Abstract

Extracorporeal membrane oxygenation (ECMO) is a new and still controversial technique in which blood, transported outside the body by tubing, is oxygenated by a machine and then returned to the body. It is used in newborn infants who are in acute respiratory failure and who may die if not treated. As with many new techniques, some experts are convinced of its importance, while others feel that it has not been adequately tested. The Committee on Fetus and Newborn, a panel of the American Academy of Pediatrics, presents in this report recommendations as to which hospital centers should establish and use ECMO. They strongly suggest that ECMO should only be used at regional medical centers that have well-established neonatal (newborn) intensive care programs with the necessary professional and technical services. Such hospitals should have an ongoing research program and an established infant follow-up program. These two aspects would aid the systematic study of the benefits and faults of ECMO. Specific guidelines for use of ECMO in individual patients should be developed by the medical center. Education of regional professionals concerned with care of the newborn should be an established function of the hospital, so that familiarization of these people with appropriate use of ECMO would not be a novel endeavor. Abstract

Seizures, including those associated with fever, are frightening to a family. Consequently, the family often seeks reassurance from their physician, who tends to prescribe medication to prevent the seizures and advise the parents that conditions will improve if the patient complies with drug therapy. However, a 1980 report from the National Institutes of Health concluded that anticonvulsant agents should only be considered for managing seizures in children under specific conditions. These included: abnormal neurologic development; long or focal seizures; recurrence of seizures within 24 hours; history of seizures unrelated to fever in a parent or sibling; or age under one year. However, the conclusions of this 10-year-old report are often not evaluated. It is not clear why children with these risks for developing seizures should be considered for anticonvulsant treatment, because these children are not at risk for death or developing neurologic or intellectual disorders. However, these children do have an increased risk of febrile (fever-related) seizures and epilepsy. One study reported that only the onset of seizures at an early age was associated with an increased risk of recurrent seizures. Febrile seizures tend to be benign even if they last more than 30 minutes. The anticonvulsant phenobarbital was recently shown to be ineffective and unsafe in preventing recurrent febrile seizures, and was associated with decreased intellectual performance. Because febrile seizures tend to be benign and phenobarbital treatment may be deleterious, the treatment of febrile seizures with anticonvulsants is not recommended.
