64. The sign of traveling wave speed in bistable dynamics.

Abstract

We are concerned with the sign of traveling wave speed in bistable dynamics. This question is related to which species wins the competition in multiple species competition models. It is well-known that the wave speed is unique for traveling wave connecting two stable states. In this paper, we first review some known results on the sign of wave speed in bistable two species competition models. Then we derive rigorously the sign of bistable wave speed for a special three species competition model describing the competition in two different circumstances: (1) two species are weak competitors and one species is a strong competitor; (2) three species are very strong competitors. It is interesting to observe that, under certain conditions on the parameters, two weaker competitors can wipe out the strongest competitor. [ABSTRACT FROM AUTHOR]

Abstract

We propose a stable numerical scheme for a Cahn-Hilliard type equation with long-range interaction describing the micro-phase separation of diblock copolymer melts. The scheme is designed by using the discrete variational derivative method, one of structure preserving numerical methods. The derivation of the discrete variational derivative of a discretized energy functional is simplified by using a suitable discrete L2 space and fractional powers of a discrete approximation of the Laplace operator. The proposed scheme has the same characteristic properties, mass conservation and energy dissipation, as the original equation does. We are concerned with bifurcations of steady states for a model system of phase separation, which is introduced by Eguchi-Oki-Matsumura (EOM). The system consists of coupled two evolution equations and admits steady state solutions with different ergies. We analyze the bifurcation phenomena of these steady states with respect to the principal parameter, which is related to the temperature.

Abstract

Progressive telomere shortening with aging was studied in the normal liver tissue of 94 human subjects aged between 0 and 101 years old to determine the rate of telomere loss in 1 year. Telomere length demonstrated age-related shortening with reduction of 55 base pairs (bp) per year. The mean telomere length in five neonates was 12.9 [+ or -] 2.6 kilobase pairs (kbp), and that in one centenarian was 8.3 kbp. Mean telomere lengths by age group were 13.2 [+ or -] 2.0 kbp ([is less than or equal to] 8 years; 10 subjects), 7.8 [+ or -] 1.9 kbp (40-79 years; 29 subjects), and 7.5 [+ or -] 2.0 kbp ([is greater than or equal to] 80 years; 53 subjects), with reduction thus appearing to show slowing on the attainment of middle age. The difference of mean telomere lengths for two groups with or without advanced malignancies of other than liver origin was not significant in the older two groups. Despite the slow turnover of liver tissue, the overall reduction rate of telomere length decrease in 1 year was almost the same as that of digestive tract mucosa, with its very rapid renewal.

Background. Pleomorphic adenoma (PA) is a benign tumour that mainly arises from salivary glands, and PA of the external auditory canal (EAC) is very rare. The objective of this study was to clarify the clinical presentation and treatment of PA of the EAC. Method. The authors present a case of PA arising from the EAC together with a literature review. Results. A 40-year-old man complained of hearing loss and foreign-body sensation of the right ear. Clinical and radiological examinations revealed a well-defined tumour limited to the EAC, with no connection to the parotid gland. Preoperative fine-needle aspiration cytology findings were characteristic of PA. The tumour was removed en bloc with the overlying skin. Conclusion. PA of the EAC is very rare, and methods to rule out malignancy before treatment are lacking. Thus, long-term follow-up is necessary, because malignant tumours are common in the EAC and PA has malignant potential.

Abstract

In the past decade, the serum free light chain (FLC) immunoassays have become widely available enabling greater sensitivity in the diagnosis and management of monoclonal light chain diseases. Here, we describe a rare case of serum free light chain only myeloma with cytoplasmic IgM. A 75-year-old woman presented with a progressively worsening lumbosacral pain. FDG PET/CT images showed increased FDG uptake in the sacral mass, vertebral bodies, and ribs. Laboratory data found hypogammaglobulinemia and the bone marrow aspirate revealed only 2.2% of plasma cells. The serum and urine protein electrophoresis did not detect a monoclonal band. However, the serum FLC immunoassays reported an abnormal kappa/lambda ratio (0.001) indicating the presence of monoclonal lambda FLC. The sacral tumor biopsy revealed proliferation of plasma cells and immunohistochemical staining showed that the plasma cells were positive for CD138, IgM, and lambda light chain but negative for CD20. This case may have previously been described as a nonsecretory IgM myeloma but recently would be identified as free light chain only myeloma. The immunohistochemical and genetic features of the clonal plasma cells in free light chain only myeloma need to be further investigated to better understand the relevance and incidence of this myeloma type.

A case of leiomyoma of the urinary bladder in a 40-year-old man is reported. Enucleation of the tumor was performed, because transurethral resection-biopsy revealed a bladder leiomyoma. Ninety nine cases of a bladder leiomyoma have been reported in Japan, but male cases are rare.

前立腺肥大症111例, 膀胱頸部硬化症6例, 前立腺肥大症+膀胱頸部硬化症3例, 女性の排尿困難1例の計121例に対して, α1受容体遮断薬であるYM 617を0.1～0.4 mg/日投与した.1)患者の印象および主治医による自覚症状改善度の改善率は投与期間が増すにしたがって高くなり, 16～20週以降はほぼ一定の値に維持された.他覚所見改善度の改善率は4週以降ほぼ一定の値に維持された.2)患者の印象および自覚症状改善度において増量効果が認められた.3)副作用は3例にみられたが, 重篤なものはなかった.血圧・脈拍, 臨床検査値に影響をおよぼさなかった.以上, YM 617は長期間投与しても効果は維持され, 安全性も高いことから, 前立腺肥大症に代表される下部尿路通過障害患者に対し有用性の高い薬剤である, The efficacy and safety of YM617, a new alpha 1-blocker, were evaluated in 121 patients with urinary obstruction of the lower urinary tract in the long-term administration study for 1 year. A total of 121 patients, i.e., 111 with benign prostatic hypertrophy (BPH), 6 with bladder neck sclerosis (BNS), 3 with BPH with BNS and 1 with female urinary obstruction, were orally administered 0.1 to 0.4 mg of YM617 once daily. Patients showing improvement of patient's impression and of subjective symptoms judged by the physician were increased until the 16th to 20th week, and it had been maintained there after. Objective findings were improved to a constant level after the 4th week. Patient's impression and subjective symptoms were improved in a certain dose-response relationship Side effects were observed in 3 patients but none were serious. YM617 proved to be useful in the treatment of patients with urinary obstruction of the lower urinary tract like BPH because it maintained the efficacy and safety in the long-term administration.

Abstract

Background: Congenital biliary dilatation (CBD) is a congenital malformation involving both dilatation of the extrahepatic bile duct and pancreaticobiliary maljunction. Persistent reflux of pancreatic juice injures the biliary tract mucosa, resulting in chronic inflammation and higher rates of carcinogenesis in the biliary tract, including the gallbladder. Telomeres are repetitive DNA sequences located at the ends of chromosomes. Chromosomal instability due to telomere dysfunction plays an important role in the carcinogenesis of many organs. This study was performed to determine whether excessive shortening of telomeres occurs in the gallbladder mucosa of patients with CBD.Methods: Resected gallbladders were obtained from 17 patients with CBD, ten patients with cholecystolithiasis without pancreatic juice reflux, and 17 patients with normal gallbladders (controls) (median age of each group of patients: 37, 50, and 53 years, respectively). The telomere lengths of the gallbladder epithelium were measured by quantitative fluorescence in situ hybridization using tissue sections, and the normalized telomere-to-centromere ratio (NTCR) was calculated.Results: The NTCRs in the CBD, cholecystolithiasis, and control groups were 1.24 [interquartile range (IQR) 1.125-1.52], 1.96 (IQR 1.56-2.295), and 1.77 (IQR 1.48-2.53), respectively. The NTCR in the CBD group was significantly smaller than that in the cholecystolithiasis and control groups (p = 0.003 and 0.004, respectively), even in young patients.Conclusions: Our findings indicate that telomere shortening in the gallbladder mucosa plays an important role in the process of carcinogenesis in patients with CBD. These results support the recommendation of established guidelines for prophylactic surgery in patients with CBD because CBD is a premalignant condition with excessive telomere shortening.
