85. BioGlue®: A Protective Barrier After Pericardiotomy.

Abstract

Background: Repeat operation on the heart composes about 20% of procedures in contemporary practice of cardiac surgery. A sheet of material providing a barrier against cardiac adhesion to the sternum would be desirable. Methods: Anterior pericardiectomy was performed in rats. BioGlue® milled to a 0.4 mm sheet was applied to the anterior surface of the heart in 16 rats; Surgicel® plus liquid BioGlue® in seven; Surgicel alone in three; and nothing (control) in eight. The operative site was reexamined for gross evidence of adhesion, scarring, and residual BioGlue® 1, 3, and 6 months later. Results: There was formation of a loose connective tissue barrier containing blood vessels without scar formation in all animals treated with milled BioGlue®. Surgicel® plus BioGlue® resulted in a barrier containing more denser connective tissue with collagen fibers. Surgicel® alone resulted in a similar barrier. No barrier formed in the control experiments. Conclusions: A sheet of milled BioGlue® applied over the surface of the heart but not attached to it after partial pericardiectomy has been shown to stimulate formation of a loose connective tissue barrier containing blood vessels. This barrier is unique compared to dense fibrous scar which usually forms after opening the pericardium for cardiac operations. [ABSTRACT FROM AUTHOR]

92. The effect of autologous blood salvage techniques upon bank blood usage and the cost of routine coronary revascularization.

Abstract

Preservation of autologous blood during cardiac surgery may reduce the need for homologous blood transfusions. We reviewed our experience for patients undergoing primary coronary revascularization to determine the effect of the use of the Haemonetics Cell-Saver upon blood bank resources and upon the cost of operation.The quantity of homologous blood required by two groups of patients was compared. One group of 46 patients had operation prior to use of the Cell- Saver ; the other group of 31 patients was entered into Cell-Saver protocols. The mean number of homologous blood units transfused per patient fell strikingly (p < 0.0001) from 4.2 before to 0.5 after introduction of the Cell- Saver. Of the 31 patients in the Cell-Saver protocol, 71 % required no homologous blood while they received 2.5 units of autologous blood processed by the Cell-Saver. Related to this, the mean number of units prepared by typing and compatibility testing in anticipation of surgery fell from 10 units to five. The projected cost to the patient fell 23%. There were no adverse effects from the use of the Cell-Saver.We conclude that the use of the Cell-Saver is justified not only to reduce the potential risks of homologous blood transfusion, but also to reduce the strain upon blood bank resources and the patient cost of primary coronary revascularization. 100. Bypass of superior vena cava

Abstract

Ten patients had operations for obstruction of the superior vena cava (SVC) with SVC syndrome. Four patients had fibrosing mediastinitis and six had bronchogenic carcinoma. A composite spiral vein graft was placed between the left jugular-subclavian vein and the right atrium to bypass the completely occluded SVC. The graft was constructed from the patient's own saphenous vein, which was split longitudinally and wrapped around a stent in spiral fashion. The edges of the vein were sutured together to form a large autogenous conduit. All patients were immediately relieved of SVC obstructive symptoms and signs. All grafts were patent at 7 days to 18 months, as determined by conventional or radionuclide venography or contrast-enhanced computerized axial tomography. The fact that SVC syndrome has not returned in any patient implies long-term patency. All patients with benign disease continue asymptomatic 3 months to 6 years after operation and have returned to gainful employment. All patients with SVC obstruction resulting from cancer have died 1 to 21 months (mean 10.7 months) postoperatively. Spiral vein bypass graft provides effective treatment for SVC obstruction with immediate and long-term relief of SVC syndrome.
