For many years now our research team has been involved in an effort (both theoretical and technological) that can be labeled as an attempt to investigate the notion of cooperation from the 'participation' or 'contribution' perspective. From our perspective, it encompasses a set of situations in which different actors identified or unidentified, ratified or not, distributed in space and time, contribute to a sometimes ill-defined collective goal, using most of the time low-overhead web-based technologies. Doing so, people participate to a collective design that aims at generating a bunch of perpetually moving collective knowledge and decisions submitted to discussion, negotiation and sometimes dismissal. To avoid the design of services as a repeated 'one-shot' process we have gradually built a transverse software infrastructure that can be used in various projects aiming to design participatory services using complex knowledge and cooperations. Thanks to this “non-disposable” infrastructure, the participatory services designed take profit from the scientific outcomes of each previous project.

- Abstract
Aim: This retrospective analysis focused on the effect of treatment with EVE/EXE in a real-world population outside of clinical trials. We examined the efficacy of this combination in terms of PFS and RR related to dose intensity (5 mg daily versus 10 mg daily) and tolerability. Methods: 163 HER2-negative ER+/PgR+ ABC patients, treated with EVE/EXE from May 2011 to March 2016, were included in the analysis. The primary endpoints were the correlation between the daily dose and RR and PFS, as well as an evaluation of the tolerability of the combination. Secondary endpoints were RR, PFS, and OS according to the line of treatment. Patients were classified into three different groups, each with a different dose intensity of everolimus (A, B, C). Results: RR was 29.8% (A), 27.8% (B) (p = 0.953), and not evaluable (C). PFS was 9 months (95% CI 7–11) (A), 10 months (95% CI 9–11) (B), and 5 months (95% CI 2–8) (C), p = 0.956. OS was 38 months (95% CI 24–38) (A), median not reached (B), and 13 months (95% CI 10–25) (C), p = 0.002. Adverse events were stomatitis 57.7% (11.0% grade 3–4), asthenia 46.0% (6.1% grade 3–4), hypercholesterolemia 46.0% (0.6% grade 3–4), and hyperglycemia 35.6% (5.5% grade 3–4). The main reason for discontinuation/interruption was grade 2–3 stomatitis. Conclusions: No correlation was found between dose intensity (5 vs. 10 mg labeled dose) and efficacy in terms of RR and PFS. The tolerability of the higher dose was poor in our experience, although this had no impact on efficacy.

Background The recurrence of hepatitis C viral infection is common after liver transplant, and achieving a sustained virological response to antiviral treatment is desirable for reducing the risk of graft loss and improving patients’ survival. Aim To investigate the long-term maintenance of sustained virological response in liver transplant recipients with hepatitis C recurrence. Methods 436 Liver transplant recipients (74.1% genotype 1) who underwent combined antiviral therapy for hepatitis C recurrence were retrospectively evaluated. Results The overall sustained virological response rate was 40% (173/436 patients), and the mean follow-up after liver transplantation was 11 ± 3.5 years (range, 5–24). Patients with a sustained virological response demonstrated a 5-year survival rate of 97% and a 10-year survival rate of 93%; all but 6 (3%) patients remained hepatitis C virus RNA-negative during follow-up. Genotype non-1 (p = 0.007), treatment duration >80% of the scheduled period (p = 0.027), and early virological response (p = 0.002), were associated with the maintenance of sustained virological response as indicated by univariate analysis. Early virological response was the only independent predictor of sustained virological response maintenance (p = 0.008). Conclusions Sustained virological response achieved after combined antiviral treatment is maintained in liver transplant patients with recurrent hepatitis C and is associated with an excellent 5-year survival.

It has been recently suggested that the risk of graft loss after liver transplantation (LT) may increase in female HCV patients. The aim of the study was to examine gender differences in HCV therapy tolerance and outcome in LT patients treated for HCV recurrence. A retrospective study was conducted on liver recipients with HCV recurrence, who were given antiviral therapy from 2001 to 2009 in 12 transplant centers in Italy. Sustained virological response (SVR), adherence-to-therapy, and side effects were evaluated. A multivariate logistic regression model was used after adjusting for possible confounders. The data regarding 342 treated patients were analyzed. SVR was reported in 38.8% of patients. At baseline, male and female did not differ in HCV viral load, histology, or rate of diabetes. SVR was lower in females than in males (29.5% vs. 42.1%; P=0.03). - Abstract
Background. Subcutaneous administration of hepatitis B immunoglobulin (HBIg) is effective in preventing hepatitis B virus (HBV) recurrence after liver transplantation, but early conversion to subcutaneous administration is undocumented. Methods. In a prospective study, patients transplanted for terminal liver disease due to HBV infection who were HBV DNA-negative at transplant were switched by week 3 posttransplantation from intravenous to subcutaneous HBIg (500 or 1000 IU weekly or fortnightly, adjusted according to serumanti-HBs trough level) if they were HBsAg- and HBV-DNA negative at time of switch. All patients concomitantly received nucleos(t)ide analogue antiviral therapy. Primary endpoint was failure rate by month 6, defined as serum anti- HBs of 100 IU/L or less or HBV reinfection despite serum anti-HBs greater than 100 IU/L. Results. Of 49 patients treated, 47 (95.9%) continued treatment until month 6. All patients achieved administration by a caregiver or self-injection by week 14. No treatment failures occurred. Mean anti-HBs declined progressively to month 6, plateauing at a protective titer of approximately 290 IU/L. All patients tested for HBV DNA remained negative (45/45). Only 1 adverse event (mild injection site hematoma) was assessed as treatment-related. Conclusions. Introduction of subcutaneous HBIg administration by week 3 posttransplantation, combined with HBV virostatic prophylaxis, is effective and convenient for preventing HBV recurrence.

Questo articolo affronta la problematica del mettere in relazione le conoscenze di base necessarie per l’insegnamento delle discipline scientifiche con le effettive competenze professionali degli insegnanti. Viene presentato uno studio sulla percezione delle competenze professionali da parte di 16 docenti esperti di Scienze e di 132 studenti delle Scuole di Specializzazione all’Insegnamento (S.S.I.S.). Seguendo i metodi dell’analisi qualitativa, sono stati analizzati diversi tipi di dati e la loro analisi ha permesso una categorizzazione delle competenze individuate dai docenti esperti nelle tre aree della classificazione di Shulman (Conoscenza Pedagogica, Conoscenza del Contenuto Disciplinare e Conoscenza Pedagogica del Contenuto). Background: Patients undergoing liver transplantation for hepatocellular carcinoma within the Milan criteria (single tumour ≤5 cm in size or ≤3 tumours each ≤3 cm in size, and no macrovascular invasion) have an excellent outcome. However, survival for patients with cancers that exceed these criteria remains unpredictable and access to transplantation is a balance of maximising patients' chances of cure and organ availability. The aim of this study was to explore the survival of patients with tumours that exceed the Milan criteria, to assess whether the criteria could be less restrictive, enabling more patients to qualify as transplant candidates, and to derive a prognostic model based on objective tumour characteristics, to see whether the Milan criteria could be expanded. Methods: Data on patients who underwent transplantation for hepatocellular carcinoma despite exceeding Milan criteria at different centres were recorded via a web-based survey completed by specialists from each centre. The survival of these patients was correlated retrospectively with the size of the largest tumour nodule, number of nodules, and presence or absence of microvascular invasion detected at pathology. Contoured multivariable regression Cox models produced survival estimates by means of different combinations of the covariates. The primary aim of this study was to derive a prognostic model of overall survival based on tumour characteristics, according to the main parameters used in the Tumour Node Metastasis classification. The secondary aim was the identification of a subgroup of patients with hepatocellular carcinoma exceeding the Milan criteria, who achieved a 5-year overall survival of at least 70%-ie, similar to the outcome expected for patients who meet the Milan criteria. Findings: Over a 10-month period, between June 25, 2006, and April 3, 2007, data for 1556 patients who underwent transplantation for hepatocellular carcinoma were entered on the database by 36 centres. 1112 patients had hepatocellular carcinoma exceeding Milan criteria and 444 patients had hepatocellular carcinoma shown not to exceed Milan criteria at post-transplant pathology review. In the group of patients with hepatocellular carcinomas exceeding the criteria, the median size of the largest nodule was 40 mm (range 4-200) and the median number of nodules was four (1-20). 454 of 1112 patients (41%) had microvascular invasion and, for those transplanted outside the Milan criteria, 5-year overall survival was 53·6% (95% CI 50·1-57·0), compared with 73·3% (68·2-77·7) for those that met the criteria. Hazard ratios (HR) associated with increasing values of size and number were 1·34 (1·25-1·44) and 1·51 (1·21-1·88), respectively. The effect was linear for size, whereas for number of tumours, the effect tended to plateau above three tumours. The effect of tumour size and number on survival was mediated by recurrence (b=0·08, SE=0·12, p=0·476). The presence of microvascular invasion doubled HRs in all scenarios. The 283 patients without microvascular invasion, but who fell within the Up-to-seven criteria (hepatocellular carcinomas with seven as the sum of the size of the largest tumour [in cm] and the number of tumours) achieved a 5-year overall survival of 71·2% (64·3-77·0). Interpretation: More patients with hepatocellular carcinoma could be candidates for transplantation if the current dual (yes/no) approach to candidacy, based on the strict Milan criteria, were replaced with a more precise estimation of survival contouring individual tumour characteristics and use of the up-to-seven criteria. Medulloblastomas (MBs) are malignant tumors of the cerebellum. They commonly affect children, no more than 30% occurring in individuals older than 16 years, and are considered embryonic tumors since they originate from precursors present during cerebellar development. In particular, MBs may derive from cerebellar granule cell precursors (GCPs), which form a layer of proliferating committed cells in the subpial surface of the cerebellum during the fetal and the postnatal period. Neuroepithelial cells of the cerebellar ventricular zone (CVZ), displaced around the midline of the neuroectodermal tube, may constitute the other cells of origin of MBs.1 The idea that MBs have a double origin is molecularly supported by the expression of the neurotrophin receptor p75NTR,2 the calcium-binding protein calbin-din-D28k,3 and the human atonal homolog 1 (ATOH1) gene in distinct subsets of these tumors. ATOH1 is a basic helix-loop-helix (bHLH) transcription factor (TF), and its mouse homolog, Math1, is exclusively expressed in proliferating GCPs during cerebellar development. Math1 expression seems necessary to uncommitted precursors of the CVZ to differentiate first as GCPs and finally as postmitotic, cerebellar granule cells.4 ATOH1 is expressed only in a subgroup of MBs, suggesting that not all these tumors originate from the GCPs.5–7 Interestingly, MBs with ATOH1 transcript are commonly located in the hemispheres rather than in the vermis, as should be expected if they effectively originate from the GCPs.8 Since MBs expressing ATOH1 afflict almost exclusively adult patients, GCPs may be the cells of origin of adult MBs. On the other hand, there is no molecular marker, functionally analogous to ATOH1, identifying MBs deriving from precursor cells of the CVZ. Indeed, calbindin-D28k, which is considered a marker of MBs originating from the CVZ, is a calcium-binding protein, and it is also expressed in mature neurons. In contrast, neurogenin-1 (NEUROG1) is a proneural bHLH TF that functions as a determinant of neuronal identity and is expressed in a subset of interneuronal precursors of the murine spinal cord in a pattern complementary to Math1.9 Its role in cerebellar development, if any, was not yet investigated, but the expression of the human homolog, NEUROG1, was detected in a subset of human MBs and associated with a poor prognosis.10 Moreover, during development of the murine brain, Ngn1 is transiently expressed, and no transcript is detectable in adult nervous tissue, reinforcing its similarity to Math1.11 A distinct histogenesis may be associated with activation of different proliferative signaling pathways, as the SHH and WNT pathways. SHH is a glycoprotein secreted from Purkinje cells during development of the cerebellar cortex that interacts with the transmembrane PTCH receptor expressed by GCPs, stimulating their proliferation. PTCH mutations were found in sporadic MBs and in Gorlin syndrome, an autosomal-dominant cancer syndrome characterized by the propensity to develop multiple neoplasms, including basal cell carcinomas and MBs. The final common effector of the SHH-dependent signaling pathway is TF GLI1.12 In murine models, Gli1 plays an important role in the formation of MBs,13 even though its alteration does not appear to be a general requirement.14 On the other hand, the role of the SHH pathway in the development of neuroepithelial cells of the CVZ is unclear.15,16 Since MB cells conserve many characters of their cells of origin, it is likely that the SHH pathway does not play a relevant role in MBs originating from progenitors of the CVZ. The WNT pathway, on the contrary, may play a pivotal role in the pathogenesis of this latter subgroup. Dysregulation of the WNT-dependent signaling pathway has been involved in sporadic MBs and in Turcot syndrome, another autosomal-dominant cancer syndrome characterized by malignant tumors of the CNS associated with familial polyposis of the colon.17 Many WNT proteins and corresponding human frizzled-homolog receptors (FZDs) have been described, but much less is known about their role in the development of the human cerebellum. The WNT pathway is activated when WNT ligands bind to FZDs, causing stabilization of β-catenin and its translocation into the nucleus. In the nucleus, β-catenin activates transcription of a number of oncogenic targets, including MYC, although transcriptional activation by a β-catenin– independent pathway may be a mechanism for MYC overexpression in a subset of MBs.18 Here, we examined the mRNA expression of NEUROG1 in human MBs that were characterized for the expression of ATOH1, and the mRNA expression of Ngn1 in mouse embryonal cerebellum. In addition, we investigated the mRNA expression of GLI1 as marker of the SHH pathway activation, and nuclear β-catenin staining, β-catenin mutations, and/or mRNA expression of MYC as indicators of the WNT pathway activation. A link between the activation of these pathways and the mRNA expression of ATOH1 and NEUROG1 was assessed. Finally, in a smaller series of human MBs, we analyzed mRNA expression of OTX2, a TF that has been shown to be a further positive marker for MBs that originated from GCPs.19
