Abstract

Recently biotechnologic progress has, through the technique of the recombinant DNA, allowed a low cost production of large amount of several growth factors. Such a large availability has made possible to either carry out deeper investigations on the physiopathology of the hemopoietic regulation and perform new therapeutic approaches under different pathologic conditions. The most interesting acquisition concerning the biology of hemopoiesis to which such investigations have addressed us is the inadequacy of the protocols adopted till now. Such protocols considered only a simple vision of an elective action of a given growth factor during an exact maturation period of a determined cell colony. On the contrary, it was possible to point out a close network of inter-relationship among the different factors, which sometime impedes a clear distinction for each single factor, between actions of competence and progression in the cell maturation phenomena. However, the present uncertainty pertaining to the regulation of hemopoiesis has not impede the performance of clinical trials with positive findings in several pathologic conditions. The administration of recombinant erythropoietin has for example allowed to intervene in a resolutive way on the anemia in uremic subjects, and seems giving satisfactory results also in subjects with non renal origin anemic conditions. Satisfactory results were also obtained through the use of the Granulocyte-Macrophage CSF and of the Granulocyte-CSF, which by preventing neutropenia have allowed the performance of more adequate chemotherapeutic protocols in neoplastic subjects. New interesting perspectives are now coming for the use of Interleukin-3 in the treatment of the aplastic anaemia.

Abstract

The behaviour of aldosterone output was evaluated in isolated and superfused bovine adrenal glands during superfusion with human alpha atrial natriuretic peptide on its own or with angiotensin II or a antagonist dopaminergic drug: metoclopramide. H alpha-ANP even in high concentrations did not reduce the basal amount of aldosterone released from bovine adrenal glands, nor did it modify aldosterone response to metoclopramide, but it partially inhibited aldosterone stimulation by angiotensin II. These data suggest that atrial natriuretic factor may affect sodium secretion through the modulation of aldosterone secretion.

Abstract

Administration of ACTH beta 1-24 (0.25 mg, i.m.) in six healthy subjects reduces blood prolactin response to sulpiride (100 mg, i.m.). Dexamethasone (2 mg, orally, given at 11.00 p.m. of preceding day) blocks blood prolactin response to insulin-induced hypoglycemia (0.20 U. kg of "Actrapid" Novo, i.v.) in six voluntary healthy subjects. Intravenous naloxone (0.4 mg/Kg of body weight) blocks in man blood prolactin response to insulin-induced hypoglycemia. On the basis of the above data we can hypothesize that ACTH and dexamethasone have influenced by a feed-back mechanism the lypocorticotropic system. Such system by beta-endorphin release can modulate the secretion of prolactin. Thus inhibition of the lipocorticotropic system may be responsible for the reduced response of the prolactin system to stimulation by sulpiride or hypoglycemic stress.

Abstract

Angiotensin II increases aldosterone production of isolated and superfused bovine adrenal glands. Indomethacin shows a bi-phasic effect on the production of the above-mentioned hormone, inhibitory at low doses (0,2 microgram/ml), stimulatory at high doses (5.0 microgram/ml). Preincubation with this drug impedes the increase of aldosterone production induced by angiotensin II. It is likely that the steroidogenetic effect of angiotnesin II is carried out through the PGs.

The authors studied the blood aldosterone, cortisol and kalium levels, the plasma renin activity and the aldosterone urinary excretion following L-DOPA (0.50 g per os) administration in seven patients affected by hypertension and secondary hyperaldosteronism. The results suggest that L-DOPA administration, by stimulating the dopaminergic biosynthesis. Nevertheless such a conclusion needs further investigations.

Abstract

Atrial natriuretic peptide (ANP) was assayed on the 1st, 3rd, 7th and 10th day of extrauterine life and the levels of the hormone were checked after the 2nd month in 12 infants. After an initially rapid increase, with a peak of the 3rd day (45.7 +/- 7 pg/ml the 1st day of extrauterine life and 175 +/- 5 pg/ml on the 3rd day), we can recognise a progressive decrease until the measurement made at 2 months (61.5 +/- 5 pg/ml). The reported data show the important role of ANP in adaption of life after birth. This can be considered as a compensatory response because it represents an attempt to maintain the balance of positive sodium which is essential for the growth of the newborn.

Abstract

The Authors examined the behaviour of the humeral arterial pressure, of the heart frequency and of the muscular blood flow in inferior limbs. This study was performed before and after treatment with Captopril (SQ 14225), 100 mg per os, a selective inhibitor of the converting enzyme. Both resting blood flow (RBF) and maximal blood flow (NBF) were studied by Xenon 133 clearance. The A.A. observed that the drug induced a significant (P less than 0.01) increase of both RBF and MBF. Therefore, the Authors hypothesize that the Captopril is able to produce peripheral vasodilatation, without any significant variation of the cardiac index and frequency. Such an effect is considered to depend not only upon the inhibited angiotensin II production, but also upon stimulation of bradikinins and prostaglandins.

The effect of intracerebroventricular (i.c.v.) injection of synthetic rat atrial natriuretic polypeptide (alpha-rANP) on drinking behavior was studied in normotensive rats, alpha-rANP (0.2, 0.4 or 0.8 micrograms in 5 microliter) caused a dose-dependent dipsogenic effect which was abated by i.c.v. pretreatment with saralasine (9 micrograms in 5 microliter). These results suggest that alpha-rANP possesses dipsogenic effects in water repleted rats and that brain angiotensin is involved. In addition, our data indicate that, at least as far as the effect of cerebral ANP is concerned, there are some differences between alpha-rANP and human atrial natriuretic polypeptide.

SUMMARY The effect of L-dopa (0·50 g, orally) on serum aldosterone, cortisol and potassium levels, plasma renin activity, and on urinary aldosterone excretion has been studied in normotensive subjects, before and after dexamethasone-induced ACTH suppression. L-dopa caused a further reduction in the levels of aldosterone but not cortisol, in the dexamethasone treated subjects. It is therefore suggested that the inhibitory action of L-dopa on aldosterone secretion is mediated by the dopaminergic system.

Abstract

Acetylcholine in the central nervous system appears to be involved in some aspects of hypertension. Clonidine and methyldopa may inhibit acetylcholine (Ach) release in several brain areas. The present study was therefore designed to determine whether a depletion of brain Ach could modify the antihypertensive effect of clonidine in freely moving spontaneously hypertensive (SHR), Grollman hypertensive (GHR) and DOCA-salt hypertensive (DHR) rats. Intravenous injection of clonidine (15, 30 and 75 micrograms/kg) reduced mean arterial pressure (MAP) and heart rate (HR) in all hypertensive animals. The hypotensive effect was more marked in SHR than in DHR and GHR. The effect was also reproducible when the drug dose was repeated 3 h later in rats pretreated with saline (5 microliters) in the lateral cerebral ventricle (i.c.v.). When clonidine administration was repeated in hypertensive animals 3 h after i.c.v. hemicholinium-3 (20 micrograms/5 microliters), the decrease in MAP and HR was significantly reduced compared with that observed in the same animals after the first injection. The data suggest that the antihypertensive effect of clonidine depends partially upon the integrity of central cholinergic neurons.

We studied the effects of infusion of dopamine (4 microgram/kg/min for 120 min) alone or with domperidone infusion (235 microgram/min for 120 min) on aldosterone secretion before and after stimulation by ACTH (0,5 mg i.v.) in normal subjects. We also studied the effects, of pimozide (12 mg p.o.), of domperidone infusion (235 microgram/min for 120 min) before and after stimulation with ACTH (0,5 mg i.v.) on aldosterone secretion in two other sets of normal subjects. The infusion of dopamine does not modify the basal secretion of aldosterone whereas in other trials it caused reduction (P less than 0,05) of blood aldosterone response to ACTH versus controls. Domperidone and pimozide, D2 antagonist, do not modify the aldosterone secretion before and after ACTH stimulation. Furthermore domperidone does not remove inhibition of dopamine on aldosterone response to ACTH. We can exclude an action of central or peripheral D2 dopaminergic receptors in aldosterone secretion in man. Dopamine, which does not cross the blood brain barrier, reduced the blood aldosterone response to ACTH; for those reasons we can hypothesize that receptors D1, stimulated by dopamine modulate aldosterone secretion.

Abstract

PRA (in the supine and erect subject) and urinary sodium before and after acute treatment (100 mg i.m.) and chronic treatment (100 mg per os for 8 days) were examined in two groups of subjects with no signs of cardiovascular or renal disease. It was found that indomethacin, which is a well-known inhibitor of prostaglandin synthesis, reduced PRA levels in basal conditions and as a result of postural stimulation, without causing any appreciable change in urinary sodium. Regulation of the PRA on the part of the renal prostaglandins is postulated and its probable mechanisms discussed.

Abstract

The role of PGA on the release of renin in the anesthetized dog has been studied. The infusion of prostaglandin for 60 minutes at a dose unable to cause pressor modifications (2 microgram/kg/min) was shown to increase the plasma renin activity (PRA); this effect was inhibited by propranolol and strengthened by aminophylline.

Abstract

The correlation between the alterations of free intracellular calcium concentrations and the essential arterial hypertension has been largely investigated. Calmodulin, a cytoplasmic protein with low molecular weight, is one of the factors known to be able to affect the activity of calcium-dependent enzymes. The authors have investigated the effect of calcium and calmodulin on the plasmatic membrane of intact erythrocytes in a group of patients with essential arterial hypertension. To this purpose, the ionophor A23187, propranolol at low concentrations and a few calcium channel blocking drugs, alone or associated with calmodulin have been used. The results demonstrate that calmodulin, capable of blocking calcium outside the cell, can exert its effect only when propranolol is also present in the erythrocytes of normotensive but not in the hypertensive patients. The authors discuss some pathogenetic hypotheses.

Abstract

Purpose: Indole-3-acetic acid is a protein-bound indolic uremic toxin deriving from tryptophan metabolism. Increased levels are associated with higher thrombotic risk and both cardiovascular and all-cause mortality. An emerging biomarker of cardiovascular disease is the monocyte-to-high-density lipoprotein ratio (MHR). The main purpose of this study was to investigate the association of indole-3-acetic acid with MHR and other markers of cardiovascular risk in patients with chronic kidney disease (CKD)., Methods: We enrolled 61 non-dialysis CKD patients and 6 dialysis patients. Indole-3-acetic acid levels were measured with ELISA technique., Results: In the whole cohort of 67 patients, indole-3-acetic acid was directly related to Ca × P (ρ = 0.256; P = 0.0365) and MHR (ρ = 0.321; P = 0.0082). In the 40 patients with previous cardiovascular events, indole-3-acetic acid correlated with uric acid (r = 0.3952; P = 0.0116) and MHR (ρ = 0.380; P = 0.0157). MHR was related with fibrinogen (ρ = 0.426; P = 0.0010), arterial hypertension (ρ = 0.274; P = 0.0251), C-reactive protein (ρ = 0.332; P = 0.0061), gender (ρ = - 0.375; P = 0.0017; 0 = male, 1 = female), and CKD stage (ρ = 0.260; P = 0.0337). A multiple regression analysis suggested that indole-3-acetic acid might be an independent predictor of MHR., Conclusion: This study shows a significant association between indole-3-acetic acid and MHR. Prospective studies are required to evaluate if decreasing indole-3-acetic acid concentrations may reduce MHR levels and cardiovascular events and improve clinical outcomes., (© 2022. Abstract

Purpose: To evaluate the relation between CD34 + cells count in maternal blood and potential development of fetal congenital renal abnormalities., Materials and Methods: We enrolled 16 women that gave birth to newborns carrying congenital renal malformations over a 3-year period and 48 women with uncomplicated pregnancies (controls) in a 1:3 ratio (three controls per case)., Results: CD34 + cells in the maternal peripheral blood were significantly lower in the group of women who gave birth to newborns carrying congenital renal malformations compared to the controls ( p  < .0001)., Conclusions: CD34 + cells in maternal blood could be validated as a potential marker to predict the development of possible kidney malformations.

Abstract

Background: Granulomatous interstitial nephritis in sarcoidosis (sGIN) is generally clinically silent, but in <1% causes acute kidney injury (AKI)., Methods: This Italian multicentric retrospective study included 39 sarcoidosis-patients with renal involvement at renal biopsy: 31 sGIN-AKI, 5 with other patterns (No-sGIN-AKI), 3 with nephrotic proteinuria. We investigate the predictive value of clinical features, laboratory, radiological parameters and histological patterns regarding steroid response. Primary endpoint: incident chronic kidney disease (CKD) beyond the 1°follow-up (FU) year; secondary endpoint: response at 1°line steroid therapy; combined endpoint: the association of initial steroid response and outcome at the end of FU., Results: Complete recovery in all 5 No-sGIN-AKI-patients, only in 45% (13/29) sGIN-AKI-patients (p=0.046) (one lost in follow-up, for another not available renal function after steroids). Nobody had not response. Primary endpoint of 22 sGIN-AKI subjects: 65% (13/20) starting with normal renal function developed CKD (2/22 had basal CKD; median FU 77 months, 15-300). Combined endpoint: 29% (6/21) had complete recovery and final normal renal function (one with renal relapse), 48% (10/21) had partial recovery and final CKD (3 with renal relapse, of whom one with basal CKD) (p=0.024). Acute onset and hypercalcaemia were associated to milder AKI and better recovery than subacute onset and patients without hypercalcaemia, women had better endpoints than men. Giant cells, severe interstitial infiltrate and interstitial fibrosis seemed negative predictors in terms of endpoints., Conclusions: sGIN-AKI-patients with no complete recovery at 1°line steroid should be treated with other immunosuppressive to avoid CKD, in particular if males with subacute onset and III stage-not hypercalcaemic AKI., Competing Interests: Special thanks to Jacqueline Rodriguez, who revised the manuscript for English language, and to Claudia Giuliani, for graphic support.
