92. Is standardised 18F-FDG uptake value an outcome predictor in patients with stage III non-small cell lung cancer?

Abstract

Purpose: Recent studies have demonstrated the relevance of 18F-FDG uptake as an independent prognostic factor for recurrence of operable non-small cell lung cancer (NSCLC). This corresponds with the experimental finding that FDG uptake correlates with the proliferative activity of tumour cells (Higashi et al., J Nucl Med 2000;41:85-92). On the basis of these observations, we studied the influence of FDG uptake on prognosis and occurrence of distant metastases in patients with advanced NSCLC. Methods: One hundred and fifty-nine patients with NSCLC of UICC stage IIIA or IIIB were included in the study. In all patients, neoadjuvant treatment was planned to achieve operability. FDG PET was performed as an additional staging procedure prior to the initiation of therapy. Clinical outcome data in terms of overall survival, disease-free survival and incidence of distant metastases could be obtained for 137 patients and were correlated with the average standardised uptake value of the tumour (SUVavg). Furthermore, other factors influencing SUVavg and patient outcome (histological tumour type, grading, UICC stage, tumour size) were analysed. Results: SUVavg was significantly influenced by tumour histology, UICC stage and tumour size. No significant difference could be shown for grading. In 38 out of the 159 patients (24%), FDG PET revealed previously unsuspected distant metastases. The incidence of distant metastases significantly correlated with SUVavg. Overall survival tended to decrease with increasing SUVavg; however, significance was only reached when a cut-off of 12.0 was applied (p=0.05). Conclusion: FDG uptake is an independent prognostic factor in patients with UICC stage III NSCLC, although less distinctively so than has been reported for stage I/II tumours. [ABSTRACT FROM AUTHOR]

94. Validation of continuous-wave Doppler measurements of mitral valve gradients during exercise — a simultaneous Doppler-catheter study.

Abstract

In patients with mitral stenosis, continuous-wave Doppler measurements of the maximal transmitral inflow velocity can be converted into transvalvular pressure gradients using the modified Bernoulli equation. Because of close correlations between Doppler- and catheter-measured gradients this method has become a valuable tool in non-invasive evaluation of mitral stenosis at rest. However, in some patients, exercise studies are necessary to determine the haemodynamic significance of the valve stenosis. The accuracy of continuous-wave Doppler in this setting has not yet been validated. Thus, in 20 selected patients with pure or predominant mitral stenosis, continuous-wave Doppler echo-cardiography was performed during left- and right-heart catheterization. At rest and during submaximal bicycle exercise, Doppler and pressure measurements were simultaneously performed. The Doppler gradient was calculated according to the modified Bernoulli equation while the mean manometric gradient was determined from the simultaneous pulmonary wedge and left ventricular pressure curves. Exercise caused a significant increase of cardiac output (4·8+ 1·3 to 5·7±1·31 min) and heart rate (59·7±12·0 to 95·3±14·3 beats minmin). The mean Doppler gradient increased from 6·8 ± 2·1 to 12·2 ± 3·2 mmHg. The manometric gradient showed a comparable increase of 9·5 ± 2·4 to 17·2 ± 3·7 mmHg, respectively. Correlation between Doppler and manometric data was close (y = 0·79x − 0·67,r = 0·90, SEE= 0·97 mmHg) at rest and still good during exercise (y = 0·71x − 0·10, r = 0·82, SEE= 1·97 mmHg). Thus, in some patients with borderline resting gradients and valve areas, exercise Doppler might allow further identification of the haemodynamic severity of mitral stenosis. 95. Diagnosis and quantification of aortic regurgitation by pulsed Doppler echocardiography in patients with mitral valve disease.

Abstract

To test the ability of pulsed Doppler echocardiography (PDE) to detect and quantify aortic regurgitation (AR), 55 consecutive patients {14–74 years) with aortic and mitral valve disease were examined clinically and by echocardiography before cardiac catheterisation. The severity of AR was determined angiographically (I–IV) and compared to the extent of the regurgitant jet in the left ventricle measured by PDE. In 13 of 55 patients (3 with mitral stenosis, 3 with mitral incompetence, 3 with combined mitral lesions, 3 with aortic stenosis, one with aortic and mitral stenosis) neither angiography nor PDE showed AR (specificity 100%). Apart from 3 patients with poor echo quality PDE correctly detected AR in 39 of 42 patients (sensitivity 93%). Clinical examination (62%), mode M-mode (62%) and both methods combined (81%) were significantly less sensitive than PDE, especially in mild AR (P < 0.008). The PDE degree of AR closely correlated with angiography (corrected contingency coefficient 0.91). Differentiation between AR III and IV was not possible. Mitral valve disease did not affect quantification of AR (n = 20 patients).Conclusions: Pulsed Doppler echocardiography is better than auscultation and M-mode echocardiography in the diagnosis of aortic regurgitation, especially in grades I and II. PDE can reliably discriminate between three degrees of aortic regurgitation (I–III). Mitral valve disease does not affect quantification of aortic regurgitation by PDE. [ABSTRACT FROM PUBLISHER]
