PubMedID: 26479761 Subacute sclerosing panencephalitis (SSPE) is a chronic infection of the central nervous system caused by the measles virus (MV). Its prevalence remains high in resource poor countries and is likely to increase in the Northern Europe as vaccination rates decrease. Clinical knowledge of this devastating condition, however, is limited. We therefore conducted this multinational survey summarizing experience obtained from more than 500 patients treated by 24 physicians in seven countries. SSPE should be considered in all patients presenting with otherwise unexplained acquired neurological symptoms. In most patients, the diagnosis will be established by the combination of typical clinical symptoms (characteristic repetitive myoclonic jerks), a strong intrathecal synthesis of antibodies to MV and typical electroencephalogram findings (Radermecker complexes). Whereas the therapeutic use of different antiviral (amantadine, ribavirin) and immunomodulatory drugs (isoprinosine, interferons) and of immunoglobulins has been reported repeatedly, optimum application regimen of these drugs has not been established. This is partly due to the absence of common diagnostic and clinical standards focusing on neurological and psychosocial aspects. Carbamazepine, levetiracetam, and clobazam are the drugs most frequently used to control myoclonic jerks. We have established a consensus on essential laboratory and clinical parameters that should facilitate collaborative studies. 62. 36 Component Seismic Data: Investigating Translational and Rotational Components in Exploration Seismology

Abstract

In conventional multicomponent seismic exploration, the wavefield is recorded by measuring translational motions in three directions using three-component sensors. A novel approach in land seismic acquisition is to additionally record the rotational components of the wavefield around the various Cartesian axes and to combine them with translational measurements. Additional rotational measurements provide the opportunity to locally extract valuable information on the propagating wavefield, that either cannot be obtained from conventional translational measurements alone or are challenging to extract. For example, rotational data facilitate wavefield separation, shear-wave (S-wave) imaging, and ground roll suppression because of the direct link between rotation and the S-wave component, but also enable local instantaneous phase velocity estimation. At the Earth’s free surface, rotational motions can be expressed in terms of spatial seismic wavefield gradients. Wavefield gradients can be estimated by differencing the outputs of closely spaced three-component translational sensors. The same approach can be adapted to source arrays: differencing of recordings from closely spaced translational (directed) sources can be used to simulate rotational sources that primarily emit S-waves. The combination of three components of translation and three components of rotation on both the source and the receiver side leads to a total of 36 measurable seismic components. In this thesis, I first verify that array-derived rotational rates, estimated using spatial seismic wavefield gradients, correspond to direct rotation measurements from rotational sensors. Then, I investigate the value of 36-component seismic data using synthetic as well as real field data. I show that rotational components around the vertical axis mainly contain horizontally polarized S-waves. Abstract

Amniotic fluid N-acetyl-beta-D-glucosaminidase (NAG) activity was measured in 21 normal pregnancies to determine reference values, and in 10 pregnancies with fetuses who had prenatally recognised renal and urinary tract malformations. The normal values obtained at 17-19 and 28-31 gestational weeks did not differ, and an activity of 105 units NAG/mmol creatinine was the upper limit of the normal range. Raised NAG activities were found in four fetuses with oligohydramnios and severe bilateral renal disease, and also in two infants with a normal amount of amniotic fluid, only unilateral renal lesions, and a normal renal function after birth. Three of four cases with normal NAG activities had malformations without kidney damage, but one baby with prune belly syndrome was subsequently found to have dysplastic kidneys and renal failure. Our preliminary results suggest that a high level of NAG activity in the amniotic fluid indicates unilateral or bilateral kidney damage but is not proof of intrauterine renal failure.

Abstract

A new method for the ex vivo perfusion of organs from large mammals is described. Gas exchange and dialysis are carried out simultaneously with a low-flux polysulfon dialysis module. The dialysate (e.g. Tyrode solution) is aerated with a mixture of oxygen and carbon dioxide to ensure gas exchange with the blood. Dialysis is carried out in a closed thermostatically controlled system. Monitoring of ultrafiltration is maintained by continuously weighing the blood reservoir and adjusting an afferent and efferent blood pump. Initial results obtained with isolated pig kidneys demonstrate the suitability of the new method for use as a model for the replacement of animal experiments. Theoretically, clinical application in the area of in vivo regional organ perfusion may also be possible.
