Cyclic movements, such as walking/cycling, require the activity of spinal-circuits, the central-pattern-generators (CPG). To our knowledge little work has been done to investigate the activation of these circuits, e.g., the muscular and kinematic activity during cycling initiation. This study aims to detail the muscle output properties as a function of the initial lower limb-position using a simple cycling paradigm. Therefore, subjects were required to pedal on a cycle-ergometer in seated position starting at different-crank-angles (0–150°). Surface-electromyography was recorded from the gluteus major (GL), vastus lateralis (VL), and gastrocnemius medialis (GM), while crank position was recorded using a linear-encoder. Gluteus major peak-activity (PA) occurred at 65.0±12.4° when starting with 0° initial crank position (ICP), while occurred maximally at 110.5±2.9 when starting with 70° ICP. Vastus lateralis PA occurred at 40.7±8.8° with 0° ICP, whereas with 70° ICP PA occurred at 103.4±4.0°. Similarly, GM PA occurred at 112.0±10.7° with 0° ICP, whereas with 70° ICP PA occurred at 142.5±4.2° PA. Gluteus major and gastrocnemius medialis showed similar PA phase shifts, which may suggest they are controlled by same local circuitry, in agreement with their common spinal origin, i.e., motoneurons pool in S1–S2.

Deep brain stimulation (DBS) is associated with substantial improvements in motor symptoms of PD. Emerging evidence has suggested that nonlinear measures of complexity may provide greater insight into the efficacy of anti-PD treatments. This study investigated sample entropy and complexity index values in individuals with PD when DBS was OFF compared to ON. Five individuals with PD using DBS performed a four-minute treadmill walking task while 3D kinematics were collected over two periods of 30 s. Participants were tested in the DBS-ON and DBS-OFF conditions. Sample entropy (SE) and complexity index (CI) values were calculated for ankle, knee and hip joint angles. Paired samples t-tests were used to compare mean SE and CI values between the DBS-OFF and DBS-ON conditions, respectively. No differences in SE or CI were observed between the DBS-ON and DBS-OFF conditions at the ankle. At the knee, the DBS-ON was associated with greater SE and CI values than the DBS-OFF condition. At the hip, DBS-ON was associated with greater SE and CI values than the DBS-OFF condition. DBS enhances complexity of movement at the hip and knee joints while complexity at the ankle joint is not significantly altered. Greater complexity of knee and hip joint motion may represent increased adaptability and a greater number of available strategies to complete the gait task.

J Strength Cond Res 32(12): 3425-3431, 2018-Age-related changes in the neuromuscular system underlie reduced performance and injury but may be mitigated through regular physical activity. It was hypothesized that older (OLD) compared with young (YOUNG) adults would exhibit greater vertical and joint stiffness when running at 3.35 m·s. Nine YOUNG and 10 OLD runners performed over ground running trials while three-dimensional biomechanics were recorded. Ankle and knee joint angles, moments and stiffness values were compared between YOUNG & OLD. YOUNG had smaller vertical stiffness (p = 0.01; YOUNG: 32.8 ± 3.6; OLD: 38.1 ± 5.7) and greater joint stiffness than OLD at the ankle (p = 0.04; YOUNG: 0.134 ± 0.021; OLD: 0.118 ± 0.017) and knee (p = 0.01; YOUNG: 0.119 ± 0.016; OLD: 0.098 ± 0.014). YOUNG exhibited greater peak knee flexion angles (p = 0.04; YOUNG: 43.4 ± 6.5°; OLD: 39.1 ± 2.6°), and peak ankle plantarflexion (p = 0.02; YOUNG: -2.8 ± 0.4 Nm·kg; OLD: -2.5 ± 0.1 Nm·kg) and knee extension moments (p < 0.01; 2.6 ± 0.3 Nm·kg; OLD: 2.1 ± 0.2 Nm·kg) than OLD whereas no differences were observed in peak ankle dorsiflexion angles (p = 0.44; YOUNG: 23.6 ± 4.2°; OLD: 23.4 ± 2.1°). The findings of this study suggest that OLD compared with YOUNG adults adopt altered lower extremity biomechanics. These altered running biomechanics by seek to minimize the metabolic cost of running or may be a function of reduced lower extremity strength and power.

Balance is a key component to the assessment of concussion injury; however, the efficacies of clinical tests used are currently under debate. It is questionable whether currently accepted methods of standing balance assessment quantify balance disturbances sufficiently to support decisions on recovery and return to play. Recent evidence of balance abnormalities postconcussion that linger beyond the typical 3- to 5-day recovery period support arguments that currently available standing balance tests are not sensitive enough to determine recovery of function. This article discusses the current clinical tests used in the assessment of concussion in adults and their limitations and the evidence supporting continued balance dysfunction. Implications for the future of balance assessment in concussed adults and recommendations to clinicians for best practices are presented.

Bradykinesia is a cardinal symptom of Parkinson’s disease (PD). Both aging and disease are shown to be associated with decreased adaptability to environmental stresses characterized by reduced complexity (or increased regularity) of biorhythms. The purpose of this study was to investigate the regularity of movement in individuals with PD, and the effect of dopaminergic medication. Nine subjects with PD and eight controls performed wrist flexion/extension movements at maximal velocity and range of motion. Subjects with PD were tested under two medication conditions. Approximate entropy (ApEn) was calculated to assess the regularity of the movement, with the smaller value associated with the greater regularity. Data revealed that subjects with PD had lower ApEn values than controls. Medication did not alter the ApEn values. These findings demonstrate that impaired voluntary movement in individuals with PD is associated with increased regularity of movement and this exaggerated regularity appears less sensitive to anti-PD medication.

Ankle osteoarthritis (OA) compromises ankle joint stability in regards to static balance. Unilateral and bilateral osteoarthritis patients often exhibit different limb-loading patterns during static balance tasks. Symmetrical loading has been posited to improve balance performance. Therefore, the purpose of this study was to quantify balance performance in both ankle osteoarthritis patient groups. Twenty-two unilateral and twenty-one bilateral ankle osteoarthritis patients performed three 10-s quiet standing trials with their feet together. Ground reaction force data were collected from force platforms with one under each foot. Center of pressure excursion in the anteroposterior and mediolateral directions as well as the resultant center of pressure were calculated using custom Matlab software. A 2 × 2 repeated measures ANOVA with Cohen's d were used to analyze the differences between groups (unilateral vs. bilateral) and between limbs (affected vs. unaffected) (α = 0.05). No significant differences were found between limbs or groups in either the anteroposterior or mediolateral direction for any measured variable. Though not statistically different, moderate to large effect sizes were observed for mean resultant distance between unilateral and bilateral (d = 0.096, d: 1.0) as well as anteroposterior excursion (p = 0.077, d: 1.731) and mean velocity (p = 0.084, d: 1.50) between affected and unaffected limbs. These large effect sizes suggested clinically relevant differences may exist, particularly in the anteroposterior direction. These findings may suggest that center of pressure is a better measure of postural strategy while center of mass measures may be more representative of postural steadiness. © 2017 Orthopaedic Research Society. Use of the Nintendo Wii™ as a balance assessment and rehabilitation tool continues to grow. One advantage of the Wii is that games can serve as a virtual reality training tool; however, a disadvantage of the Wii is the human-machine interface and the learning effect over multiple trials. The purpose of this study was to assess changes in postural control during Wii gameplay over a series of trials in novice players.Thirty-one university athletes (aged 18-25 years) completed four trials of the Nintendo Wii Fit™ soccer heading (SH) balance game. Center of pressure (COP) was calculated in the anterior-posterior (AP) and medial-lateral (ML) directions for each 70-second time trial at 1000 Hz. COP was assessed using six linear and two nonlinear measures. Repeated measures analysis of variances compared COP measures over the four trials.Significant differences in COP magnitude and velocity were found between trials 1 and 2 in the ML direction. No significant effects of trial were found in the AP direction. In contrast, a measure of the overall area of COP using an ellipse method revealed a significant reduction to COP area between trials 3 and 4. No significant differences between trials were observed in nonlinear measures.These results demonstrate how magnitude and velocity measures of COP control stabilize after the first trial of Wii SH game play in novice young adults. As Wii rehabilitation focuses on individuals with balance difficulties, an important consideration when using the game as an assessment tool is that more than four trials may be required to capture learning in these populations. In addition, contrasting results from ellipse measurement methods point to the use of multiple measures for robust description of COP behavior. This work provides understanding of normative postural control responses with further research in clinical populations needed.

Evidence suggests that Repetitive Head Impacts (RHI) directly influence the brain over the course of a single contact collision season yet do not significantly impact a player's performance on the standard clinical concussion assessment battery. The purpose of this study was to investigate changes in static postural control after a season of RHI in Division I football athletes using more sensitive measures of postural control as compared to a non-head contact sports. Fourteen Division I football players (CON) (age=20.4±1.12years) and fourteen non-contact athletes (NON) (2 male, 11 female; age=19.85±1.21years) completed a single trial of two minutes of eyes open quiet upright stance on a force platform (1000Hz) prior to athletic participation (PRE) and at the end of the athletic season (POST). All CON athletes wore helmets outfitted with Head Impact Telemetry (HIT) sensors and total number of RHI and linear accelerations forces of each RHI were recorded. Center of pressure root mean square (RMS), peak excursion velocity (PEV), and sample entropy (SampEn) in the anteroposterior (AP) and mediolateral (ML) directions were calculated. CON group experienced 649.5±496.8 mean number of impacts, 27.1±3.0 mean linear accelerations, with ≈1% of total player impacts exceeded 98g over the course of the season. There were no significant interactions for group x time RMS in the AP (p=0.434) and ML (p=0.114) directions, PEV in the AP (p=0.262) and ML (p=0.977) directions, and SampEn in the AP (p=0.499) and ML (p=0.984) directions. In addition, no significant interactions for group were observed for RMS in the AP (p=0.105) and ML (p=0.272) directions, PEV in the AP (p=0.081) and ML (p=0.143) directions, and SampEn in the AP (p=0.583) and ML (p=0.129) directions. These results suggest that over the course of a single competitive season, RHI do not negatively impact postural control even when measured with sensitive non-linear metrics.

Running during sports and for physical activity often requires changes in velocity through acceleration and deceleration. While it is clear that lower extremity biomechanics vary during these accelerations and decelerations, the work requirements of the individual joints are not well understood. The purpose of this investigation was to measure the sagittal plane mechanical work of the individual lower extremity joints during acceleration, deceleration, and steady-state running. Ten runners were compared during acceleration, deceleration, and steady-state running using three-dimensional kinematics and kinetics measures. Total positive and negative joint work, and relative joint contributions to total work were compared between conditions. Total positive work progressively increased from deceleration to acceleration. This was due to greater ankle joint work during acceleration. While there was no significant change in total negative work during deceleration, there was a greater relative contribution of the knee to total negative work with a subsequent lower relative ankle negative work. Each lower extremity joint exhibits distinct functional roles in acceleration compared with deceleration during level running. Deceleration is dominated by greater contributions of the knee to negative work while acceleration is associated with a greater ankle contribution to positive work.

Electromyography is a commonly used method to determine relative effort and neuromuscular drive to skeletal muscle. A limitation of the interpretation of EMG within the literature is the many methods used to determine the intensity of muscle activation. In the current study, ten healthy young adults performed a level walking task while EMG was recorded from the tibialis anterior, medial gastrocnemius and fibularis longus. The EMG data were rectified and smoothed using the root mean squared (RMS). Peak RMS (pRMS), mean RMS (mRMS) and integrated EMG (iEMG) were normalized to the peak value within the subject and were used to determine EMG amplitude. A 3×3 repeated measures analysis of variance was used to determine significant differences between the methods of determining EMG amplitude. The findings of the current study show that pRMS produced significantly lower EMG amplitudes than mRMS or iEMG values. Furthermore, mRMS and iEMG produced nearly identical normalized EMG amplitudes. Based on the findings of this study and the components of each measurement of EMG amplitude, it is suggested to use mRMS to determine EMG amplitude.

To examine lower extremity joint contributions to a landing task in high-(HA) and low-arched (LA) female athletes by quantifying vertical stiffness, joint work and relative joint contributions to landing. Methods Twenty healthy female recreational athletes (10 HA and 10 LA) performed five barefoot drop landings from a height of 30 cm. Three-dimensional kinematics (240 Hz) and ground reaction forces (960 Hz) were recorded simultaneously. Vertical stiffness, joint work values and relative joint work values were calculated using Visual 3D and MatLab. Results HA athletes had significantly greater vertical stiffness compared to LA athletes (p = 0.013). Though no differences in ankle joint work were observed (p = 0.252), HA athletes had smaller magnitudes of knee (p = 0.046), hip (p = 0.019) and total lower extremity joint work values (p = 0.016) compared to LA athletes. HA athletes had greater relative contributions of the ankle (p = 0.032) and smaller relative contributions of the hip (p = 0.049) compared to LA athletes. No differences in relative contributions of the knee were observed (p = 0.255). Conclusions These findings demonstrate that aberrant foot structure is associated with unique contributions of lower extremity joints to load attenuation during landing. These data may provide insight into the unique injury mechanisms associated with arch height in female athletes.

High- (HA) and low-arched athletes (LA) experience distinct injury patterns. These injuries are the result of the interaction of structure and biomechanics. A suggested mechanism of patellofemoral pain pertains to frontal plane knee moments which may be exaggerated in LA athletes. We hypothesize that LA athletes will exhibit greater peak knee abduction moments than high-arched athletes. Methods Twenty healthy female recreational athletes (10 HA and 10 LA) performed five over-ground barefoot walking and five barefoot running trials at a self-selected velocity while three-dimensional kinematics and ground reaction forces were recorded. Peak knee abduction moments and time-to-peak knee abduction moments were calculated using Visual 3D. Results High-arched athletes had smaller peak knee abduction moments compared to low-arched athletes during walking (KAM1: p = 0.019; KAM2: p = 0.015) and running (p = 0.010). No differences were observed in time-to-peak knee abduction moment during walking (KAM1: p = 0.360; KAM2: p = 0.085) or running (p = 0.359). Conclusions These findings demonstrate that foot type is associated with altered frontal plane knee kinetics which may contribute to patellofemoral pain. Future research should address the efficacy of clinical interventions including orthotics and rehabilitation programs in these athletes.

Background Short-leg walking boots are a common intervention for acute and chronic lower extremity injury. Few studies have examined the neuromuscular adaptations associated with short-leg walking boots and no previous study has investigated timing characteristics of muscle activation during gait. The purpose of the current study was to examine the timing and amplitudes of muscle activation of the extrinsic ankle musculature during walking in two types of short-leg walking boots. Methods Eleven healthy young adults performed five level walking trials at a self-selected pace in each of three conditions: normal walking, Gait Walker and Equalizer short-leg walking boots. Ground reaction forces were collected from a force platform while surface electromyography (EMG) was collected from the tibialis anterior, peroneus longus and medial gastrocnemius. EMG signals were rectified and smoothed using the root mean squared with a 20-ms smoothing window and were normalized to the largest mean of the normal walking trials. A repeated measures analysis of variance was used to assess the effect of short-leg walking boots on the onset, duration and amplitude of muscle activation. Results Short-leg walking boots were generally associated with earlier onsets of muscle activation and longer durations of muscle activation. However, there was no reduction in EMG amplitude. Conclusion The findings of this study show that the induced alterations in muscle activation patterns may limit the short-leg walking boots.

Parkinsonian rigidity is characterized by an increased resistance of a joint to externally imposed motion that remains uniform with changing joint angle. Two candidate mechanisms are proposed for the uniformity of rigidity, involving neural-mediated excitation of shortening muscles, i.e., shortening reaction (SR), or inhibition of stretched muscles, i.e., stretch-induced inhibition (SII). To date, no study has addressed the roles of these two phenomena in rigidity. The purpose of this study was to differentiate these two phenomena, and to quantify the potential contribution of each to wrist joint moment in 17 patients with parkinsonian rigidity, in both Off- and On-medication states. Joint position, torque, and EMGs of selected muscles were collected during externally imposed flexion and extension motions. Moments of shortened and stretched muscles were estimated using a biomechanical model. Slopes of the estimated torque–angle curve were calculated for shortened and stretched muscles, separately. A mixed model ANOVA was performed to compare the contribution between the two mechanisms. During flexion, slopes were significantly (P = 0.003) smaller for SR than for SII, whereas during extension, slopes for SII were significantly (P = 0.003) smaller. Results showed that both SR and SII contributed to rigidity. Which mechanism predominates appeared to be associated with the direction of movement. The findings provide new insights into the biomechanical underpinnings of this common symptom in Parkinson’s disease.

This study examined the neuromuscular adaptations of young and old adults in response to a challenge to balance during gait. 12 young (22.2 ± 2.0 yr.) and 10 old adults (78.8 ± 6.3 yr.) performed five level walking trials with and without a challenge to balance which consisted of adding a load bilaterally to the participants' ankles and increasing gait speed (1.5 m/sec. and 1.7 m/sec). Onset, duration, and amplitude of muscle activity in the vastus lateralis, rectus femoris, biceps femoris, and semimembranosus were calculated. Old adults responded to the added load by increasing the activation intensity of their vastus lateralis and semimembranosus disproportionately when compared with young adults. Also, old adults activated their knee flexors earlier and all muscles for a longer period of time than young adults. These findings suggest old adults alter both magnitude of activation and duration of lower extremity musculature in response to increased inertial and velocity demands, and these response characteristics differ from those of young adults.

— Professional cyclists often adopt a competition-start standing posture, which has been shown to improve performance. The biomechanical basis of this is unclear, and might be due to a greater mechanical advantage or increased key muscle activity. Previous observations in steady state cycling showed greater activation of the tibialis anterior, erector spinae , and biceps brachii when adopting a standing vs. seated-riding posture. Little is known regarding the effect of riding posture on activation during a standing start. Eleven cyclists performed standing starts in seated and standing-postures using stationary-cycle and on the track. Electromyography of the gastrocnemius medialis , tibialis anterior, erector spinae , and biceps brachii was recorded during first and subsequent pedal strokes. Results showed that the gastrocnemius medialis did not modify activity. The tibialis anterior, erector spinae, and biceps brachii activity was increased during the standing posture compared to seated, only during the first pedal stroke. These increased activation intensities were accompanied by a corresponding 10% increase in bike speed during the first 5 meters following a standing start in the standing posture compared to the seated one. Adopting a standing posture during a standing start improves performance through greater initial acceleration.Keywords: sEMG, cycling, performance

Background High velocity thrust (HVT) cervical techniques have been associated with serious vertebral artery (VA) trauma. Despite numerous studies, the nature of this association is uncertain. Previous studies have failed to demonstrate haemodynamic effects on the VA in simulated pre-thrust positions. No study has investigated haemodynamic affects during or immediately following HVT, nor sufficiently controlled for the influence of the thrust. Objectives To investigate the immediate effects of HVT of the atlanto-axial joint upon haemodynamics in the sub-occipital portion of the vertebral artery (VA3). Design Randomized Controlled Trial. Method Twenty-three healthy participants (14 women, 9 men; mean age 40, range 27–69 years of age) were randomly assigned to two groups: an intervention group (MANIP, n = 11) received HVT to the atlanto-axial segment whilst a control group (CG, n = 12) was held in the pre-manipulative hold position. Colour-flow Doppler ultrasound was used to measure VA3 haemodynamics. Primary outcome measures were peak systolic (PSV) and end diastolic velocities (EDV) of three cardiac cycles measured at neutral (N1), pre-HVT (PreMH), post-HVT (PostMH), post-HVT-neutral (N2) positions. Results Test-retest reliability for the Doppler measures demonstrated intra-class correlation coefficient (ICC) of 0.99 (95% CI 0.98–1.0) for PSV and 0.91 (95% CI 0.84–0.96) for EDV. Visually, EDV were lower in the MANIP group than in the CONTROL group across the four measurements. However, there were no significantly different changes (at p ≤ 0.01) between the MANIP and CONTROL groups for any measurement variable. Conclusions HVT to the atlanto-axial joint segment does not affect the haemodynamics of the sub-occipital portion of the vertebral artery during or immediately following HVT in healthy subjects.

Context: An estimated 300 000 sport-related concussion injuries occur in the United States annually. Approximately 30% of individuals with concussions experience balance disturbances. Common methods of balance assessment include the Clinical Test of Sensory Organization and Balance (CTSIB), the Sensory Organization Test (SOT), the Balance Error Scoring System (BESS), and the Romberg test; however, the National Collegiate Athletic Association recommended the Wii Fit as an alternative measure of balance in athletes with a concussion. A central concern regarding the implementation of the Wii Fit is whether it is reliable and valid for measuring balance disturbance in athletes with concussion. Objective: To examine the reliability and validity evidence for the CTSIB, SOT, BESS, Romberg test, and Wii Fit for detecting balance disturbance in athletes with a concussion. Data Sources: Literature considered for review included publications with reliability and validity data for the assessments of balance (CTSIB, SOT, BESS, Romberg test, and Wii Fit) from PubMed, PsycINFO, and CINAHL. Data Extraction: We identified 63 relevant articles for consideration in the review. Of the 63 articles, 28 were considered appropriate for inclusion and 35 were excluded. Data Synthesis: No current reliability or validity information supports the use of the CTSIB, SOT, Romberg test, or Wii Fit for balance assessment in athletes with a concussion. The BESS demonstrated moderate to high reliability (interclass correlation coefficient = 0.87) and low to moderate validity (sensitivity = 34%, specificity = 87%). However, the Romberg test and Wii Fit have been shown to be reliable tools in the assessment of balance in Parkinson patients. Conclusions: The BESS can evaluate balance problems after a concussion. However, it lacks the ability to detect balance problems after the third day of recovery. Further investigation is needed to establish the use of the CTSIB, SOT, Romberg test, and Wii Fit for assessing balance in athletes with concussions.

High- (HA) and low-arched (LA) athletes have an exaggerated risk of injury. Ankle joint stiffness is a potential underlying mechanism for the greater rate of injury within these two functionally different groups. An alternative candidate mechanism of injury in HA and LA athletes pertains to the efficacy of the foot as a rigid lever during propulsion. The purpose of this study was to quantify the differences in ankle dynamic joint stiffness, and ankle braking work and ankle propulsive work during stance phase of running. Methods Ten HA and ten LA athletes performed five barefoot running trials while ground reaction forces and three-dimensional kinematics were recorded. Ankle dynamic joint stiffness was calculated as the slope of the ankle joint moment–ankle joint angle plot during load attenuation. Ankle braking and propulsive work values were calculated for the stance phase. Results HA athletes had significantly greater ankle dynamic joint stiffness and significantly smaller ankle net and propulsive work than LA athletes. Conclusions These data demonstrate that HA and LA athletes exhibit unique biomechanical patterns during running. These patterns may be related to lower extremity injury.

Background: Malalignment and dysfunction of the foot have been associated with an increased propensity for overuse and traumatic injury in athletes. Several multisegment foot models have been developed to investigate motions in the foot. However, it remains unknown whether the kinematics measured by different multisegment foot models are equivocal. The purpose of the present study is to examine the efficacy of two multisegment foot models in tracking aberrant foot function. Methods: Ten high-arched and ten low-arched female athletes walked and ran while ground reaction forces and three-dimensional kinematics were tracked using the Leardini and Oxford multisegment foot models. Ground reaction forces and joint angles were calculated with Visual 3D (C-Motion Inc, Germantown, MD). Repeated-measures analyses of variance were used to analyze peak eversion, time to peak eversion, and eversion excursions. Results: The Leardini model was more sensitive to differences in peak eversion angles than the Oxford model. However, the Oxford model detected differences in eversion excursion values that the Leardini model did not detect. Conclusions: Although both models found differences in frontal plane motion between high- and low-arched athletes, the Leardini multisegment foot model is suggested to be more appropriate as it directly tracks frontal plane midfoot motion during dynamic motion. Hamstrings injury is a common occurrence in athletic performance. These injuries tend to occur during a deceleration or landing task suggesting the negative work may be a key component in hamstrings injury. The purpose of this study was to investigate the muscular activity (EMG) of the biceps femoris (BF) in different phases (concentric vs. eccentric) of a Counter Movement Jump (CMJ), Squat Jump (SJ) and the Braking Phase (BP) of a landing task. Twelve female volleyball players performed 5 CMJs, SJs and BPs while surface EMG was recorded using a MuscleLab (BoscoSystemTM, Norway). EMG values were normalized to an maximal voluntary contraction. A repeated measures analysis of variance (ANOVA) was used to compare mean normalized EMG values of the concentric and eccentric portions of the CMJ with the BP and SJ. The ANOVA revealed significantly lower BF activation in the concentric and eccentric portions of the CMJ compared to the BP (64%, p p = 0.02), respectively. These findings suggest that the CMJ relies on a greater contribution of elastic tissues during the concentric and eccentric portions of the movement and thus requires less muscle activation of the BF.

The biomechanical management of bioenergetics of runners when running uphill was investigated. Several metabolic and mechanical variables have been studied simultaneously to spread light on the locomotory strategy operated by humans for effective locomotion. The studied variables were: heart rate, heart rate variability, oxygen intake and blood lactate, metabolic cost, kinematics, ground reaction force and muscular activity. 18 high-level competitive male runners ran at 70% VO2max on different uphill slope conditions: 0%, 2% and 7%. Modifications were significant in almost all variables studied, and were more pronounced with increasing incline. Step frequency/length and ground reaction force are adjusted to cope with both the task of uphill progression and the available (limited) metabolic power. From 0% to 7% slope, step frequency and ground reaction force and metabolic cost increased concurrently by 4%, 12% and 53%, respectively (with a 4% step length decrease as well). It is hypothesised that this biomechanical management is allowed by an environment-body communication performed by means of specific muscular activity.

The medial longitudinal arch plays a major role in determining lower extremity kinematics. Thus, it is necessary to understand the dynamics of the arch structure in response to load. The purpose of this study was to examine arch function in high- and low-arched feet during a vertical loading condition. Ten high- and ten low-arched females performed five trials in a sit-to-stand exercise. Ground reaction force (1200 Hz) and three-dimensional kinematics (240 Hz) were collected simultaneously. The high-and low-arched athletes had no differences in vertical deformation of the arch. High-arched participants were less everted than the low-arched athletes; furthermore, the high-arched athletes had smaller mid-forefoot eversion excursions. Differences between the high-arched and low-arched athletes occur through and motion at the mid-forefoot joint.

Quantify the effects of increased amplitude and rate of muscle stretch on parkinsonian rigidity.Eighteen subjects with Parkinson's disease participated in this study. Subjects' tested hand was passively displaced through 60° and 90° ranges of wrist flexion and extension at velocities of 50°/s and 280°/s in both treated and untreated conditions. Joint angular position, resistance torque, and surface electromyography (EMG) of the wrist flexors and extensors were recorded. Rigidity was quantified by normalized work scores and normalized angular impulses for flexion and extension, separately. Reflex responses of stretched and shortened muscles were quantified by mean EMG and EMG ratio. A series of ANOVAs was performed to determine the effect of amplitude, velocity and medication on selected variables.Both work scores and angular impulses revealed that the larger displacement amplitude and the higher velocity were associated with significantly greater rigidity, increased EMG ratio and mean EMG of stretched muscles. Dopaminergic medication was not associated with a reduction in rigidity.Parkinsonian rigidity is modulated by the amplitude and rate of muscle stretch.These findings shed light on the biomechanical underpinnings and physiological characteristics of rigidity and may inform clinical rigidity assessment in Parkinson's disease.

Increased rigidity with contralateral activation is not due to ipsilateral muscle activation. abstract Objective: Quantify the enhancement of parkinsonian rigidity associated with a contralateral activation maneuver. Methods: Twelve subjects with PD and eight controls participated in the study protocol. Subjects' tested hand was displaced by a servo-motor throughout wrist flexion and extension motions of 60 without and with a concurrent gripping activation in the contralateral hand, referred to as Passive and Active condi- tions, respectively. Subjects with PD were tested in both OFF-MED and ON-MED states. Rigidity was quantified by integrating torque with position during both flexion and extension (torque resistance). ANOVA was performed to assess the effect of contralateral activation on rigidity. Results: PD patients had significantly (0.038) enhanced torque resistance in OFF-MED compared to healthy controls and ON-MED. In the Active condition, differences in torque resistance were magnified (p = 0.002). Medication substantially reduced differences in torque resistance between controls and PD patients in the Passive and Active conditions. Conclusions: A contralateral activation maneuver substantially increases rigidity in patients with PD, spe- cifically the OFF-MED state. Rigidity is reduced with the application of dopaminergic medication, even with the presence of a contralateral activation maneuver. Significance: These data support the use of a contralateral activation maneuver as a tool in the diagnosis of PD.
