treatment recommendations (e.g. bracing, specific activities, etc.) and tailored home exercises targeting the GIs identified with the knee kinesiography results. Patients from group 3- also had a one-hour educational session on knee OA self-management and two follow-up group meetings with a therapist (to answer their questions, regulate the nature and intensity of their exercises, etc.). For all patients, we assessed the presence of 14 known GIs in knee OA at baseline and 6-month follow-up

Abstract

Objectives Growing attention has been given to considering sex and gender in health research. However, this remains a challenge in the context of retrospective studies where self-reported gender measures are often unavailable. This study aimed to create and validate a composite gender index using data from the Canadian Community Health Survey (CCHS). Methods According to scientific literature and expert opinion, the GENDER Index was built using several variables available in the CCHS and deemed to be gender-related (e.g., occupation, receiving child support, number of working hours). Among workers aged 18–50 years who had no missing data for our variables of interest (n = 29,470 participants), propensity scores were derived from a logistic regression model that included gender-related variables as covariates and where biological sex served as the dependent variable. Construct validity of propensity scores (GENDER Index scores) were then examined. Results When looking at the distribution of the GENDER Index scores in males and females, they appeared related but partly independent. Differences in the proportion of females appeared between groups categorized according to the GENDER Index scores tertiles (p < 0.0001). Construct validity was also examined through associations between the GENDER Index scores and gender-related variables identified a priori such as choosing/avoiding certain foods because of weight concerns (p < 0.0001), caring for children as the most important thing contributing to stress (p = 0.0309), and ability to handle unexpected/difficult problems (p = 0.0375). Conclusion The GENDER Index could be useful to enhance the capacity of researchers using CCHS data to conduct gender-based analysis among populations of workers.
