Abstract

BackgroundOtitis media (OM) susceptibility has significant heritability; however, the role of rare variants in OM is mostly unknown. Our goal is to identify novel rare variants that confer OM susceptibility.MethodsWe performed exome and Sanger sequencing of >1000 DNA samples from 551 multiethnic families with OM and unrelated individuals, RNA-sequencing and microbiome sequencing and analyses of swabs from the outer ear, middle ear, nasopharynx and oral cavity. We also examined protein localisation and gene expression in infected and healthy middle ear tissues.ResultsA large, intermarried pedigree that includes 81 OM-affected and 53 unaffected individuals cosegregates two known rare A2ML1variants, a common FUT2variant and a rare, novel pathogenic variant c.1682A>G (p.Glu561Gly) within SPINK5(LOD=4.09). Carriage of the SPINK5missense variant resulted in increased relative abundance of Microbacteriaceae in the middle ear, along with occurrence of Microbacteriaceae in the outer ear and oral cavity but not the nasopharynx. Eight additional novel SPINK5variants were identified in 12 families and individuals with OM. A role for SPINK5in OM susceptibility is further supported by lower RNA counts in variant carriers, strong SPINK5 localisation in outer ear skin, faint localisation to middle ear mucosa and eardrum and increased SPINK5expression in human cholesteatoma.ConclusionSPINK5variants confer susceptibility to non-syndromic OM. These variants potentially contribute to middle ear pathology through breakdown of mucosal and epithelial barriers, immunodeficiency such as poor vaccination response, alteration of head and neck microbiota and facilitation of entry of opportunistic pathogens into the middle ear.

63. Genome-wide association analysis reveals variants on chromosome 19 that contribute to childhood risk of chronic otitis media with effusion

Abstract

To identify genetic risk factors of childhood otitis media (OM), a genome-wide association study was performed on Finnish subjects, 829 affected children, and 2118 randomly selected controls. The most significant and validated finding was an association with an 80 kb region on chromosome 19. It includes the variants rs16974263 (P = 1.77 x 10(-7), OR = 1.59), rs268662 (P = 1.564 x 10(-6), OR = 1.54), and rs4150992 (P = 3.37 x 10(-6), OR = 1.52), and harbors the genes PLD3, SERTAD1, SERTAD3, HIPK4, PRX, and BLVRB, all in strong linkage disequilibrium. In a sub-phenotype analysis of the 512 patients with chronic otitis media with effusion, one marker reached genome-wide significance (rs16974263, P = 2.92 x 10(-8)). The association to this locus was confirmed but with an association signal in the opposite direction, in a UK family cohort of 4860 subjects (rs16974263, P = 3.21 x 10(-4), OR = 0.72; rs4150992, P = 1.62 x 10(-4), OR = 0.71). Thus we hypothesize that this region is important for COME risk in both the Finnish and UK populations, although the precise risk variants or haplotype background remain unclear. Our study suggests that the identified region on chromosome 19 includes a novel and previously uncharacterized risk locus for OM.

Abstract

Tutkimuksen tarkoitus: Tarkoituksena oli tehdä kirjallisuuskatsaus suunielun syövistä, jossa tauti kuvaillaan pääpiirteittäin. Lisäksi viime vuosina Helsingissä hoidetuista suunielun syöpäpotilaista tehtiin analyysi, jonka tavoitteena oli analyysin perusteella kuvata HPV-positiivisen ja toisaalta HPVnegatiivisen suunielun syöpää sairastavan potilaan taudinkuva, riskitekijät, sekä levinneisyyteen vaikuttavat seikat. Tässä tutkimuksessa HPV-infektion yhteyttä suunielun syöpään on arviotu p16ilmentymisen perusteella, sillä HPV-infektioon liittyy usein p16-yliekspressio. Materiaalit ja menetelmät: Kirjallisuuskatsausta varten etsittiin PubMedistä artikkeleita aiheeseen liittyen, käyttäen esimerkiksi hakusanoja ”HPV” ja ”Oropharyngeal cancer”. Analyysia varten kerätty potilasaineisto koostuu HUS Korva-, nenä- ja kurkkutautien klinikan tuumorimeetingeissä 1.3.2012 – 28.2.2014 diagnostisoiduista uusista suunielusyöpäpotilaista. Potilastietoja kerätään sähköisistä hoitokertomustiedoista, sekä HUSLAB Patologian laitoksen rekistereistä. Aineiston perusteella tehtiin kolme taulukkoa: potilaan kliiniset piirteet (esimerkiksi tupakointi, oireet) tuumorin p16 ilmentymisen mukaan, tuumorin ominaisuudet (esimerkiksi TNMluokitus) p16 ilmentymisen mukaan ja potilaan ensimmäinen suunniteltu hoito tuumorin p16 ilmentymisen mukaan. Tulokset: Potilaista (N=126) suurin osa oli miehiä ja kaikkien potilaiden keski-ikä oli 62.4 vuotta. P16-värjäys tehtiin 110:lle ja niistä positiivisia oli 80. Tupakoimattomilla potilailla todettiin ilmentyvän enemmän p16-postiivisia tuumoreita. Niillä potilailla, joilla alkoholin suurkulutusrajat täyttyivät, todettiin olevan vähemmän p16-positiivisia tuumoreita. P16-positiivisen tuloksen omaavilla potilailla enemmistöllä oireiden laatu oli patti kaulalla ja p16-negatiivisilla potilailla enemmistöllä oireena oli kipu. P16-negatiivisen tuumorin omaavilla potilailla etäpesäkkeiden esiintymistä alueellisissa imusolmukkeissa (N-luokitus) vaikutti olevan vähemmän

Abstract

Conclusion This is the first controlled study of balloon sinuplasty's long-term efficacy with the follow-up time over 5 years. The results are in accordance with a previous 2-year-follow-up study. Both techniques retained the efficacy and patient satisfaction on average 6 years after the surgery. Background Endoscopic sinus surgery (ESS) and balloon sinuplasty are considered as a treatment for chronic rhinosinusitis (CRS) after a failure of conservative therapy. High cost and lack of long-term follow-up studies restrain the use of balloon sinuplasty. Objective The aim of this study was to compare long-term efficacy and satisfaction in CRS patients who had undergone maxillary sinus operation with either balloon sinuplasty or ESS technique. Previous or additional sinonasal operations were exclusion criteria. Materials and methods Study patients were recruited from 208 CRS-patients who underwent either ESS or balloon sinuplasty. Patients with nasal polyposis (gradus >= 2), previous sinonasal surgery, unilateral disease, or immune deficiency were excluded. Altogether 45 patients in the ESS group and 40 patients in the balloon group were included. Of these, 30 and 28, respectively, answered to a phone interview held on average 6 years after primary surgery. Symptom reduction and long-term satisfaction were evaluated by using symptom scores of 19 parameters altogether. Results Both groups experienced improvement in symptoms and were equally satisfied with the operation. The number of patient-reported acute exacerbations was higher among the balloon dilated patients. Also, the reduction of thick nasal discharge was less evident in the balloon sinuplasty group. Four patients in the balloon sinuplasty group underwent revision surgery. There were no revisions in the ESS group.

Tietojärjestelmätiede, kandidaatin tutkielma Ohjaaja: Leppänen, Mauri Viime vuosikymmenen aikana ohjelmistokehityksessä on tapahtunut suuria edistysaskeleita. Erityisesti ketterät kehittämismenetelmät ovat kasvattaneet suosiotaan. Yksi tärkeimmistä ketterien menetelmien periaatteista on asiakkaan tarpeiden tyydyttäminen. Lisäksi tuotteen käyttöliittymän tulee olla helposti käytettävä. On kuitenkin todettu, että ketterät menetelmät eivät välttämättä itsessään kykene varmistamaan ohjelmistojen käytettävyyttä. Alan kirjallisuudessa on tutkittu tapoja, joilla voitaisiin yhdistää ketterän kehittämisen ja käyttäjäkeskeisen suunnittelun parhaimpia puolia. Tämän tutkielman tarkoituksena on muodostaa lukijalle käsitys ketterästä kehittämisestä, käyttäjäkeskeisestä suunnittelusta sekä niiden integroimisesta. Työssä keskitytään yleisimpiin ketteriin menetelmiin, Scrumiin ja XP:hen, sekä niiden prosesseihin ja rooleihin. Tutkielman tuloksia voidaan hyödyntää sellaisissa ohjelmistoprojekteissa, joissa ketterien menetelmien lisäksi halutaan korostaa käytettävyyden näkökulmaa.

Abstract

Background Predisposition to childhood otitis media (OM) has a strong genetic component, with polymorphisms in innate immunity genes suspected to contribute to risk. Studies on several genes have been conducted, but most associations have failed to replicate in independent cohorts. Methods We investigated 53 gene polymorphisms in a Finnish cohort of 624 cases and 778 controls. A positive association signal was followed up in a tagging approach and tested in an independent Finnish cohort of 205 cases, in a British cohort of 1269 trios, as well as in two cohorts from the United States (US); one with 403 families and the other with 100 cases and 104 controls. Results In the initial Finnish cohort, the SNP rs5030717 in the TLR4 gene region showed significant association (OR 1.33, P=.003) to OM. Tagging SNP analysis of the gene found rs1329060 (OR 1.33, P=.002) and rs1329057 (OR 1.29, P=.003) also to be associated. In the more severe phenotype the association was stronger. This finding was supported by an independent Finnish case cohort, but the associations failed to replicate in the British and US cohorts. In studies on TLR4 signaling in 20 study subjects, the three-marker risk haplotype correlated with a decreased TNF alpha secretion in myeloid dendritic cells. Conclusions The TLR4 gene locus, regulating the innate immune response, influences the genetic predisposition to childhood OM in a subpopulation of patients. Environmental factors likely modulate the genetic components contributing to the risk of OM.

72. Tumorvolumen als prognostischer Marker bei p16-positiven und p16-negativen Patienten mit Oropharyngealkarzinom unter Therapie mit definitiver intensitätsmodulierter Strahlentherapie.

Abstract

Purpose: To investigate the impact of primary gross tumor volume (pGTV) and nodal gross tumor volume (nGTV) in oropharyngeal squamous cell carcinoma (OPSCC) and the difference in their role between human papillomavirus (HPV)-positive and HPV-negative patients.Methods: The patient cohort consists of 91 OPSCC patients treated with definitive radiochemotherapy or radiotherapy using intensity-modulated radiotherapy (IMRT). All patients had a minimum follow-up of 31 months. Volume measurements were made from computer tomography (CT) scans and HPV status was assessed by p16 immunohistochemistry. The end points were as follows: overall survival (OS), disease-free survival (DFS) and locoregional control (LRC).Results: pGTV was a significant independent prognostic factor for overall survival (OS; p = 0.020) in p16-negative patients. nGTV of p16-negative tumors had significant prognostic value in all end points in multivariate analyses. High-stage (III-IVc) p16-negative tumors were only associated with significantly poorer OS (p = 0.046) but not with poorer LRC or DFS when compared with the low-stage (I-II) tumors. nGTV of p16-positive tumors was an independent prognostic factor for DFS (p = 0.005) and LRC (p = 0.007) in multivariate analyses.Conclusion: pGTV may serve as an independent prognostic factor in p16-negative patients and nGTV may serve as an independent prognostic factor both in p16-positive and p16-negative patients treated with radiochemotherapy or radiotherapy using IMRT. Tumor volume may have an impact on selecting patients for de-escalation protocols in the future, both in p16-positive and p16-negative patients. [ABSTRACT FROM AUTHOR]

Abstract

Objectives: Oropharyngeal squamous cell carcinoma (OPSCC) is divided in two different disease entities depending on HPV involvement. We investigated differences in presenting symptoms and clinical findings in patients with HPV-positive and -negative OPSCC tumors. Methods: Altogether 118 consecutive patients diagnosed with primary OPSCC between 2012 and 2014 at the Helsinki University Hospital were included. HPV-status of the tumors was assessed by PCR detection of HPV DNA and immunostaining with p16-INK4a antibody. Results: Fifty-one (47.7%) of the patients had HPV-positive and 56 (52.3%) HPV-negative tumors. Forty- nine (49/51, 96.1%) of the HPV+ tumors were also p16+ showing high concordance. The most common presenting symptom among HPV+/p16+ patients was a neck mass (53.1%), whereas any sort of pain in the head and neck area was more frequently related to the HPV-/p16- (60.0%) group. HPV+/p16+ tumors had a tendency to locate in the tonsillar complex and more likely had already spread into regional lymph nodes compared with HPV-/p16- tumors. Smoking and heavy alcohol consumption were significantly more common among HPV-/p16- patients but also rather common among HPV+/p16+ patients. Conclusions: This analysis of symptoms and signs confirm that OPSCC can be dichotomized in two distinct disease entities as defined by HPV status. [ABSTRACT FROM AUTHOR]

Abstract

Conclusion:In terms of operation time, anesthesia method, and low complication rate, ESS and balloon sinuplasty seemed comparable. The advantages of balloon sinuplasty were shown to be shortness of sick leave, possibility to be performed as an in-office procedure, and lower adhesion formation. Background:Endoscopic sinus surgery (ESS) has been considered as a treatment of choice for persistent chronic rhinosinusitis (CRS). During the last decade balloon sinuplasty has been introduced as an alternative technique to dilate the ostium. Although balloon sinuplasty is considered relatively safe and efficient, comparative evidence of its putative intra-operative and post-operative advantages remain limited. Objectives:The aim of this retrospective controlled study was to evaluate intra-operative factors and early post-operative outcomes among CRS patients who had undergone maxillary sinus operation with either balloon sinuplasty or ESS technique. Materials and methods:Data were collected from 208 patients with CRS treated either with ESS or balloon sinuplasty during the years 2008–2010. Intra- and peri-operative factors were collected from patient records of the patients who met the inclusion criteria (n = 39 in ESS group andn = 36 in balloon sinuplasty group). Results:There was no significant difference in operation time and anesthesia method between the two groups. No complications occurred with either technique. All ESS procedures and 67% of the balloon sinuplasty procedures were done in the hospital setting, whereas 33% of the balloon sinuplasty procedures were done in the office setting. The duration of sick leave and the number of patients with adhesions were significantly higher in the ESS group compared to the balloon sinuplasty group. [ABSTRACT FROM PUBLISHER]

77. Finland's Security Solution in the Future

Abstract

Finland's current security strategy will not prove beneficial for the nation without profound and concrete changes. Nordic and EU cooperation have been presented as alternatives to NATO membership. The complexity of taking into account the public opinion, Finnish political relation and the economic relations of both to Russia and the EU is point of the equation. As an EU-member state it cannot ignore any of these factors and it has to reconsider its foreign and security policy to coordinate the upcoming decisions over the European security arrangements. This paper examines different strategic security possibilities for Finland: 1) to rely on EU resources as a primary means for national security, 2) to rely on NATO capabilities, and 3) to rely on Nordic cooperation. The solution can also be a practical combination of all three. This paper concludes with recommendations based on consequences of each option.

79. The prognostic role of blood lymphocyte subset distribution in patients with resected high-risk primary or regionally metastatic melanoma

Abstract

Udgivelsesdato: 2007-Oct, The aim of this study was to investigate whether the profile of peripheral blood lymphocyte subsets of patients with high-risk malignant melanoma is associated with prognosis. Blood samples were systematically obtained from 31 patients with high-risk melanoma eligible for the Nordic Melanoma Cooperative Group adjuvant interferon study. The frequencies of peripheral blood lymphocyte subsets were monitored by flow cytometry using CD3, CD4, CD8, CD56, and CD69 monoclonal antibodies. Patients with low proportions of CD3+CD4+CD69+ cells and of CD3+CD56+ cells before treatment had an improved disease-free survival compared to those with high proportions [77.7 vs. 16.8 mo, hazard ratio (HR) 0.25, confidence interval (CI) 0.09-0.71, P=0.005 and 77.2 vs. 16.0 mo, HR: 0.25, CI 0.086-0.73, P=0.001, respectively]. Low pretreatment levels of these cell populations also correlated with a better overall survival (79.2 vs. 22.6 mo, HR: 0.17, CI 0.05-0.52, P=0.0005 and 78.2 vs. 21.4 mo, HR: 0.2, CI 0.07-0.59, P=0.001, respectively). In the multivariate analysis both the pretreatment proportion of CD3+CD4+CD69+ cells (P=0.01, HR: 0.21, CI 0.07-0.67) and CD3+CD56+ cells (P=0.01, HR: 0.22, CI 0.062-0.65) were independent prognostic factors for overall survival. Our data show that both the proportions of CD3+CD4+CD69+ cells and of CD3+CD56+ cells seem to have a prognostic potential in the natural course of melanoma. These results need to be confirmed in larger studies.
