Abstract: We report results of a search for flavor-changing neutral current (FCNC), lepton flavor, and lepton-number violating decays of the D0 (and its antiparticle) into three and four bodies. Using data from Fermilab charm hadroproduction experiment E791, we examine modes with two leptons (muons or electrons) and a rho(0), K( *0), or straight phi vector meson or a nonresonant pi(pi), Kpi, or KK pair of pseudoscalar mesons. No evidence for any of these decays is found. Therefore, we present branching-fraction upper limits at 90% confidence level for the 27 decay modes examined (18 new).

Abstract: We present the first direct measurements of the pion valence quark momentum distribution which is related to the square of the pion light-cone wave function. The measurements were carried out using data on diffractive dissociation of 500 GeV/c $��^-$ into di-jets from a platinum target at Fermilab experiment E791. The results show that the $|q\bar {q}>$ light-cone asymptotic wave function, which was developed using perturbative QCD methods, describes the data well for $Q^2 \sim 10 ~{\rm (GeV/c)^2}$ or more. Abstract: From a sample of 1172 +/- 61 D(+)--pi(-)pi(+)pi(+) decays, we find gamma(D(+)--pi(-)pi(+)pi(+))/gamma(D(+)--K-pi(+)pi(+)) = 0.0311 +/- 0.0018(+0.0016)(-0.0026). Using a coherent amplitude analysis to fit the Dalitz plot of these decays, we find strong evidence that a scalar resonance of mass 478(+24)(-23) +/- 17 MeV/c(2) and width 324(+42)(-40) +/- 21 MeV/c(2) accounts for approximately half of all decays.

Abstract: From a sample of 848+/-44 D(+)(s)--pi(-)pi(+)pi(+) decays, we find gamma(D(+)(s)--pi(-)pi(+)pi(+))/gamma(D(+)(s)--straight phipi(+)) = 0.245+/-0.028(+0.019)(-0.012). Using a Dalitz plot analysis of this three body decay, we find significant contributions from the channels rho(0)(770)pi(+), rho(0)(1450)pi(+), f(0)(980)pi(+), f(2)(1270)pi(+), and f(0)(1370)pi(+). We also present the values obtained for masses and widths of the resonances f(0)(980) and f(0)(1370).

Abstract: We have studied the diffractive dissociation into di-jets of 500 GeV/c pions scattering coherently from carbon and platinum targets. Extrapolating to asymptotically high energies (where t_{min} approaches 0) we find that when the per-nucleus cross-section for this process is parameterized as $\sigma = \sigma_0 A^{\alpha}, \alpha$ has values near 1.6, the exact result depending on jet transverse momentum. Abstract: Using the large hadroproduced charm sample collected in experiment E791 at Fermilab, we report the first directly measured constraint on the decay-width difference Delta Gamma for the mass eigenstates of the D0-D0bar system. We obtain our result from lifetime measurements of the decays D0 --> K-pi+ and D0 --> K-K+, under the assumption of CP invariance, which implies that the CP eigenstates and the mass eigenstates are the same. The lifetime of D0 --> K-K+ (the CP-even final state is ��_KK = 0.410 +/- 0.011 +/- 0.006 ps, and the lifetime of D0 --> K-pi+ (an equal mixture of CP-odd and CP-even final states is tau_Kpi = 0.413 +/- 0.003 +/- 0.004 ps. The decay-width difference is Delta Gamma = 2(Gamma_KK - Gamma_Kpi) = 0.04 +/- 0.14 +/- 0.05 ps^-1. Abstract: We report the results of a search for flavor-changing neutral current, lepton-flavor violating, and lepton-number violating decays of D+, Ds, and D0 mesons (and their antiparticles) into modes containing muons and electrons. Using data from Fermilab charm hadroproduction experiment E791, we examine the pi,l,l and K,l,l decay modes of D+ and Ds and the l+l- decay modes of D0. No evidence for any of these decays is found. Therefore, we present branching-fraction upper limits at 90% confidence level for the 24 decay modes examined. Abstract: A double-sided silicon strip detector with a radiation-tolerant design was fabricated and characterized in a sequence of beam tests at KEK using 4 GeV/ c pions. The detectors were combined with newly designed, fast, lower power, bipolar amplifier-shaper-discriminator chips and CMOS digital pipeline chips to record hit-no hit signals in the strips. Efficiencies, noise occupancies, and spatial resolutions were measured before and after the proton irradiation at an equivalent fluence of 1 × 10 14 p/cm 2 , depending on angle of track incidence and strip-pitches. The median pulse height distribution, derived from the threshold scans of the efficiency, allowed to extract the response of the detector. A 1 T magnetic field enabled us to determine the Hall mobilities of electrons and holes.

Abstract: Detailed computer simulation studies of a tracking detector originally designed for high luminosity operation at the Superconducting Super Collider have been carried out. Detector simulation and track reconstruction techniques appropriate to the extremely high charged particle rates corresponding to luminosities of up to 10 34 cm −2 s −1 are described. The detector performance is evaluated and compared to design requirements stemming from the physics goals of the SDC experiment. The results have implications for the design of tracking detectors at future high luminosity hadron colliders such as the Large Hadron Collider (LHC).

Abstract: 1. Interleukin-1 beta (IL-1 beta) is a potent stimulant of inducible nitric oxide synthase (iNOS) mRNA and nitric oxide (NO) production in vascular smooth muscle (VSM) cells in culture. These studies investigate the role of adenosine 3':5'-cyclic monophosphate (cyclic AMP) in this process. 2. Dibutyryl cyclic AMP (db cyclic AMP, 0.1-1 mM), forskolin (1-10 microM) and the phosphodiesterase inhibitor, Ro 20-1724 (1-10 microM), all of which increase intracellular cyclic AMP, had no effect on NO production when added alone but markedly enhanced NO production by IL-1 beta-stimulated VSM cells in a dose-dependent manner. Consistent with a cyclic AMP-mediated action, isoprenaline (1-10 microM) increased NO production from IL-1 beta-stimulated cells. Dibutyryl cyclic GMP (db cyclic GMP) had no effect at concentrations up to 1 mM. 3. Pursuing these observations, iNOS protein levels were examined by Western blot analysis and iNOS mRNA levels were measured by reverse transcription and amplification of the resultant cDNA using the polymerase chain reaction. In addition to enhancing NO production, db cyclic AMP increased iNOS protein and mRNA above that produced by IL-1 beta alone. 4. These data demonstrate a major effect of cyclic AMP on cytokine-induced NOS activity in VSM cells, mediated at least in part by regulating synthesis of iNOS, and has implications for the pathogenesis and management of septic shock.

Abstract: This paper presents our first measurement of the F 2 structure function in neutral-current, deep inelastic scattering using the ZEUS detector at HERA, the ep colliding beam facility at DESY. The data correspond to an integrated luminosity of 24.7 nb −1 . Results are presented for data in range of Q 2 from 10 GeV 2 to 4700 GeV 2 and Bjorken x down to 3.0 × 10 −4 . The F 2 structure function increases rapidly as x decreases.

Abstract: This paper reports a search for excited electrons at the HERA electron-proton collider. In a sample corresponding to an integrated luminosity of 26 nb−, no evidence was found for any resonant state decaying into e−γ, νW− or e−Z0. Limits on the coupling strength of an excited electron have been determined for masses between 45 and 225 GeV. This study also reports the observation of the wide-angle eγ Compton scattering process.

Abstract: As part of a program to develop a silicon strip central tracking detector system for the Superconducting Super Collider (SSC), the effects of radiation damage in silicon detectors and their associated front-end readout electronics are being studied. The authors report on the results of neutron and proton irradiations at the Los Alamos National Laboratory and gamma -ray irradiations at UC Santa Cruz. Individual components on single-sided AC-coupled silicon strip detectors and on test structures were tested. Circuits fabricated in a radiation-hard CMOS process and individual transistors fabricated using dielectric isolation bipolar technology were also studied. Bulk damage to the silicon itself is seen as the limiting factor in the lifetime of a detector system. In particular, it is the acceptor site creation in the active volume of the silicon detector that will limit the lifetime to approximately 10 yr for the innermost detectors. >

Abstract: The effect of pirenperone, a putative 5-HT2 receptor antagonist, on various 5-HT-mediated behavioural responses has been examined. The head twitch response in mice, induced by administration of carbidopa (25 mg/kg) followed by 5-hydroxytryptophan (5-HTP) (200 mg/kg), was inhibited in a dose-dependent manner by pirenperone, with an ED50 of 76 micrograms/kg. The appearance of head weaving, forepaw treading and hind-limb abduction, which followed the administration of tranylcypromine (5 mg/kg) plus L-tryptophan (100 mg/kg) or quipazine (50 mg/kg) to rats, was also inhibited by pretreatment with pirenperone (100 micrograms/kg). Pirenperone did not alter the rate of 5-HT synthesis in the rat brain. Whilst pirenperone (100 micrograms/kg) did decrease methamphetamine-induced locomotor activity in rats, a dose of haloperidol producing a similar inhibition of this response did not alter the 5-HT-mediated behaviour. It is suggested, therefore, that the currently used 5-HT-induced behavioural models are 5-HT2 receptor-mediated.

Abstract: The Mark II detector has been upgraded in preparation for its role as the first detector to take data at the Stanford Linear Collider. The new detector components include the central drift chamber, the time-of-flight system, the coil, the endcap electromagnetic calorimeters and the beam energy and luminosity measuring devices. There have also been improvements in detector hermeticity. All of the major components were installed for a test run at the PEP storage ring (√s = 29 GeV) in 1985. This paper describes the upgraded detector, including its trigger and data acquisition systems, and gives performance figures for its components. Future improvements are also discussed.

Abstract: Production rates of multijet hadronic final states are studied ine + e − annihilation at 29 GeV center of mass energy. QCD shower model calculations with exact first order matrix element weighting at the first gluon vertex are capable of reproducing the observed multijet event rates over a large range of jet pair masses. The method used to reconstruct jets is well suited for directly comparing experimental jet rates with parton rates calculated in perturbative QCD. Evidence for the energy dependene of α s is obtained by comparing the observed production rates of 3-jet events with results of similar studies performed at higher center of mass energies.

Abstract: We use data from the Mark II experiment at PEP to search for the process B→h 0 X for m h 0 between 50 and 210 MeV/ c 2 . No evidence for the Higgs boson is seen in this mass range. The limit obtained rules out the standard Higgs boson for masses between 70 and 210 MeV/ c 2 and significantly constrains extensions of the Higgs sector.

Abstract: We have studied the energy-energy correlation in e+e- annihilation into hadrons at s =29 GeV using the Mark II detector at the SLAC storage ring PEP. We find to O(s2) that s=0.1580.0030.008 if hadronization is described by string fragmentation. Independent fragmentation schemes give s=0.100.14, and give poor agreement with the data. A leading-log shower fragmentation model is found to describe the data well. © 1988 The American Physical Society.

Abstract: Multihadronic e/sup +/e/sup -/ annihilation events at a center-of-mass energy of 29 GeV have been studied with both the original (PEP 5) Mark II and the upgraded Mark II detectors. Detector-corrected distributions from global shape analyses such as aplanarity, Q/sub 2/-Q/sub 1/, sphericity, thrust, minor value, oblateness, and jet masses, and inclusive charged-particle distributions including x, rapidity, p/sub perpendicular/, and particle flow are presented. These distributions are compared with predictions from various multihadron event models which use leading-logarithmic shower evolution or QCD matrix elements at the parton level and string or cluster fragmentation for hadronization. The new generation of parton-shower models gives, on the average, a better description of the data than the previous parton-shower models. The energy behavior of these models is compared to existing e/sup +/e/sup -/ data. The predictions of the models at a center-of-mass energy of 93 GeV, roughly the expected mass of the Z/sup 0/, are also presented.

Abstract: The central drift chamber in the Mark II detector at the SLAC linear collider has been instrumented with 100 MHz flash-ADCs. Pulse digitization provides particle identification through the measurement of average ionization loss in the chamber. We present the results of a study of system performance and outline the systematic corrections that optimize resolution. The data used are from a short test run at PEP with one third of the FADCs installed, and an extensive cosmic ray sample with the fully instrumented chamber.

Abstract: We have searched for events with new-particle topologies in 390 hadronic Z decays with the Mark II detector at the SLAC Linear Collider. We place 95%-confidence-level lower limits of 40.7 GeV/c^2 for the top-quark mass, 42.0 GeV/c^2 for the mass of a fourth-generation charge - 1/3 quark, and 41.3 GeV/c^2 for the mass of an unstable Dirac neutral lepton.

Abstract: The weak acid uncoupler, carbonyl cyanide m-chlorophenylhydrazone, carries protons across lipid membranes. As predicted by the carrier model, at low pH, the current changes immediately following a jump in applied potential and then remains constant. By contrast at high pH, the currents relax from an initial value to a lower value as the carrier anions redistribute in the membrane. These relaxations are slower than those seen with other lipid-soluble anions which presumably explains why they had not been detected previously.

Abstract: We report results of a search for CP violation in the singly Cabibbo-suppressed decays D+ -> K- K+ pi+, phi pi+, K*(892)0 K+, and pi- pi+ pi+ based on data from the charm hadroproduction experiment E791 at Fermilab. We search for a difference in the D+ and D- decay rates for each of the final states. No evidence for a difference is seen. The decay rate asymmetry parameters A(CP), defined as the difference in the D+ and D- decay rates divided by the sum of the decay rates, are measured to be: A(CP)(K K pi) = -0.014 +/- 0.029, A(CP)(phi pi) = -0.028 +/- 0.036, A(CP)(K*(892) K) = -0.010 +/- 0.050, and A(CP)(pi pi pi) = -0.017 +/- 0.042., 13 pages, 5 figures, 1 table; Elsevier LaTeX

80. Oxidative DNA damage promotes vascular aging associated with changes in extracellular matrix-regulating proteins.

Abstract: Aims: Vascular aging is characterized by vessel stiffening, with increased deposition of extracellular matrix (ECM) proteins including collagens. Oxidative DNA damage occurs in vascular aging, but how it regulates ECM proteins and vascular stiffening is unknown. We sought to determine the relationship between oxidative DNA damage and ECM regulatory proteins in vascular aging., Methods and Results: We examined oxidative DNA damage, the major base excision repair (BER) enzyme 8-Oxoguanine DNA Glycosylase (Ogg1) and its regulators, multiple physiological markers of aging, and ECM proteomics in mice from 22-72w. Vascular aging was associated with increased oxidative DNA damage, and decreased expression of Ogg1, its active acetylated form, its acetylation regulatory proteins P300 and CBP, and the transcription factor Foxo3a. Vascular stiffness was examined in vivo in control, Ogg1-/-, or mice with vascular smooth muscle cell-specific expression of Ogg1+ (Ogg1) or an inactive mutation (Ogg1KR). Ogg1-/- and Ogg1KR mice showed reduced arterial compliance and distensibility, and increased stiffness and pulse pressure, whereas Ogg1 expression normalised all parameters to 72w. ECM proteomics identified major changes in collagens with aging, and downregulation of the ECM regulatory proteins Protein 6-lysyl oxidase (LOX) and WNT1-inducible-signaling pathway protein 2 (WISP2). Ogg1 overexpression upregulated LOX and WISP2 both in vitro and in vivo, and downregulated Transforming growth factor β1 (TGFb1) and Collagen 4α1 in vivo compared with Ogg1KR. Foxo3a activation induced Lox, while Wnt3 induction of Wisp2 also upregulated LOX and Foxo3a, and downregulated TGFβ1 and fibronectin 1. In humans, 8-oxo-G increased with vascular stiffness, while active OGG1 reduced with both age and stiffness., Conclusions: Vascular aging is associated with oxidative DNA damage, downregulation of major BER proteins, and changes in multiple ECM structural and regulatory proteins. 81. An Investigation Into What Factors Influence Patterns of Clinical Presentation in Adult-Onset Celiac Disease.

Abstract: Introduction Anxiety and depression are common in patients with celiac disease (CD), and many psychosocial explanations have been considered. However, as the gut-brain axis is becoming increasingly understood, biological mechanisms have been proposed, including vitamin or mineral deficiencies and gut inflammation. Aim To investigate associations between anxiety/depression and symptom severity, vitamin status, and gut inflammation in untreated adult patients presenting with a serologic indication of celiac disease. Methods The Hospital Anxiety and Depression Scale (HADS), Celiac Symptom Index (CSI), and Perceived Stress Scale (PSS) questionnaires were administered to 17 patients over a 14-month period. Duodenal biopsies were obtained to determine histological Marsh scores. Iron, B12, folate, vitamin D, and thyroid function tests were reviewed. Results HADS-Anxiety (HADS-A) scores correlated with symptom severity (rs = 0.62, P = 0.008), but not with any hematological investigations or degree of intestinal inflammation. No patients scored highly for depression. Iron deficiency was the most common deficiency observed (n = 6). Greater symptomatology was associated with female sex (females versus males: average CSI scores, 32.1 versus 23.6; t17 = 2.1, P < 0.05), younger age at presentation (rs = -0.55, P = 0.02), and lower Marsh score (Marsh 0 versus Marsh 3C: mean scores, 36 versus 24.5; t5 = 6.2, P = 0.009). Conclusions The anxiety experienced by patients with CD at presentation is likely a reactive form due to gastrointestinal symptoms rather than a biological process specific to CD. Older patients tend to present less symptomatically, highlighting the need for screening of at-risk individuals. The degree of villous atrophy does not correlate well with clinical presentation. To inform policy, stakeholders and management decisions on global threats to aquatic ecosystems, 195 assessors representing 120 risk assessment areas across all six inhabited continents screened 819 non-native species from 15 groups of aquatic organisms (freshwater, brackish, marine plants and animals) using the Aquatic Species Invasiveness Screening Kit. This multi-lingual decision-support tool for the risk screening of aquatic organisms provides assessors with risk scores for a species under current and future climate change conditions that, following a statistically based calibration, permits the accurate classification of species into high-, medium- and low-risk categories under current and predicted climate conditions. The 1730 screenings undertaken encompassed wide geographical areas (regions, political entities, parts thereof, water bodies, river basins, lake drainage basins, and marine regions), which permitted thresholds to be identified for almost all aquatic organismal groups screened as well as for tropical, temperate and continental climate classes, and for tropical and temperate marine ecoregions. In total, 33 species were identified as posing a 'very high risk' of being or becoming invasive, and the scores of several of these species under current climate increased under future climate conditions, primarily due to their wide thermal tolerances. The risk thresholds determined for taxonomic groups and climate zones provide a basis against which area-specific or climate-based calibrated thresholds may be interpreted. Abstract: The recent genome-wide analysis of carotid-femoral pulse wave velocity (PWV) identified a significant locus within the 14q32.2 gene desert. Gene regulatory elements for the transcriptional regulator B-cell CLL/lymphoma 11B (BCL11B) are within this locus and an attractive target for the gene association. We investigated the functional impact of these gene desert SNPs on BCL11B transcript in human aorta to characterize further its role in aortic stiffness. To do this, we used a large repository of aortic tissues (n = 185) from an organ transplant program and assessed ex vivo stiffness of the aortic rings. We tested association of three lead SNPs from the GWAS meta-analysis with ex vivo aortic stiffness and BCL11B aortic mRNA expression: rs1381289 and rs10782490 SNPs associated significantly with PWV and showed allele-specific differences in BCL11B mRNA. The risk alleles associated with lower BCL11B expression, suggesting a protective role for BCL11B. Despite strong association, we could not detect BCL11B protein in the human aorta. However, qPCR for CD markers showed that BCL11B transcript correlated strongly with markers for activated lymphocytes. Our data confirm the significance of the 14q32.2 region as a risk locus for aortic stiffness and an upstream regulator of BCL11B. The BCL11B transcript detected in the human aorta may reflect lymphocyte infiltration, suggesting that immune mechanisms contribute to the observed association of BCL11B with aortic stiffness.

Abstract: Lead ions (Pb 2+ ) contamination in drinking water, a major source of lead poisoning to the general population, is typically detected by bulky and costly laboratory analytical instrument. A mobile analytical device for rapid Pb 2+ sensing is a growing demand. Herein, we report smartphone nanocolorimetry (SNC) as a new technique to detect and quantify dissolved Pb 2+ in drinking water. Specifically, we have employed a single-step sedimentation approach by mixing a controlled quantity of chromate ion (CrO 4 2- ) to react with Pb 2+ containing solutions to form highly insoluble lead chromate (PbCrO 4 ) nanoparticles as vivid yellow precipitates. This is followed by microscopic color detection and intensity quantitation at nanoscale level using dark-field smartphone microscopy. The sum of the intensity of yellow pixels bears a highly reproducible relationship with Pb 2+ concentration between 1.37 and 175 ppb in deionized water and 5-175 ppb in city tap water. In contrast to traditional colorimetric techniques analyzing bulk color changes, SNC achieves unparalleled sensitivity by combining nanocolorimetry with dark-field microscopy and mobilized the metal ions detection by integrating the detection into the smartphone microscope platform. SNC is rapid and low-cost and has the potential to enable individual citizens to examine Pb 2+ content in drinking water on-demand in virtually any environmental setting.

88. Evolution and update on current devices for prosthetic breast reconstruction.

Abstract: Over the past decade, the leading breast reconstruction modality has shifted from autologous tissue to implants. This trend reversal is multi-factorial but includes increasing bilateral mastectomies and the more widespread acceptance of implants due to stringent quality and safety regulatory surveillance by the US Food and Drug Administration (FDA). Since 2012, the US FDA has approved several new implant styles, shapes and textures, increasing the choices for patients and surgeons. Predictable, superior aesthetic results after prosthetic breast reconstruction are attainable, but require thoughtful planning, precise surgical technique and appropriate device selection based on several different patient and surgeon parameters, such as patient desires, body mass index, breast shape, mastectomy flap quality and tissue based bio-dimensional assessment. This article briefly reviews historic devices used in prosthetic breast reconstruction beginning in the 1960s through the modern generation devices used today. We reflect on the rigorous hurdles endured over the last several decades leading to the approval of silicone gel devices, along with their well-established safety and efficacy. The various implant characteristics can affect feel and performance of the device. The many different styles and features of implants and expanders are described emphasizing surgical indications, advantages and disadvantages of each device.

Abstract: Purpose: To assess the impact of using an advanced electronic health record (EHR) on hospital quality and patient satisfaction., Method: This retrospective, cross-sectional analysis was conducted in 2012 to evaluate the association between advanced EHR use (Healthcare Information Management Systems Society [HIMSS] Stage 6 or 7 as of December 2012) and estimated process and experience of care scores for hospitals under the Medicare Hospital Value-Based Purchasing Program, using data from the American Hospital Association for 2008 to 2010. Generalized linear regression models were fit to test the association between advanced EHR use with process of care and experience of care, controlling for hospital characteristics. In a second analysis, the models included variables to account for HIMSS stage of advanced EHR use., Results: The study included 2,988 hospitals, with 248 (8.3%) classified as advanced EHR users (HIMSS Stage 6 or 7). After controlling for hospital characteristics, advanced EHR use was associated with a 4.2-point-higher process of care score (P < .001). Hospitals with Stage 7 EHRs had 11.7 points higher process of care scores, but Stage 6 users had scores that were not substantially different from those of nonadvanced users. There was no significant difference in estimated experience of care scores by level of advanced EHR use., Conclusions: This study evaluated the effectiveness of the U.S. federal government's investment in hospital information technology infrastructure. Results suggest that the most advanced EHRs have the greatest payoff in improving clinical process of care scores, without detrimentally impacting the patient experience.

Abstract: Background: Neonatal extracorporeal membrane oxygenation (ECMO) patients are particularly vulnerable to the effects of uncompensated insensible water loss resulting in hypernatraemia. There exists a long-standing relationship between hypernatraemia and varying degrees of cerebral dysfunction. The aim of this study is to explore the degree to which free water loss occurs across a commonly used ECMO oxygenator, the polymethylpentene (PMP) membrane Hilite 2400LT (Medos, Medizintechnik AG, Stolberg, Germany). The secondary aim is to assess to what extent the addition of heat and/or humidity ameliorates this water loss., Methods: An ECMO circuit consisting of a centrifugal pump and a Hilite 2400LT oxygenator was primed with crystalloid and albumin. Each experimental trial was carried out in triplicate, with gas flow rates of 1, 3 and 4.8 L/min being investigated. Fluid loss was assessed at six time points over a 24-hour period., Results: Water loss increased significantly from 1 to 3 L/min gas flow (p=0.05) and from 3 to 4.8 L/min gas flow (p=0.025). The mean water loss differences between the differing gas flow trials per L/min gas flow were non-significant (72.4 ±3.9 ml/24 hrs). The effect of heating the gas to 37 °C did not significantly alter water loss, whereas heat and humidity reduced water loss significantly (p=0.009)., Conclusions: Insensible water loss from a Hilite 2400LT oxygenator is approximately 72 ml/day per L/min gas flow over 24 hrs. Heating and humidifying the gas reduces the fluid loss significantly to approximately 8 ml/L/min gas flow over 24 hrs (p=0.009).

However, measuring esthetic outcomes using conventional two-dimensional photography has inherent limitations visualizing in three dimensions such as the face., Study Design: This study used three-dimensional imaging to visualize facial images preoperatively and postoperatively in patients undergoing Contour threadlift procedures., Methods: In patients undergoing Contour threadlift placement for midfacial rejuvenation, we prospectively imaged and photographed patients preoperatively and postoperatively. From three-dimensional images, we measured three-axis vector movement of the facial soft tissue in millimeters. Three control subjects (n = 6, bilateral sides) who had not undergone any procedure were a control group to confirm stability of the images over time. Patients also completed a questionnaire regarding their results and overall experience., Results: We studied 6 (n = 12, bilateral procedures) undergoing Contour thread placement. Analyzed images revealed a trend of midface soft tissue flattening of nasolabial contour over 3 months. Average nasolabial flattening or tightening was 2.3 mm (P > .05) in the Contour patient group. The flattening of the midfacial region returned to baseline postprocedure at 90 days. Three-dimensional imaging for the control subjects demonstrate less than 0.2 mm change (P < .05) in the nasolabial region weekly over a period of 3 weeks., Conclusions: After 3 months, patients undergoing Contour thread placement had three-dimensional imaging measurements suggesting a return to baseline values. Three-dimensional imaging may increase accuracy in quantifying change after surgery specifically with contour differences; film and digital photography capture in only two dimensions. The use of three-dimensional imaging may be applicable to numerous clinical settings.

Abstract: Most chronic wounds can be classified into three major types: pressure ulcers, venous ulcers, and diabetic ulcers. The authors propose a unifying hypothesis of chronic wound pathogenesis based on four main causative factors: local tissue hypoxia, bacterial colonization of the wound, repetitive ischemia-reperfusion injury, and an altered cellular and systemic stress response in the aged patient. Traditional strategies for the treatment of chronic wounds have shown limited success. The authors explore potential treatment regimens specifically aimed at each individual determinant of chronic wound pathogenesis. Furthermore, they explore a combined therapeutic approach that collectively targets all the components of chronic wound pathology. These innovative ideas and therapies could be of substantial interest for clinicians and researchers, while further offering significant benefit to patients with chronic wounds.

Abstract: Reconstructive surgeons focus their efforts on refining and developing techniques that optimize oncologic outcome and recreate the most natural breast mound possible. The introduction of perforator flaps offers the potential for faster and more complete recovery, while increasing technical complexity and the risks associated with microsurgical reconstruction. Despite appealing long-lasting results, autologous reconstruction is not the ideal reconstructive choice for every patient. Tissue/expander reconstruction can produce a satisfactory aesthetic outcome and is the reconstruction of choice for most women undergoing breast reconstruction. Immediate autologous reconstruction at the time of mastectomy best resembles the lost breast and favorably withstands radiation, compared with other reconstructive methods. However, the best method for integrating reconstruction and radiation is being actively debated. Placing a tissue expander at the time of mastectomy allows a skin-sparing approach and gives the patient a chance to see how she will feel with an implant. This approach avoids radiation damage to tissue reconstruction flaps, saving them for use later if necessary. This approach, using a tissue expander as a first step, is growing in popularity.

Abstract: Peroxisome proliferator-activated receptor gamma (PPARgamma) is a ligand-dependent transcription factor that belongs to the nuclear receptor family that plays a critical role in adipocyte differentiation and lipid metabolism. Here we report for the first time that PPARgamma is expressed in human renal cortical collecting ducts (CCD), segments of the nephor involved in regulation of sodium and water homeostasis via action of the epithelial sodium channel (ENaC). ENaC activity is regulated by the hormones aldosterone and insulin, primarily through co-ordinate actions on serum and glucocorticoid regulated kinase 1 (SGK1). We show that SGK1 activity is stimulated by treatment of a human CCD cell line with PPARgamma agonists, paralleled by an increase in SGK1 mRNA that is abolished by pretreatment with a specific PPARgamma antagonist, and that this leads to increased levels of cell surface ENaCalpha. Electrophoretic mobility shift assays suggest that these effects are caused by binding of PPARgamma to a specific response element in the SGK1 promoter. Our results identify SGK1 as a target for PPARgamma and suggest a novel role for PPARgamma in regulation of sodium re-absorption in the CCD via stimulation of ENaC activity. This pathway may play a role in sodium retention caused by activation of PPARgamma in man.

96. Membrane oxygenator exhaust capnography for continuously estimating arterial carbon dioxide tension during cardiopulmonary bypass.

Abstract: Typically, the standard practice for measuring the arterial blood carbon dioxide tension (PaCO2) during cardiopulmonary bypass (CPB) is to take intermittent blood samples for analysis by a bench blood gas analyzer. Continuous inline blood gas monitors are available but are expensive. A potential solution is the capnograph, which was evaluated by determining how accurately the carbon dioxide tension in the oxygenator exhaust gases (PECO2) predicts PaCO2. A standard capnograph monitoring line was attached to the exhaust port of the membrane oxygenator. During CPB, the capnograph reading and arterial blood temperature were recorded at the same time as routine arterial blood gases were taken. One hundred fifty-seven blood samples were collected from 78 patients. A good correlation was found between the PECO2 and the temperature corrected PaCO2 (r2 = 0.833, P < .001). There was also a reasonable degree of agreement between the PECO2 and the temperature corrected PaCO2 during all phases of CPB: accuracy (bias or mean difference between PaCO2 and PECO2) of -1.2 mmHg; precision (95% limits of agreement) of +/- 4.7 mmHg. These results suggest that oxygenator exhaust capnography may be a simple and inexpensive adjunct to the bench blood gas analyzer in continuously estimating PaCO2 of a clinically useful degree of accuracy during CPB.

97. Coronary artery bypass grafting: an off-pump versus on-pump review.

Abstract: There has been a proliferation in the number of coronary artery bypass grafts (CABG) being performed without the use of cardiopulmonary bypass (CPB). However, the benefits of off-pump coronary artery grafting (OPCAB) are still being determined. The aim of this retrospective review was to compare the perioperative outcomes of CPB patients with OPCAB patients and to identify the patients most likely to benefit from the off-pump procedure. We reviewed the perioperative data of all isolated CABG patients at two metropolitan hospitals for the period of August 2000 to September 2001. The two groups (OPCAB vs. CPB) were further divided into subgroups identifying patients by their predicted mortality (higher-risk and lower-risk) and the number of distal graft anastomoses received (1, 2, 3, 4, or 5). A p value less than .05 was considered significant. Out of the total of 882 patients, 46.2% were OPCAB cases. Both CPB and OPCAB groups were similar in terms of demographics and predicted risk of mortality. Intraoperatively, OPCAB patients had fewer distal graft anastomoses (2.4 +/- 1.0 vs. 3.2 +/- 1.0, p < .001). Postoperatively, patients in the OPCAB group had less chest drainage (889 +/- 588 vs. 989 +/- 662 mls, p < .001), sustained fewer strokes (0.2 vs. 1.9%, p < .05), were transfused less (15.4 vs. 32.5%, p < .001) and were discharged earlier (7.3 +/- 5.6 vs. 8.5 +/- 9.1 days, p < .05). For higher-risk patients, OPCAB was associated with fewer reoperations for bleeding (1.3 vs. 6.4%, p < .05), a lower stroke rate (0 vs. 3.2%, p < .05), and a trend toward lower mortality (7.1 vs. 15.1%, p = .08). However, lower-risk OPCAB patients' stroke incidences (0.5% OPCAB group vs. 1.4% CPB group), and mortality rates (0.5 vs. 0.5%) were similar. Comparisons by number of grafts performed revealed that only the single-grafted OPCAB patients had statistically fewer postoperative complications, reduced chest drainage, and a shorter intensive care stay. Differences between either operation groups in transfusion rates were only statistically significant for the one to three grafted patients, while postoperative stays were similar for patients having four grafts. These results suggest that OPCAB is associated with a reduction in mortality and morbidity, particularly within the higher-risk patients. However, the benefits of OPCAB diminished with an increasing number of distal anastomoses performed.

Abstract: We have studied the diffractive dissociation into dijets of 500 GeV/c pions scattering coherently from carbon and platinum targets. Extrapolating to asymptotically high energies (where t(min)-->0), we find that when the per-nucleus cross section for this process is parametrized as sigma = sigma0Aalpha, alpha has values near 1.6, the exact result depending on jet transverse momentum. These values are in agreement with those predicted by theoretical calculations of color-transparency.

Abstract: We present the first direct measurements of the pion valence-quark momentum distribution which is related to the square of the pion light-cone wave function. The measurements were carried out using data on diffractive dissociation of 500 GeV/c pi(-) into dijets from a platinum target at Fermilab experiment E791. The results show that the /q&q> light-cone asymptotic wave function describes the data well for Q2 approximately 10 (GeV/c)(2) or more. We also measured the transverse momentum distribution of the diffractive dijets.

100. A genome-wide search for susceptibility loci to human essential hypertension.

Abstract: We undertook a systematic search of the entire human genome with the affected sibling-pair model to identify major susceptibility loci to essential hypertension. Affected nuclear families (n=263) were recruited and divided according to definite or probable genetic contribution to hypertension depending on number of hypertensive siblings. The largest nuclear families were first screened with a set of microsatellite markers. Regions on the genome with P<0.05 were tested against the second set of smaller families. An exclusion map was generated to identify regions in which hypertension-causing genes are unlikely to reside. Sibling-pair linkage analysis identified a single locus on chromosome 11q (P<0.004) in the first pass. A second pass with nuclear families that had only affected sibling pairs was, as expected, insufficient to support linkage to 11q. Multipoint exclusion-linkage analysis showed that 3 genetic loci are necessary to explain familial aggregation of essential hypertension. Our preliminary findings suggest that no single region within the human genome contains genes with a major contribution to essential hypertension. We show that the disease is indeed polygenic, with each gene providing a relatively small risk. Our exclusion map will help future investigators to concentrate on areas likely to contain these genes. The region on chromosome 11 is the first to point to a new candidate gene for hypertension that has arisen out of a genome search, but replication of these results at a higher significance is necessary before positional cloning can be justified.
