Abstract

Purpose This study was intended to determine whether a simulation-based education addressed to physicians was able to increase the proportion of hospitalized elderly with atrial fibrillation prescribed with oral anticoagulants (OACs) compared with the usual practice. Methods We conducted a cluster randomized trial (from April 2015 to September 2018) on 32 Italian internal medicine and geriatric wards randomized 1:1 to intervention or control arms. The physicians of wards randomized to intervention received a computer-based e-learning tool with clinical scenarios (Dr Sim), and those of wards randomized to control received no formal educational intervention. The primary outcome was the OAC prescription rate at hospital discharge in the intervention and control arms. Results Of 452 patients scrutinized, 247 were included in the analysis. Of them, 186 (75.3%) were prescribed with OACs at hospital discharge. No difference was found between the intervention and control arms in the post-intervention phase (odds ratio, 1.46; 95% confidence interval [CI], 0.81-2.64). The differences from the pre- to post-intervention phases in the proportions of patients prescribed with OACs (15.1%; 95% CI, 0%-31.5%) and with direct oral anticoagulants (DOACs) (20%; 95% CI, 0%-39.8%) increased more in the intervention than in the control arm. Conclusions This simulation-based course did not succeed in increasing the rate of elderly patients prescribed with OACs at hospital discharge compared with the usual practice. Notwithstanding, over time there was a greater increase in the intervention than in the control arm in the proportion of patients prescribed with OACs and DOACs. Abstract

The "mediastinal syndrome" is an oncological urgency but, in some situations as in the case report described below, it can present itself in a subtle and inextricable way with symptoms and signs that, if singularly evaluated, can be nonspecific. In particular, we report the case of facial edema and a rash that, developing in an unusual and progressive way during about 2 months, concealed a voluminous mediastinal formation due to the localization of non-Hodgkin B-cell lymphoma that caused a "superior caval vein syndrome".

76. Barotrauma during non-invasive ventilation for acute respiratory distress syndrome caused by COVID-19: a balance between risks and benefits.

Abstract

Ventilatory support is vital for the management of severe forms of COVID-19. Non-invasive ventilation is often used in patients who do not meet criteria for intubation or when invasive ventilation is not available, especially in a pandemic when resources are limited. Despite non-invasive ventilation providing effective respiratory support for some forms of acute respiratory failure, data about its effectiveness in patients with viral-related pneumonia are inconclusive. Acute respiratory distress syndrome caused by severe acute respiratory syndrome-coronavirus 2 infection causes life-threatening respiratory failure, weakening the lung parenchyma and increasing the risk of barotrauma. Pulmonary barotrauma results from positive pressure ventilation leading to elevated transalveolar pressure, and in turn to alveolar rupture and leakage of air into the extra-alveolar tissue. This article reviews the literature regarding the use of non-invasive ventilation in patients with acute respiratory failure associated with COVID-19 and other epidemic or pandemic viral infections and the related risk of barotrauma. Abstract

Background: There is uncertainty whether venoarterial extracorporeal membrane oxygenation (VA-ECMO) should be used in older patients with cardiopulmonary failure after cardiac surgery. Methods: This was a retrospective multicenter study of 781 patients who required postcardiotomy VA-ECMO for cardiopulmonary failure after adult cardiac surgery from 2010 to 2018 at 19 cardiac surgery centers. A parallel systematic review with meta-analysis of the literature was performed. Results: The hospital mortality in the overall Postcardiotomy Venoarterial Extracorporeal Membrane Oxygenation (PC-ECMO) series was 64.4%. A total of 255 patients were 70 years old or older (32.7%), and their hospital mortality was significantly higher than in younger patients (76.1% vs 58.7%; adjusted odds ratio, 2.199; 95% confidence interval [CI], 1.536 to 3.149). Arterial lactate level greater than 6 mmol/L before starting VA-ECMO was the only predictor of hospital mortality among patients 70 years old or older in univariate analysis (82.6% vs 70.4%; P = 0.029). Meta-analysis of current and previous studies showed that early mortality after postcardiotomy VA-ECMO was significantly higher in patients aged 70 years or older compared with younger patients (odds ratio, 2.09; 95% CI, 1.59 to 2.75; 5 studies including 1547 patients; I², 5.9%). The pooled early mortality rate among patients aged 70 years or older was 78.8% (95% CI, 74.1 to 83.5; 6 studies including 617 patients; I², 41.8%). Two studies reported 1-year mortality (including hospital mortality) of 79.9% and 75.6%, respectively, in patients 70 years old or older. Conclusions: Advanced age should not be considered a contraindication for postcardiotomy VA-ECMO. However, in view of the high risk of early mortality, meaningful scrutiny is needed before using VA-ECMO after cardiac surgery in older patients.

Abstract

Electrolyzed water (EW) has attracted much recent attention as a high-performance, new technology for its potential use in the food industry. However, the risk of postharvest EW treatments of winegrapes destined for winemaking is the chloroanisoles formation in the final product. In the present study, we investigated the impact of postharvest grape EW and storage treatments on the occurrence of these compounds in wine, 2,4,6-trichloroanisole (TCA) being the main compound responsible for the cork taint off-flavor due to its extremely low perception threshold. The results revealed that the presence of TCA in the wines depended on the concentration of residual free chlorine in the must from the EW treatment. Particularly, TCA was not detected in wine when active chlorine concentrations higher than 0.005 mg/L were present in the must. Chloroanisole and chlorophenol levels in wine were strongly affected not only by EW but also by grape storage conditions (temperature, time, sunlight exposition). The results showed that the 24-hours grape storage at a controlled temperature of 20 °C in the dark, after EW treatment, resulted in the highest TCA concentrations in wines (7.3 ± 2.7 ng/L), while storage and withering in sunlight seemed to mitigate its presence in wine. This could suggest that microbiological formation of chloroanisoles may have been influenced by the storage temperature and germicidal effect of UV light. Biochemical mechanism of chloroanisoles production in grapes and wine is unknown, but the combination of residual free chlorine on the grape surface and the controlled storage conditions in the dark may have positively influenced the bio-formation of chloroanisoles and therefore their presence in wine.

Abstract

OBJECTIVE: the role of microbes and chronic inflammation in the pathogenesis of Alzheimer' disease (AD) has been postulated by many authors. On the other hand, several studies have reported the main role of H. pylori infection and/or GUT microbiota alteration in promoting chronic inflammation, thus possibly influencing both occurrence and evolution of AD. In this article, we analyze the most important and recent studies performed on this field both on humans and animals and provide possible pathogenic explanations. RESULTS: all main and most recent animal, human, epidemiological and in-silico studies, showed a role of H. pylori and/or dysbiosis in AD, mostly through the promotion of systemic chronic inflammation and/or by triggering molecular mimicry mechanisms. In particular, H. pylori infection seems to be related to a poorer cognitive performance. CONCLUSIONS: Indeed, bacteria have been shown to affect neurodegeneration by promoting inflammation, inducing molecular mimicry mechanisms and accumulation of Aß into the brain. These findings open the way for H. pylori eradicating trials and/or GUT microbiota remodulating strategies. Therefore, further studies are now needed in order to test whether antibiotics, pre and/or probiotics may exert a beneficial effect in the prevention of AD.

Abstract

Dengue virus infection, an arthropod-borne virus transmitted by Aedesaegypti mosquitos, has severe health risks in tropical and sub-tropical areas worldwide [1, 2]. Specific test for Dengue infections are rarely performed, limiting the establishment of an appropriate therapy. Many challenges on fighting Dengue infection still need to be addressed, including the development of reliable, label-free and cost-effective diagnosis techniques. In this article, we present a new optical platform to assay dengue virus in the acute phase of infection.

This paper explores how recessions affect industry evolution and competitive dynamics in two-sided markets, breaking through the high entry barriers in place, allowing competitors with a superior price-performance proposition to improve their market performance, and even leapfrog the market leader. It extends traditional perspectives on platform competition that emphasize challenges associated with complementors, ecosystem governance, technology performance, and innovation, including an often forgotten but omnipresent and relevant factor: the macroeconomic context surrounding firms and industries. Abstract

Aims: Although oral anticoagulants (OACs) are effective in preventing stroke in older people with atrial fibrillation (AF), they are often underused in this particularly high-risk population. The aim of the present study was to assess the appropriateness of OAC prescription and its associated factors in hospitalized patients aged 65 years or older. Methods: Data were obtained from the retrospective phase of Simulation-based Technologies to Improve the Appropriate Use of Oral Anticoagulants in Hospitalized Elderly Patients With Atrial Fibrillation (SIM-AF) study, held in 32 Italian internal medicine and geriatric wards. The appropriateness of OAC prescription was assessed, grouping patients in those who were and were not prescribed OACs at hospital discharge. Multivariable logistic regression was used to establish factors independently associated with the appropriateness of OAC prescription. Results: A total of 328 patients were included in the retrospective phase of the study. Of these, almost 44% (N = 143) were inappropriately prescribed OACs, being mainly underprescribed or prescribed an inappropriate antithrombotic drug (N = 88). Among the patients prescribed OACs (N = 221), errors in the prescribed doses were the most frequent cause of inappropriate use (N = 55). Factors associated with a higher degree of patient frailty were inversely associated with the appropriateness of OAC prescription. Conclusions: In hospitalized older patients with AF, there is still a high prevalence of inappropriate OAC prescribing. Characteristics usually related to frailty are associated with the inappropriate prescribing. These findings point to the need for targeted interventions designed for internists and geriatricians, aimed at improving the appropriate prescribing of OACs in this complex and high-risk population.

Abstract

Background & Aims: Diverticular Disease (DD) is a common clinical condition with a dramatic increasing of the prevalence among industrialized countries. Based on the most used classification, DD may be divided into asymptomatic diverticulosis, symptomatic uncomplicated diverticular disease and complicated diverticular disease. Since recent studies pointed out the role of GUT microbiota imbalance in promoting diverticular formation and inflammation, we have designed a systematic review focusing on the possible role of probiotics in the management of this condition. Methods: According to PRISMA, we identified studies on DD patients treated with probiotics, by searching on Pubmed, Embase, Cochrane and ResearchGate. Results: 13 studies were included in this review based on our selection criteria: 3 double-blind randomized placebo-controlled, 6 open randomized, and 4 non-randomized open studies. Conclusion: This is the first systematic review providing an updated measure of evidence on the efficacy of probiotics in a different phase of DD. Even though the majority of studies are still preliminary, current data show a possible clinical application of certain probiotic strains in all stages of DD. Further investigation is then required to better understand when and how probiotics can be used in different phases of DD.

Abstract

Background: The evidence of the benefits of using venoarterial extracorporeal membrane oxygenation (VA-ECMO) after coronary artery bypass grafting (CABG) is scarce. Methods: We analyzed the outcomes of patients who received VA-ECMO therapy due to cardiac or respiratory failure after isolated CABG in 12 centers between 2005 and 2016. Patients treated preoperatively with ECMO were excluded from this study. Results: VA-ECMO was employed in 148 patients after CABG for median of 5.0 days (mean, 6.4, SD 5.6 days). In-hospital mortality was 64.2%. Pooled in-hospital mortality was 65.9% without significant heterogeneity between the centers (I2 8.6%). The proportion of VA-ECMO in each center did not affect in-hospital mortality (p = 0.861). No patients underwent heart transplantation and six patients received a left ventricular assist device. Logistic regression showed that creatinine clearance (p = 0.004, OR 0.98, 95% CI 0.97–0.99), pulmonary disease (p = 0.018, OR 4.42, 95% CI 1.29–15.15) and pre-VA-ECMO blood lactate (p = 0.015, OR 1.10, 95% CI 1.02–1.18) were independent baseline predictors of in-hospital mortality. One-, 2-, and 3-year survival was 31.0%, 27.9%, and 26.1%, respectively. Conclusions: One third of patients with need for VA-ECMO after CABG survive to discharge. In view of the burden of resources associated with VA-ECMO treatment and the limited number of patients surviving to discharge, further studies are needed to identify patients who may benefit the most from this treatment.

99. Metodo innovativo per calcolare le curve di laminazione nelle operazioni di laminazione attraverso la simulazione numerica

Abstract

Il “segreto” di un efficiente processo di produzione di un anello laminato sta nella capacità di laminare ilpezzo mantenendo centrato l’anello e di far crescere il diametro in maniera controllata, evitando ovalità edifetti di forma. Nel processo reale questo lavoro è svolto direttamente dal controllo numerico che agiscesull’avanzamento del mandrino, sulla rotazione dei coni e sul posizionamento dei rulli di centratura. Il tutto èsvolto in modo da seguire le curve di laminazione impostate dall’operatore sul supervisore della macchina.La macchina nel seguire e rispettare le curve impostate monitora il processo e la forma dell’anello attraversoalcuni sistemi di misurazione laser e sensori di carico. Tali dati vengono usati anche per correggere lacinematica degli utensili in funzione dei dati acquisiti in tempo reale. Negli ultimi anni sono stati utilizzatinumerosi metodi per simulare questo processo, sempre con il limite che le curve di laminazione debbanoessere inserite manualmente dall’utilizzatore del software di calcolo e non possano cambiare durantela simulazione. Questo articolo è un riassunto del lavoro fatto da Muraro Spa assieme a Transvalor S.A.,l’autore del software FEM Forge® ed Enginsoft Spa, distributore per l’Italia del software Forge® e centrodi competenza nella simulazione del processo di produzione. Lo scopo di questo lavoro era sviluppare unsoftware in grado di leggere in tempo reale, durante il calcolo, la posizione di alcuni sensori virtuali presentinella simulazione (laser virtuale di misura) e trasmettere informazioni come la posizione degli assi, leforze esercitate dalla macchina al pezzo e altri parametri del processo ad una routine (black-box) esterna.Quest’ultima è in grado di calcolare le correzioni della cinematica di tutti gli assi della macchina, riscriverequeste correzioni direttamente in Forge® e proseguire il calcolo della simulazione numerica. Questo metodopermette, durante la simulazione, di correggere ad ogni step di calcolo la cinematica degli assi impostati nellasimulazione, come realmente succede nelle macchine reali di laminazione degli anelli. La logica della blackboxdi pilotaggio della simulazione è la stessa usata nella macchina reale per pilotare gli assi. La differenzasta nel fatto che nella macchina reale tale software di pilotaggio risieda nel CNC della macchina. Questoapproccio garantisce che i risultati ottenuti nella simulazione siano molto più vicini a quelli reali. Il prossimopasso del lavoro sarà estendere l’applicazione di questa interfaccia ad altri modelli di macchine speciali, maanche a differenti tipi di presse normalmente usate per deformare materiali metallici o non metallici.

Abstract

Extracorporeal membrane oxygenation (ECMO) is an extracorporeal technique of which provides respiratory support oxygen to patients with severe lung disease which no longer responds to conventional intensive therapy, including invasive mechanical ventilation. ECMO in Italy is usually located in general Intensive Care Units (ICUs), and patients are not managed by the specialist Pneumologists. We show the first year results of the experience of the University Hospital of Trieste where ECMO is located in the Cardiac Surgery ICU and there is a multidisciplinary team which includes Pneumologists cooperating to the management of the patients treated with ECMO due to pulmonary diseases or severe pulmonary hypertension. From January 2010, following the pandemic influenza H1N1, were treated 8 patients for refractory acute respiratory failure (2 women and 6 men, 35 years old on average, 3 with H1N1 infection, 2 with cystic fibrosis, 1 with Wegener's disease, 1 with ARDS and 1 with pulmonary hypertension), with severely lungs disease no longer responsive to conventional intensive therapy. Out of these 8 patients in ECMO 4 died, 3 were discharged from hospital and 1 proceeded to successfully pulmonary transplantation. In Italy and in Europe, it is uncommon to find a multidisciplinary team including Pneumologist to treat patients using ECMO due to severely lungs disease. The Pneumology experience of Trieste was made possible thanks to the expertise reached managing patients with severe acute hypoxemic respiratory failure in the Intensive Respiratory Care Unit.
