Abstract

Nurses in the Neonatal Intensive Care Unit (NICU) have an important role in implementing family-centered care (FCC). The aim of the study was to explore the lived experiences of NICU nurses on implementing FCC. An interpretative phenomenological study was conducted and 11 employed nurses were interviewed from April 2015 to February 2016. The data were analyzed through the Diekelmann, Allen, and Tanner approach. Four main themes of "strain to achieve stability," "bewildered by taking multiple roles," "accepting the family," and "reaching bright horizon" were extracted. This study provided deeper understanding about nurses' perceptions of FCC implementation. In Muslim developing countries, FCC implementation is challenging and nurses are under extra pressure because of a shortage in nursing workforce; however, having positive experiences with family participation and valuing theism beliefs allowed them to support family involvement. Support of nurses to take FCC strategies in the NICU is needed by officials overseeing the health care system. [ABSTRACT FROM AUTHOR]

56. Effect of Clustered Nursing Care on Sleep Behaviors of the Preterm Neonates Admitted to the Neonatal Intensive Care Unit.

Abstract

Background: Premature neonates admitted to the neonatal intensive care unit (NICU) undergo sleep disorder due to various manipulations. The present study aimed to investigate the effect of clustered nursing care on sleep behaviors in premature neonates admitted to NICUs. Methods: This clinical trial study was conducted on 60 neonates selected through convenience sampling method out of the infants admitted to the NICU. First, in the control group, a constant nurse took care of the neonates in a complete shift. The nurse observed the neonates sleep behaviors every two min for 45 min and recorded them in the questionnaire. The intervention group was investigated one week after the control group. The neonates 'sleep behaviors were observed and recorded every two min by Prechtl instrument. All the data were analyzed by the Mann- Whitney U test using SPSS software version 16. Results: The mean sleep times in the control group were as follow: quiet sleep 5 min and 86 millisecond, active sleep 21 min and 50 milliseconds, quiet wake 4 min and 6 milliseconds,alertness9 min and 6 milliseconds, and cry1 min and 76 millisecond .On the other hand, in the neonates who received clustered care in the intervention group, the mean timings were as follow: quiet sleep time 19 min and 33 millisecond, active sleep 24 min and 66 millisecond, quiet wake 1 min and 76 millisecond,alertness2 min and 76 millisecond, and cry0.13 min. According to the mentioned times, it could be concluded that the neonates in the test group had a quiet and active sleep (P <0.05). Conclusion: Findings of this study demonstrate that cluster care can significantly increase the time of quiet and active sleep in the newborns. Based on the result, it is recommended that this kind of care be provided in the NICU program, as well as in the syllabus of students and nursing retraining. [ABSTRACT FROM AUTHOR]

Abstract

Presentations from the annual workshops of the Center for Qualitative psychology in Sassary (Italy) in 2010 and in Heidelberg (Germany) in 2011 have been combined in this volume. The book has twelve chapters, organized in two parts: Reflections on Values and Diversity (Part I), Methodological Impacts (Part II). All chapters are related to a specific aspect of diversity and have been presented at either one of the workshops in Italy or in Germany. The term "diversity" is often used to describe differences, but it also refers to the notion of inclusiveness, - an inclusiveness of individuals who are diverse in ability /disability, age, economic status, ethnicity, gender, language, national origin, race, religion, and sexual orientation. This inclusivity can be a framework for managing diversity. Social justice and respect for all individuals are important values in workplaces,where people seek to create a culture of inclusion that actively supports diversity in people, who live, work, and serve together. Attaining social justice and respect requires all members of the community to recognize that diversity in all its dimensions contributes to workplaces and other learning environments, thereby enriching the community, and improving opportunities for human understanding. A common issue in the contributions to this volume is to understand and apply research ethics as a dialogical practice with research subjects.

Abstract

Drug misuse is increasing and diversifying in Iran. This study is the first to explore in detail the impact on, and ways of coping used by, spouses of addicted men in Iran. Semi-structured interviews were conducted with 24 spouses. Four main themes were identified in the data: heart-breaking news of the husband's addiction; coping alone; progressive deterioration and suffering; and disruption of family relationships and finances. Reactions upon learning of the husband's addiction involved shock and collapse, and/or fear, disbelief and confusion. Spouses tried to hide the problem and to solve the problem alone, feeling for a long time as if they were 'walking in the dark' without any social support and exposed to stigma. As time had gone on they had experienced distress and turmoil, and mixed feelings towards, and loss of trust and confidence in, their husbands, whose behaviour was increasingly unreliable. This had led to impairment in the relationship, and financial stress, and a general degradation and disruption of normal family life, leading to strain for spouses, akin to a state of burnout. Present findings confirm the conclusions of similar research conducted in other countries, that substance misuse in the family can have devastating effects for spouses, on children and all aspects of family life. The situation for Iranian wives may be compounded by the relatively closed nature of family life, and the existence of culturally based attitudes, including shame, towards 'family defects' such as addiction. Abstract

Background: Sensitive and responsive maternal caregiving behavior strengthens infant self-regulatory capacities (HL), but this regulatory role may be diminished in some mothers with second-trimester prenatal exposure to depression and/ or anxiety (MDA). This study examined maternal and infant behavior during infant heel lance (HL) when mothers had or did not have MDA. Ethological methods and micro-analytic approaches capable of distinguishing and comparing time-based patterning in maternal and infant behavior were used to clarify biological mechanisms, such as MDA, that may underlie observed behavior. Aims were to examine group differences in caregiving behavior between mothers with and without MDA 5 min Pre-HL and 5 min Post-H, and relationships between MDA, maternal caregiving behavior and infant pain behavior self-regulation, concurrently. Methods: At second trimester, mothers were assessed for symptoms of mild-severe depression or anxiety. Mothers whose scores exceeded predetermined cut-off scores on one or more of the mental health measures were allocated to the MDA-exposure group, those below to the non-MDA-exposure group. Reliable observers, blinded to MDA status and study phases, coded video records of the caregiving behavior of each study mother for the full duration of the 5 min Pre-HL and 5 min Post-HL study phases. Group differences and associations between mean measures of maternal mental health scores, time-based measures of maternal behavior, and time-based measures of infant pain behavior regulation (previously coded) were concurrently analyzed using comparative and correlational statistics. Results: MDA-exposed mothers spent significantly more time not embracing, engaging or responding to infant cues than maternal controls Pre-HL and Post-HL. MDA was associated with atypical maternal caregiving behavior, which in turn was related to atypical infant pain behavior self-regulation during and after the HL. Conclusion: Our findings have implication for practice. We recommend inclusion of mothers with MDA and their infants in interventions that strengthen the early mother-infant interaction and mother’s regulatory caregiving role. MDA and maternal caregiving behavior must be considered in future infant pain studies to examine if they confound effectiveness of mother driven caregiving interventions for neonatal pain. We highlight the importance of examining maternal mental health throughout the perinatal and postnatal trajectory, and particularly the newborn period.
