PubMedID: 30982817 Objective: Heart failure (HF) is an important health issue of the 21st century and the prevalence in Turkey has been reported as 2.9%. A national profile, frequency data, characteristics of different phenotypes, and risk factors have not yet been well established. The Snapshot Evaluation of Heart Failure Patients in Turkey (SELFIE-TR) was an analysis of a representative sample of HF patients from Turkey. Methods: A total of 23 centers with at least 2 cardiologists from the 12 NUTS-1 regions of Turkey were invited to participate in the research. The contributing centers shared the data of a consecutive enrollment of HF patients, as confirmed by an investigator, on a pre-selected day of each week for the month of October or November of 2015. Results: The mean age of the entire cohort was 63.3±13.3 years (male/female ratio: 751/303, 71.3%/28.7%). There were 712 acute HF patients and 342 chronic HF patients. The total number of HF patients with reduced ejection fraction (HFrEF), heart failure with mid-range ejection fraction, and heart failure with preserved ejection fraction was 801 (75%), 176 (16.7%), and 77 (7.3%), respectively. Objectives: Evidence suggests that lysosomal acid lipase deficiency (LAL-D) is often underdiagnosed because symptoms may be nonspecific. We aimed to investigate the prevalence of LAL-D in children with unexplained liver disease and to identify demographic and clinical features with a prospective, multicenter, cross-sectional study. Methods: Patients (aged 3 months-18 years) who had unexplained transaminase elevation, unexplained hepatomegaly or hepatosplenomegaly, obesity-unrelated liver steatosis, biopsy-proven cryptogenic fibrosis and cirrhosis, or liver transplantation for cryptogenic cirrhosis were enrolled. A Web-based electronic data collection system was used. LAL activity (nmol/punch/h) was measured using the dried blood spot method and classified as LAL-D(0.37). Asecond dried blood spot sample was obtained from patients with intermediate LAL activity for confirmation of the result. Results: A total of 810 children (median age 5.6 years) from 795 families were enrolled. İzole medial kompartman artrozunun cerrahitedavisinde; unikompartmantal diz artroplastisi (UDP), yüksek tibial osteotomi(YTO) ve bazen de total diz artroplastisi (TDP) alternatif yöntemlerdir. Buçalışmada benzer endikasyonları olan YTO ve UDP sonuçları klinik ve yaşamkalitesi açısından karşılaştırmalı olarak değerlendirilmiştir. Temmuz 2003 veŞubat 2010 tarihleri arasında medialkompartman artrozu tanısı ile sirkülereksternalfiksatörle yüksek tibialosteotomi ve unikondiler diz artroplastisiyapılan toplam 47 hastanın 51 dizi çalışmaya alınmıştır. 31 hastanın (2 erkek,29 kadın) 35 dizine sirküler eksternalfiksatörle YTO, 16 hastanın(16 kadın) 16dizine UDP yapıldı. Hastalar; yaş, artroz derecesi, preoperatif ve postoperatifdiz KSS, HSS ve SF-36 skorları açısından karşılaştırıldı. Ameliyat öncesi veameliyat sonrası değerler karşılaştırıldığında SF-36, HSS ve KSS skorlamalarıaçısından anlamlı farklılık saptanmadı. Her iki grubun ameliyat sonrasıskorlarında anlamlı iyileşme elde edilirken, iyileşme miktarlarında iki gruparasında anlamlı fark saptanmadı. UDP grubunda (YTO grubuna göre) SF-36’nınağrı ve emosyonel rol parametrelerinde daha hızlı ve daha fazla iyileşme eldeedildiği görüldü.Klinik ve yaşam kalitesideğerlendirmelerinin sonuçları dikkate alındığında medial kompartman osteoartritinintedavisinde her iki tekniğin de efektif ve memnuniyet verici olduğugörülmüştür., The surgical treatment alternatives for isolatedmedial compartmental knee osteoarthritis are high tibial osteotomy (HTO),unicompartmental knee arthroplasty (UKA) and rarely total knee arthroplasty(TKA). In this study results of UKA and HTO are compared in regard to clinicalresults and quality of life. Between July 2003 and February 2010, 51 knees of 47patients were operated for medial osteoarthritis of the knee. 35 knees of 31patients (2 male and 29 female) underwent high tibial osteotomy using circularexternal fixator and 16 knees of 16 patients (16 female) underwentunicompartmental knee arthroplasty. The groups were compared according to age,degree of arthrosis, preoperative and postoperative HSS, KSS and SF-36 scores. There was no significant diffirence between the HTOand UKA groups in regard to SF-36, HSS and KSS parameters in preoperative andpostoperative periods. Within each group there was significant increase in allparameters in the postoperative period, but there was no diffirence between thetwo groups. In UKA group the increase in pain and emotional role parameters ofSF-36 were more prominent and rapid. In conclusion, in regard to clinical results andquality of life results both treatment modalities were effective andsatisfactory in the treatment of medial compartmental knee osteoarthritis.

Fibroblast-adapted rhesus CMV–vectored vaccines protect macaques from SIV challenge and elicit unconventional CD8 T cell responses. In contrast, Murray et al. show that humans vaccinated with fibroblast-adapted human CMV vaccines generate conventional CD8 T cell responses., Cytomegalovirus (CMV)-based vaccines have shown remarkable efficacy in the rhesus macaque model of acquired immune deficiency syndrome, enabling 50% of vaccinated monkeys to clear a subsequent virulent simian immunodeficiency virus challenge. The protective vaccine elicited unconventional CD8 T cell responses that were entirely restricted by MHC II or the nonclassical MHC I molecule, MHC-E. These unconventional responses were only elicited by a fibroblast-adapted rhesus CMV vector with limited tissue tropism; a repaired vector with normal tropism elicited conventional responses. Testing whether these unusual protective CD8 T responses could be elicited in humans requires vaccinating human subjects with a fibroblast-adapted mutant of human CMV (HCMV). In this study, we describe the CD8 T cell responses of human subjects vaccinated with two fibroblast-adapted HCMV vaccines. Most responses were identified as conventional classically MHC I restricted, and we found no evidence for MHC II or HLA-E restriction. These results indicate that fibroblast adaptation alone is unlikely to explain the unconventional responses observed in macaques.

Methods: Patient data was collected using an Internet-based survey. Total of 588 patients were enrolled from 36 participating medical centers from across the country. Results: Mean age was 62±13 years and 38% of the patients were female. Ratio of de novo AHF to study cohort was 24%. Coronary heart disease and hypertension were found in 61% and 53% of the patients, respectively. Valvular heart disease was underlying cause in 46% of heart failure patients. Most frequent factor associated with decompensation was noncompliance with treatment, observed in 34% of patients. Systolic blood pressure was 125±28 mmHg and heart rate was 93±22 beats/minute in the cohort. Most common fndings on physical examination were inspiratory fne crackles (84%), peripheral edema (64%), and cold extremities in 34%. Mean ejection fraction (EF) measured at admission was 33±13%. Preserved EF (=%40) was present in 20% of patients. At admission, 60%, 46%, and 40% of patients were using angiotensin-converting enzyme inhibitor/angiotensin receptor blocker, beta-blocker, or aldosterone antagonist, respectively. Death, stroke, and myocardial infarction were reported with frequency of 3.4%, 1.6%, and 2%, respectively, as in-hospital events. Conclusion: Compared to some other research data collected around the world, AHF patients in Turkey were younger, more frequently had valvular heart disease as underlying cause, and were more noncompliant with medical treatment, but overall mortality was lower. Galyum-67 (Ga-67) sintigrafisi nadir olmayan sıklıkta diffüz büyük B hücreli lenfoma, akciğer kanseri, malign melanom, hepatoselüler kanser, testis tümörleri, baş-boyun kanserleri gibi onkolojik vakalarda tanı ve tedaviye yanıt amacı ile kullanılan sintigrafik yöntemdir. Ga-67 sintigrafisi onkolojik vakalar dışında sarkoidoz ve bazı enflamatuar hastalıklarda da tanı ve tedaviye yanıtı değerlendirmede kulanılabilmektedir. Bu olgu sunumunda amaç sarkoidoz ön tanısı ile Ga-67 sintigrafisi çekimi için bölümümüze refere edilen hastada sol memede Ga-67 tutulumu izlememiz üzerine yapılan histopatolojik incelemede invaziv duktal karsinom tespit ettiğimizi bildirmektir

Aim: Shock index and Glascow coma scale(GCS) are the parameters used for the follow-up of trauma patient's prognosis. In case of trauma increased reactive oxygen species and oxidative stres induced increase in lipid peroxidation causes deterioration of the cell wall structure, and an increase of cell and tissue damage can be seen. We aimed to investigate the relationship between oxidative stress and paraoxonase, as an antioxidant, and prognosis of the trauma patients. Material and Method: Seventyone trauma patients who were admitted between July - September 2011 were included. Patients were classified according to GCS; Mild (61), moderate (6), and severe (3). Paraoxonase, oxidative stress index (OSI), total antioxidant capacity (TAC) and total oxidative stress (TOS) were compared with the shock index and the GCS. Results: We didn't found any difference between GCS groups and paraoxonase, TAS, TOS, OSI (p> 0.05). Also there wasn't any correlation between shock index and paraoxonase, TAC, TOS, OSI. Aim: The purpose of the present study was to investigate the changes of malondialdehyde (MDA), total antioxidant status (TAS), total oxidant status (TOS) levels; histopathological alterations; morphology, concentration and motilities of the sperm in post ischemic reperfused (I/R) testis tissue. Materials and Methods: Forty adult male Wistar rats were carried out and were randomized to five groups; (1) Control group, (2) Ipsilateral left testis ischemia, (3) Melatonin plus ipsilateral left testis ischemia, (4) Contra lateral right testis ischemia, 5. Melatonin plus contralateral right testis ischemia. After 1 h ischemia and 24 h perfusion; MDA, TAS and TOS levels were measured, histopathological alterations were determined using by Johnsen's score (JS) and sperm morphology, concentration, motility were examined. Results: MDA,TAS and TOS levels of the testis tissue did not change in all groups (p > 0.05 for all). JS was decreased in I/R group and melatonin treatment reversed histopathological changes and increased JS both in ipsilateral and contralateral testis. Abnormal sperm rate significantly increased in I/R group and melatonin administration changed abnormal sperm rate to normal. Conclusions: As a result, the present study demonstrated that testicular damage occurs following I/R without an increase of MDA, TAS and TOS levels. WOS: 000421963600007, PubMed: 25782122, Objectives: Atrial fibrillation (AF) is one of the most common causes of preventable ischemic stroke and is related to increased cardiovascular morbidity and mortality. There is a lack of data in Turkey on the use of new oral anticoagulants (NOACs), and time in therapeutic INR range (TTR) in vitamin K antagonist users and AF management modality. In this multi-center trial, we aimed to analyze, follow and evaluate the epidemiological data in non-valvular AF patients. Study design: Four thousand one hundred consecutive adult patients from 42 centers with at least one AF attack identified on electrocardiography will be included in the study. Patients with rheumatic mitral valve stenosis and prosthetic valve disease will be excluded from the study. At the end of one year, the patients will be evaluated in terms of major cardiac end points (death, transient ischemic attack, stroke, systemic thromboembolism, major bleeding and hospitalization). Results: First results are expected in June 2015. Data about major cardiovascular end-points will be available in January 2016. Conclusion: The rates and kind of oral anticoagulant use, TTR in vitamin K antagonist users and main management modality applied in non-valvular AF patients will be determined by AFTER-2 study. In addition, the rate of major adverse events (MACEs) and the independent predictors of these MACEs will be detected (AFTER-2 Study ClinicalTrials. gov number, NCT02354456.).

PubMed ID: 26574757, Acute heart failure (AHF) is a life threatening clinical syndrome with a progressively increasing incidence in general population. Turkey is a country with a high cardiovascular mortality and recent national statistics show that the population structure has turned to an 'aged' population. As a consequence, AHF has become one of the main reasons of admission to cardiology clinics. This consensus report summarizes clinical and prognostic classification of AHF, its worldwide and national epidemiology, diagnostic work-up, principles of approach in emergency department, intensive care unit and ward, treatment in different clinical scenarios and approach in special conditions and how to plan hospital discharge. © 2015 by Turkish Society of Cardiology.

Acute heart failure syndrome is a heterogeneous group, which requires distinct therapeutic approaches. Diuretics and/or vasodilators for patients with high blood pressure and inotropic agents for patients with low left ventricular ejection fraction and evidence of hypoperfusion are kept in the forefront. Early initiation of therapy along with accurate and early diagnosis in acute heart failure reduces mortality and morbidity significantly. Therefore, it is critically important to develop algorithms, which could easily be followed by all physicians. Bu çözüm yolları içerisinde en dikkat çekici ve sonuca yatkın olan ise arabuluculuk yöntemidir. İnsanlık tarihi ile paralel bir şekilde gelişme gösteren arabuluculuk, neredeyse tüm uygarlıklarda tercih edilen bir yöntem olmuştur. Arabuluculuk uyuşmazlık halindeki iki devletin, üçüncü bir uluslararası hukuk kişisi tarafından bir araya getirilmesi ve barış için çözüm üretilmesi sürecidir. Bu sürecin başarılı bir şekilde sonuçlanmasında arabulucunun tarafsızlığı, statüsü ve taraflar üzerindeki baskı gücü önemli bir rol oynamaktadır. Within these peacfeul ways, most remarkable and liable to result is mediation. As developing parallel with the history of humanity, mediation has been a preffered method in almost all civilizations. Mediation is a process of bringing together and finding solutions to the conflict parties by a third party person. To have a successful conclusion in this process, the impartiality, the status and the leverage of the mediator plays an important role. The third party person as a mediator, may be a state and may also be an international organization or natural person. One of the most important characteristics of mediation is the mediator can not give a decision that is binding on parties. In this regard, mediator only brings the conflict parties together and offers solutions. Mediation way is more preferable than other peaceful solutions in such reasons as the time savings of mediation, being expert persons as a mediator, reducing the burden of the judicial organs and being easier to protect confidentiality.
