Abstract

Empowerment Squared’s programming for newcomer youth and their families is reliant on volunteer support. Empowerment Squared aims to recruit more consistent, long-term volunteers to maintain continuity of their programs and build capacity. Empowerment Squared’s volunteer base is currently made up of largely undergraduate students, who tend to be unable to volunteer over the summer months and during their exam periods, causing a gap in available volunteers to support the programs. By better understanding what attracts volunteers to their programs and what factors engage and retain them, Empowerment Squared hopes to double their current volunteer base and better engage and retain their current volunteers. The research team conducted a review of the literature on volunteer recruitment, engagement and retention, focus groups with Empowerment Squared staff, an online survey of volunteers, and volunteer interviews. Our results indicate that volunteers are motivated to sign up to volunteer and continued to volunteer with Empowerment Squared for a variety of altruistic, social and material reasons and stop volunteering with the program due to situational factors (e.g., scheduling, time and/or location). Recommendations for Empowerment Squared include altering their word of mouth and club and group-based recruitment strategies, recruiting program alumni, providing specialized training to volunteers, considering volunteering motivation in their engagement strategies, and formalizing the volunteer feedback process.

75. The additive association of indoor cigarette and marijuana smoking on potential exposure to fine particles

Abstract

Air particle monitors were placed in 298 homes of families with at least 1 cigarette smoker and 1 child under the age of 14. After monitors continuously measured fine particle counts (0.5 to 2.5 microns) for at least 7 days, participants were interviewed about past 7-day frequency of cigarette and marijuana smoking, other PGEs (e.g., burning candles, burning food) and ventilation activities such as use of exhaust fans. Dichotomized survey responses (any vs. none) from 193 homes that provided data for all relevant PGEs and ventilation activities were used along with mean air particle counts from the 7-day period.

Huyen Nguyen, a graduate of the University of California, Irvine, is a senior programming analyst at Western Growers. The interview focuses on her experiences before the fall in April 1975, settling in the U.S., and adjusting to life a new life in the U.S.. Her stories detail what she had in Vietnam, what she lost, and what she had to work for. She went from a life living in a well‐to‐do family with a prestigious reputation to a refugee who had to work her hardest to make a comfortable living in a new country. 93. Impact of Social Determinants of Health and Demographics on Refill Requests by Medicare Patients Using a Conversational Artificial Intelligence Text Messaging Solution: Cross-Sectional Study

Abstract

BackgroundNonadherence among patients with chronic disease continues to be a significant concern, and the use of text message refill reminders has been effective in improving adherence. However, questions remain about how differences in patient characteristics and demographics might influence the likelihood of refill using this channel. ObjectiveThe aim of this study was to evaluate the efficacy of an SMS-based refill reminder solution using conversational artificial intelligence (AI; an automated system that mimics human conversations) with a large Medicare patient population and to explore the association and impact of patient demographics (age, gender, race/ethnicity, language) and social determinants of health on successful engagement with the solution to improve refill adherence. MethodsThe study targeted 99,217 patients with chronic disease, median age of 71 years, for medication refill using the mPulse Mobile interactive SMS text messaging solution from December 2016 to February 2019. All patients were partially adherent or nonadherent Medicare Part D members of Kaiser Permanente, Southern California, a large integrated health plan. Patients received SMS reminders in English or Spanish and used simple numeric or text responses to validate their identity, view their medication, and complete a refill request. The refill requests were processed by Kaiser Permanente pharmacists and support staff, and refills were picked up at the pharmacy or mailed to patients. Descriptive statistics and predictive analytics were used to examine the patient population and their refill behavior. Qualitative text analysis was used to evaluate quality of conversational AI. ResultsOver the course of the study, 273,356 refill reminders requests were sent to 99,217 patients, resulting in 47,552 refill requests (17.40%). This was consistent with earlier pilot study findings. Of those who requested a refill, 54.81% (26,062/47,552) did so within 2 hours of the reminder. There was a strong inverse relationship (r10=−0.93) between social determinants of health and refill requests. Abstract

This paper describes Kaiser Permanente's (KP) enterprise-wide medical terminology solution, referred to as our Convergent Medical Terminology (CMT). Initially developed to serve the needs of a regional electronic health record, CMT has evolved into a core KP asset, serving as the common terminology across all applications. CMT serves as the definitive source of concept definitions for the organization, provides a consistent structure and access method to all codes used by the organization, and is KP's language of interoperability, with cross-mappings to regional ancillary systems and administrative billing codes. The core of CMT is comprised of SNOMED CT, laboratory LOINC, and First DataBank drug terminology. These are integrated into a single poly-hierarchically structured knowledge base. Cross map sets provide bi-directional translations between CMT and ancillary applications and administrative billing codes. Context sets provide subsets of CMT for use in specific contexts. Our experience with CMT has lead us to conclude that a successful terminology solution requires that: (1) usability considerations are an organizational priority; (2) "interface" terminology is differentiated from "reference" terminology; (3) it be easy for clinicians to find the concepts they need; (4) the immediate value of coded data be apparent to clinician user; (5) there be a well defined approach to terminology extensions. Over the past several years, there has been substantial progress made in the domain coverage and standardization of medical terminology. KP has learned to exploit that terminology in ways that are clinician-acceptable and that provide powerful options for data analysis and reporting.
