Abstract

В истории учений о производственной деятельности известны ссылки представителей институционализма на предопределяющую роль технологии в жизни общества. В работе представлен новый взгляд на проблему технологического детерминизма. Обоснована необходимость овладения общим и в то же время самым существенным представлением о технологии. Изложены первые шаги в этом направлении, что позволило вскрыть сущность производственного процесса, технологии производства, а также детерминирующую взаимосвязь технологии со свойствами промышленных материалов и продукции, с экономикой производства, с перечнем и уровнем производственных затрат, с естествознанием, с уровнем благосостояния и развития общества. In the works of representatives of institutionalism on production activities references to predetermining character of technology in the life of society occur. In the article a new view on the problem of technological determinism is presented. Also necessity of mastering a general but at the same time the most essential idea of technology is substantiated. First steps in this direction are viewed, which made it possible to reveal the nature of the production process and production technology. The article also reveals predetermining interrelation of technology with the properties of the industrial materials and products, with production economics, with the list and level of production expenses, as well as with natural history, society’s standard of well-being and level of development.

Abstract

Background: Zoledronic acid (ZOL) is an important component of therapy for patients with metastatic bone disease (MBD) to reduce the risk of skeletal-related events (SREs). We evaluated overall survival (OS) in patients with MBD secondary to solid tumours included in placebocontrolled ZOL trials. Patients and methods: Exploratory analyses were performed using databases from three randomised trials of ZOL versus placebo. 1126 patients (ZOL, n=731; placebo, n=395) with complete baseline data for 18 predefined parameters were evaluated for OS. Relative risks (RRs) with 95% confidence intervals were assessed using stratified and adjusted Cox regression models. Baseline covariates defining patient populations with significantly different effects of ZOL treatment on OS (identified by stepwise backward elimination) were included in multivariate models. Results: Although OS was similar between the overall treatment groups, ZOL significantly improved OS in the subset of patients (n=423; 38%) with elevated baseline NTX (Z100 nmol/mmol creatinine; RR, 0.692; P=.0028). Notably, this effect was independent of SRE prevention. Additional covariates associated with OS benefits with ZOL (e.g. low albumin, SRE history, elevated lactate dehydrogenase, shorter cancer duration) were characteristic of advanced disease. Conclusion: These exploratory analyses suggest a beneficial effect of ZOL on OS in patients with highly aggressive or advanced MBD. © 2013 Elsevier GmbH. 73. Clinical assessment of intralaryngeal ultrasonography.

Abstract

Objectives: To assess the clinical use of intralaryngeal ultrasonography. Study Design: A retrospective study was conducted of 16 patients with laryngeal lesions treated by laryngomicrosurgery. Methods: An intraluminal ultrasonic tomography apparatus was connected to a radial scanning 30-MHz miniaturized probe. Under general anesthesia, ultrasonic images were obtained using the filling method. Results: In cases of benign disease, such as vocal cord nodules or polyps, the layers of the mucosa could be identified. Characteristic internal echoes were noted in cases of hemorrhagic polyps, vocal cord cysts, and vocal cord cancer. In case of hemorrhagic polyps, hyperechoic regions were noted within the lesions. In cases of vocal cord cysts, internal echoes were absent, and posterior echoes were mildly enhanced. In cases of vocal cord cancer, infiltration beyond the mucosa could be visualized. Conclusions: Laryngeal lesions can be diagnosed by intralaryngeal ultrasonography using the filling method. Although it does not replace the combination of conventional endoscopy and a critical evaluation of the clinical symptoms of the individual disease, it can profitably complement them. Intralaryngeal ultrasonography can help in determining the extent of tumor involvement during microscopic laser surgery performed under general anesthesia. Abstract

ANG II induces secretion and activation of transforming growth factor-beta (TGF-beta) by glomerular mesangial cells. However, the mechanisms that operate this are unclear. Thrombospondin-1 (TSP-1), which is produced by mesangial cells in damaged glomeruli, is one of several molecules known to activate the latent TGF-beta1 complex. Therefore, we examined whether the ANG II-induced activation of latent TGF-beta1 in human mesangial cells (HMC) operates via TSP-1. The addition of ANG II (1-100 nM) to HMC significantly increased TSP-1 mRNA within 6 h, followed by an increase in TSP-1 protein production as shown by Western blot analysis of cells and immunoassay of the culture supernatant. Production of ANG II-induced TSP-1 mRNA and protein was completely inhibited by an ANG II type 1 (AT 1)-receptor antagonist but was unaffected by an AT 2-receptor antagonist. Use of a TSP-1-specific blocking peptide demonstrated that the ANG II-induced activation of latent TGF-beta 1 operates via TSP-1. Next, we investigated the role of ERK1/2, p38 MAPK, and JNK in ANG II-induced TSP-1 production in HMC. The addition of the upstream ERK1/2 inhibitor PD-98059 did not affect ANG II-induced TSP-1 production, whereas addition of either the p38 MAPK inhibitor SB-203580 or the JNK inhibitor SP-600125 significantly reduced TSP-1 production. In conclusion, this study has demonstrated that ANG II-induced activation of latent TGF-beta1 in HMC operates via TSP-1. Furthermore, ANG II-induced TSP-1 production is dependent on p38 MAPK and JNK signaling.
