Water scarcity is the most significant factor limiting coffee production, although some cultivars can still have important drought tolerance. This study analyzed leaf transcriptomes of two coffee cultivars with contrasting physiological responses, Coffea canephora cv. CL153 and Coffea. arabica cv. Icatu, subjected to moderate (MWD) or severe water deficits (SWD). We found that MWD had a low impact compared with SWD, where 10% of all genes in Icatu and 17% in CL153 reacted to drought, being mainly down‐regulated upon stress. Drought triggered a genotype‐specific response involving the up‐regulation of reticuline oxidase genes in CL153 and heat shock proteins in Icatu. Responsiveness to drought also included desiccation protectant genes, but primarily, aspartic proteases, especially in CL153. A total of 83 Transcription Factors were found engaged in response to drought, mainly up‐regulated, especially under SWD. Together with the enrollment of 49 phosphatases and 272 protein kinases, results suggest the involvement of ABA‐signaling processes in drought acclimation. The integration of these findings with complementing physiological and biochemical studies reveals that both genotypes are more resilient to moderate drought than previously thought and suggests the existence of post‐transcriptional mechanisms modulating the response to drought.

Introduction: Involuntary admission rates differ between gender across various countries. In several European Union countries, men are more frequently involuntarily admitted, while an opposite trend, associating women with involuntary care, has been observed in countries like Switzerland, Brazil, and China. Objectives: Considering the contradictory evidence about gender and involuntary care in the literature, we aim to analyze the gender patterns of involuntary care in Centro Hospitalar Médio Tejo's Psychiatric Acute Unit, exploring the gender differences in diagnosis among involuntary patients. Methods: We stored and analyzed the data using Microsoft Excel and IBM SPSS Statistics. We studied psychiatry admissions at Centro Hospitalar Médio Tejo, Portugal over 2 years. The Acute Psychiatric Unit, located within a general hospital, has 24 beds, and offers acute mental healthcare services to adults aged 18 and above, serving a coverage area of approximately 251,000 residents. As part of our data collection process for all admissions to the Acute Psychiatry Unit, we recorded information such as gender, age, diagnosis at discharge, treatment type (voluntary or involuntary), and length of stay. Results: From January 1, 2021, to December 31, 2022, there were 686 psychiatry admissions at Centro Hospitalar Médio Tejo, of which 125 (18,2%) were involuntary. The admission rates were approximately 136.6 per 100,000 people annually, with 24.9 being involuntary admissions per 100,000 people annually. In our analysis of involuntary admissions, women had a lower rate of such admissions, making up 6.4%, while men had a higher rate at 11.8%. No other gender identity was mentioned. Schizophrenia-related disorders were the primary cause for involuntary admissions for both genders, with 67.9% for men and 50% for women. Mood disorders were the second most common reason for involuntary admission, accounting for around 40.9% of cases for women and a significantly lower 16% for men. Involuntarily hospitalized patients exhibited longer lengths of stay independently of the gender. Men hospitalized involuntarily tended to be younger, whereas for women, involuntary hospitalizations were associated with older ages. Conclusions: In conclusion, our study reveals gender differences in psychiatric involuntary admissions, with more men being involuntarily admitted than women. Schizophrenia group disorders were the most common diagnoses among male and female involuntary patients. Furthermore, all hospitalized women exhibited a higher prevalence of mood disorders, a trend that was more pronounced among those admitted involuntarily. These gender trends match the overall patterns seen in the epidemiology of schizophrenia and mood disorders. Additionally, women with schizophrenia generally exhibit better social functioning than men, which may explain the lower needs of involuntary hospitalization. Background: The incidence of gastroenteropancreatic neuroendocrine neoplasms (GEP-NENs) has been increasing in the last five decades, but there is no large-scale data regarding these tumours in Portugal. We conducted a cross-sectional, multicentric study in main Portuguese centers to evaluate the clinical, pathological, and therapeutic profile of GEP-NENs. Methods: From November, 2012, to July, 2014, data from 293 patients diagnosed with GEP-NENs from 15 centers in Portugal was collected and registered in an online electronic platform. Results: Median age at diagnosis was 56.5 (range: 15-87) years with a preponderance of females (54.6%). The most frequent primary sites were the pancreas (31.1%), jejunum-ileum (24.2%), stomach (13.7%), and rectum (8.5%). Data regarding hormonal status was not available in most patients (82.3%). Stratified by the tumour grade (WHO 2010 classification), we observed 64.0% of NET G1, 24.7% of NET G2, and 11.3% of NEC. Poorly differentiated tumours occurred mainly in older patients (p = 0.017), were larger (p < 0.001), and presented more vascular (p = 0.004) and lymphatic (p = 0.001) invasion. At the time of diagnosis, 44.4% of GEP-NENs presented metastatic disease. Surgery (79.6%) and somatostatin analogues (30.7%) were the most frequently used therapies of GEP-NENs with reported grading. Conclusion: In general, Portuguese patients with GEP-NENs presented similar characteristics to other populations described in the literature. This cross-sectional study represents the first step to establish a national database of GEP-NENs that may aid in understanding the clinical and epidemiological features of these tumours in Portugal. Fly ash and slag are supplementary cementitious materials (SCMs) commonly used to mitigate alkali-silica reaction (ASR). However, future supply of these SCMs is at risk due to a global push to reduce coal-fired energy production and increased steel recycling. Thus, an immediate need to identify alternative SCMs is critical. In order to establish the efficacy of an SCM in its ability to mitigate ASR, an understanding of the chemical processes involved in ASR mitigation is required. This study aims to better understand the mechanisms behind ASR mitigation by comparing the amount of silicon (Si) and aluminium (Al) released by SCMs under AMBT conditions, investigating the interaction of the dissolved SCM species in the system (i.e. formation of reaction products) and how these correlate to explain the differences in SCM dosage requirements for effective mitigation. Results show that the ability of SCMs to release Si is as follows: SF>MK>FA>SL which correlates well with the dosage required to mitigate ASR. This indicates that the efficacy of SCMs in mitigating ASR is primarily due to their ability to release Si in solution. Formation of sodium aluminium silicate hydrate (N-A-S-H) in fly ash and metakaolin and formation of calcium aluminium silicate hydrate (C-A-S-H) in slag post alkali immersion were also observed. This indicates the ability of aluminium to bind silicon and precipate alkali in the process (effectively reducing solution alkali concentration) and highlights its beneficial effect on ASR mitigation. Further, in systems saturated with calcium, Ca is bound instead of Na suggesting the occurence of competitive reactions and subsquent alkali recycling. Calcium, therefore, does not appear to have a beneficial effect on ASR mitigation.

- Abstract
The second law of thermodynamics, through exergy analysis, is commonly applied to quantify process inefficiencies in metallurgical reactors, however, it is not yet being used to understand physical processes and changes in particle-based systems. Correlating the state of mixing of particle texture and homogeneous liquid mixtures is of importance. This paper applies the exergy analysis and excess entropy method to two sets of experiments highlighting the differential breakage as microwave pre-treatment is applied to a gold-copper ore. Grinding kinetic properties were measured following the top-size fraction method and calculated using the population balance model. The approach combines the mixing entropy on the system level (streams) and the entropy for multicomponent particle systems, using automated mineralogy data to quantify the effects of intergrowth and improvements in grinding performance. This is a first step towards understanding mineral processing not only in terms of energy conservation (first law of thermodynamics) but also in terms of the quality of energy available at multicomponent systems (second law of thermodynamics). When applied to comminution processes, this methodology enables us to understand the change in particle composition (its degree of liberation) as well as changes in particle size, being an important measure of process efficiency and selectivity.

In this study, were used residual Açaí seeds as a biosorbent for two textile dyes (Acid Yellow 219 (AY) and Acid Red 361 (AR)), in single and binary solutions. Further, kinetic and isothermal studies were carried to evaluate if the chemical modifications in the Açaí seeds would promote an increase in biosorption. Kinetic assays for all solutions showed that more than 50 % of the dyes were removed in the first 15 minutes. The acid treatment of the biosorbent increased the removal of acid dyes in solution by 46 % and 32 %, respectively. In conclusion, chemically modified Açaí seeds showed high capacity for the removal of textile dyes in single (AY qe = 1.40 mg g-1; AR qe = 1.76 mg g–1) and multi-component solutions (AY MIX qe = 2.09 mg g–1; AR MIX qe = 2.23 mg g–1). These results demonstrate that Açaí seeds can be applied in an inexpensive strategy for removing pollutants (single or binary solutions) from wastewater. The Alkali Silica Reaction (ASR) is a deleterious reaction in concrete that poses significant durability concern worldwide. As a preventative measure, total alkali content of general purpose Portland cements can be limited. In Australia, like in other countries, general purpose cement is limited with a conservative alkali content of 0.6%, this may be unnecessary, as low risk non-reactive aggregates and SCM blends are effective in reducing ASR prevalence. Indeed there is a growing argument to transition to risk assessed methods in choosing cement alkali levels. ASTM in the USA has employed a prescriptive approach to selecting preventative measures that incorporates a variety of cement alkali contents without compromising on safety. Similar balanced alkali approaches such as those recommended in Europe, Canada and New Zealand may be applicable in Australia. Raising alkali limits to a level greater than 0.6% in cement used in conjunction with alternative mitigation techniques would reduce the economic and environmental impact associated with alkali removal during cement production. This literature review discusses the Australian approach to alkali limits in contrast to the methods used around the world and explores the continuing research into alkali’s mechanistic contribution to ASR.

The use of supplementary cementitious materials (SCMs) for mitigating alkali-silica reaction (ASR) is the most common and practical approach adopted by concrete producers since the early 1950s [1]. However, with the future supply of commonly available SCMs such as fly ash and ground granulated blast furnace slag set to decline, alternative materials for mitigating ASR needs to be considered. The objective of this experimental work is to investigate the potential of using ground reactive aggregate fines as SCM substitutes to mitigate ASR. The mechanism of mitigation has been investigated using characterization and expansion tests assessed under AMBT conditions. Mortar bars containing 0%, 10%, 25% and 40% ground reactive aggregate fines by mass of cement replacement were prepared for modified accelerated mortar bar testing. The results obtained indicated that a reduction in ASR expansion was achieved with increasing ground reactive aggregate fines content. Further characterization including XRF and ICP-OES analyses were carried out on ground reactive aggregate fines to understand the efficacy of these materials as potential additives for ASR mitigation.

This study reports on the efficacy of supplementary cementitious materials (SCMs) to mitigate ASR when used in conjunction with cement of higher alkali contents (up to 1%Na2Oeq). The expansion of concrete prisms immersed in simulated pore solution was studied in order to address the limitations of conventional ASR testing methods AMBT and CPT. The alkali concentration in the simulated pore solution was derived from the 28-day age pore solution of pastes with equivalent composition as the concrete binder of interest. The concrete prisms were prepared using Australian sourced materials: 2 types of reactive aggregates (dacite and rhyolite), 2 types of SCMs (fly ash and slag) and cement with 0.6% Na2Oeq original alkali content that was boosted with alkali to obtain 1.0% Na2Oeq. The concrete prisms were stored at 38 °C and at 60 °C. Expansion results show that 25% fly ash and 50% slag are both sufficient to mitigate ASR even with cements with alkali content up to 1.0%Na2Oeq. Extensive amount of ASR products were observed in concretes without SCMs. Small amount of ASR products were also observed in concrete with SCMs, even if there was no notable expansion observed.

Alkali in concrete pore solution and reactive silica in aggregate are integral features required for alkalisilica reaction (ASR). When high amounts of alkali are present, expansive ASR gel forms that cause cracking of concrete. Thus, limits have been imposed, restricting allowable alkali contents for use in concrete. However, these limits are known to be generalised with a single limit specified for all aggregate types. This study investigates the reactivity potential and critical alkali threshold for individual aggregates and aggregate combinations, by increasing alkali content (0.60-1.25% Na2Oe) in concrete, varying exposure temperature (38-80°C) and extending test duration. A combination of RILEM recommended methods and modified versions of the standard accelerated mortar bar test (AMBT) and concrete prism test (CPT) expansion test methods have been used. The key findings of this study suggest that the potential exists for specifying a determined alkali threshold in concrete based on the reactivity classification of aggregates used, thus, allowing a relaxation of the current alkali limit for concrete. This approach permits greater flexibility in the potential safe use of reactive aggregates in concrete. Furthermore, this study shows that the determination of an aggregate’s reactivity and potential to ASR is not only highly dependent on its chemical composition and the alkali content in the concrete but also the test method, exposure temperature and test storage age used to assess changes in expansion.

The second law of thermodynamics, through exergy analysis, is commonly applied to quantify process inefficiencies in metallurgical reactors, however, it is not yet being used to understand physical processes and changes in particle-based systems. Correlating the state of mixing of particle texture and homogeneous liquid mixtures is of importance. This paper applies the exergy analysis and excess entropy method to two sets of experiments highlighting the differential breakage as microwave pre-treatment is applied to a gold-copper ore. Grinding kinetic properties were measured following the top-size fraction method and calculated using the population balance model. The approach combines the mixing entropy on the system level (streams) and the entropy for multicomponent particle systems, using automated mineralogy data to quantify the effects of intergrowth and improvements in grinding performance. This is a first step towards understanding mineral processing not only in terms of energy conservation (first law of thermodynamics) but also in terms of the quality of energy available at multicomponent systems (second law of thermodynamics). When applied to comminution processes, this methodology enables us to understand the change in particle composition (its degree of liberation) as well as changes in particle size, being an important measure of process efficiency and selectivity.

PURPOSE As cancer surgery restarts after the first COVID-19 wave, health care providers urgently require data to determine where elective surgery is best performed. This study aimed to determine whether COVID-19–free surgical pathways were associated with lower postoperative pulmonary complication rates compared with hospitals with no defined pathway. PATIENTS AND METHODS This international, multicenter cohort study included patients who underwent elective surgery for 10 solid cancer types without preoperative suspicion of SARS-CoV-2. Participating hospitals included patients from local emergence of SARS-CoV-2 until April 19, 2020. At the time of surgery, hospitals were defined as having a COVID-19–free surgical pathway (complete segregation of the operating theater, critical care, and inpatient ward areas) or no defined pathway (incomplete or no segregation, areas shared with patients with COVID-19). The primary outcome was 30-day postoperative pulmonary complications (pneumonia, acute respiratory distress syndrome, unexpected ventilation). RESULTS Of 9,171 patients from 447 hospitals in 55 countries, 2,481 were operated on in COVID-19–free surgical pathways. Patients who underwent surgery within COVID-19–free surgical pathways were younger with fewer comorbidities than those in hospitals with no defined pathway but with similar proportions of major surgery. After adjustment, pulmonary complication rates were lower with COVID-19–free surgical pathways (2.2% v 4.9%; adjusted odds ratio [aOR], 0.62; 95% CI, 0.44 to 0.86). This was consistent in sensitivity analyses for low-risk patients (American Society of Anesthesiologists grade 1/2), propensity score–matched models, and patients with negative SARS-CoV-2 preoperative tests. The postoperative SARS-CoV-2 infection rate was also lower in COVID-19–free surgical pathways (2.1% v 3.6%; aOR, 0.53; 95% CI, 0.36 to 0.76). CONCLUSION Within available resources, dedicated COVID-19–free surgical pathways should be established to provide safe elective cancer surgery during current and before future SARS-CoV-2 outbreaks.

Metallurgical simulation and evaluation of the resource efficiency of whole production processes are of key importance for a sound environmental impact assessment. An exergy dissipation analysis is suitable for quantifying the theoretical limits for a process and pinpoint hotspots for improvement along the value chain. The production of NdFeB permanent magnets is evaluated through a simulation-based life cycle assessment and exergetic analysis, comprising 107 unit operations, 361 flows and 209 compounds. This methodology highlights areas with the greatest potential for improvements in terms of technology and environmental impact, shedding light on the true resource efficiency and minimum exergy dissipation for the production of permanent magnets, present in several low-carbon technologies. A maximum exergy efficiency of 60.7% shows that there is a limit for sustainability, which can be improved by technological improvements and recovery of waste streams, showing the inconvenient truth that the resource efficiency will never reach 100%.

- Abstract
Background: Survival outcomes are poor for patients with metastatic urothelial carcinoma who receive standard, first-line, platinum-based chemotherapy. We assessed the overall survival of patients who received durvalumab (a PD-L1 inhibitor), with or without tremelimumab (a CTLA-4 inhibitor), as a first-line treatment for metastatic urothelial carcinoma. Methods: DANUBE is an open-label, randomised, controlled, phase 3 trial in patients with untreated, unresectable, locally advanced or metastatic urothelial carcinoma, conducted at 224 academic research centres, hospitals, and oncology clinics in 23 countries. Eligible patients were aged 18 years or older with an Eastern Cooperative Oncology Group performance status of 0 or 1. We randomly assigned patients (1:1:1) to receive durvalumab monotherapy (1500 mg) administered intravenously every 4 weeks; durvalumab (1500 mg) plus tremelimumab (75 mg) administered intravenously every 4 weeks for up to four doses, followed by durvalumab maintenance (1500 mg) every 4 weeks; or standard-of-care chemotherapy (gemcitabine plus cisplatin or gemcitabine plus carboplatin, depending on cisplatin eligibility) administered intravenously for up to six cycles. Randomisation was done through an interactive voice–web response system, with stratification by cisplatin eligibility, PD-L1 status, and presence or absence of liver metastases, lung metastases, or both. The coprimary endpoints were overall survival compared between the durvalumab monotherapy versus chemotherapy groups in the population of patients with high PD-L1 expression (the high PD-L1 population) and between the durvalumab plus tremelimumab versus chemotherapy groups in the intention-to-treat population (all randomly assigned patients). The study has completed enrolment and the final analysis of overall survival is reported. The trial is registered with ClinicalTrials.gov, NCT02516241, and the EU Clinical Trials Register, EudraCT number 2015-001633-24. Findings: Between Nov 24, 2015, and March 21, 2017, we randomly assigned 1032 patients to receive durvalumab (n=346), durvalumab plus tremelimumab (n=342), or chemotherapy (n=344). At data cutoff (Jan 27, 2020), median follow-up for survival was 41·2 months (IQR 37·9–43·2) for all patients. In the high PD-L1 population, median overall survival was 14·4 months (95% CI 10·4–17·3) in the durvalumab monotherapy group (n=209) versus 12·1 months (10·4–15·0) in the chemotherapy group (n=207; hazard ratio 0·89, 95% CI 0·71–1·11; p=0·30). In the intention-to-treat population, median overall survival was 15·1 months (13·1–18·0) in the durvalumab plus tremelimumab group versus 12·1 months (10·9–14·0) in the chemotherapy group (0·85, 95% CI 0·72–1·02; p=0·075). In the safety population, grade 3 or 4 treatment-related adverse events occurred in 47 (14%) of 345 patients in the durvalumab group, 93 (27%) of 340 patients in the durvalumab plus tremelimumab group, and in 188 (60%) of 313 patients in the chemotherapy group. The most common grade 3 or 4 treatment-related adverse event was increased lipase in the durvalumab group (seven [2%] of 345 patients) and in the durvalumab plus tremelimumab group (16 [5%] of 340 patients), and neutropenia in the chemotherapy group (66 [21%] of 313 patients). Serious treatment-related adverse events occurred in 30 (9%) of 345 patients in the durvalumab group, 78 (23%) of 340 patients in the durvalumab plus tremelimumab group, and 50 (16%) of 313 patients in the chemotherapy group. The incidence of gastroenteropancreatic neuroendocrine neoplasms (GEP-NENs) has been increasing in the last five decades, but there is no large-scale data regarding these tumours in Portugal. We conducted a cross-sectional, multicentric study in main Portuguese centers to evaluate the clinical, pathological, and therapeutic profile of GEP-NENs.From November, 2012, to July, 2014, data from 293 patients diagnosed with GEP-NENs from 15 centers in Portugal was collected and registered in an online electronic platform.Median age at diagnosis was 56.5 (range: 15-87) years with a preponderance of females (54.6%). The most frequent primary sites were the pancreas (31.1%), jejunum-ileum (24.2%), stomach (13.7%), and rectum (8.5%). Data regarding hormonal status was not available in most patients (82.3%). Stratified by the tumour grade (WHO 2010 classification), we observed 64.0% of NET G1, 24.7% of NET G2, and 11.3% of NEC. Poorly differentiated tumours occurred mainly in older patients (In general, Portuguese patients with GEP-NENs presented similar characteristics to other populations described in the literature. This cross-sectional study represents the first step to establish a national database of GEP-NENs that may aid in understanding the clinical and epidemiological features of these tumours in Portugal.

- Abstract
Rare Earth Magnets (REM), especially the NdFeB type, are essential components in high-performance electric motors and wind turbines, playing an important role in the shift towards a low-carbon energy matrix. However, little work has been done to understand how the production of REM can be in line with the global sustainable transition. To overcome this lack and help with future research, as well as decision-making, this paper provides a literature overview of which aspects of sustainability are being investigated in the REM supply chain, and how each of them contributes to achieving Sustainable Development Goals (SDG). This research is developed through a consistent analysis of 44 peer-reviewed publications, followed by an analysis of strengths, weaknesses, opportunities, and threats. Four main subjects of studies were identified: environmental impact; social impact; economic aspects and circular economy. Most of the studies focus on computing the environmental impact through life cycle assessment and discussing techniques towards exploring the circular economy concept. In addition to contributing to a greener economy, the majors identified strengths of REM are the great potential of its supply chain in reducing primary resource extraction, since REM recovery and recycling seem to be viable, and the promising techniques to minimize environmental impacts along the rare earth elements production chain.

Metallurgical simulation and evaluation of the resource efficiency of whole production processes are of key importance for a sound environmental impact assessment. An exergy dissipation analysis is suitable for quantifying the theoretical limits for a process and pinpoint hotspots for improvement along the value chain. The production of NdFeB permanent magnets is evaluated through a simulation-based life cycle assessment and exergetic analysis, comprising 107 unit operations, 361 flows and 209 compounds. This methodology highlights areas with the greatest potential for improvements in terms of technology and environmental impact, shedding light on the true resource efficiency and minimum exergy dissipation for the production of permanent magnets, present in several low-carbon technologies. Introduction: Sustained alcohol intake, when combined with incomplete treatment, can result in chronic structural changes in the Central Nervous System, including generalized cortical and cerebellar atrophy, amnesic syndromes like Korsakoff's syndrome, and white matter disorders such as Central Pontine Myelinolysis and Marchiafava-Bignami syndrome. It is crucial to prevent these complications due to their potential for irreversible and debilitating consequences. For Wernicke-Korsakoff syndrome, early recognition and thiamine administration for prevention are paramount, as it arises from thiamine deficiency due to malnutrition caused by persistent alcohol use. In the case of Central Pontine Myelinolysis, which is caused by abrupt fluctuations in serum osmolality, controlled sodium correction is essential. Objectives: Through a clinical case and a review of published literature, this study aims to reflect on the importance of preventing neurological injuries associated with chronic alcohol consumption, specifically Wernicke-Korsakoff Syndrome and Central Pontine Myelinolysis. Methods: A literature review was conducted by searching for articles on PubMed using the terms "Alcohol Use Disorder," "Wernicke-Korsakoff syndrome," and "Central pontine myelinolysis." A clinical case is presented, featuring a 50-year-old patient with alcohol use disorder who developed Wernicke-Korsakoff syndrome and Central Pontine Myelinolysis. Considering this case, we reflect on the primary approaches that could have been beneficial in preventing these complications and propose a straightforward method for doing so. Results: A 50-year-old patient presented with poor general condition, characterized by low weight, significant loss of strength in the limbs and arms, and incoherent speech with anterograde amnesia and confabulation. This condition had progressed to a point where the patient could no longer walk, perform basic self-care tasks such as bathing, dressing, and eating independently, underscoring the severity of his condition. The diagnoses of Wernicke-Korsakoff syndrome and Central Pontine Myelinolysis were established based on clinical manifestations and the presence of hyperintense lesions observed in the central pons on T2/FLAIR axial MRI scans. This clinical case highlights the importance of proper and precocious prevention of complications in patients with alcohol use disorder. The foremost step in preventing these complications is to treat alcohol dependence effectively, even when faced with patient resistance. It's vital to remain vigilant about potential complications and implement suitable prophylactic measures. Conclusions: The devastating effects of complications arising from Alcohol Use Disorder, such as Wernicke-Korsakoff syndrome and Central Pontine Myelinolysis, underscore the importance of enhanced attention that clinicians should provide when approaching these patients at all clinical interactions.
