51. Abstract 17162: Prediction of Functional Severity of Coronary Bifurcation Lesions by Means of Angiographically-Derived Score

Abstract

Background: There is uncertainty about relation between clinical and angiographic characteristics and functional significance of bifurcation stenosis. Methods: We analysed patients from FIESTA registry (ClinicalTrials.gov:NCT01724957). Subjects (>18 years) with stable angina, bifurcation lesions with diameter ≥2.5mm and ≤4.5 mm, SB diameter≥2.0 mm were included. We excluded patients with ST-segment elevation myocardial infarction, left main disease, hemodynamic instability. Provisional stenting was the default strategy in all patients. Fractional flow reserve(FFR) was performed using the PrimeWire or PrimeWire Prestige(Volcano Corp., USA). Bifurcation lesion with FFR above 0.80 were deferred from PCI. Results: A total of 171 patients, mean age 67±10 years, 66% males were included in the analysis. Of them 78(46%) had functionally significant bifurcation lesion (FSL) versus 93(54%) with non-significant lesion (nFSL). There were no differences (FSLvs.nFSL) in baseline characteristics: dyslipidemia(88%vs96%), diabetes(44%vs.32%), smoking (52%vs40%), past MI (24%vs15%), previous PCI(54%vs49%), atrial fibrillation (17%vs29%), peripheral artery disease (10%vs9%), renal failure(29%vs31%) –all p>0.05. On logistic-regression analysis independent predictors of functional significance were: SYNTAX score≥11 (OR=5.523,CI:1.666-18.311,p=.005), lesion length≥25mm (OR=21.737,CI:4.963-95.202,p Conclusion: Different degree of stenosis in proximal main vessel and distal side branch, lesion length, SB myocardium territory and SYNTAX score are significant predictors of functional severity of bifurcation lesion. A score based on those parameters was developed with excellent discriminatory ability.

Abstract

This article is dedicated to the creation of a biometric system of the identification by facial image. The conceptual model and the mathematical model based on image processing methods (Gabor and Daubechies wavelet transform) were developed, also steps of image processing in the program and the forming of the feature vector by calculating the image statistical characteristics were described during the analysis. Considering that the primary efficiency outcome of the system is the identification accuracy, which is performed by the system, it became necessary to carry out the experiments to determine these parameters, specifically the methods that are appropriate to use in the system. The results of the experimentation indicated that the system performs the most accurate identification outcome with the use of Daubechies wavelet transform for the image processing, the standard deviation and the variance for the feature vector forming, and Euclidean distance, the squared Euclidean distance or the Canberra distance as a metric of the image classification. Using these parameters, the created system performed the 92,5% accuracy of identification.

Abstract

The phenomenon of diffraction occurs when a wave encounters an obstacle in its path, as a result of which it changes its rectilinear propagation in space. Wave diffraction is described in the literature as: Fresnel diffraction (when the target is close to the receiver) and Fraunhofer diffraction (when the target is far from the receiver). A diffraction model from opaque objects in a near and far observation zone is presented and investigated. The diffraction pattern was interpreted using a Fourier transform. The relationships between the object parameters, the distance from the object to the receiver and the wavelength of the signal are studied. The obtained diffraction model can be used to study the phenomenon of direct signal propagation in bistatic radar systems as well as to detect and evaluate the parameters of moving objects.

Abstract

This article discusses the possibility of reducing the levels of single-phase short circuits in HV networks. Since over 80% of all short circuits are single-phase. The dimensional condition for the equipment in the HV switchgears is a three-phase short circuit, as a result of which the currents of single-phase short circuits are inadmissible to be greater than the currents of three-phase short circuit. Reducing the currents of single-phase short circuit will increase the service life of the circuit breakers.

Abstract

In the recent years the attention to Brain-Computer Interface (BCI) devices and their potential for decoding human brain signals have risen considerably. The achieved results find application in the spheres of medicine, Smart IoT, machinery management, automobiles etc. In this article our team research the impact of additional visual stimulation on the accuracy of the classification of human brain signals. Experimental data is obtained by using Emotiv Epoc 14+. The data is processed in OpenVibe platform. The results of the research show considerable quality improvement in classifier training when additional visual stimulation is introduced. This leads to improved accuracy of incoming signal classification when applied in practice.

Abstract

The purpose of the paper is research and development of font visual classification system, that will allow users to work with fonts more effectively, specifically with the use of the required font fetching on pre-screening, employment of the wide range of filters and font management tools application.There were reviewed basic fonts classification approaches with the definition of pivot advantages and disadvantages. After assessing these approaches, it was decided to deploy neural network with the font symbols images as an input.As a result, neural networks driven computer fonts classification system, that allows to classify fonts by slope, basic style and weight according to their graphical representation, was developed and integrated into the font organizer. High classification accuracy of the developed neural network model was confirmed by control tests. Percentage of correctly classified fonts during the system testing reaches 91% in determining weight, 92% in determining basic style, 96% in determining slope. The developed system can be applied to tasks of fonts classification solution as supplement for digital documents structure determination, and as system of font selection for design creation.

This article presents a 2D visualization of stellar spectra, obtained in Rozhen NAO.The aim is to convert one-dimensional arrays into two-dimensional images with the possibility of adjusting the degree of gray. This allows us to visualize the curves of the radial velocities and to determine their half-amplitudes even more precisely. The results of the observed stars NSVS 254037 and TYC3621-711 are presented.

Abstract

There are a lot of possibilities to control devices remotely nowadays. Doing this with brain waves is not a science fiction anymore. The first steps for device control by brain are already made. The main challenge is identification and classification of the human waves corresponding to the movement, emotions and so on. Brain-Computer Interfaces (BCI) devices provide a connection between the user's brain and the computer. These can be observed in our daily life, such as Medical Applications, Neuroergonomics and Smart Environment, Neuromarketing and Advertising, Education and Self-Regulation, Games and Entertainment, Security and Authentication, etc. To reach the best result a reliable classification of BCI signals is needed. Controlling machines and making science fiction with the power of our mind is a reality today. This study deals with increasing accuracy of the processing and classification of the Brain Wave Signals. This approach is based on pre-determination of the channels with useful information and consequently precise classification of the signals. In this experiment, the BCI device Emotiv 14-Epoc is used for collecting brain signals. They are received from BCI devices. The signals classified are based on the Mixed Linear Programming (MLP). This is an approximation to reach the goal- precise identification of brain signals. The results indicate the necessity of approaches that enhance the characteristics: capabilities and quality. The main results of the presented work show the accuracy of signals identification is improved from 60% to 90% when preliminary determination of the channels is done.

Abstract

The Brain-Computer Interfaces (BCI) is a technology that allows users to submit command to devices by focusing on which action the user wants to do. Using appropriate classification methods, brainwaves data can be used as a digital command to the computer, eliminating the need from classic GUI and commands to control devices. But one of the main problems in the processing of these signals is the preliminary determination of exactly which signals carry useful information containing the necessary commands. The purpose of this study is the show result from one approach to preliminary classification brain wave signals and to distinguish them with great precision when performing activities. In this research, we use an EMOTIV Epoc (14 channel) device. The results of the study show the need for pre-processing of signals coming from BCI devices. This can significantly reduce the amount of data for further processing since we can exclude further processing of data from those channels that do not contain important information. As a result, the processing speed and the percentage of correct recognizable commands increase. © 2019 IEEE.

Abstract

Digital, Background: Two glucagon-like peptide-1 (GLP-1) receptor agonists reduced renal outcomes in people with type 2 diabetes at risk for cardiovascular disease. We assessed the long-term effect of the GLP-1 receptor agonist dulaglutide on renal outcomes in an exploratory analysis of the REWIND trial of the effect of dulaglutide on cardiovascular disease. Methods: REWIND was a multicentre, randomised, double-blind, placebo-controlled trial at 371 sites in 24 countries. Men and women aged at least 50 years with type 2 diabetes who had either a previous cardiovascular event or cardiovascular risk factors were randomly assigned (1:1) to either weekly subcutaneous injection of dulaglutide (1·5 mg) or placebo and followed up at least every 6 months for outcomes. Urinary albumin-to-creatinine ratios (UACRs) and estimated glomerular filtration rates (eGFRs) were estimated from urine and serum values measured in local laboratories every 12 months. The primary outcome (first occurrence of the composite endpoint of non-fatal myocardial infarction, non-fatal stroke, or death from cardiovascular causes), secondary outcomes (including a composite microvascular outcome), and safety outcomes of this trial have been reported elsewhere. In this exploratory analysis, we investigate the renal component of the composite microvascular outcome, defined as the first occurrence of new macroalbuminuria (UACR >33·9 mg/mmol), a sustained decline in eGFR of 30% or more from baseline, or chronic renal replacement therapy. Analyses were by intention to treat. This trial is registered with ClinicalTrials.gov, number NCT01394952. Findings: Between Aug 18, 2011, and Aug 14, 2013, 9901 participants were enrolled and randomly assigned to receive dulaglutide (n=4949) or placebo (n=4952). At baseline, 791 (7·9%) had macroalbuminuria and mean eGFR was 76·9 mL/min per 1·73 m2 (SD 22·7). During a median follow-up of 5·4 years (IQR 5·1-5·9) comprising 51 820 person-years, the renal outcome developed in 848 (17·1%) participants at an incidence rate of 3·5 per 100 person-years in the dulaglutide group and in 970 (19·6%) participants at an incidence rate of 4·1 per 100 person-years in the placebo group (hazard ratio [HR] 0·85, 95% CI 0·77-0·93; p=0·0004). Abstract

Digital, Background Three different glucagon-like peptide-1 (GLP-1) receptor agonists reduce cardiovascular outcomes in people with type 2 diabetes at high cardiovascular risk with high glycated haemoglobin A1c (HbA1c) concentrations. We assessed the effect of the GLP-1 receptor agonist dulaglutide on major adverse cardiovascular events when added to the existing antihyperglycaemic regimens of individuals with type 2 diabetes with and without previous cardiovascular disease and a wide range of glycaemic control. Methods This multicentre, randomised, double-blind, placebo-controlled trial was done at 371 sites in 24 countries. Men and women aged at least 50 years with type 2 diabetes who had either a previous cardiovascular event or cardiovascular risk factors were randomly assigned (1:1) to either weekly subcutaneous injection of dulaglutide (1·5 mg) or placebo. Randomisation was done by a computer-generated random code with stratification by site. All investigators and participants were masked to treatment assignment. Participants were followed up at least every 6 months for incident cardiovascular and other serious clinical outcomes. The primary outcome was the first occurrence of the composite endpoint of non-fatal myocardial infarction, non-fatal stroke, or death from cardiovascular causes (including unknown causes), which was assessed in the intention-to-treat population. This study is registered with ClinicalTrials.gov, number NCT01394952. Findings Between Aug 18, 2011, and Aug 14, 2013, 9901 participants (mean age 66·2 years [SD 6·5], median HbA1c 7·2% [IQR 6·6–8·1], 4589 [46·3%] women) were enrolled and randomly assigned to receive dulaglutide (n=4949) or placebo (n=4952). During a median follow-up of 5·4 years (IQR 5·1–5·9), the primary composite outcome occurred in 594 (12·0%) participants at an incidence rate of 2·4 per 100 person-years in the dulaglutide group and in 663 (13·4%) participants at an incidence rate of 2·7 per 100 person-years in the placebo group (hazard ratio [HR] 0·88, 95% CI 0·79–0·99; p=0·026). Abstract

[Full article is in English]English: This article proposes a novel two-level instrument for analytically robust distinction of typological differences in the area of rule of law (ROL) among the European countries. The first level of analysis uses the EC’s first anti-corruption report data for a cumulative calculation of the country’s rank of anti-corruption potential. This procedure is repeated on a higher level of aggregation, wherein the EC’s anti-corruption ranking is combined with the rankings of the World Press Freedom and Rule of Law Indices. Thus, the authors create a database for the second level quantitative instrument, which is a variation of cluster analysis. The stable clusters obtained do not coincide with conventional blocks of countries that figure in the academic literature.. The instrument for the measurement of the ROL ranking could be even more important for the assessment of the advancement of the candidate-states toward their full compliance with the EU ROL standards.Spanish: Este artículo propone un instrumento doble nivel para un análisis detallado de las diferencias tipológicas del estado de derecho entre países europeos. El primer nivel utiliza el primer informe anti-corrupción de la CE para calcular de manera cumulativa la clasifi cación del potencial anti-corrupción de cada país. Abstract

The publication presents the results of empirical research on web technologies used in the home pages of the Finland commercial banks authorized under Finland Legislation to carry on commercial banking business and supervised by the Finland Financial Supervisory Authority (FIN-FSA). The home pages of 9 commercial banks were studied. Our survey reveals details about the proportion of the web technologies used in the respective web sites. The mean home page size is around 2.5 MB (±3.7 SD, median - 1.3) and the share of the images is around 41%. The share of the other web components is as follows: JavaScript programs - around 35%, CSS files - 10%, Font files - 7%. The size of the main HTML file is only 7% of the size of whole web page. The mean total number of requests that the web browser makes in order to display the home page is around 64 separate requests - 23 requests for images, 19 for JavaScript, 7 for CSS files, 6 for Font files. Only one of the surveyed web sites is using HTTP/2. The rest are using as minimum HTTP 1.1. Near 56% of the sites are using Symantec SSL certificates and again near 56% are using 2 years long certificates.

Abstract

The main task of the “relay” protection system is to provide trip commands in case of faults and to prevent the operation of the electrical equipment in dangerous modes. The set of the protection functions and the system structure have an impact on the reliability and respectively dependability of the protected structure. In some smaller node-point electrical facilities like step-up substations for PVPPs and wind-farms, the set of protection functions required by the respective regulations can be limited. The multifunctional features of new generations of Intelligent Electronic Devices (IEDs) and the options to use IEC 61850 based communication, protection and automation, provide opportunities for improvement of the busbar protection system.

Abstract

This paper analyses data modeling and approaches for finding regularities in large sets of data, as well as their application. The applications of the presented approaches are based on data derived from a relatively large information system (University of Library Studies and Information Technologies, Sofia, Bulgaria). This paper presents models and techniques to determine optimal speed of data processing and define critical points. The results are also analyzed and discussed. At the end of the paper we provide an instance with a large sample of the real observations and all results are illustrated with examples.

Abstract

Novel heavy metal complexes: Sr(5-NH2-phen)4(NO3)(OH)(H2O)2 (1) (synthesized via a static self-assembly process) and Sn(phen)(NO3)(OH)(H2O) (2), Sn(5-NH2-phen)(OH)(Cl)(H2O) (3), Pb(5-NH2-phen)(NO3)2(H2O) (4) (obtained via metal competitive reactions under mild conditions) were reported. The coordination compounds were characterized by elemental analysis, FTIR-spectroscopy and FAB-mass spectrometry. Their cytotoxicity was measured by MTS-test towards human tumour (MDA-MB-231, HT-29, HeLa, HepG2) and non-tumour diploid (Lep-3) cell lines. The most pronounced cytotoxic effect on all cancer lines showed 1 and 4 at their high concentrations as well as 1 at its lower ones (≤ 4×10−2 mg). Therefore, strontium complex of 5-amino-o-phenanthroline (1) exhibited the widest antitumour spectrum activity, having no toxicity to non-tumour cells at quantities ≤ 4×10−2 mg. The computed EC50 values of 1–4 against MDA-MB-231, HT-29, HeLa, HepG2 varied from 1.40×10−3 to 6.31×10−6 M. Towards Lep-3 substances 2–4 showed IC50 7.52×10−4 − 0.44 M. Substance 1 possess EC50=1.26×10−7 M to the non-tumour cells.

Abstract

Synthesis and impact on the tumour growth of palladium(II) complex of 5-amino-1,10-phenanthroline Pd(5-NH 2 -phen) 2 (NO 3 ) 2 and the protonated dimer (phen) 2 (H + )(BF 4 − ) have been described. In the reported experiments a cancerous (100% lethality) myeloid subcutaneous tumour (with Graffi-tumour origin) in hamsters was used. The animals were injected i.p. with different doses of the substances. The longest mean survival time (1.65 times longer than the controls) was achieved when the substance Pd(5-NH 2 -phen) 2 (NO 3 ) 2 was injected into the animals. One of the animals even survived until the 71st day, which is 2.2 times longer than the controls. The compound (phen) 2 (H + )(BF 4 − ) prolonged the mean life-time of the animals 1.4 times in comparison to the controls. On the other hand, the Pd(II) complex of 1,10-phenanthroline, Pd(phen) 2 (H 2 O)(NO 3 ) 2 , did not reveal any antitumour activity. Our experience concerning the effect of other drugs on this tumour has shown a survival time no longer than 4–5 d after the death of the controls.

Abstract

This paper focuses on the overt and tacit role of the state and other stakeholders in defining objectives, concepts and approaches to citizenship education. It suggests a new approach for understanding citizenship education based on the assumption that trans-rational dimensions of human existence – interests, institutional culture, and traditions – are not just external environment, influenced by citizenship education, but they are imbedded in its contents, character and dynamics. This approach is summed up by the metaphor of ‘battle’, since citizenship education is revealed not as a pre-given univocal entity but as a developing matter that results from social combats among different stakeholders. It is argued that there is a common tendency in different countries for the state to take over the citizenship education advocating ‘disciplinary citizenship’. Abstract

We conducted a case–control study to determine the contribution of polymorphisms in CYP2C8 (CYP2C8*3) and CYP2J2 (CYP2J2*7) to increased risk of coronary artery disease and essential hypertension in Bulgarians. The current analysis included 192 unrelated hypertensive patients, 261 patients with angiographically documented CAD (153 with myocardial infarction and 108 without myocardial infarction), and 496 population controls. The CYP2C8*3 and CYP2J2*7 polymorphisms were genotyped by TaqMan SNP Genotyping Assay. PLINK version 1.07 was used for the statistical analysis. No overall association was observed for the studied polymorphisms with coronary artery disease and essential hypertension. The frequency of -50T mutant allele of CYP2J2*7 was significantly higher in male with coronary artery disease without history of myocardial infarction (OR 2.16 95% CI 1.04–4.48 p = 0.035) compared to population control group, but this association did not survive after Bonferroni correction (p adj = 0.07). A significant association of CYP2C8*3 allele with increased risk of essential hypertension has found in men (OR 2.12 95% CI 1.18–3.81 p = 0.015) and this relationship remained significant after adjustment for multiple comparisons (p adj = 0.03). This is the first study showing significant gene-sex interaction for CYP2C8*3 with twofold increase in the relative risk of essential hypertension and a similar tendency for CYP2J2*7 associated with coronary artery disease without myocardial infarction in Bulgarian males. The association is not seen in females and in the whole group of patients. This result could be partly explained by the effect of estrogens on the vascular tone of coronary arteries and CYP2C8 gene expression.

Abstract

Three new complexes of 1,10-Phenanthroline (phen), [Ca(phen)2(H2O)2(NO3)](NO3) (1), [Sr(phen)2(H2O)2(NO3)](NO3) (2) and [Ca(phen)2(H2O)2(NO3)][BF4] (3) were synthesized and characterized by UV, FT IR, Raman, and MS spectra. Their crystal structures were also determined. In 1 and 2 only one of the (NO3)− is coordinated to the central atom. The other (NO3)− is away from the metal ion and was replaced by [BF4]− under the conditions of a competitive reaction. The two phenanthroline molecules also occupy nonequivalent positions showing typical π – π interactions in their crystal packing.

Abstract

The very headline of this presentation hints at least two things. First, what is meant is the history of American sociology, though it is some what awkward to say so outright. Second, the history of American sociology is accomplished, in an impor tant sense, but one should not say that so out‐right, either. In philosophy, as Wittgenstein advised, whereof we can not speak, thereof we must be silent. A different rule reigns in sociology: whereof we can hint at, thereof we must prove. My first task, there fore, is to prove that no matter how embarrassing it may seem one canspeak of a his tory of American sociology after all. My second task is to prove that it has already been on a course of development specifically characteristic of it. And, finally, my last point will be that this specific course is brought to its desired end. Every thing in its own time. Talking about the history of American sociology is extremely risky. But it is the risk that it makes it worth trying. Part of the risk stems from the issue if there is a history of sociology at all, as well as from the issue if there is American sociology in the proper sense of the word. Apart from this, there is the consideration that it may be the American nature proper of that sociology that makes it the least likely to have its own history. First things first.
