Abstract

Background: We sought to determine if ripple oscillations (80-120Hz), detected in intracranial EEG (iEEG) recordings of epilepsy patients, correlate with an enhancement or disruption of verbal episodic memory encoding. Methods: We defined ripple and spike events in depth iEEG recordings during list learning in 107 patients with focal epilepsy. We used logistic regression models (LRMs) to investigate the relationship between the occurrence of ripple and spike events during word presentation and the odds of successful word recall following a distractor epoch, and included the seizure onset zone (SOZ) as a covariate in the LRMs. Results: We detected events during 58,312 word presentation trials from 7,630 unique electrode sites. The probability of ripple on spike (RonS) events was increased in the seizure onset zone (SOZ, p<0.04). In the left temporal neocortex RonS events during word presentation corresponded with a decrease in the odds ratio (OR) of successful recall, however this effect only met significance in the SOZ (OR of word recall 0.71, 95% CI: 0.59-0.85, n=158 events, adaptive Hochberg p<0.01). Ripple on oscillation events (RonO) that occurred in the left temporal neocortex non-SOZ also correlated with decreased odds of successful recall (OR 0.52, 95% CI: 0.34-0.80, n=140, adaptive Hochberg , p<0.01). Spikes and RonS that occurred during word presentation in the left middle temporal gyrus during word presentation correlated with the most significant decrease in the odds of successful recall, irrespective of the location of the SOZ (adaptive Hochberg, p<0.01). Conclusion: Ripples and spikes generated in left temporal neocortex are associated with impaired verbal episodic memory encoding.

Its expression, as for some heparan sulfate proteoglycans expression, has been linked to hepatocellular carcinoma (HCC) at mRNA level and immunohistochemistry staining on biopsy samples. This study aims to evaluate the prognostic value of serum levels of SULF2 in patients with alcoholic cirrhosis with or without HCC.Patients and Methods: Two hundred and eighty-seven patients with alcoholic cirrhosis were enrolled in this study: 164 without HCC, 57 with early HCC, and 66 with advanced HCC at inclusion. We analyzed the association between SULF2 serum levels and prognosis using Kaplan–Meier method and univariate and multivariate analysis using a Cox model.Results: Child-Pugh C Patients have higher serum levels of SULF2 than Child-Pugh A patients. Serum levels of SULF2 were also higher in patients with advanced HCC compared with the other groups. In patients with advanced HCC, high serum levels of SULF2 were associated with less favorable overall survival. Combination of SULF2 with Glypican 3 (GPC3) and Syndecan 1 (SDC1) serum levels enhanced the ability to discriminate worst prognostic in advanced HCC.Conclusion: SULF2 along with GPC3 and SDC1 serum levels have been shown to be associated with a prognostic value in advanced HCC.Keywords: sulfatase 2, proteoglycans, prognosis, hepatocellular carcinoma, cirrhosis

55. Imaging and histological features of tumor biopsy sample predict aggressive intrasegmental recurrence of hepatocellular carcinoma after radiofrequency ablation

Abstract

Abstract Aggressive intrasegmental recurrence (AIR) is a form of local recurrence associated with a dismal prognosis and defined by multiple nodules or by an infiltrative mass with a tumor thrombus, occurring in the treated segment, after radiofrequency ablation (RFA) for hepatocellular carcinoma (HCC). We aimed to identify radiological and/or histological characteristics of tumor biopsy predictive of AIR. We retrospectively analyzed patients treated by No-Touch multi-bipolar RFA (mbpRFA) for a first HCC with a systematic per-procedural tumor biopsy positive for diagnosis of HCC. The first recurrence was classified as non-aggressive local recurrence, AIR or intrahepatic distant recurrence. 212 patients were included (168 men; mean age 67.1 years; mean tumor size 28.6 mm, 181 cirrhosis). AIR occurred in 21/212 patients (10%) and was associated with a higher risk of death (57% in patients with AIR vs 30% without AIR, p = 0.0001). Non-smooth tumor margins, observed in 21% of the patients and macro-trabecular massive histological subtype, observed in 12% of the patients were independently related to a higher risk of AIR (HR: 3.7[1.57;9.06], p = 0.002 and HR:3.8[2.47;10], p = 0.005 respectively). Non smooth margins at imaging and macro-trabecular massive histological subtype are associated with AIR and could be considered as aggressive features useful to stratify therapeutic strategy.

Abstract

Background: Understanding the public awareness, attitudes, and practices is needed to successfully control the COVID-19 pandemic. With the second wave of the pandemic spreading and reinstatement of lockdown in many countries, there is an urgent need to understand the knowledge, attitudes, and practices (KAP) of the people (especially the high-risk groups) to control mortality and morbidity. Aims: To explore the KAP of the general population of India and in particular, the at-risk groups, i.e., aged >55 years and those with comorbidities (diabetes, hypertension, chronic kidney disease, ischemic heart disease, and chronic obstructive pulmonary disease) who are at a high risk for developing severe COVID-19 infection and death. Patients and Methods: In this cross-sectional study, the questionnaires were administered through online survey links. The questionnaire had four sections, one for demographics and three each for KAP. The KAP scores were compared for a priori defined questions using Chi-square test, t-test, or ANOVA appropriately. Bonferroni corrected P < 0.05 was considered statistically significant. Results: Of the1259 responses, 83 (6.6%) were above 55 years and 193 (15.3%) had comorbidities. Higher education and income groups were more informed about COVID-19 disease (F > 18.5, P < 0.002). Participants in the at-risk groups had the same level of KAP scores as the general public. While a seemingly higher proportion of the at-risk group followed use-of-mask (n = 206, 94%) and 20 seconds of handwashing (n = 142, 65%), it was not significantly higher compared to the general population (P's > 0.4). Of the 1259 participants, 7%–10% of them did not practice social distancing at all times and 5%–7% were not wearing a mask at all times. Only 538 (42.7%) participants believed that this COVID-19 pandemic will be successfully controlled. Conclusions: This study highlights that at-risk groups are not better prepared compared to the general public, which is the need of the hour to prevent over-crowding of health systems and possibly mitigating COVID-19-related mortality.

Abstract

Purpose: This study aimed at establishing the normative data for the thickness of macular layers on Spectralis Spectral-domain optical coherence tomography (SD-OCT) in healthy Indian eyes and testing the effects of age, gender, central corneal thickness (CCT), and intraocular pressure (IOP) on such values. Methods: This cross-sectional study was done on 308 eyes of 159 healthy subjects. OCT scans were obtained using the posterior pole asymmetry scan protocol. From the thickness map, data were grouped into nine Early Treatment Diabetic Retinopathy Study (ETDRS) macular sectors. Correlation between retinal thickness and age/IOP/CCT was done using Pearson correlation. Correcting for age as a covariate, multivariate regression analysis was done to know which retinal layers showed significant differences in thickness between males and females. Results: The mean age was 46.06 ± 13.06 years (range: 20–75 years). Significant central subfield (CSF) thickening with age was noted in retinal nerve fiber layer (RNFL), inner nuclear layer (IPL), inner nuclear layer (INL), outer plexiform layer (OPL), and outer nuclear layer (ONL) (P < 0.04). The average thickness of the outer ring reduced with age in the ganglion cell layer (GCL)/IPL/INL (P = 0.001). Women had thinner inner and outer retinal thickness than men in all ETDRS rings (P < 0.001). There was no interocular asymmetry (P > 0.05) and no correlation between IOP/CCT and retinal layer thickness. Conclusion: In CSF, age-related thickening was noted in RNFL, IPL, INL, OPL, and ONL. The average inner ring thickness decreased with age in GCL and IPL and increased in the RPE layer. The average outer ring thickness decreased with age in GCL, IPL, and INL layers and increased in OPL. The average IR and OR thickness was significantly less in women compared to men in all sub-fields. There was no correlation between IOP/CCT and retinal layer thickness.

Abstract

Temporal changes in Earth's tectonic style play a crucial role in understanding the planet's evolution. Modern-style tectonics is characterized by the formation of basaltic crust at divergent plate boundaries and its subsequent recycling at subduction zones, accompanied by wedge mantle formation and arc magmatism. It is commonly believed that the secular cooling of the mantle modified the tectonic style from stagnant lid or heat pipe on an early hotter Earth to horizontal tectonics during Meso-Neoarchean. However, various field, petrographic, and geochemical studies suggest that the onset of plate tectonics ranges from Hadean to Neoproterozoic. In this study, we re-evaluate the primary magma temperature (mantle potential temperature, Tp) of the upper ambient mantle, spanning from Eoarchean to Neoproterozoic. We used basalts from several Archean and Proterozoic greenstone belts worldwide, along with Proterozoic ophiolites, to (re)calculate the Tp using the FRACTIONATE-PT method. We observed a Tp range of 1444–1639°C during the Archean and 1414–1611°C during the Proterozoic. These findings indicate a strong correlation with previously estimated Tp values obtained from PRIMELT3 method. This further highlights strong internal consistency among different methods and supports models of a hot ambient mantle during the Archean and Proterozoic. We further reviewed numerical models regarding the effect of mantle temperature on the viability of early Earth plate tectonics. Such model results, composition of Archean continental crust, recent crustal growth models, and field evidence are consistent with the operation of plate tectonics on an early hotter Earth. The transition from a hotter mantle to a colder one from Eo-Neoarchean resulted in thinner oceanic lithosphere and a less depleted lithospheric mantle. Subduction of this thinner oceanic lithosphere led to modern-style tectonics. Abstract

High permittivity SrTiO3 for the realization of all-dielectric metamaterials operating at terahertz frequencies was fabricated. A comparison of different processing routes evidences that Spark Plasma Sintering is the most effective sintering process to yield high density ceramic with high permittivity. We compare this sintering process with two others. The elaborated samples are characterized in the low frequency and in the terahertz frequency ranges. Their relative permittivities are compared with that a reference \sto single crystal. Abstract

Abstract Background In HCV-infected patients with advanced liver disease, the direct antiviral agents-associated clinical benefits remain debated. We compared the clinical outcome of patients with a previous history of decompensated cirrhosis following treatment or not with direct antiviral agents from the French ANRS CO22 HEPATHER cohort. Methods We identified HCV patients who had experienced an episode of decompensated cirrhosis. Study outcomes were all-cause mortality, liver-related or non-liver-related deaths, hepatocellular carcinoma, liver transplantation. Secondary study outcomes were sustained virological response and its clinical benefits. Results 559 patients met the identification criteria, of which 483 received direct antiviral agents and 76 remained untreated after inclusion in the cohort. The median follow-up time was 39.7 (IQR: 22.7–51) months. After adjustment for multivariate analysis, exposure to direct antiviral agents was associated with a decrease in all-cause mortality (HR 0.45, 95% CI 0.24–0.84, p = 0.01) and non-liver-related death (HR 0.26, 95% CI 0.08–0.82, p = 0.02), and was not associated with liver-related death, decrease in hepatocellular carcinoma and need for liver transplantation. The sustained virological response was 88%. According to adjusted multivariable analysis, sustained virological response achievement was associated with a decrease in all-cause mortality (HR 0.29, 95% CI 0.15–0.54, p
