Abstract

Chapter 2 of the book "Clinical Adult Neurology," Third Edition, is presented. The second chapter deals with the importance of electroencephalography (EEP) and evoked potential (EP) studies in relation to the neurology practice. The role of EEG in the differential diagnosis of epilepsy is explored. It has been concluded that EEG is critical in the outpatient management of epilepsy, dementias and coma while EP is significant in optic neuritis, hearing loss and acoustic neuroma, among other conditions.

Abstract

Epilepsy is a common disorder that may be caused by wide ranging etiologies. Historically a significant number of people with epilepsy have idiopathic epilepsy without a definitive etiology. More recently, auto-antibodies have been discovered to be associated with numerous conditions affecting the nervous system, including epilepsy. With this increased understanding of the potential for immune-based etiologies for epilepsy it is important to recognize the associated EEG patterns in such cases. 90. Protracted letal Confusion in Elderly Patients.

Abstract

Background: Ictal confusion, particularly if protracted, often presents a diagnostic challenge. Objectives: To define protracted ictal confusion in elderly patients and to characterize its features and outcome. Design: Case series. Setting: Neurology outpatient and emergency departments at 2 tertiary care centers. Patients: Consecutive series of 22 ambulatory patients with acute ictal confusion. Main Outcome Measures: Duration of ictal confusion was correlated with age and lesions noted on cerebral images and videoelectroencephalographic studies. Results: The ictal basis underlying confusion was not recognized for up to 5 days in 22 patients (mean±SD age, 70 ±8.5 years). Twenty patients had partial complex status epilepticus, and 2 patients had newly diagnosed primary generalized status epilepticus. Motor movements were not present in either group, although reduced mood states and ictal neglect were noted in some patients. Fifteen patients had previous episodes (2-10) of protracted ictal confusion. Once identified, treatment reversed confusion, and eventually patients were discharged to home, although a few patients sustained persistent reduction in baseline cognition. Conclusions: Protracted ictal confusion is often not considered in the ambulatory elderly patient, with resulting delay in diagnosis. Electroencephalographic and videoelectroencephalographic studies performed while the patient is experiencing symptoms are crucial to early diagnosis and timely management. [ABSTRACT FROM AUTHOR]

91. The Importance of Cardiac Monitoring in the Epilepsy Monitoring Unit: A Case Presentation of Ictal Asystole

Abstract

ABSTRACT.Ictal asystole may be a potent marker for epilepsy patients at high risk for sudden unexpected death in epilepsy (SUDEP). The use of inpatient long-term video-electroencephalographic (VEEG) monitoring coupled with simultaneous continuous cardiac telemetry is an important tool to detect ictal asystole as well as other significant ictal cardiac arrhythmias. In this paper a case of ictal asystole detected during VEEG is presented. Routine 12-lead EKG was normal upon admission. After antiepileptic medication was tapered, the patient had a typical complex partial seizure with oral automatisms at onset followed by secondary generalization. Ictal onset was noted in left temporal lobe with subsequent spread to the right temporal region. A 20 second period of asystole began just prior to the secondary generalization. During this admission the patient underwent a potentially life-saving pacemaker implantation. The use of cardiac telemetry and baseline EKG are suggested for patients admitted into epilepsy monitoring units as part of the standard epilepsy monitoring protocol.
