68. Simulation Optimization Gantry Call-Back Control Method for Proton Therapy Systems

Abstract

Proton therapy, a highly targeted radiation treatment for tumors, is experiencing high demand in recent years. Many studies focused more on improving patient throughput and proton beam utilization than treatment quality, especially in a multiple-gantry proton therapy facility that shares a proton beam accelerator. The beam sharing causes significantly long wait for a patient to receive beam treatment. This long beam wait in a gantry negatively impacts treatment quality and patient satisfaction. To reduce beam wait time while maximizing utilization, we propose a simulation optimization gantry call-back control (SOGCC) method to determine the optimal time to call a patient back to the gantry so that beam request conflicts can be improved without sacrificing much beam utilization. Simulation experiments are conducted to compare the proposed SOGCC method with other methods, such as the naive method, the gatekeeper method, and the rollout-based gantry call-back control (RGCC) method, and show a satisfactory performance of the SOGCC method. The simulation experiment also demonstrates how variation of setup time, beam treatment time and transition time, can undermine the performance of proton therapy systems, and the SOGCC method is able to account for the variation more efficiently. Evidently, the SOGCC method is a promising method to improve proton therapy system care delivery. Note to Practitioners—Proton therapy is an advanced radiation treatment technology that has been demonstrated effective in clinical settings. In current practice, its workflow efficiency and patient experience have not yet received enough attention. Proton therapy practitioners are seeking a high beam utilization and low beam wait time, which are difficult to achieve due to variation of time on each procedure. In this paper, we intend to improve the proton therapy delivery process by making use of real-time system state. Simulation is applied to predict the system dynamics and performance measures, and thus the optimal time to call patients back to gantry can be determined. This proposed method can be integrated into proton therapy systems to reduce beam request conflicts and patients beam wait time with small impact on beam utilization.

Abstract

Giardia, Cryptosporidium, Salmonella, and Shigella have emerged as waterborne pathogens of concern. Thirty water samples were collected from 10 small water systems in mountainous areas of Taiwan and investigated for the occurrence of these pathogens. In addition, the basic characteristics of each small water system were recorded, and each water sample was tested for several water quality parameters. The occurrence frequency was 10.0% for Giardia spp., 3.3% for Cryptosporidium spp. and Shigella flexneri, and 16.7% for Salmonella enterica. The highest correlation was found between the presence of Salmonella enterica and low water temperature, followed by the correlation between water turbidity and fecal coliform concentration. A low correlation was found between the presence of Giardia and the concentration of heterotrophic bacteria. The water samples from small water systems with filtration devices had a lower occurrence of pathogen microorganisms than those without filtration devices. The proportion of water samples with pathogenic microorganisms increased with the consumer population of the water systems, and the pathogen occurrence differed among the regions of the sampling sites. In order to prevent parasitic infection, the use of disinfection devices in small water systems would be needed. Abstract

Structural design simultaneously governed by earthquakes and environmental vibrations has received a lot of attention in recent years. Base-isolated composite structures are typically used in the above-mentioned structural design. The corresponding analysis involves validating structural safety under earthquakes and human comfort under environmental vibrations through a time-history analysis. Thus, a reasonable damping model is essential. In this work, the representatives of viscous damping model and rate-independent damping model, namely the Rayleigh damping model and uniform damping model, were adopted to investigate the influence of damping models on the time-history analysis of such structural designs. The energy dissipation characteristics of the above-mentioned damping models were illustrated via a dynamic test of recycled aggregate concrete specimens. A case study was performed on a base-isolated steelconcrete composite structure. The dynamic responses under the excitation of earthquakes and environmental vibrations were compared using different damping models. The uniform damping model was found to be more flexible than the Rayleigh damping model in dealing with excitations with different frequency components. The uniform damping model is both theoretically advantageous and easy to use, demonstrating its potential in dynamic analysis of structures designed simultaneously governed by earthquakes and environmental vibrations. Abstract

Organic–2D material heterostructures have attracted intensive research interest due to their intriguing properties, with a wide range of potential applications in multifunctional flexible electronic and optoelectronic devices. Central to the realization of such devices is a fundamental understanding of the electronic structures at organic–2D material heterointerfaces. The energy level alignment (ELA) at the interface is of paramount importance because it determines the charge transfer barriers between the two materials in contact. In this paper, we discuss the physical mechanisms determining the ELAs, with special attention on interfacial charge transfer at the heterostructures. We review the current understanding of electronic properties at the heterointerfaces formed by the integration of organics with graphene and 2D transition metal dichalcogenides (TMDs), and conclude with a perspective on the future development of organic–2D material heterostructure. This study examines whether government ownership can affect bank risk. We use Moody's individual and issuer ratings as proxies for banks' operating and default risks. We find that whether government ownership increases or reduces default risk depends on where the banks are from and when the banks obtain government support. We find that government injections of funds into banks depends on this government's income level and whether the distressed banks are in crisis periods. Abstract

Due to its opportunistic spectrum sharing capability, cognitive radio (CR) has been proposed as a fundamental solution to alleviate the contradiction between spectrum scarcity and inefficient utilization of licensed spectrum. In CR system (CRS), to efficiently utilize the spectrum resource, one important issue is to allocate the sensing and transmission duration reasonably. In this paper, the evaluation metric of energy efficiency, which represented the total number of bits that were delivered with per joule of energy consumed, is adopted to evaluate the proposed scheme. We study a joint design of energy efficient sensing and transmission durations to maximize energy efficiency capacity (EEC) of CRS. The tradeoff between EEC and sensing and transmission durations are formulized as an optimization problem under constraints on target detection probability of secondary users (SUs) and toleration interference threshold of primary users (PUs). To obtain the optimal solution, optimizing sensing duration and transmission duration will be first performed separately. Then, a joint optimization iterative algorithm is proposed to search the optimal pair of sensing and transmission durations. Analytical and simulation results show that there exists a unique duration pair where the EEC is maximized, and that the EEC of the proposed joint optimization algorithm outperforms that of existed algorithms. Furthermore, the simulation results also reveal that the performance of the proposed low complexity iterative algorithm is comparable with that of the exhaustive search scheme.
