OBJECTIVE To describe the sociodemographic profile and analyze the migratory characteristics of the members of the Residency Programs in Family Medicine in 2020 in Brazil. METHODS The study follows a cross-sectional observational design of a quantitative nature from the perspective of the members of the Residency Programs in Family Medicine. Questionnaires adapted for each participating group were developed, applied through an online platform. RESULTS Most participants are female and white. Most supervisors and preceptors were residents of Residency Programs in Family Medicine, however, there are some who are not specialists in the field. Most participants are based in capitals or metropolitan regions. In relation to retention, 41.1% of supervisors and 73.1% of preceptors are affiliated to a program in the same municipality where they lived. For most resident physicians, the place of residence coincides with the place of birth and/or graduation (57.4%), and 48.5% are in the same place of graduation. CONCLUSIONS The research reinforces the need for policies to promote the migration of residents to Residency Programs in Family Medicine outside capital cities and metropolitan regions, as well as encouraging the retention of graduates trained outside large urban centers so that they can contribute to distribution and provision of doctors where they are still needed. RESUMO OBJETIVO Caracterizar o perfil sociodemográfico e analisar as características migratórias dos integrantes dos Programas de Residência em Medicina de Família e Comunidade em 2020 no Brasil. MÉTODOS O estudo segue um delineamento observacional transversal de natureza quantitativa a partir da perspectiva dos integrantes dos Programas de Residência em Medicina de Família e Comunidade. Foram desenvolvidos questionários adaptados para cada grupo participante, aplicados por meio de plataforma on-line. RESULTADOS A maioria dos participantes é do sexo feminino e de cor branca. A maioria dos supervisores e preceptores foi residente de Programas de Residência em Medicina de Família e Comunidade, contudo, há alguns que não são especialistas na área. A maior parte dos participantes está vinculada às capitais ou regiões metropolitanas. Em relação à fixação, 41,1% dos supervisores e 73,1% dos preceptores estão vinculados a um programa no mesmo município onde foram residentes. Para a maioria dos médicos residentes, o local da residência coincide com o local de nascimento e/ou graduação (57,4%), sendo que 48,5% estão no mesmo local de graduação. CONCLUSÕES A pesquisa reforça a necessidade de políticas de promoção da migração de residentes para Programas de Residência em Medicina de Família e Comunidade fora das capitais e regiões metropolitanas, bem como estimula a fixação dos egressos formados fora dos grandes centros urbanos para que eles possam contribuir com a distribuição e com o provimento de médicos onde ainda é necessário.

The sodium (Na+)/hydrogen (H+) exchanger 3 (NHE3) is one of the most important Na+/H+ antiporters in the small intestines of the gastrointestinal tract and the proximal tubules of the kidney. The roles of NHE3 in the regulation of intracellular pH and acid–base balance have been well established in cellular physiology using in vitro techniques. Localized primarily on the apical membranes in small intestines and proximal tubules, the key action of NHE3 is to facilitate the entry of luminal Na+ and the extrusion of intracellular H+ from intestinal and proximal tubule tubular epithelial cells. NHE3 is, directly and indirectly, responsible for absorbing the majority of ingested Na+ from small and large intestines and reabsorbing >50% of filtered Na+ in the proximal tubules of the kidney. However, the roles of NHE3 in the regulation of proximal tubular Na+ transport in the integrative physiological settings and its contributions to the basal blood pressure regulation and angiotensin II (Ang II)-induced hypertension have not been well studied previously due to the lack of suitable animal models. Recently, novel genetically modified mouse models with whole-body, kidney-specific, or proximal tubule-specific deletion of NHE3 have been generated by us and others to determine the critical roles and underlying mechanisms of NHE3 in maintaining basal body salt and fluid balance, blood pressure homeostasis, and the development of Ang II-induced hypertension at the whole-body, kidney, or proximal tubule levels. The objective of this invited article is to review, update, and discuss recent findings on the critical roles of intestinal and proximal tubule NHE3 in maintaining basal blood pressure homeostasis and their potential therapeutic implications in the development of angiotensin II (Ang II)-dependent hypertension.

El exceso de grasa visceral constituye un factor de riesgo independiente y se correlaciona más fuertemente con los cambios metabólicos cuando se compara con la grasa abdominal total o subcutánea. El objetivo del estudio fue desarrollar y validar una ecuación predictiva para estimar el tejido adiposo visceral (TAV) a partir de parámetros antropométricos y bioquímicos. Estudio metodológico desarrollado a partir de datos de un diseño transversal involucrando individuos de ambos sexos, con edad ≥20 años, donde se evaluó el IVA (por tomografía computarizada); parámetros antropométricos: índice de masa corporal (IMC), circunferencia de la cintura (AC), circunferencia de la cadera (HC), circunferencia del cuello (NC) y circunferencia del muslo (WCx); bioquímicos: glucosa en ayunas (FG), hemoglobina glucosilada (HbA1C), triglicéridos (TG), colesterol total (TC) y fracciones, proteína C reactiva (PCR) y ácido úrico (AU); y datos demográficos: sexo, edad y raza. Se evaluaron un total de 160 individuos con edades entre 20 y 80 años y una media de 53,2±13,9 años. La ecuación predictiva desarrollada para estimar el TAV en hombres fue TAV= -2307.8 + 5.8 (edad) -34.1(raza) + 722.3 (talla) + 24.2(PC) - 3.2( GJ) + 52.8(HbA1c) + 29.3(AU) ( R2 ajustado: 67,6%). Para mujeres, TAV = - 109,0 + 1,1 (edad) - 2,9 (HC) + 0,3 (TG) + 5,8 (IMC) + 118 (PC) (R2 ajustado: 48,1 %). La capacidad predictiva de las ecuaciones desarrolladas en relación con el VAT determinado por tomografía computarizada fue del 64,1% y 49,6% para hombres y mujeres, respectivamente. Las ecuaciones propuestas y validadas pueden ser utilizadas para estimar el VAT cuando no se disponga de pruebas de imagen, siendo una herramienta de más fácil aplicabilidad y mayor practicidad

Considera-se, no presente trabalho, a voz como parte da comunicação e meio de fundamentação lingüística da criança nas relações sociais. O objetivo do estudo é analisar o grupo como modalidade de intervenção fonoaudiológica junto a crianças disfônicas, bem como as possibilidades de atuação que ele propicia. O banco de dados da pesquisa corresponde a registros em vídeo e transcrições de um processo terapêutico com um grupo de seis crianças disfônicas, durante seis meses. Os resultados mostram as construções conjuntas de conhecimentos, a repercussão dos conceitos trabalhados em grupo na vida das crianças, o importante papel do terapeuta, as trocas de experiências, a dificuldade na realização de exercícios e técnicas individuais no contexto grupal e as modificações na qualidade vocal de crianças com disfonias de grau leve com o decorrer do processo. Conclui-se que o grupo é um espaço rico e promissor, importante para o desenvolvimento de noções que as crianças têm da voz e de conscientizações para hábitos saudáveis, além de proporcionar modificações na qualidade vocal das crianças Abstract: The in-group audio-phonologic work with dysphonic children is still not much diffused, there is very little literature approaching the subject. The voice is considered, here, as part of communication and means of linguistic basing of the child in the social relations. The goal of the research is to analyze the group as a kind of audio-phonologic intervention along with dysphonic children, as well as the possibilities of acting that it provides. The database of the research is the therapeutic process of a group of six dysphonic children during six months. The results show the joined constructions of knowledge, the concepts' repercussion developed in-group in the children's life, the important role of the therapist, the exchanging of experiences, the difficulty in the achievement of the exercises and individual techniques in the group context and the positive changes in the vocal quality with dysphonic of slight level during the process. It comes to the conclusion that the group is a rich and promising 'space', important for the development of notions the children have towards the voice and conscientiousness for eliminating bad habits and collection of healthy habits, besides providing positive changes regarding the children's vocal quality Mestrado Saúde da Criança e do Adolescente Mestre em Saúde da Criança e do Adolescente
