61. Rare cause of life-threatening hypercalcemia in an infant: a case report

Abstract

National audience; BACKGROUND: 25-Hydroxyvitamin D 24-hydroxylase (CYP24A1) deficiency is a rare cause of autosomal recessive infantile hypercalcemia due to vitamine D hypersensitivity. CASE PRESENTATION: We report the case of a 2-year-old boy who presented with severe hypercalcemia-hypercalciuria and a bilateral nephrocalcinosis. Laboratory investigations detected a collapsed parathormone and a highly elevated 1α,25-dihydroxycholecalciferol along with an increased phosphate excretion (hypophosphatemia and hyperphosphaturia). An adapted management with two courses of palmidronic acid and an eviction of vitamin D and calcium allowed to stabilize him. A homozygous p.Leu409Ser pathogenic variant on CYP24A1 gene resulting in a collapsed 25-Hydroxyvitamin D24-hydroxylase activity was found. A normal development is possible with a meticulous clinical, biological and nutritional management and monitoring. CONCLUSIONS: Vitamin D hypersensitivity is challenging during childhood, especially due to the need to avoid vitamin D while requiring a close nutritional monitoring to maintain a normal growth. Biomarkers such as vitamin D metabolite ratio and 24,25(OH)2D3 along with ionized calcium and nutritional management can contribute to properly follow patients with vitamin D hypersensitivity.

Abstract

In the search for more efficient low-temperature catalysts able to oxidise biorecalcitrant pollutants in water, the selective substitution of La3+ by Ti3+ cations was reported to boost the UV-A light driven activity of LaFeO3 orthoferrites as H2O2-mediated photo-CWPO catalysts. La1−xTixFeO3 catalysts with 0 < x < 0.11 were obtained by a modified Pechini sol–gel route via the addition of a solid source of titanium during the synthesis. By using a span of crystallized and amorphous TiO2, the influence of the titanium source on the substitution rate in the orthoferrite network and on the performances of the catalysts under UV-A light was demonstrated using the degradation of 4-chlorophenol as test reaction. The amorphous content of the TiO2 precursor is proposed to be the key factor driving the substitution of La3+, the largest substitution of 11 % being obtained using a dried sol-gel TiO2 precursor. We suggest that the substitution proceeds during the thermal treatment via a solid-solid diffusion between TiO2 and the amorphous LaFeO3 before it crystallises. The catalyst robustness was influenced by the TiO2 nature. Small size Ti-LaFeO3 crystallites strongly lowered or blocked the Fe release, while no improvement was observed for large size crystallites vs. the pristine material, what was associated to a poor homogenization of the titanium in the network. We showed that the mineralization activity was proportional to the substitution rate of La1−xTixFeO3 catalysts, while a similar relationship could be drawn for the degradation activity only in the case of highly robust catalysts with pure heterogeneous surface reactions. © 2023 Elsevier B.V.

Abstract

This special issue is the second published after the Global Symposium on Soil Erosion (GSER, 15-17 May 2019, Rome, Italy) and includes contributions dealing with the 2nd theme of the GSER: Policies and practices in action to address soil erosion. While there is a good scientific understanding of the physical measures that can be used to prevent or mitigate soil erosion, the main constraints to progress often relate to policy development and or implementation as well as socio-economic aspects that provide limitations to implementation of sustainable soil management (SSM) practices including those directed to control erosion. There are no right or wrong answers to which policy or approach is most effective. Some combination of approaches needs to be adopted that work in the particular political, cultural, and socio-economic environment under consideration. The papers included in this special issue provide examples, from the national to local level, that could be adapted, or used, to improve uptake and implementation of SSM practices to prevent or reduce soil erosion. Regardless of what policy or plan is developed there has to be effective interaction with the local farmers and land managers as they are key to implementing any actions that will make a practical difference on the ground. Effective policies cannot be developed or implemented without bringing the land managers “on board” and the needs and limitations of the local farmers must be thoroughly understood and considered in any policy or plan development.
