Desmoplastic fibroma is a rare benign intraosseous tumor with locally aggressive behavior. The mandible is the most frequent site of involvement. It causes bone destruction with a tendency to invade the soft-tissues if untreated. It has a high propensity for local recurrence if treated inadequately. We report a case of desmoplastic fibroma of the mandible treated with a wide margin of excision.

The plant propagules migrate into denuded or conditionally created habitats by variety of means and grow, capable species establishes their population successfully and rest abolish. The present study was aimed to identify potential species in such habitats by evaluating naturalized community in conditionally crated habitats i.e. debris dumping sites of a hydroelectric power project in Western Himalayas, India for phytoretoration (eco-restoration) purpose. The data on phytosociological attributes of herbaceous community was collected from both debris dumping area (D) and undumped natural area (N) in the fringe, by quadrat method (1 x 1 m dimension). A total of 54 species from debris dumping sites and 128 species from undumped natural area (N) are recorded in this study. The invasive alien species predominates at dumping sites which covered 37% of the species richness, 50.99% of density, 76.67% of basal cover and 63.15% of dominance (IVI). Thus, invasive species are opportunistic in the process of phytorestoration in degraded habitats, which may not be beneficial for the better functioning of ecosystem but some of them can be considered as potential preliminary soil binder at such cases (dumping area). The development agencies must have an eco-restoration plan for such dumping zones which magnetized the encroachments of invasive alien species and play a pivotal role in degrading the natural ecosystem.

The present study was under taken to characterize and identify prominent species of Xanthomonas causing bacterial leaf spot disease on pepper and their genetic diversity in India. Thirty one isolates of Xanthomonads isolated from infected leaf and fruit samples of tomato, chilli and bell pepper from 5 different states of India, viz. Himachal Pradesh, Karnataka, Uttarakhand, Tamil Nadu and Haryana were characterized as translucent, yellow, raised colonies on YGCA medium, Gram negative, rod shaped, positive in starch hydrolysis, variation in pectin utilization. These isolates showed symptoms on chilli cv. Pusa Jwala and tomato cv. Pusa Ruby and they were grouped into pepper-tomato group (XCVPT), belonged to Xanthomonas euvesicatoria. Two sets of primers, i.e. Bs-XeF and Bs- XeR and Xeu 2.4 and Xeu 2.5 specific to X. euvesicatoria were used for further confirmation and the primers amplified DNA of all the isolates at 173bp and 208 bp respectively. Genetic diversity of 31 isolates of X. euvesicatoria along with four out group bacteria P. fluorescence DTPF-3, X. campestris pv. campestris Xcc-4, and Xcc-C23 and B. subtilis DTBS-5 was done by REP, ERIC and BOX- PCRs fingerprinting. Out of 12 DNA types, maximum isolates of X. euvesicatoria (12 isolates) belong to DNA type 2 isolated from tomato plants from different states of India and formed separated group isolated from pepper. High level of genetic diversity among the X. euvasicatoria isolates was observed. All out group bacteria were separated from X. euvesicatoria isolates.

Thirty-eight isolates of Ralstonia solanacearum were isolated from wilted eggplants from seven states of India, i.e. Uttarakhand, Himachal Pradesh, Jharkhand, Odisha, West Bengal, Karnataka and Meghalaya on TTC medium to study their genetic diversity using BOX, ERIC and REP-PCR’s. All isolates of R. solanacearum produced fluidal, irregular pinkish-white colonies and they belonged to biovar 3, race 1 and phylotype I. The study also included five out-group bacteria (Bacillus subtilis DTBS-5, Pseudomonas fluorescence DTPF-3, Xanthomonas campestris pv. campestris Xcc-C4, X. citri subsp. citri Xac-C63 and X. oryzae pv. oryzae Xoo-4). The isolates of R. solanacearum and out group bacteria were grouped into10 DNA types at 75% similarity coefficient. Maximum 11 isolates of R. solanacearum isolated from West Bengal were grouped into DNA Type 2. The isolates collected from Uttarakhand grouped into DNA types 6, 7, 8 and 10. However, all out-group bacteria were clearly separated from the isolates of R. solanacearum as DNA type 9. Multilocus sequence analysis of 18 isolates of R. solanacearum along with sequences obtained from NCBI database of 2 out- group bacteria (Burkholderia multivoransATCC-17616 and X. campestris pv. campestris ATCC-33913) was done using sequences of hrpB (hypersensitive response gene), fliC (flagelline protein) and egl (endoglucanase gene). Sequence comparisons showed high level of variability within these three regions of the genome involved in pathogenicity. On the basis of phylogenetic analysis, the selective pressure on virulence-related genes of R. solanacearum was identified. This genetic diversity presents major challenges for developing effective long-term management strategies to control the wilt disease of eggplant.

Black rot caused by Xanthomonas campestris pv. campestris (Xcc) is a very important disease of cauliflower (Brassica oleracea botrytis group) resulting into 10–50% yield losses every year. Since there is a dearth of availability of resistance to black rot disease in B. oleracea (C genome), therefore exploration of A and B genomes was inevitable as they have been reported to be potential reservoirs of gene(s) for resistance to black rot. To utilize these sources, interspecific hybrid and backcross progeny (B1) were generated between cauliflower “Pusa Sharad” and Ethiopian mustard “NPC-9” employing in vitro embryo rescue technique. Direct ovule culture method was better than siliqua culture under different temperature regime periods. Hybridity testing of F1 inter-specific plants was carried out using co-dominant SSR marker and Brassica B and C genome-specific (DB and DC) primers. Meiosis in the di-genomic (BCC) interspecific hybrid of B. oleracea botrytis group (2n = 18, CC) × B. carinata (2n = 4x = 34, BBCC) was higly disorganized and cytological analysis of pollen mother cells revealed chromosomes 2n = 26 at metaphase-I. Fertile giant pollen grain formation was observed frequently in interspecific F1 hybrid and BC1 plants. The F1 inter-specific plants were found to be resistant to Xcc race 1. Segregation distortion was observed in BC1 generation for black rot resistance and different morphological traits. The At1g70610 marker analysis confirmed successful introgression of black rot resistance in interspecific BC1 population. This effort will go a long way in pyramiding gene(s) for resistance against black rot in Cole crops, especially cauliflower and cabbage for developing durable resistance, thus minimize dependency on bactericides.

The present communication deals with the distribution, diversity and phytosociology of an important medicinal shrub Berberis lycium along Neeru stream, a major left bank tributary of river Chenab with an elevational gradient of 848 m to 2200 m. Berberis lycium, during our phytosociological surveys has emerged as the most dominant species amongst its understorey associates. The highest values of IVI (180.2 & 160.6) in the riparian zone (Zone-I) were recorded for site 7 (left bank, Dranga) and site 8 (right bank upslope, Zone-II, Amiranagar), respectively. The diversity parameters revealed the highest value (H’ 2.60) at Zone-II along the left bank. The soil parameters (pH, moisture content, Nitrogen, Phosphorous & Potassium) too had a profound effect on the distribution of the plant species, which has been discussed in the paper.

We report the synthesis of Pt nanoparticles and their burrowing into silicon upon irradiation of a Pt–Si thin film with medium-energy neon ions at constant fluence (1.0 × 1017 ions/cm2). Several values of medium-energy neon ions were chosen in order to vary the ratio of the electronic energy loss to the nuclear energy loss (Se/Sn) from 1 to 10. The irradiated films were characterized using Rutherford backscattering spectroscopy (RBS), atomic force microscopy (AFM), scanning electron microscopy (SEM), X-ray diffraction (XRD) and high resolution transmission electron microscopy (HRTEM). A TEM image of a cross section of the film irradiated with Se/Sn = 1 shows ≈5 nm Pt NPs were buried up to ≈240 nm into the silicon. No silicide phase was detected in the XRD pattern of the film irradiated at the highest value of Se/Sn. The synergistic effect of the energy losses of the ion beam (molten zones are produced by Se, and sputtering and local defects are produced by Sn) leading to the synthesis and burrowing of Pt NPs is evidenced. The Pt NP synthesis mechanism and their burrowing into the silicon is discussed in detail.

In paper (2004) Chang studied an inventory model under a situation in which the supplier provides the purchaser with a permissible delay of payments if the purchaser orders a large quantity. Tripathi (2011) also studied an inventory model with time dependent demand rate under which the supplier provides the purchaser with a permissible delay in payments. This paper is motivated by Chang (2004) and Tripathi (2011) paper extending their model for exponential time dependent demand rate. This study develops an inventory model under which the vendor provides the purchaser with a credit period; if the purchaser orders large quantity. In this chapter, demand rate is taken as exponential time dependent. Shortages are not allowed and effect of the inflation rate has been discussed. We establish an inventory model for deteriorating items if the order quantity is greater than or equal to a predetermined quantity. We then obtain optimal solution for finding optimal order quantity, optimal cycle time and optimal total relevant cost. Numerical examples are given for all different cases. Sensitivity of the variation of different parameters on the optimal solution is also discussed. Mathematica 7 software is used for finding numerical examples.

Primary carcinoma of ureter is an uncommon malignancy. Of which, mostly are transitional cell carcinomas followed by squamous cell carcinomas and adenocarcinomas being the rarest histopathology encountered. We report a case of adenocarcinoma ureter in a middle-aged male along with its clinical scenario. A 62-year-old male, presented with complaints of lower urinary tract symptoms. Computerized tomography urogram showed a soft tissue lesion at the right ureterovesical junction. Cystoscopic biopsy reported villous adenoma. Diethylene triamine pentaacetic acid scan reported nonfunctioning right kidney. He underwent laparoscopic right nephroureterectomy, and histopathology reported adenocarcinoma of the right lower third of ureter, with positive distal and close radial margins. The patient received external beam radiation to the postoperative bed and lymph nodes, and he is disease-free till date.

Calcifying odontogenic cyst (COC) is an unusual and unique lesion with characteristics of a solid neoplasm and of a cyst. It shows considerable amount of histopathological diversity, with variable clinical behavior such as cystic, neoplastic and infiltrating malignant behavior. It is a rare developmental odontogenic cyst with notable presence of histopathological features, which include a cystic lining demonstrating characteristic "Ghost" epithelial cells with a propensity to calcify. Odontomes are best known as hamartomatous benign tumors rather than true neoplasms, arising from odontogenic tissues. Histologically, they are classified as compound and complex variety. We report a rare case of COC of anterior maxilla with complex odontome in a 12-year-old female child.

Introduction: Awake intubation via Fiberoptic Bronchoscope (FB) is the gold standard for management of difficult airway but patients had to face problems like oxygen desaturation, tachycardia, hypertension and anxiety due to awake state. This study was conducted to assess feasibility of Fastrach Laryngeal Mask Airway (FLMA) to manage difficult airway as a conduit for intubation as well as for ventilation. Materials and Methods: After ethical approval and informed consent, 60 patients with difficult airway were randomly enrolled in FB group and FLMA group. In FB group, patients were sedated with midazolam/fentanyl. Airway anaesthetization of oropharynx was done with xylocaine spray and viscous and larynx and trachea by superior laryngeal nerve block and transtracheal block respectively. In FLMA group, initially patients were induced with propofol for FLMA insertion then succinylcholine was given for Tracheal Intubation (TI). The first TI attempt was done blindly via the FLMA and all subsequent attempts were performed with fiberoptic guidance. Haemodynamic monitoring was done during induction, intubation, immediately post insertion and there after at five minutes interval for 30 minutes. Results: All patients in the FLMA group were successfully ventilated (100%). In both the groups 28 (93.33%) patients were successfully intubated. However, first/second/third attempt intubation rate in FLMA vs FB group was 15 (50%) vs 13 (43.3%), 8 (26.66%) vs 10 (33.33%) and 5 (16.66%) in both groups respectively. Townes-Brocks syndrome (TBS) is an autosomal dominant disorder with multiple malformations and variable expression. Major findings include external ear anomalies, hearing loss, limb deformity, imperforate anus, and renal malformations. Intelligence is usually normal, although mild moderate mental retardation has been reported. It can be difficult to differentiate clinically between hemifacial microsomia (HFM) and Townes-Brocks syndrome (TBS). The distinction is important because TBS is inherited as an autosomal dominant trait, whereas HFM is sporadic. We report a classical case of TBS with overlapping features of hemifacial microsomia in an 11 year old boy who presented with unilateral anotia with hearing defect, renal agenesis and imperforate anus by birth.

Xanthomonas campestris pv. Campestris (Xcc) causing black rot disease of crucifer crops, is a serious worldwide problem resulting in >50% loss under environmental conditions favorable to the pathogen. For the characterization of Xanthomonas campestris pv. campestris races to standardize the Indian cultivars of Brassica spp. total of 184 representatives lines of the family crucifereae from of five economically important Brassica species, i.e. B. carinata, B. juncea, B. napus, B. oleracea, and B. rapa grown in India were inoculated by 75 strains of Xcc, belonging to three races 1, 4 and 6 in 2012-13 and 2013-14. Disease reaction of these cultivars was compared with the standard differential hosts of crucifer for disease incidence and diseases severity. Indian cultivars of B. juncea (Pusa Bold, Pusa Varuna, Pusa Vijay, Pusa Mustard-21, Pusa Mustard-25, Pusa Sag), B. olerecea var. botrytis (Pusa Sukti), B. olerecea var. capitata (KGMR) and B. rapa (Pusa Swarnima) were at par with the standard differentials of respective Brassica species and thus could be suitable to useas alternative differential hosts for characterization of Xcc races. 20 cultivars of Brassica spp. grown in India were further validated as differential crucifer host to characterize the races of Xcc on the molecular basis using four set of primers, viz. CAPS (ACS2) CAPS (BTPT) and CAPS (NDPK3) and among them, Br 019305 primer amplified at 906 bp in all cultivars, while primer CAPS (ACS2), CAPS (BTPT) and CAPS (NDPK3) primers were amplified multiple bands. Combined phylogenetic UPGMA study at 75% similarity coefficient, 19/20 accessions were divided into three groups and distinguished resistant and susceptible cultivars against black rot disease.

An integrated management of bacterial wilt of tomato against race 1 biovar III of Ralstonia solanacearum was conducted with eleven treatment combinations on resistant (cv Swarna Lalima) and susceptible (cv. Pusa Ruby) of tomato from 2006 to 2009-10 during rainy seasons. The pooled analysis of different treatments × variety × date of sampling were showed significanteffects on Ralstonia population, wilt incidence and yield of the crop. This treatment also reduced R. solanacearum population and increased the yield 47.2% in cv. Swarna Lalima (343.7 q/ha) with B: C ratio (1.49:1) over resistant check.

Introduction: Buprenorphine is a semi-synthetic derivative of thebaine; its low concentration is sufficient to provide effective pain relief. Aim: To evaluate the efficacy of transdermal buprenorphine patch in postoperative pain management. Materials and Methods: After ethical approval and taking informed consent from the patients, they were randomized into three groups (n=30 in each group) using a computer generated random number table. Group A: placebo patch; Group B: buprenorphine (10mg) patch and Group C: buprenorphine (20mg) patch. Haemodynamic and analgesic effects were compared by using analysis of variance (ANOVA) followed by Turkey’s post hoc test. The proportion of side effects was compared using the Chi-square test. Results: Haemodynamic changes were not statistically different in all the three groups A, B and C, whereas at the end of surgery VAS score of Group A subjects was significantly higher (4.93+0.98) as compared to Group B (1.73+0.64) and Group C (1.40+0.50). On 2nd postoperative day, no pain was reported by the Group C patients and on 4th day after surgery, no pain was reported by Group B patients. Conclusion: The transdermal buprenorphine patch (20mg) was effective in attenuating postoperative pain, maintaining haemodynamic stability requiring no rescue analgesia, with fewer postoperative rescue analgesic requirements in low dose of buprenorphine patch (10mg) group.

The present study analyzes the biochemical effects of exogenous iodine on the antioxidant defense system of soybean (Glycine max L.) plant when grown under heat stress (35ºC). The parameters studied under oxidative stress generation were superoxide anion production, hydrogen peroxide formation and malondialdehyde production. The enzymatic defense was investigated by analyzing the levels of superoxide dismutase, ascorbate peroxidase, catalase and glutathione reductase. For the partial expression of 1-cys peroxiredoxin, the genomic DNA was extracted and primers were designed to amplify the 3’ end region of the PER-1 gene locus 17780502 to 17781727 of genome size 61Mbp, 304 base pairs: PER1-F 5’-ATGCCAGGGATCACACTAGG-3’and PER1-R 5’-ACAAGACAGACCAAGGAGCT-3’ and subjected to PCR. The heat stress generated maximum malondialdehyde without application of iodate and production of superoxide anion was maximum in 80 µM iodate concentration. The H2O2 production was reported highest in 40 µM iodate concentration, Enzymatic defense was highest in 20 and 80 µM dosages of iodate. We amplified 1-cys peroxiredoxin (1-Cys Prx) gene, an important antioxidant enzyme, in soybean plant using primer of an Arabidopsis 1-Cys Prx gene to confer tolerance against heat stress.

Background: The aim of this double-blind, prospective, randomized, controlled study was to compare the effect of addition of ketamine; fentanyl and saline with propofol anesthesia on hemodynamic profile and laryngeal mask airway (LMA) insertion conditions in oral clonidine premedicated children. Methods: 180 children (age 2 - 10 years) were at first given oral clonidine (4 μg/kg) 90 minutes before operation, and then were randomly allocated to receive either ketamine 0.5 mg/kg (n=60), fentanyl 1 μg/kg (n=60) or 0.9% normal saline (n=60) before induction with propofol 3.0 mg/kg. Insertion of LMA was performed within 1 minute of injection of propofol. Heart rate and mean blood pressure were noted 1 min before induction (baseline), immediately after induction, before and after insertion of LMA for up to 3 min. Following LMA insertion, 6 subjective end points were noted-mouth opening, coughing, swallowing, patient′s movement, laryngospasm, and ease of an insertion. LMA insertion summed score was prepared depending upon these variables. Results: LMA insertion summed score was nearly similar in ketamine and fentanyl group, which were significantly better than saline group (P120 secs.) was higher in fentanyl group compared to ketamine and saline group. Conclusion: Even in oral clonidine premedicated children, addition of ketamine with propofol provides hemodynamic stability and comparable conditions for LMA insertion like fentanyl propofol with significantly less prolonged apnea.

Pleomorphic adenomas are slow growing, well-demarcated benign neoplasms that constitute more than 80% of benign parotid tumours. Deep lobe parotid pleomorphic adenomas are rare tumors that present a diagnostic and therapeutic challenge. Approximately 10-12% of pleomorphic adenomas of the parotid are thought to arise from the deep lobe of parotid. Due to anatomical relations and restrictive boundaries of the deep lobe, parapharyngeal extensions may remain asymptomatic until reaching a very large size. These tumors have a variety of distinct clinical presentations, most commonly arising from the portion of the gland deep to the facial nerve lateral to the mandible. We present a case of deep lobe parotid gland pleomorphic adenoma involving the parapharyngeal space in a 38 year old female.

The northeast region of India is one of the world’s most significant biodiversity hotspots. One of the richest bird areas in India, it is an important route for migratory birds and home to many endemic bird species. This paper describes a literature-based dataset of species occurrences of birds of northeast India. The occurrence records documented in the dataset are distributed across eleven provinces of India, viz.: Arunachal Pradesh, Assam, Bihar, Manipur, Meghalaya, Mizoram, Nagaland, Sikkim, Tripura, Uttar Pradesh and West Bengal. The geospatial scope of the dataset represents 24 to 29 degree North latitude and 78 to 94 degree East longitude, and it comprises over 2400 occurrence records. These records have been collated from scholarly literature published between1915 and 2008, especially from the Journal of the Bombay Natural History Society (JBNHS). The temporal scale of the dataset represents bird observations recorded between 1909 and 2007. The dataset has been developed by employing MS Excel. The key elements in the database are scientific name, taxonomic classification, temporal and geospatial details including geo-coordinate precision, data collector, basis of record and primary source of the data record. The temporal and geospatial quality of more than 50% of the data records has been enhanced retrospectively. Where possible, data records are annotated with geospatial coordinate precision to the nearest minute. This dataset is being constantly updated with the addition of new data records, and quality enhancement of documented occurrences. The dataset can be used in species distribution and niche modeling studies. It is planned to expand the scope of the dataset to collate bird species occurrences across the Indian peninsula.

Background: Adjuvants have been used to prolong analgesic effects of epidural local anaesthetics. We studied two different doses of neostigmine. Patients & Methods: A randomized double blind study was conducted on ninety adult females scheduled for lower intra abdominal surgeries. The study was designed to compare two doses of epidural neostigmine co administered with lignocaine, with regard to its analgesic efficacy and its effect on sedation in postoperative period. Patients were divided into three groups of 30 each. Group I received lignocaine 1% (9ml) with normal saline (1ml), group II lignocaine1% (9ml) with neostigmine 100μg in saline (1ml) and group III received lignocaine 1% (9ml) with neostigmine 200μg in NS (1ml). Group I served as a control. In operating room, after putting epidural catheter, general anesthesia was administered with propofol (2mg kg-1), succinylcholine (2mg kg-1) and maintained with O2, N2O, relaxant technique. At the end of surgery, patients were reversed. Epidural analgesic medication was administered to after proper recovery from anesthesia. Intensity of pain relief on VAS, duration of analgesia, level of sensory block, motor blockade, sedation by sedation score and complications were assessed. Results: The addition of neostigmine resulted in significant longer duration of analgesia (dose independent) and sedation (dose dependent). Sensory and motor blockade were identical in all three groups. There was no incidence of respiratory depression, pruritus, bradycardia or hypotension in any group. Two patients in control group and one, receiving neostigmine (200μg), developed nausea/vomiting. Conclusion: Co administration of epidural neostigmine and lignocaine appears to be a useful technique for postoperative analgesia as it increases the duration of analgesia and provides desirable sedation at the same time.

For treatment of intra and postoperative pain, no drug has yet been identified that specifically inhibits nociception without associated side effects. Magnesium has antinociceptive effects in animal and human models of pain. The current prospective randomised double-blind study was undertaken to establish the effect of addition of magnesium or clonidine, as adjuvant, to epidural bupivacaine in lower abdominal and lower limb surgeries. A total of 90 American Society of Anesthesiology (ASA) grade I and II patients undergoing lower abdominal and lower limb surgeries were enrolled to receive either magnesium sulphate (Group B) or clonidine (Group C) along with epidural bupivacaine for surgical anaesthesia. All patients received 19 ml of epidural bupivacaine 0.5% along with 50 mg magnesium in group B, 150 mcg clonidine in Group C, whereas in control group (Group A), patients received same volume of normal saline. Onset time, heart rate, blood pressure, duration of analgesia, pain assessment by visual analogue score (VAS) and adverse effects were recorded. Onset of anaesthesia was rapid in magnesium group (Group B). In group C there was prolongation of duration of anaesthesia and sedation with lower VAS score, but the incidence of shivering was higher. The groups were similar with respect to haemodynamic variables, nausea and vomiting. The current study establishes magnesium sulphate as a predictable and safe adjunct to epidural bupivacaine for rapid onset of anaesthesia and clonidine for prolonged duration of anaesthesia with sedation.

JUSTIFICATIVA E OBJETIVOS: A percepção dos pacientes a respeito da utilidade do Ambulatório de Avaliação Pré-anestésica e o fluxo de pacientes não foram extensamente estudados no mundo em desenvolvimento. O objetivo deste estudo foi analisar esse aspecto. MÉTODO: Foi realizada pesquisa aplicando-se questionário entre pacientes atendidos no Ambulatório de Avaliação Pré-anestésica de um hospital terciário de ensino em Trinidad. A título de comparação, esse questionário também foi distribuído aos pacientes atendidos no Ambulatório de Cirurgia Geral. Os parâmetros demográficos, incluindo idade, sexo, nível escolar e estado físico ASA foram anotados. Outros dados registrados incluíam o fluxo de pacientes e detalhes a respeito da equipe médica. RESULTADOS: Dos 220 pacientes atendidos no Ambulatório de Avaliação Pré-anestésica, 92,7% participaram do estudo. A validade do questionário foi apoiada pelo coeficiente alfa de Cronbach (0,67). O tempo médio para o encaminhamento do Ambulatório de Cirurgia para a Clínica Pré-anestésica foi de 50 dias; a espera média na clínica foi de 2,7 horas e da espera pela intervenção cirúrgica após a aceitação pelo ambulatório foi de 13 dias. As opiniões dos pacientes a respeito dos benefícios da clínica e tempo de espera não foram influenciadas pela idade nem pelo nível educacional. Na opinião dos pacientes, o atendimento no Ambulatório de Avaliação Pré-anestésica foi satisfatório, mas os tempos de espera foram considerados longos. CONCLUSÕES: Na opinião dos pacientes, a consulta no Ambulatório de Avaliação Pré-anestésica antes do procedimento cirúrgico foi útil e o atendimento foi considerado satisfatório.JUSTIFICATIVA Y OBJETIVOS: La percepción de los pacientes respecto de la utilidad del Ambulatorio de Evaluación Preanestésica como también el flujo de pacientes, no fueron extensamente estudiados en el mundo en desarrollo. El objetivo de este estudio fue analizar ese aspecto. MÉTODOS: Se realizó una investigación aplicando un cuestionario entre los pacientes atendidos en el Ambulatorio de Evaluación Preanestésica de un hospital subcontratado de enseñanza en Trinidad. Como comparación, ese cuestionario también se distribuyó a los pacientes atendidos en el Ambulatorio de Cirugía General. Los parámetros demográficos, incluyendo edad, sexo, nivel escolar y estado físico ASA se registraron. Otros datos registrados incluían el flujo de pacientes y los detalles respecto del equipo médico. RESULTADOS: De los 220 pacientes atendidos en el Ambulatorio de Evaluación Pré-anestésica, un 92,7% participaron del estudio. La validez del cuestionario fue apoyada por el coeficiente alfa de Cronbach (0,67). El tiempo promedio para la derivación al ambulatorio de cirugía para la Clínica Preanestésica fue de 50 días; el tiempo promedio de espera en la clínica fue de 2,7 horas y el de espera por la cirugía después de la aceptación por parte del ambulatorio fue de 12 días. Las opiniones de los pacientes respecto de los beneficios de la clínica y tiempo de espera no tuvieron ningún influjo por la edad ni por el nivel cultural. Según los pacientes, la atención en el Ambulatorio de Evaluación Preanestésica fue benéfica, pero los tiempos de espera fueron considerados extensos. CONCLUSIONES: Según los pacientes, la consulta en el Ambulatorio de Evaluación Preanestésica antes del procedimiento quirúrgico fue útil y la atención fue considerada satisfactoria.BACKGROUND AND OBJECTIVES: Perception of the patients regarding the utility of the Preanesthetic Clinics and flow time in clinics has not been widely studied in the developing world. The present study aims to study this aspect. METHODS: A self-administered 15-item questionnaire survey was conducted among patients attending the Preanesthetic Clinics at a tertiary care teaching hospital in Trinidad. The questionnaire was also distributed to the patients attending the General Surgical Clinic for comparison. Another questionnaire was distributed among the staff of the Preanesthetic Clinic. Patient demographics including age, gender, and educational status and American Society of Anesthesiologists physical status were noted. Other data recorded were patient flow time and details of attending staff. RESULTS: Of the 220 patients who attended the Preanesthetic Clinics, 92.7% participated in the study. The reliability of the questionnaire was supported by Cronbach's alpha coefficient (0.67). The median time for referral from the surgical clinic to Preanesthetic Clinic was 50 days, median waiting time in the clinic was 2.7 hours, and the median waiting time for surgery after acceptance in the clinic was 13 days. The patients' opinions regarding the benefits of the clinic, length of the waiting time was independent of their age and educational status. Patients felt that attending the Preanesthetic Clinic was beneficial and not costly to them, although the waiting times were found to be longer. CONCLUSIONS: Patients perceive that attending the Preanesthetic Clinic has been useful before the surgical procedure and the care they received in the clinic was satisfactory

Black rot caused by Xanthomonas campestris pv. campestris (Pam.) Dowson is the most destructive disease of cauliflower causing huge loss to the farmers throughout the world. Since there are limited sources of resistance to black rot in B. oleracea (C genome Brassica), exploration of A and B genomes of Brassica was planned as these were thought to be potential reservoirs of black rot resistance gene(s). In our search for new gene(s) for black rot resistance, F2 mapping population was developed in Brassica carinata (BBCC) by crossing NPC-17, a susceptible genotype with NPC-9, a resistant genotype. Out of 364 Intron length polymorphic markers and microsatellite primers used in this study, 41 distinguished the parental lines. However, resistant and susceptible bulks could be distinguished by three markers At1g70610, SSR Na14-G02 and At1g71865 which were used for genotyping of F2 mapping population. These markers were placed along the resistance gene, according to order, covering a distance of 36.30 cM. Intron length polymorphic markers At1g70610 and At1g71865 were found to be linked to black rot resistance locus (Xca1bc) at 6.2 and 12.8 cM distance, respectively. This is the first report of identification of markers linked to Xca1bc locus in Brassica carinata on B-7 linkage group. Intron length polymorphic markers provided a novel and attractive option for marker assisted selection due to high cross transferability and cost effectiveness for marker assisted alien gene introgression into cauliflower.

Xanthomonas campestris pv. campestris (Xcc) is causal agent of black rot disease in crucifers. Genetic diversity, 28 isolates of Xcc collected from cole crops (cauliflower, cabbage, broccoli and knol khol), turnip, brown and yellow mustard crops from northern region of India was performed by using rep-PCR fingerprinting (BOX, REP and ERIC) and random amplified polymorphic DNA (RAPD). Five DNA type groups were formed by using rep-PCR fingerprinting. In RAPD study, six primers showed monomorphic bands and remaining six primers (OPB-10, OPB- 15, UBC-245, D-11, UBC-220 and OPA-11) exhibited polymorphic amplified product in all isolates of Xcc. Xcc- C281 and Xcc-C261 and Xcc-C268 were formed separate group as DNA types, 1, 2, 3, 4, 5 and 6 respectively. Genetic variability was found among the isolates of Xcc based on their hosts. They were categorized into eight groups at DNA typing at ~50% similarity coefficient and formed 8 DNA type groups at ~ 50% similarity coefficient. Eighteen isolates of Xcc were grouped under DNA type 7, whereas in DNA type 8 (4 isolates).

Odontogenic fibromyxoma is a rare odontogenic tumor of mesenchymal origin. It is composed of large amounts of intercellular substance rich in acid mucopolysaccharides, making it locally very aggressive and with high recurrence rates following conservative excision. Its histological and radiological features make it difficult to differentiate from other odontogenic tumors and, occasionally, may be misinterpreted as a malignant lesion. Although the origin of a myxoma is still obscure, its origin from the dental follicle seems to be the most reasonable explanation. We report a case of odontogenic fibromyxoma of maxilla in a 32-year-old female patient.

Background: Feeling of pain is one of the most important emotional determinants which dominate the perception of females who undergo the process of labour and delivery. Patient controlled epidural labour analgesia (PCEA) is convenient and safer technique for this purpose. Very few studies compared clonidine and fentanyl with ropivacaine in labour analgesia in past. This study was undertaken to compare fentanyl and clonidine in PCEA. Aims: To compare low concentration ropivacaine with or without fentanyl or clonidine for labour analgesia and its effect on maternal and foetal safety. Settings and Design: Prospective, double blind, randomized, comparative study. Materials and Methods: Ninety primegravida in labour were divided into three groups (n=30) and patient controlled epidural labour analgesia was given to them: Initial bolus of 10ml of ropivacaine 0.125% in Group I; with fentanyl 2 µg/ml in Group II and with clonidine 1µg/kg in Group III. Subsequently each group received ropivacaine 0.125% through patient controlled epidural analgesia (PCEA) as background infusion of 5 ml/hr with lockout interval time of 10min and subsequent bolus of 5ml. Hemodynamic parameters, sensory level, motor block and pain relief were noted. Total analgesic dose of local anaesthetic and feto-maternal adverse effects were also recorded. Results: At baseline, groups were matched demographically, hemodynamically as well as for intensity of pain. There was a statistically significant decrease in hemodynamic parameters from baseline in all groups with maximum reduction in group III. A significant difference among groups in VAS was observed at zero min and from 120min till 240min intervals and lowest values were in Group III. No significant difference was observed among the groups for mode of delivery and expulsive efforts. Total analgesic dose and PCA bolus requirement was maximum in Group I and minimum in Group III and the difference was statistically significant among groups. Six (20%) patients had shivering in Group II and hypotension was recorded in only 1 (3.3%) patient of Group III. Conclusion: Ropivacaine 0.125% was effective in decreasing labour pain without any motor blockade. Clonidine 1µg/kg was superior to fentanyl 2µg/ml as an adjuvant in PCEA for labour without any significant feto-maternal adverse effects.

Abstract Introduction Simulated exercises provide a safe space for trainees to hone their clinical and decision-making skills in pediatric critical care. This resource includes a series of case-based scenarios on respiratory failure, cardiovascular instability, neurological emergencies, and pediatric procedural sedation for use with a high-fidelity human simulator. Target learners include residents, medical students, and critical care nurses in pediatric critical care. Methods This resource includes an instructor's guide on how to prepare for and implement the training program, as well as guidelines on appropriate methods for debriefing and evaluation of teaching effectiveness at completion of the training exercise. Results At Tulane University this simulation has been taught to third- and fourth-year medical students since 2011 during a pediatric clinical competency seminar. The simulation has also been offered to pediatric interns at the end of their first year before their pediatric intensive care unit rotation begins, and is offered to nurses during an annual Pediatric Transport Course. This work has been very effective based on positive feedback from trainees. Discussion This course has been successfully implemented for teaching medical students, pediatric residents, and pediatric intensive care nurses. This course provides the avenue for trainees to hone their clinical and decision making skills in pediatric critical care through simulated exercises; therefore, it has the potential for improving patient safety and limiting medical errors resulting from poor clinical and inadequate training and decision making skills.

Genetic diversity of Ralstonia solanacearum, causal agent of bacterial wilt of tomato was assessed by using three different molecular methods such as random amplified polymorphism DNA (RAPD), BOX-PCR and hrp (hypersensitive response and pathogenicity) gene sequence analysis technique. Twelve isolates of Ralstonia solanacearum belonging to biovar 3 (9 isolates) and 4 (3 isolates) were collected from Northern parts of India including Himachal Pradesh and Uttarakhand states from infected tomato plants. Out of 16 primers used in RAPD fingerprinting, four primers (OPA-2, OPA-11, OPC-5, OPE-7) showed monomorphic bands and remaining 12 primers exhibited polymorphic amplified products of both the biovars of R. solanacearum. The primer OPE-10 showed the highest level of genetic diversity among the isolates. Ten isolates of R. solanacearum were classified into two clusters at 20 per cent similarity coefficient and cluster 1 represented all isolates of biovar 3 (UTT-23, UTT-10, UTT-26, HPT11, UTT-9, UTT-32, HPC-3) and cluster 2 comprised the biovar 4 (UTT-22, HPT-3, UTT-24). BOX-PCR fingerprint of R. solanacearum clearly distinguished biovar 3 and 4 grouped into two distinct clusters at 40% similarity coefficient. Cluster 1 represented all isolates of R. solanacearum biovar 3 and cluster 2 comprised the biovar 4 isolates. The isolates of R. solanacearum have genetic diversity in hrpB gene, but it could not differentiate the isolates of biovar 3 and 4. However, the biovars 3 and 4 of R. solanacearum can be genetically distinguished by using BOX- PCR and specific primer of RAPD.

Aims. The aim is to present a case of rare giant granular cell ameloblastoma and to review the pertinent literature highlighting the molecular aspects of its pathogenesis by analyzing the expression of CD-68, Bcl-2, and β-catenin. Methods. H and E stained sections showed large odontogenic islands showing peripheral ameloblast-like cells and central stellate reticulum-like cells with extensive granular cell transformation surrounded by fibrous stroma. Polyclonal rabbit anti-CD 68, anti-Bcl2, and anti-β-catenin were stained immunohistochemically. Results. CD-68 showed a moderate to strong staining intensity in granular cells. Moderate staining of Bcl-2 was expressed by the peripheral columnar cells of tumor islands and negative in the granular cells. Expression of β-catenin was generally weak, except for only the focal areas that showed a moderate staining intensity and weak in peripheral cells. Conclusion. The present case of giant granular cell ameloblastoma is a rare entity. Development of monstrous size is indicative of ameloblastomas persistent growth. Granular cell transformation in ameloblastomas probably occurs as a consequence of extensive molecular changes. Immunohistochemical studies help us to know the pathogenesis of this granular cell ameloblastoma. Therefore, an effort has been made here to study the expression of Bcl-2, CD-68, and β-catenin.

Systemic arterial hypertension in children has traditionally been thought to be secondary in origin. Increased incidence of risk factors like obesity, sedentary life-styles, and faulty dietary habits has led to increased prevalence of the primary arterial hypertension (PAH), particularly in adolescent age children. PAH has become a global epidemic worldwide imposing huge economic constraint on health care. Sudden acute increase in systolic and diastolic blood pressure can lead to hypertensive crisis. While it generally pertains to secondary hypertension, occurrence of hypertensive crisis in PAH is however rare in children. Hypertensive crisis has been further subclassified depending on presence or absence of end-organ damage into hypertensive emergency or urgency. Both hypertensive emergencies and urgencies are known to cause significant morbidity and mortality. Increasing awareness among the physicians, targeted at investigation of the pathophysiology of hypertension and its complications, better screening methods, generation, and implementation of novel treatment modalities will impact overall outcomes. In this paper, we discuss the etiology, pathogenesis, and management of hypertensive crisis in children. An extensive database search using keywords was done to obtain the information.

Nature and magnitude of genetic diversity was assessed in 170 wild raspberry genotypes based on eight quantitative characteristics, viz., fruit weight, fruit length, fruit breadth, TSS, acidity, reducing sugars, non-reducing sugars and Vitamin C. A survey was conducted in three north-western Himalayan states of Himachal Pradesh, Jammu&Kashmir and Uttarakhand. The species was found to be distributed between 760 and 1950m AMSL, 30°10'159" to 33°04'693"N and 74°44'076" to 78°25'681"E. The non-hierarchical cluster analysis resulted in 12 clusters of genotypes. The cluster pattern did not exhibit any interrelation between geographical isolation and genetic diversity. Of the 170 genotypes, 31 fell in Cluster XII, 27 in Cluster V, 19 in Cluster I, 17 in Cluster IX, 16 in Cluster VIII, 15 in Cluster XI, 13 in Cluster II, 12 in Cluster VII, 10 in Cluster III, six in Cluster X, three in Cluster VI and one genotype in Cluster IV. Genotypes falling under Clusters III, VI , VI can be used as parents in hybridization programmes for improving important traits like TSS, fruit weight and acidity respectively.

The present investigation was carried out in various districts of Himachal Pradesh, Jammu&Kashmir and Uttarakhand States falling under north-western Himalayan region of India. As a result of sustained exploration, 170 wild raspberry genotypes were marked and studied for berry quality attributes. Variation ranged from 0.25 g-0.93 g for berry weight. Berry length varied between 6.31 mm and 14.46 mm, while, berry breadth was 7.02 mm to 15.91 mm. Variation in Total Soluble Solids (TSS) in berry ranged between 9.6oB and 18.6oB whereas, acidity in berries ranged between 1.02 and 1.72%. The range of variation was 2-4.90% for reducing sugars, 4.2° - 11.6° for non-reducing sugars and 2.4- 5.2 mg/100 g for ascorbic acid. Berry weight had significant and positive correlation with its length and its breadth. Berry length exhibited positively significant correlation with berry breadth.
