Abstract

Introduction: Gastric cancer (GC) is the fourth leading cause of cancer-related death worldwide with limited therapeutic options. The aim of this study was to analyze the value of adding surgery to the first-line treatment in patients with oligometastatic GC (OGC). Methods: This retrospective study included patients with OGC who underwent induction chemotherapy followed by surgery of both primary tumor and synchronous metastasis between April 2012 and April 2022. Endpoints were overall survival (OS) and relapse-free survival (RFS) analyzed by the Kaplan-Meier method. Prognostic factors were assessed with the Cox model. Results: Data from 39 patients were collected. All cases were referred to our multidisciplinary tumor board (MTB) to evaluate the feasibility of radical surgery. After a median follow-up of 33.6 months (mo.), median OS was 26.6 mo. (95% CI 23.8-29.4) and median RFS was 10.6 mo. (95% CI 6.3-14.8). Pathologic response according to the Mandard criteria (TRG 1-3, not reached versus 20.5 mo. for TRG 4-5; HR 0.23, p=0.019), PS ECOG <= 1 (26.7 mo. for PS <= 1 versus 11.2 mo. for PS >1; HR 0.3, p=0.022) and a low metastatic burden (26.7 mo. for single site versus 12.9 mo. for >= 2 sites; HR 0.34, p=0.039) were related to good prognosis. No major intraoperative complications nor surgery-related deaths occurred in our series. Discussion: A sequential strategy of preoperative chemotherapy and radical surgical excision of both primary tumor and metastases was demonstrated to significantly improve OS and RFS. Multidisciplinary evaluation is mandatory to identify patients who could benefit from this strategy.

Abstract

Simple Summary: Pressurized intraperitoneal aerosol chemotherapy (PIPAC) is a new surgical procedure used in the treatment of peritoneal surface malignancies (PSMs). Despite encouraging clinical results, the high costs of PIPAC and the need to repeat the treatment multiple times make the procedure difficult to sustain economically. A retrospective study was conducted in order to evaluate the cost of hospitalization for patients undergoing PIPAC treatment at a high-volume center in Italy. Currently, the Italian National Health System only partially covers the hospitals' costs. Therefore, there is an urgent need for an economical redefinition of this procedure, by creating dedicated reimbursement codes. PIPAC is a new surgical procedure and a viable treatment option for PSM patients, due to promising therapeutic outcomes, minimal invasiveness, limited surgical morbidity, and systemic toxicity side effects. However, its implementation throughout hospitals is hard to obtain due to its fragile economical sustainability. A retrospective health economic analysis was conducted in order to evaluate the cost of hospitalization for patients undergoing PIPAC treatment at Fondazione Policlinico Universitario Agostino Gemelli, IRCCS, in Rome. The average cost of a PIPAC procedure was defined based on the cost of surgery (cost of surgical material, operating room, intraperitoneal chemotherapy), hospital stay, diagnostic examinations, and drugs used during the stay. A total of 493 PIPAC procedures were performed on 222 patients with peritoneal metastases or primary peritoneal cancer from 2017 to 2023. Since the mean remuneration for each PIPAC hospitalization is €5916 and the mean expenditure per hospitalization is €6538, this results in an operating profit per PIPAC hospitalization of −€622. The reimbursement of PIPAC treatment by the Italian National Health System currently only partially covers the hospital's costs. Development of specific codes and adequate reimbursement for PIPAC by recognizing this procedure as a proper treatment for peritoneal carcinomatosis is essential. [ABSTRACT FROM AUTHOR]

Abstract

The emergence of tele-exercise as a response to the impact of technology on physical activity has opened up new possibilities for promoting physical health. By integrating innovative technologies and open-source platforms, tele-exercise encourages people to stay active. In our latest analysis, we delved into the scientific literature surrounding the use of tele-exercise technologies in training healthy individuals. After conducting an extensive search on the PubMed database using the keywords "tele-exercise" and "physical activity" (from 2020 to 2023), we identified 44 clinical trials that were applicable to tele-exercise, but less than 10% of them were aimed at healthy individuals, precisely 9.09% (four out of forty-four studies analyzed). Our review highlights the potential of tele-exercise to help maintain physical fitness and psychological well-being, especially when traditional fitness facilities are not an option. We also underscore the importance of interoperability, standardization, and the incorporation of biomechanics, exercise physiology, and neuroscience into the development of tele-exercise platforms. Nevertheless, despite these promising benefits, research has shown that there is still a significant gap in the knowledge concerning the definition and evaluation of training parameters for healthy individuals. Abstract

Simple Summary: Peritoneal disease in gastric cancer has a poor prognosis, with a median survival of 3–6 months and a 5-year survival rate of 0%. Despite multiple advancements in therapeutics, the National Comprehensive Cancer Network (NCCN) guidelines recommend systemic chemotherapy or best supportive care for GC with peritoneal dissemination. According to several studies, CRS + HIPEC could provide survival advantages in gastric cancer peritoneal metastasis compared to pSC. Background: Peritoneal carcinomatosis is one of deadliest metastatic patterns of gastric cancer, being associated with a median overall survival (OS) of 4 months. Up to now, palliative systemic chemotherapy (pSC) has been the only recommended treatment. The aim of this study is to evaluate a potential survival benefit after CRS + HIPEC compared to pSC. Methods: A systematic review was conducted according to the PRISMA guidelines in March 2024. Manuscripts reporting patients with peritoneal carcinomatosis from gastric cancer treated with CRS + HIPEC were included. A meta-analysis was performed, comparing the survival results between the CRS + HIPEC and pSC groups, and the primary outcome was the comparison in terms of OS. We performed random-effects meta-analysis of odds ratios (ORs). We assessed heterogeneity using the Q2 statistic. Results: Out of the 24 papers included, 1369 patients underwent CRS + HIPEC, with a median OS range of 9.8–28.2 months; and 103 patients underwent pSC, with a median OS range of 4.9–8 months. CRS + HIPEC was associated with significantly increased survival compared to palliative systemic chemotherapy (−1.8954 (95% CI: −2.5761 to −1.2146; p < 0.001). Abstract

Background: Pressurized intraperitoneal aerosol chemotherapy (PIPAC) is a novel intraperitoneal drug delivery method of low-dose chemotherapy as a pressurized aerosol in patients affected by peritoneal cancer of primary or secondary origin. We performed a systematic review and meta-analysis with the aim of assessing the feasibility, safety, and efficacy of PIPAC. Methods: A systematic literature search was performed using Medline and Web of Science databases from 1 January 2011, to inception, to 31 December 2021. Data were independently extracted by two authors. The Newcastle-Ottawa Scale was used to assess the quality and risk of bias of studies. Meta-analysis was performed for pathological response, radiological response, PCI variation along treatment, and for patients undergoing three or more PIPAC. Pooled analyses were performed using the Freeman–Tukey double arcsine transformation, and 95% CIs were calculated using Clopper–Pearson exact CIs in all instances. Results: A total of 414 papers on PIPAC were identified, and 53 studies considering 4719 PIPAC procedure in 1990 patients were included for analysis. The non-access rate or inability to perform PIPAC pooled rate was 4% of the procedures performed. The overall proportion of patients who completed 3 or more cycles of PIPAC was 39%. Severe toxicities considering CTCAE 3–4 were 4% (0% to 38.5%). In total, 50 studies evaluated deaths within the first 30 postoperative days. In the included 1936 patients were registered 26 deaths (1.3%). The pooled analysis of all the studies reporting a pathological response was 68% (95% CI 0.61–0.73), with an acceptable heterogeneity (I2 28.41%, p = 0.09). In total, 10 papers reported data regarding the radiological response, with high heterogeneity and a weighted means of 15% (0% to 77.8%). PCI variation along PIPAC cycles were reported in 14 studies. Abstract

Clinical prediction models for spine surgery applications are on the rise, with an increasing reliance on machine learning (ML) and deep learning (DL). Many of the predicted outcomes are uncommon; therefore, to ensure the models' effectiveness in clinical practice it is crucial to properly evaluate them. This systematic review aims to identify and evaluate current research-based ML and DL models applied for spine surgery, specifically those predicting binary outcomes with a focus on their evaluation metrics. Overall, 60 papers were included, and the findings were reported according to the PRISMA guidelines. A total of 13 papers focused on lengths of stay (LOS), 12 on readmissions, 12 on non-home discharge, 6 on mortality, and 5 on reoperations. The target outcomes exhibited data imbalances ranging from 0.44% to 42.4%. A total of 59 papers reported the model's area under the receiver operating characteristic (AUROC), 28 mentioned accuracies, 33 provided sensitivity, 29 discussed specificity, 28 addressed positive predictive value (PPV), 24 included the negative predictive value (NPV), 25 indicated the Brier score with 10 providing a null model Brier, and 8 detailed the F1 score. Additionally, data visualization varied among the included papers. This review discusses the use of appropriate evaluation schemes in ML and identifies several common errors and potential bias sources in the literature. Embracing these recommendations as the field advances may facilitate the integration of reliable and effective ML models in clinical settings.

Abstract

Malignant mesothelioma is a rare aggressive tumour of the mesothelium with a propensity to spread locally and, rarely, to distant organs. The latest advances in its diagnosis and treatment have led to an increase in unusual disease presentations. Although a direct invasion of the perineum has been previously described in a men, a malignant mesothelioma spreading to the perianal region was never reported in a women. We presented a rare case of malignant mesothelioma recurrence spreading from the peritoneal cavity to the perineum through the rectovaginal space.

Abstract

Despite the recent increase in Virtual Reality (VR) technologies employed for training manufacturing operators on industrial robotic tasks, the impact of VR methods compared to traditional ones is still unclear. This paper presents an experimental comparison of the two training approaches, with novice operators performing the same manufacturing tasks with a VR robot and with a real robot. The hardware selected is an ABB IRB 120 industrial robot, a HTC Vive head mounted display to operate it, besides a corresponding VR model developed in Unity. Twenty-four students performed two actions — drawing and “pick and place” -– in tasks with increasing difficulty, with both the VR model and the real robot. Completion time and task pass rate are adopted to estimate the learning efficiency, while a questionnaire evaluates the users’ satisfaction. The results show that students using VR overall need less elapsed time to complete all tasks, and they record a higher pass rate. The questionnaire answers show that 83% of participants find the VR model helpful in familiarizing with the real robot, and 75% are in favor of using the virtual tool for training novice operators. Users also report that moving the real robot is more complex than the virtual one; adjusting the speed is harder and the possibility of causing damage is worrisome, whereas the VR robot feels safer to operate and easier to drive. The majority of students are satisfied with the design of the tasks, and feel content with the experience. The main finding is that learning from a VR model allows to master driving a real robot quickly and easily. VR training is more useful than conventional methods because it reduces the learning time, allows for training without hindering production, lowers the risk perception, and improves safety for operators and industrial equipment. Abstract

The main aim of this investigation was to determine the effect of high contextual interference (HCI) and low contextual interference (LCI) on motor learning of falling techniques. Thirty-five kinesiology students (21 males and 14 females; mean ± SD, age = 19.4 ± 0.69 years) were randomly assigned to the HCI or LCI practice group. The participants' task was to learn two judo falling techniques on both sides over 3 weeks. The two-way analysis of variance found no difference between LCI and HCI in the performance at the pretest, posttest, retention, and transfer. Both groups improved posttest and retention performance. Finally, differences were found for both groups between the falling performance in the posttest and the application test (except for the right yoko ukemi fall in the HCI). Lower application test scores led to the conclusion that the 3-week treatment was insufficient to reach the application level of the falling techniques. [ABSTRACT FROM AUTHOR]

Abstract

On August 21, 2017, the volcanic island of Ischia was struck by a Mw 3.9 (http://cnt.rm.ingv.it/event/16796811) earthquake which caused two victims and heavy damages on wide portions of the Casamicciola Terme, Lacco Ameno and Forio municipalities. Following the Decree 1/2018 of the Government Commission for the reconstruction of damaged areas of Ischia Island, a Grade 3 Seismic Microzonation study was committed by each municipality to private consultants, who were scientifically supported by a multidisciplinary team of geologists, geophysicists and geotechnical engineers from CNR, INGV and University of Naples "Federico II". The existing data from previous geological and geotechnical investigations were integrated with the results of new geo-lithological surveys and geophysical measurements (HVSR, MASW, Down hole tests) carried out by the consultants. The whole amount of data was quality checked, homogenized and collected in a SQLite/SpatiaLite geodatabase using a QGIS open source software by the support team. A geo-lithological map of northern and western sides of the island was then produced, and the results of routine geophysical tests were coupled with those of seismic 2D ambient vibration arrays, to obtain shear wave velocity profiles (several hundreds of meters deep) down to the seismic bedrock, made of lithic tuffs and lavas, and to define its buried morphology. This in turn enabled to draw a map of seismically homogeneous microzones of the entire study area. Cyclic torsional shear tests were carried out on 10 undisturbed soil samples, mainly taken from epiclastic cover deposits, to characterize their non-linear behavior to be adopted for seismic response analyses. These latter were carried out along 1D profiles characterizing each microzone and along selected 2D cross sections running from the Mount Epomeo to the coast. The dynamic analyses allowed to detect and map areas characterized by variable amplification factors, of spectral accelerations throughout different ranges of periods (0.1-0.5s, 04.-0.8s, 0.7-1.1 s). The zones most prone to site amplification are those corresponding to localized graben-like depressions at the toe of Mount Epomeo, such as Maio neighborhood of Casamicciola, where thickened epiclastic covers are entrenched into blocks of uplifted bedrock and where a combination of 1D stratigraphic amplification and 2D reflection and diffraction effects is expected.
