OBJETIVO: Comparar a prevalência de gravidez na adolescência e analisar variáveis sociobiológicas relacionadas ao binômio mãe-filho entre duas coortes de mães adolescentes de nascidos vivos em Ribeirão Preto (1978-1979 e 1994). MÉTODOS: Foram entrevistadas mães adolescentes logo após o parto, sendo 943 em 1978/79 e 499 em 1994, abrangendo nascidos vivos de parto único, de famílias residentes em Ribeirão Preto, SP. OBJETIVO: Descrever os procedimentos metodológicos e algumas características demógrafo-sociais e da atenção médica da população estudada em um projeto de pesquisa em saúde perinatal realizado no Município de Ribeirão Preto, SP, Brasil, em 1994, visando a realizar comparações históricas com indicadores de saúde materno-infantil. MATERIAL E MÉTODO: Entre junho de 1978 e maio de 1979 foram estudados todos os nascimentos hospitalares de Ribeirão Preto através de entrevistas com as mães, onde eram obtidos dados sobre hábitos maternos e paternos, situação social da família, atenção médica à gestação e parto, duração da gestação e dados antropométricos e de mortalidade dos recém-nascidos. Em 1994 foi realizado novo estudo com metodologia semelhante, sendo coletados dados de uma amostra de 1/3 dos nascimentos ocorridos no município naquele ano (todos os nascimentos hospitalares observados num período de 4 meses), e também a mortalidade desse grupo até um ano após o encerramento da coleta de dados do nascimento. As entrevistas eram realizadas após o parto, e o peso e comprimento da criança eram aferidos logo após o nascimento. A mortalidade infantil foi avaliada através do levantamento de todos os óbitos de crianças nascidas durante o período do projeto até um ano após seu encerramento. RESULTADOS: Foram analisados 3.663 nascimentos, dos quais 3.579 eram nascimentos únicos; destes, 2.846 eram procedentes do município estudado. Quando comparados com os dados do estudo anterior, verifica-se que houve aumento da freqüência de alguns indicadores como mães adolescentes, prematuridade, baixo peso ao nascer e parto cesáreo; por outro lado, houve diminuição da metade da mortalidade infantil e seus componentes.OBJECTIVE: To describe the methodological procedures and some sociodemographic and medical care characteristics of the population studied in a research project on perinatal health developed in the Ribeirão Preto county, SP, in 1994, in order to make historical comparisons with the indicators of mother-child health of a similar study conducted 15 years before in the same municipality. MATERIAL AND METHOD: All the hospital births that occurred in Ribeirão Preto from June 1978 to May 1979 were studied by interviewing the mothers and obtaining data about maternal and paternal habits, social situation of the family, medical care during pregnancy and delivery, duration of pregnancy, and anthropometric and mortality data about the newborn children. A new study was conducted in 1994 using the same methodology to collect data about a sample of 1/3 of the births that had occurred in the municipality in that year (all the hospital births observed during a period of 4 months), and also about the mortality of this group up to one year after the termination of birth data collection. The interviews were held after delivery and infant weight and length were measured immediately after birth. Infant mortality was evaluated by surveying the data referring to all deaths of infants born during the year of the study and up to 1 year after its termination. The data were obtained from the Municipal Health Secretariat to which copies of all death certificates emitted in the registry offices of the county are sent obligatorily. RESULTS: An investigation was made into 3,663 births, 3,579 of which were singletons; of these, 2,846 were from the municipality. Comparison with the data obtained in the previous study showed an increase in the frequency of some indicators such as adolescent mothers, prematurity, low birth weight and cesarian section; on the other hand, there was a 50% decrease in the mortality rate and its components.

BACKGROUND: Screening for violence during pregnancy is one of the strategies for the prevention of abuse against women. Since violence is difficult to measure, it is necessary to validate questionnaires that can provide a good measure of the phenomenon. The present study analyzed the psychometric properties of the World Health Organization Violence Against Women (WHO VAW) instrument for the measurement of violence against pregnant women. METHODS: Data from the Brazilian Ribeirão Preto and São Luís birth cohort studies (BRISA) were used. The sample consisted of 1,446 pregnant women from São Luís and 1,378 from Ribeirão Preto, interviewed in 2010 and 2011. Thirteen variables were selected from a self-applied questionnaire. Confirmatory factor analysis was used to investigate whether violence is a uni-or-multidimensional construct consisting of psychological, physical and sexual dimensions. The mean-and-variance-adjusted weighted least squares estimator was used. Models were fitted separately for each city and a third model combining data from the two settings was also tested. Models suggested from modification indices were tested to determine whether changes in the WHO VAW model would produce a better fit. RESULTS: The unidimensional model did not show good fit (Root mean square error of approximation [RMSEA] = 0.060, p < 0.001 for the combined model). The multidimensional WHO VAW model showed good fit (RMSEA = 0.036, p = 0.999 for the combined model) and standardized factor loadings higher than 0.70, except for the sexual dimension for SL (0.65). The models suggested by the modification indices with cross loadings measuring simultaneously physical and psychological violence showed a significantly better fit compared to the original WHO model (p < 0.001 for the difference between the model chi-squares). CONCLUSIONS: Violence is a multidimensional second-order construct consisting of psychological, physical and sexual dimensions. The WHO VAW model and the modified models are suitable for measuring violence against pregnant women.

BackgroundCesarean section (CS) has been associated with obesity, measured by body mass index (BMI), in some studies. It has been hypothesized that this association, if causal, might be explained by changes in gut microbiota. However, little is known about whether CS is also associated with increased adiposity as measured by indicators other than BMI.ObjectiveTo assess the association between CS and indicators of peripheral and central adiposity in young adults.MethodsThe study was conducted on 2,063 young adults aged 23 to 25 years from the 1978/79Ribeirão Preto birth cohort, São Paulo, Brazil. CS was the independent variable. The anthropometric indicators of adiposity were: waist circumference (WC), waist-height ratio (WHtR), waist-hip ratio (WHR), tricipital skinfold (TSF), and subscapular skinfold (SSF). The association between CS and indicators of adiposity was investigated using a Poisson model, with robust adjustment of variance and calculation of incidence rate ratio (IRR) with 95% confidence interval (95%CI), and adjustment for birth variables.ResultsFollow-up rate was 31.8%. The CS rate was 32%. Prevalences of increased WC, WHtR, WHR were 32.1%, 33.0% and 15.2%, respectively. After adjustment for birth variables, CS was associated with increased risk of adiposity when compared to vaginal delivery: 1.22 (95%CI 1.07; 1.39) for WC, 1.25 (95%CI 1.10;1.42) for WHtR, 1.45 (95%CI 1.18;1.79) for WHR, 1.36 (95%CI 1.04;1.78) for TSF, and 1.43 (95%CI 1.08;1.91) for SSF.ConclusionSubjects born by CS had a higher risk for increased peripheral and central adiposity during young adult age compared to those born by vaginal delivery. The association of CS with adiposity was consistently observed for all indicators and was robust after adjustment for a variety of early life confounders.

OBJECTIVE: This population-based birth cohort study examined whether normal weight obesity is associated with metabolic disorders in young adults in a middle-income country undergoing rapid nutrition transition. DESIGN AND METHODS: The sample involved 1,222 males and females from the 1978/79 Ribeirão Preto birth cohort, Brazil, aged 23-25 years. NWO was defined as body mass index (BMI) within the normal range (18.5-24.9 kg/m(2)) and the sum of subscapular and triceps skinfolds above the sex-specific 90th percentiles of the study sample. It was also defined as normal BMI and % BF (body fat) >23% in men and >30% in women. Insulin resistance (IR), insulin sensitivity and secretion were based on the Homeostasis Model Assessment (HOMA) model. RESULTS: In logistic models, after adjusting for age, sex and skin colour, NWO was significantly associated with Metabolic Syndrome (MS) according to the Joint Interim Statement (JIS) definition (Odds Ratio OR = 6.83; 95% Confidence Interval CI 2.84-16.47). NWO was also associated with HOMA2-IR (OR = 3.81; 95%CI 1.57-9.28), low insulin sensitivity (OR = 3.89; 95%CI 2.39-6.33), and high insulin secretion (OR = 2.17; 95%CI 1.24-3.80). Significant associations between NWO and some components of the MS were also detected: high waist circumference (OR = 8.46; 95%CI 5.09-14.04), low High Density Lipoprotein cholesterol (OR = 1.65; 95%CI 1.11-2.47) and high triglyceride levels (OR = 1.93; 95%CI 1.02-3.64). Most estimates changed little after further adjustment for early and adult life variables. CONCLUSIONS: NWO was associated with MS and IR, suggesting that clinical assessment of excess body fat in normal-BMI individuals should begin early in life even in middle-income countries.

In developed countries, children with intrauterine growth restriction (IUGR) or born preterm (PT) tend to achieve catch-up growth. There is little information about height catch-up in developing countries and about height catch-down in both developed and developing countries. We studied the effect of IUGR and PT birth on height catch-up and catch-down growth of children from two cohorts of liveborn singletons. Data from 1,463 children was collected at birth and at school age in Ribeirão Preto (RP), a more developed city, and in São Luís (SL), a less developed city. A change in z-score between schoolchild height z-score and birth length z-score ≥ 0.67 was considered catch-up; a change in z-score ≤ -0.67 indicated catch-down growth. The explanatory variables were: appropriate weight for gestational age/PT birth in four categories: term children without IUGR (normal), IUGR only (term with IUGR), PT only (preterm without IUGR) and preterm with IUGR; infant's sex; maternal parity, age, schooling and marital status; occupation of family head; family income and neonatal ponderal index (PI). Estudou-se em Ribeirão Preto, SP, Brasil, no período de 1º de junho de 1978 a 31 de maio de 1979, 98% do universo de nascidos vivos, totalizando 8878 crianças nascidas de parto único, sendo 6750 procedentes de Ribeirão Preto. Observou-se um aumento da proporção de mães adolescentes em Ribeirão Preto (14,1%) quando comparada com estudo realizado 10 anos antes (11,7%). A idade materna menor de 20 anos esteve associada com os indicadores mais desfavoráveis para a saúde perinatal, no que diz respeito à atenção médica à gestação e ao parto. Essa situação ficou mais evidente entre as mães adolescentes de classes sociais menos favorecidas, revelando que o grupo de mães adolescentes não é homogêneo, mas apresenta diferentes proporções de risco para a saúde perinatal de acordo com as frações de classe social que existem no seu interior.Ninety-eight percent of all live births occuring in the city of Ribeirão Preto, state of São Paulo (Brazil), from June 1, 1978 to May 30, 1979 were studied, for a total of 8,878 single-delivery infants, 6,750 of whom were from this town. An increase in the proportion of teenage mothers in Ribeirão Preto (14.1%) was observed in relation to a study carried out ten years before (11.7%). Maternal age of less than 20 years was associated with more unfavorable perinatal health indicators with respect to medical care and delivery. This situation was more evident among teenage mothers from underprivileged social classes, showing that the group of teenage mothers is not homogeneous but presents different risk rates for perinatal health according to the social strata of which the group is composed.

Foi realizado inquérito em Ribeirão Preto, Brasil, de junho de 1978 a maio de 1979, que constou de entrevistas com mães de nascidos vivos hospitalares, de partos únicos, correspondendo a 98 % dos nascimentos ocorridos durante o período no Município. O maior percentual de nascimentos com baixo peso ocorreu entre as mães jovens, fumantes e pertencentes às classes trabalhadoras. O maior número de mães jovens foi encontrado também nessas classes. A prevalência do hábito de fumar foi maior nas mães jovens, porém houve pouca diferença na freqüência de mulheres fumantes entre as classes sociais, variando de 25 a 30%. Observou-se que o pencentual de baixo peso foi maior para as mães trabalhadoras não fumantes do que para as mães burguesas fumantes. O modelo logito foi ajustado aos dados para estudo da possível associação múltipla entre hábito de fumar, idade materna e classe social com o peso ao nascer. Os resultados indicaram que hábito de fumar materno, idade materna e classe social foram independentes no seu efeito sobre o peso ao nascer, não se observando interação. Esses achados sugerem que a maior prevalência de baixo peso entre mães não fumantes das classes trabalhadoras em relação às mães fumantes da burguesia provavelmente refletiu a concentração de outros fatores de risco entre as mulheres das classes trabalhadoras, tais como: baixa escolaridade, cuidado pré-natal inadequado, alta paridade, diferenças no comportamento reprodutivo.An epidemiological survey was carried out in Ribeirão Preto, Brazil, from June 1978 to May 1979. Interviews were held with mother of singleton live borne children, delivered in hospitals, which accounted for 98% of all births in the area. The higher percentages of low birthweight children related to the offspring of smokers, young mother and women belonging to the working class. The majority of young women were found in the working class and the prevalence of smoking was higher in the group of women below 20 years of age. There was no statistical difference in the smoking habit as between different social classes. A larger number of low birthweight children were observed in nonsmoking women of the working class than among women smokers of the middle class. A log model was adjusted to the data in order to study the possible multiple association of smoking, maternal age and social class with birthweight. The results indicated that maternal smoking, maternal age and social class had independent effects on birthweight. The was no interaction between them. These findings suggest that the higher prevalence of low birthweight in nonsmoking mothers of the working class in relation to smoking mothers of the middle class probably reflects clustering of other risk factors-such as poor education inadequate prenatal care, high parity and differences in reproductive behavior in women of the working class.

Foi realizado um estudo sobre mortalidade neonatal e pós-neonatal e mortalidade no lº ano de vida de uma coorte de crianças nascidas vivas, de partos hospitalares em Ribeirão Preto, São Paulo, Brasil, cujas mães foram entrevistadas por ocasião do nascimento. Relacionou-se mortalidade com classe social e peso ao nascer. A distribuição desigual da mortalidade mostrou que as classes sociais se comportam como populações distintas, e o peso ao nascer não foi a causa isolada dessa desigualdade. Às classes sociais mais baixas corresponderam maiores taxas de baixo peso ao nascer e coeficientes de mortalidade mais elevados. O estudo da mortalidade infantil, sem levar em conta a classe social, pode mostrar coeficientes que subestimam a magnitude do problema nas camadas mais pobres da população.A study of infant mortality and its components was carried out in a cohort of live born children from hospitals of Ribeirão Preto, city of São Paulo, Brazil, whose mothers were interviewed shortly after deliverance. Mortality was associated to social classes and birthweight. The uneven distribution of mortality showed that social classes acted as distinct populations and that birthweight was not the only cause of this unballance. Lower social classes were associated with higher low birthweight rates and mortality coefficients. The study of infant mortality without taking into account the social differences may show coefficients that underestimate the magnitude of the problem among the poorest population segments.

Estudou-se, em Ribeirão Preto, SP, Brasil, no período de 1º de junho de 1978 a 31 de maio de 1979, 98% do universo de nascidos vivos, totalizando 8.878 crianças nascidas de parto único. As taxas de baixo peso e de peso deficiente ao nascer foram de 7,5% e de 21,1%, respectivamente. A maioria dos nascimentos de baixo peso eram crianças a termo, 50,6%, representando casos de retardo de crescimento intra-uterino. A prevalência de baixo peso nas classes sociais foi de 3,2% na burguesia empresarial, 2,8% na burguesia gerencial, 3,9% na pequena burguesia, 7,0% no proletariado e 9,5% no subproletariado, o que demonstra uma profunda disparidade. Definindo-se o baixo peso como menor ou igual a 2.500 gramas e comparando os resultados alcançados (8,3%) com os de outro estudo clássico (8,7%), observou-se que não houve redução estatisticamente significativa na prevalência de baixo peso em Ribeirão Preto, no intervalo de uma década.A survey was carried out in Ribeirão Preto, S. Paulo State, Brazil, between June 1978 and May 1979 with a view to studying the prevalence of low birth weight and its occurrence among different social classes. Data were collected from 8,878 singleton live births in eight maternity hospitals, accounting for 98% of all births in the area. Social classes were determinated by the use of a model proposed by Singer and modified for epidemiological purposes by Barros. Out of the 8,878 births, 660 (7.5%) were of low birth weight. The prevalence of deficient weight at birth (between 2,500 and 2,999 grams) was of 21.1%. Analysis indicated that 50.6% of children with low birth weight were at term and the majority of them suffered form intrauterine growth retardation. The prevalence of low birth weight according to social class was seen to be lower in the bourgeoisie classes (ranging from 2.8% to 3.9%) and higher in working classes (from 7% up to 9.5%). Low birth weight (defined as < 2,500 grams) was used for purposes of comparison with other previous surveys. The percentage was lower in this study (8.3%) than that found in the Interamerican Investigation of Mortality in Childhood (8.7%), carried out in 1968-70. No statistically significant differences in the percentage of low birth weight were found in the case of Ribeirão Preto when these two surveys were compared.

A monitorização de lactentes é extremamente importante para a prevenção de desnutrição e/ou para a detecção de problemas, cuja primeira manifestação se dá na diminuição da velocidade de crescimento. Como o tipo de aleitamento pode interferir nesse processo o objetivo deste trabalho e comparar o crescimento de lactentes alimentados com leite materno e, com leite artificial. Foram tomados o peso e o comprimento de 365 lactentes que, ao nascer, tiveram peso maior ou igual a 2500 g, em uma comunidade agroindustrial de São Paulo. Desses, foram separados os que passaram a receber leite artificial, antes dos 2 meses, daqueles que receberam aleitamento materno exclusivo até os 6 meses. Foram calculados os percentis 10, 50 e 90 do peso e do comprimento deitado, segundo o sexo, e gráficos são apresentados alisados pelo método H3H3H e comparados com os dados do NCHS. As crianças alimentadas com leite materno tiveram peso maior que as com leite artificial e as do NCHS. Em relação ao comprimento, tanto as crianças que receberam leite materno, quanto as que receberam leite artificial apresentam resultados sistematicamente menores que as curvas do NCHS.

Os autores fazem ampla revisão dos estudos e pesquisas realizadas sobre crânio em recém-nascidos e lactentes, procurando analisar e correlacionar os dados antropométricos com os vários fatores biológicos e sociais maternos.

Os autores apresentam a metodologia do planejamento, montagem, execução empírica e um protótipo de análise de pesquisa em saúde perinatal. No projeto foram estudados todos os partos hospitalares (resultando recém-nascidos vivos) no município de Ribeirão Preto, Brasil, no período de l de junho de 1978 a 31 de maio de 1979. O estudo foi feito através de entrevistas com todas as puérperas que deram o luz nos 8 hospitas-maternidades do município, e as informações obtidas referiam-se aos pais, à renda, à gestação e ao parto, incluindo a atenção médica ao recém-nascido, ao óbito da criança (quando ocorresse), à etnia e à reprodução humana. São apresentados em detalhes, os passos para a execução do projeto, bem como a conceituação da classe social e sua operacionalização, ponto básico para o modelo de análise.The authors introduce the methodology of planning, empiric execution and a model of analysis in perinatal health. Live births from nosocomical deliveries was studied at Ribeirão Preto's neighborhood, Brazil, from June 1978 to May 1979. Parturient women from 8 maternities were interviewed and information was obtained regarding parents, family income, pregnancy, delivery (including medical care to newborn), children deaths, race and human reproduction. It is showed, in detail, the stages of project execution. The model of social class used and its operations research a basic point in the analysis.

Foi descrito novo foco endêmico de esquistossomose mansônica situado na Cidade de Bebedouro, Estado de São Paulo, Brasil. Foram analisados 221 casos diagnosticados da doença segundo a idade, sexo e origem (autóctone ou não), bem como foram discutidos os fatores envolvidos no aparecimento do foco, alertando-se para a necessidade de medidas de controle.A new endemic focus of schistosomiasis mansoni in the town of Bebedouro, S. Paulo, Brazil, is described. Two hundred and twenty-one cases are analyzed according to sex, age, and origin (autochthonous or not), the factors involved in the emergence of the focus are discussed and the necessity for control measure is emphasized.

This study focused on changes in demographic, social, and health-care patterns and pregnancy outcome related to maternal age from 1978-79 to 1994 in Ribeirão Preto, São Paulo State, Brazil. Information on pregnancy outcome was obtained from two cohorts of singleton live births, 6,681 births in 1978/79 and 2,839 births in 1994. A standardized questionnaire was submitted to mothers after delivery, and demographic information was collected from official records. There was a significant increase in teenage pregnancies (from 5.1% to 7.4%) and a decrease in infant mortality (36/1,000 to 17/1,000). There were significant decreases in the proportion of mothers with low schooling (24.5% to 14.4%), smoking (28.9% to 21.%), and multiparity (14.7% to 9.0%). Prenatal coverage improved (from 23.4% to 9.0% of patients with fewer than 4 prenatal visits), while cesareans increased (from 30.6% to 50.8%), as did preterm delivery (7.2% to 13.6%) and low birthweight (7.2% to 10.6%). Despite significant improvements in some maternal characteristics, the proportion of teenage pregnancies, preterm deliveries, low birthweight, and cesareans increased, raising concerns about the health costs and consequences for mothers and infants.

ABSTRACT: Several studies have identified social inequalities in low birth weight (LBW), preterm birth (PTB), and intrauterine growth restriction (IUGR), which, in recent years, have diminished or disappeared in certain locations. Objectives: Estimate the LBW, PTB, and IUGR rates in São Luís, Maranhão, Brazil, in 2010, and check for associations between socioeconomic factors and these indicators. Methods: This study is based on a birth cohort performed in São Luís. It included 5,051 singleton hospital births in 2010. The chi-square test was used for proportion comparisons, while simple and multiple Poisson regression models with robust error variance were used to estimate relative risks. Results: LBW, PTB and IUGR rates were 7.5, 12.2, and 10.3% respectively. LBW was higher in low-income families, while PTB and IUGR were not associated with socioeconomic factors. Conclusion: The absence or weak association of these indicators with social inequality point to improvements in health care and/or in social conditions in São Luís.

O objetivo deste estudo foi determinar os valores dos percentis 3 (P3), 50 (P50 = Idade Mediana da Menarca = IMM) e 97(P97), e a amplitude entre os extremos (P97 e P3), nas escolares do Município de Barrinha, São Paulo, Brasil. Esses valores foram correlacionados com algumas condições sócio-econômicas: classe social, número de irmãos e situação de desemprego do pai. Trata-se de estudo transversal, utilizando o método "status quo", ajustado pelo Logito para o cálculo dos percentis. Questionário foi o instrumento aplicado a 1.602 escolares entre 8 e 17 anos incompletos. A IMM foi 12 anos(a) e 6 meses(m),P97 de 10a2 m e P3 de 14a10m. As meninas da classe social menos favorecida, e aquelas que referiram desemprego paterno apresentaram IMM mais tardia. Não houve diferença da IMM relacionada ao número de irmãos. A amplitude entre P97 e P3 mostrou-se ampla na amostra geral. Concluiu-se que Barrinha apresentou IMM semelhante e até abaixo de valores regionais, do Brasil e de alguns países desenvolvidos. O estudo do intervalo entre os percentis extremos mostrou ser um melhor indicador das diversidades biológicas e das desigualdades sócio-econômicas, do que a IMM isoladamente.

OBJECTIVE: Low birth weight children are unusual among well-off families. However, in Brazil, low birth weight rate was higher in a more developed city than in a less developed one. The study objective was to find out the reasons to explain this paradox. METHODS: A study was carried out in two municipalities, Ribeirão Preto (Southeastern Brazil) and São Luís (Northeastern Brazil), which low birth weight rates were 10.7% and 7.6% respectively. Data from two birth cohorts were analyzed: 2,839 newborns in Ribeirão Preto in 1994 and 2,439 births in São Luís in 1997-1998. Multiple logistic regression analysis was performed, adjusted for confounders. RESULTS: Low birth weight risk factors in São Luís were primiparity, maternal smoking and maternal age less than 18 years. In Ribeirão Preto, the associated variables were family income between one and three minimum wages, maternal age less than 18 and equal to or more than 35 years, maternal smoking and cesarean section. In a combined model including both cohorts, Ribeirão Preto presented a 45% higher risk of low birth weight than São Luís. When adjusted for maternal smoking habit, the excess risk for low birth weight in Ribeirão Preto compared to São Luís was reduced by 49%, but the confidence interval was marginally significant. Differences in cesarean section rates between both cities contributed to partially explain the paradox. CONCLUSIONS: Maternal smoking was the most important risk factor for explaining the difference in low birth weight between both cities. The other factors contributed little to explain the difference in low birth weight rates.

OBJETIVO: Identificar as principais características socioeconômicas, demográficas, antropométricas e comportamentais, bem como os resultados perinatais da gravidez na adolescência. Conhecer os tipos de serviços de saúde utilizados pelas gestantes adolescentes. MÉTODOS: A amostra de 2.429 partos de mulheres residentes em São Luís, MA, abrangeu 94% dos nascimentos hospitalares. As mulheres foram separadas em seis grupos de idade para melhor avaliação do comportamento das variáveis entre os dois grupos de adolescentes (abaixo de 18 anos; 18 a 19 anos) e entre as adolescentes e as demais mulheres. O teste do qui-quadrado foi utilizado para a comparação de proporções e a razão de prevalências foi empregada como medida de efeito. RESULTADOS: Das 2.429 mulheres, 714 eram adolescentes (29,4%). Seu coeficiente específico de fecundidade, 72,2 por mil, foi mais elevado que em outras regiões do País. As adolescentes apresentaram piores condições socioeconômicas e reprodutivas que as demais mulheres, maior proporção de pré-natal inadequado (39,2%) e muitas não tinham companheiro (34,5%). Por outro lado, tiveram menor proporção de parto cesáreo (23,0%) e de fumantes (3.5%). CONCLUSÕES: Apesar da situação socioeconômica igualmente desfavorável, as adolescentes de 18 a 19 anos apresentaram resultados perinatais semelhantes às mulheres de 25 a 29 anos. Já as menores de 18 anos tiveram maiores proporções de filhos com baixo peso ao nascer, prematuros e com maior risco de mortalidade infantil. Isto sugere que a imaturidade biológica possa estar associada a maiores taxas de prematuridade, baixo peso ao nascer e mortalidade infantil.

OBJETIVO: Identificar fatores associados à inadequação do uso da assistência pré-natal em comunidade urbana. MÉTODOS: Foi realizado estudo transversal em amostra sistemática, estratificada por maternidades, de todos os nascimentos hospitalares do município de São Luís, MA, no período de março de 1997 a fevereiro de 1998. Foram avaliados indicadores socioeconômicos e demográficos, de saúde reprodutiva, morbidade na gravidez e utilização de serviços pré-natais. Utilizou-se questionário padronizado respondido pelas puérperas antes da alta hospitalar. A adequação do uso da assistência pré-natal foi analisada pelo índice "Adequacy of Prenatal Care Utilization" (APNCU) e por um novo índice proposto, baseado nas recomendações do Ministério da Saúde, Brasil. RESULTADOS: Foram entrevistadas 2.831 puérperas, atendidas em dez unidades de saúde pública e privada. A inadequação do uso da assistência pré-natal foi de 49,2% pelo índice APNCU, e de 24,5% pelo novo índice proposto. Mulheres atendidas em serviços públicos de saúde, de baixa escolaridade e baixa renda familiar, sem companheiro ou com doença durante a gravidez, tiveram maiores percentuais de inadequação do uso do atendimento pré-natal, pelos dois índices analisados. Abstract Objectives: to analyze associations among violence against pregnant women, depressive symptoms during pregnancy and maternal depression symptoms. Methods: a sample of 1,139 mothers was conducted on a prenatal cohort study in the municipality of São Luís in Brazil. Psychological and physical violence against pregnant women were measured by the World Health Organization Violence Against Woman. Depressive symptoms during pregnancy were measured by the Escala de Depressão do Centro de Estudos Epidemiológicos (CES-D) (Depression Scale for Epidemiological Studies Center) and maternal depression symptoms were measured by the Edinburgh Postnatal Depression Scale (EPDS). The conceptual model of the structural equation modeling contained socioeconomic situation, social support, psychological and physical violence and depression during pregnancy as determinants of the maternal depression symptoms. OBJECTIVE: Data from municipal databases can be used to plan interventions aimed at reducing inequities in health care. The objective of the study was to determine the distribution of infant mortality according to an urban geoeconomic classification using routinely collected municipal data. METHODS: All live births (total of 42,381) and infant deaths (total of 731) that occurred between 1994 and 1998 in Ribeirão Preto, Brazil, were considered. Four different geoeconomic areas were defined according to the family head's income in each administrative urban zone. Foi realizado inquérito em Ribeirão Preto, Brasil, de junho de 1978 a maio de 1979, que constou de entrevistas com mães de nascidos vivos hospitalares, de partos únicos, correspondendo a 98 % dos nascimentos ocorridos durante o período no Município. O maior percentual de nascimentos com baixo peso ocorreu entre as mães jovens, fumantes e pertencentes às classes trabalhadoras. O maior número de mães jovens foi encontrado também nessas classes. A prevalência do hábito de fumar foi maior nas mães jovens, porém houve pouca diferença na freqüência de mulheres fumantes entre as classes sociais, variando de 25 a 30%. Observou-se que o pencentual de baixo peso foi maior para as mães trabalhadoras não fumantes do que para as mães burguesas fumantes. O modelo logito foi ajustado aos dados para estudo da possível associação múltipla entre hábito de fumar, idade materna e classe social com o peso ao nascer. Os resultados indicaram que hábito de fumar materno, idade materna e classe social foram independentes no seu efeito sobre o peso ao nascer, não se observando interação. Esses achados sugerem que a maior prevalência de baixo peso entre mães não fumantes das classes trabalhadoras em relação às mães fumantes da burguesia provavelmente refletiu a concentração de outros fatores de risco entre as mulheres das classes trabalhadoras, tais como: baixa escolaridade, cuidado pré-natal inadequado, alta paridade, diferenças no comportamento reprodutivo.

ABSTRACT CONTEXT AND OBJECTIVE: Alcohol consumption during pregnancy is a significant social problem that may be associated with adverse perinatal outcomes. The aim of this study was to describe alcohol consumption during pregnancy and to study its association with low birth weight, newborns small for gestational age and preterm birth. DESIGN AND SETTING: Nested cohort study, in the city of Ribeirão Preto, São Paulo, Brazil. METHODS: 1,370 women and their newborns were evaluated. A standardized questionnaire on health and lifestyle habits was applied to the mothers. Anthropometry was performed on the newborns. Alcohol consumption was defined as low, moderate or high, as defined by the World Health Organization. Adjusted logistic regression analysis was used. RESULTS: 23% of the women consumed alcohol during pregnancy. Consumption mainly occurred in the first trimester (14.8%) and decreased as the pregnancy progressed. The median alcohol intake was 3.89 g (interquartile range, IQR = 8 g) per day. In the unadjusted analysis, alcohol consumption increased the risk of low birth weight almost twofold (odds ratio, OR 1.91; 95% confidence interval, CI: 1.25-2.92). The risk was lower in the adjusted analysis (OR 1.62; 95% CI: 1.03-2.54). Alcohol consumption did not show associations with small for gestational age or preterm birth. There was greater risk of low birth weight and newborns small for gestational age and preterm birth among mothers who were both smokers and drinkers. CONCLUSIONS: The alcohol consumption rate during pregnancy was 23% and was independently associated with low birth weight, but there was no risk of newborns small for gestational age or preterm birth.

OBJECTIVE: To identify risk factors for low birth weight (LBW) among live births by vaginal delivery and to determine if the disappearance of the association between LBW and socioeconomic factors was due to confounding by cesarean section. METHODS: Data were obtained from two population-based cohorts of singleton live births in Ribeirão Preto, Southeastern Brazil. The first one comprised 4,698 newborns from June 1978 to May 1979 and the second included 1,399 infants born from May to August 1994. The risks for LBW were tested in a logistic model, including the interaction of the year of survey and all independent variables under analysis. RESULTS: The incidence of LBW among vaginal deliveries increased from 7.8% in 1978--79 to 10% in 1994. The risk was higher for: female or preterm infants; newborns of non-cohabiting mothers; newborns whose mothers had fewer prenatal visits or few years of education; first-born infants; and those who had smoking mothers. The interaction of the year of survey with gestational age indicated that the risk of LBW among preterm infants fell from 17.75 to 8.71 in 15 years. The mean birth weight decreased more significantly among newborns from qualified families, who also had the highest increase in preterm birth and non-cohabitation. CONCLUSIONS: LBW among vaginal deliveries increased mainly due to a rise in the proportion of preterm births and non-cohabiting mothers. The association between cesarean section and LBW tended to cover up socioeconomic differences in the likelihood of LBW. When vaginal deliveries were analyzed independently, these socioeconomic differences come up again.

Preeclampsia is a multifactorial disease. Among these factors, untreated hypertension during pregnancy can result in high morbidity and mortality rates and may also be related to the future development of cardiovascular diseases.Therefore, this systematic review aimed to determine the association of previous preeclampsia with the future development of cardiovascular diseases. Studies on the association between preeclampsia and future cardiovascular diseases published in the last 10 years (2009-2019) were identified from the PubMed/Medline (207 articles), Embase (nine articles), and Cochrane (three articles) databases using the keywords “preeclampsia” and “future cardiovascular diseases”, “preeclampsia” and “future heart attack”, and “preeclampsia” and “future cardiac disease”. After applying the inclusion and exclusion criteria, 15 articles were analyzed by systematic review and meta-analysis according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. This study analyzed the association between the inversion of traditional gender roles and exclusive psychological and physical/sexual intimate partner violence, in a cross-sectional study of Brazilian pregnant women, identified through prenatal services in the municipalities of São Luís, Maranhão State (n = 992) and Ribeirão Preto, São Paulo State (n = 943). The pregnant women ranged from 12 to 45 years. Inversion of traditional gender roles was assessed by calculating differences in age, education and occupation between pregnant women and their co-residing intimate partners and identifying the largest contribution to family income. The conceptual model was tested with structural equation modeling and showed acceptable fit. The prevalence of any type of intimate partner violence was 29.8% in São Luís and 20.1% in Ribeirão Preto. In both municipalities, pregnant women were more likely to suffer exclusive psychological and physical/sexual violence when they had the highest income in the family (p < 0.005). In São Luís, physical/sexual violence was more common among women who were better educated than their partners (standardized coefficient, SC = -0.466; p = 0.007). In Ribeirão Preto, exclusive psychological violence was more frequent among women who had lower status occupations than their partners (SC = 0.236; p = 0.004). Inversion of traditional gender roles is associated with exclusive psychological and physical/sexual violence against pregnant women by their co-residing intimate partners. These findings suggest that women's empowerment at an individual level does not necessarily relieve them of intimate partner abuse in social contexts where traditional gender norms persist.
