Acute pancreatitis is a frequent condition with a significant morbidity and mortality (approximately 20% are complicated forms). Peripancreatic collections are a very common complication. The 2012 Atlanta classification distinguishes between 4 types of peripancreatic collections. Acute peripancreatic fluid collections and pseudocysts develop after acute interstitial oedematous pancreatitis and acute necrotic collections. Walled-off necrosis develops after acute necrotizing pancreatitis. Although these collections are mostly asymptomatic, it is sometimes necessary to drain them when symptoms appear. Management by endoscopic ultrasound (EUS) is the gold standard. In this review of the literature, we will focus only on the pseudocysts' drainage. With the rapid development of therapeutic EUS, new stents have been developed: lumen apposing metal stent (LAMS). Several studies focused on their use in peripancreatic collections drainage and particularly in pseudocysts. The technical and clinical successes are very high for this type of stent, above 90% for these two criteria. However, the cost of these stents is significant. LAMS redefine the management of peripancreatic collections. However, it is necessary to manage this type of condition in a collegial manner, due to sometimes-serious complications at the time of insertion or afterwards. Their superiority seems undeniable in the management of walled-off necrosis, but as far as pseudocysts with fluid content are concerned, the debate remains open. New studies should be carried out to clarify the position of these stents and their complications, especially in the management of pseudocysts. [ABSTRACT FROM AUTHOR]

Résumé: L'hypertension portale segmentaire (HTPS) est une entité rare, compliquant principalement des maladies pancréatiques, bénignes ou malignes. Les patients sont le plus souvent asymptomatiques. La survenue de douleurs abdominales chroniques pouvant résulter de la splénomégalie et/ou de la maladie sous-jacente responsable de l'HTPS ont été décrites. Cependant, la complication majeure, et parfois fatale, de l'HTPS est l'hémorragie digestive sur rupture de varices œsophagiennes ou plus fréquemment de varices gastriques. Plusieurs modalités thérapeutiques peuvent être proposées, mais le traitement de référence reste la splénectomie. Plus récemment, des techniques de radiologie interventionnelle avec notamment des embolisations de l'artère splénique ± couplées à une embolisation splénique permettent de réaliser une « splénectomie non chirurgicale » et moins invasive. Enfin, l'endoscopie joue un rôle important dans la prise en charge de ces malades, car elle contribue d'une part au diagnostic de l'HTPS permet dans le même temps une approche thérapeutique avec également la possibilité de réaliser une hémostase endoscopique de varices par ligature élastique et/ou encollage. Left-sided portal hypertension (LSPH) is a rare entity caused by various entities, pancreatic diseases (mainly chronic pancreatitis) being the most common cause. LSPH is asymptomatic in most of the cases, but non-specific chronic abdominal pain have also been described. In some cases, LSPH may present with gastro-intestinal bleeding from ruptured oesophageal or gastric varices, which could be life-threatening. Different treatment options exist, but surgical splenectomy remains the treatment of choice as it cures the LSPH, by removing the blood outflow. Recently, interventional radiology procedures have been developed, such as the splenic artery embolization. Splenic artery embolization can be associated with a partially splenic embolization to potentialized the efficacity of the procedure, enabling a "non-surgical splenectomy", while maintaining normal immune function of spleen. Endoscopy also plays a crucial part in the therapeutic management of LSPH patients, by allowing the diagnosis of the causal lesion and its treatment (e.g., elastic band ligation for the oesophageal varices or tissues adhesive injection for gastric varices) at the same time. Résumé: L'infection de nécrose organisée pancréatique est une complication majeure des pancréatites aiguës graves, associée à une mortalité pouvant s'élever jusqu'à 20 à 30 %. Sa prise en charge est multidisciplinaire et repose sur un traitement anti-infectieux et un drainage de la nécrose préférentiellement « mini-invasif ». De nombreuses études se sont intéressées à l'écologie bactérienne des infections de nécrose pour guider le choix de l'antibiothérapie. Les séries récentes montrent une diminution des infections à bacilles Gram négatif d'origine digestive au profit de germes à Gram positif, une émergence des infections à bactéries multirésistantes et une augmentation des surinfections fongiques à Candida. Le recours trop précoce à une antibiothérapie dans le traitement de la nécrose organisée pancréatique stérile pourrait être en cause dans ce changement de spectre bactérien, et n'est actuellement pas recommandé. L'antibiothérapie doit idéalement être adaptée aux prélèvements microbiologiques réalisés lors du drainage des collections. La survenue d'une infection de coulée de nécrose pancréatique est une indication formelle au drainage. L'écho-endoscopie thérapeutique occupe une place centrale dans l'approche dite « step-up », qui privilégie en premier lieu le recours aux techniques les moins invasives. En effet, la nécrosectomie endoscopique est associée à une morbidité inférieure à celle des techniques chirurgicales. L'apparition récente des prothèses d'apposition luminale représente une évolution importante, dans la mesure où elles facilitent la procédure de drainage et sont associées à des taux élevés de succès techniques et cliniques. La prise en charge multidisciplinaire de l'infection de coulée de nécrose est donc la pierre angulaire de la gestion de cette affection grave parfois mortelle. L'antibiothérapie doit être débutée devant des arguments forts d'infection de coulée de nécrose afin d'éviter de sélectionner des bactéries avec un haut niveau de résistance ou des surinfections fongiques. Infected walled off necrosis (WON) is a major complication of acute pancreatitis. It is associated with a mortality that can rise up to 20 to 30%. Its treatment is based on a multidisciplinary approach and requires antibiotics associated with drainage, which has to be as less invasive as possible. Studies have been carried out to identify the main bacterias responsible for those infections and guide the antibiotherapy. The most recent series have shown less infections due do gram-negative bacilli. It has also shown an increased prevalence of gram-positive infections, the emergence of multidrug resistant bacteria, and an increase in fungal infections (Candida spp.). The overuse of antibiotics for non-infected WON might be in cause in these changes. Antibiotics should be adapted to microbiological samples taken from collections as much as possible. The occurrence of WON infection is a formal indication for drainage. Therapeutic echo-endoscopy occupies a central place in the "step-up" approach, which favours the use of the least invasive techniques first. Indeed, endoscopic necrosectomy is associated with lower morbidity than surgery. The recently development luminal apposition metal stent can ease the drainage procedure, with a higher rate of technical and clinical success. Multidisciplinary management of WON infection is therefore the cornerstone of the management of this serious and sometimes fatal condition. Antibiotic therapy should be initiated when there is strong evidence of necrotic flow infection to avoid selecting bacteria with a high level of resistance and fungal superinfections. Résumé: L'hémorragie digestive haute sur hypertension portale est une complication majeure et grave de la cirrhose, grevée d'un taux de mortalité de 10 % à 20 %. Sa prise en charge est codifiée par les recommandations de Baveno VI. Après une prise en charge médicale adéquate, une endoscopie initiale permet une hémostase dans 80 % des cas. Pour les 20 % d'échecs au traitement endoscopique, la pose d'un TIPS doit être discutée. Malheureusement, tous les centres ne disposent pas d'un accès au TIPS 24 heures/24 et 7 jours/7, raison pour laquelle des traitements endoscopiques de seconde ligne peuvent être proposés. La sonde de tamponnement est la technique la plus ancienne avec une « facilité » de pose et une bonne efficacité sur l'arrêt du saignement des varices œsophagiennes lors de la phase aiguë. Toutefois, avec un taux élevé de récidive au moment de son retrait et de nombreux effets secondaires, celle-ci est de moins en moins utilisée, d'autant plus qu'une prothèse œsophagienne couverte dédiée à la prise en charge des ruptures de varices œsophagiennes en échec de ligature élastique a été récemment développée. Celle-ci présente de meilleurs résultats en termes d'efficacité et d'effets secondaires mais son prix est un frein à son utilisation. La poudre hémostatique a également une place dans le traitement de seconde ligne car elle est simple d'application, elle conduit à un taux élevé d'hémostase endoscopique, la récidive hémorragique est faible et il existe très peu d'effets secondaires. La sclérothérapie peut être utilisée du fait d'un taux d'arrêt du saignement correct mais les récidives hémorragiques sont nombreuses, elle n'améliore ni la survie ni l'éradication des varices et son utilisation n'est pas dénuée de risques. Cet article a pour but de présenter les différentes techniques disponibles pour la prise en charge d'une hémorragie sur hypertension portale après échec d'un premier traitement endoscopique et/ou en attente de la pose d'un TIPS. Upper GI bleeding due to portal hypertension is a major and serious complication of cirrhosis, with a mortality rate of 10% to 20%. Its management is codified by the Baveno VI criteria. After adequate medical management, initial endoscopy can treat the bleeding in 80 % of cases. For the 20% of cases where endoscopic treatment fails, the placement of a TIPS should be discussed. Unfortunately, not all centers have 24/7 access to TIPS, which is why second-line endoscopic treatments can be offered and will be discussed in this article. The tamponade balloon catheter is the oldest technique with good efficacy in stopping bleeding from oesophageal varices in the acute phase. However, with a high rate of recurrence at the time of its removal and numerous side effects, it is less and less used, especially since a covered oesophageal prosthesis dedicated to the management of oesophageal varicose vein ruptures in failure of elastic ligation has been recently developed. It has better results in terms of efficacy and side effects, but its price is a barrier to its use. Haemostatic powder also has a place in second line treatment as it is simple to apply, has a high rate of endoscopic haemostasis, recurrence of bleeding is low and there are very few side effects. Finally, sclerotherapy can be used because of a good rate of bleeding arrest, but there are many recurrences of bleeding, it does not improve survival or eradication of varicose veins and its use is not without risk. The aim of this article is to present the different methods available for the management of haemorrhage due to portal hypertension after failure of a first endoscopic treatment and/or while waiting for the installation of a TIPS. Résumé: La nécrose pancréatique organisée (NOP) compliquant un épisode de pancréatite aiguë sévère grève le pronostic. De nombreuses études se sont intéressées à la prise en charge de ces patients, faisant passer d'une prise en charge chirurgicale à une approche progressive « step-up » intégrant les drainages par voie radiologique puis endoscopique. Le drainage endoscopique a été initialement réalisé avec des prothèses plastiques puis des prothèses métalliques couvertes. Cette technique a permis d'accéder via le tube digestif à la nécrose pancréatique et de développer la nécrosectomie endoscopique. De nouvelles prothèses dites d'apposition luminale sont apparues récemment. Il a été montré un important succès technique et clinique avec un taux de résolution de la nécrose pancréatique de l'ordre de 95 % et un faible taux de récidive. Un nouveau système « tout-en-un » a révolutionné la mise en œuvre de ces prothèses diminuant le nombre d'instruments et de manipulations nécessaires rendant la pose plus facile et rapide. Cependant, il a été décrit des complications sévères notamment à distance de la pose par lésion d'un vaisseau adjacent justifiant une surveillance rapprochée et un retrait précoce de la prothèse. Par ailleurs, il existe un surcoût à mettre en balance avec les succès technique et clinique. Les prothèses d'apposition luminale redéfinissent la prise en charge de la NOP. Les taux de succès technique et clinique importants mis en évidence dans les études en témoignent. Il faut cependant réserver cette technique aux centres experts afin de proposer une approche « step-up » en coordination avec les équipes de radiologie et de chirurgie mais aussi en raison de complications parfois graves lors de la pose ou au décours, et du surcoût qu'engendrent ces prothèses. Il semble intéressant que de nouvelles études soient réalisées afin de préciser la gestion de ces prothèses et de leurs complications. Walled off necrosis (WON) is a major complication of acute necrotizing pancreatitis (ANP) and strikes patients' prognosis. Many studies have been studying ANP management which has progressively evolved from exclusive surgery towards to a step-up approach including radiological and endoscopic drainages. As far as endoscopy is concerned, initial management consisted on stenting pancreatic collection with double pigtail plastic stent. Then full covered self-expandable metallic stent was developed to allow a direct access to WON passing through a standard gastroscope. It was the emergence of Direct Endoscopic Necrosectomy. More recently, a lumen apposing metal stent (LAMS) was designed to anastomose digestive tract to adjacent structure. It represents an important technical and clinical success since WON resolution was observed nine times out of ten with lower recurrence. Finally, new LAMS integrating electrocautery enhanced delivery system emerged reducing procedure steps. However, severe complications have been described, including acute hemorrhage by erosion of an adjacent vessel. These adverse events advocate close monitoring by compute tomodensitometry and early stent's removal. Unfortunately, there are no recommendations for the withdrawal of the stent. This new device has an additional cost compared to DPPS. However, it is important to keep in mind the global cost of management of WON not only endoscopic procedure to compare it with other devices. In conclusion, LAMS redefine the management of symptomatic WON and appear to provide a benefit in terms of technical and clinical success. But now, this technique should be reserved only for expert centers in order to propose a step-up approach in coordination with the radiology and surgery teams. Moreover, serious complications can occur and should be manage by expert teams. Purpose: This pilot study was designed to test the hypothesis that the use of a single oral splint may aggravate respiratory disturbance in sleep apneic patients. Materials and Methods: A group of 10 patients with a history of snoring and a recording night confirming a diagnosis of sleep apnea were included. Patients were then invited to spend 2 nights in the sleep laboratory: night 2 to establish baseline data (baseline night) and night 3, 1 week later, to assess the influence of an occlusal maxillary splint on sleep (splint night). The following variables were analyzed under blind conditions: total sleep time, sleep efficiency and number of awakenings, microarousals, apnea-hypopnea index per hour of sleep (AHI), respiratory disturbances index per hour of sleep (RDI), and percentage of sleeping time with snoring. Results: No statistically significant difference in AHI was noted between baseline and splint nights. However, four patients experienced an aggravation in apnea diagnosis category on the night they used the splint. The AHI was increased by more than 50% in 5 of the 10 patients. The RDI showed a 30% increase from baseline to splint nights. The percentage of sleeping time with snoring also increased by 40% with the splint. Conclusion: This open study suggested that the use of an occlusal splint is associated with a risk of aggravation of respiratory disturbances. It may therefore be relevant for clinicians to question patients about snoring and sleep apnea when recommending an occlusal splint. [ABSTRACT FROM AUTHOR]

Introduction: Both types of sleep-disordered breathing (SDB), obstructive and central sleep apnoea (OSA and CSA, respectively), are common in patients with heart failure and reduced ejection fraction (HFrEF). In such patients, SDB is associated with increased cardiovascular morbidity and mortality but it remains uncertain whether treating SDB by adaptive servo-ventilation (ASV) in such patients reduces morbidity and mortality.Aim: ADVENT-HF is designed to assess the effects of treating SDB with ASV on morbidity and mortality in patients with HFrEF.Methods: ADVENT-HF is a multicentre, multinational, randomized, parallel-group, open-label trial with blinded assessment of endpoints of standard medical therapy for HFrEF alone vs. with the addition of ASV in patients with HFrEF and SDB. Patients with a history of HFrEF undergo echocardiography and polysomnography. Those with a left ventricular ejection fraction ≤45% and SDB (apnoea-hypopnoea index ≥15) are eligible. SDB is stratified into OSA with ≥50% of events obstructive or CSA with >50% of events central. Those with OSA must not have excessive daytime sleepiness (Epworth score of ≤10). Patients are then randomized to receive or not receive ASV. The primary outcome is the composite of all-cause mortality, cardiovascular hospital admissions, new-onset atrial fibrillation requiring anti-coagulation but not hospitalization, and delivery of an appropriate discharge from an implantable cardioverter-defibrillator not resulting in hospitalization during a maximum follow-up time of 5 years.Conclusion: The ADVENT-HF trial will help to determine whether treating SDB by ASV in patients with HFrEF improves morbidity and mortality.
