52. TCT CONNECT-269 A First-in-Human Study of the Second-Generation, Thin-Strut, Everolimus-Eluting Bioresorbable Scaffold: Final IVUS and OCT Results From the FAST Clinical Trial.

Abstract

Background: The FAST Feasibility Study is a first-in-human, prospective, multicenter, single-arm study of the second-generation, thin-strut (105-mum thickness), everolimus-eluting Renuvia bioresorbable scaffold for the treatment of coronary artery disease. Method(s): To evaluate the acute device performance and long-term arterial response, serial intravascular ultrasound (IVUS) was performed at baseline (n = 31), 6 months (required, n = 31), and 2 years (optional, n = 16). Serial optical coherence tomography was recommended additionally with the aim of strut-level analysis. Result(s): Post-procedural device expansion (ratio of scaffold to reference lumen volume index on IVUS) of 99.4 +/- 14.7% was achieved. On optical coherence tomography, incomplete strut apposition was observed in a median of 1.3% (interquartile range: 0.0%, 3.1%) of all struts analyzed post-procedure, the majority of which had been resolved at 6 months (median: 0.0%; interquartile range: 0.0%, 0.2%) with nearly complete strut coverage (uncovered struts: median: 0.0%; interquartile range: 0.0%, 1.8%; n = 14). Neointimal volume obstruction by IVUS was 5.1 +/- 3.9% with minimal scaffold recoil (2.2 +/- 5.1%) at 6 months. Serial volumetric IVUS showed a plaque increase with a trend of adaptive remodeling over 6 months, followed by no significant change in vessel, plaque, or lumen from 6 months to 2 years (Figure). No thrombus or strut fracture including intraluminal dismantling was detected at any time point. Abstract

Transmission network modelling to infer ‘who infected whom’ in infectious disease outbreaks is a highly active area of research. Outbreaks of foot-and-mouth disease have been a key focus of transmission network models that integrate genomic and epidemiological data. The aim of this study was to extend Lau’s systematic Bayesian inference framework to incorporate additional parameters representing predominant species and numbers of animals held on a farm. Lau’s Bayesian Markov chain Monte Carlo algorithm was reformulated, verified and pseudo-validated on 100 simulated outbreaks populated with demographic data Japan and Australia. The modified model was then implemented on genomic and epidemiological data from the 2010 outbreak of foot-and-mouth disease in Japan, and outputs compared to those from the SCOTTI model implemented in BEAST2. The modified model achieved improvements in overall accuracy when tested on the simulated outbreaks. When implemented on the actual outbreak data from Japan, infected farms that held predominantly pigs were estimated to have five times the transmissibility of infected cattle farms and be 49% less susceptible. The farm-level incubation period was 1 day shorter than the latent period, the timing of the seeding of the outbreak in Japan was inferred, as were key linkages between clusters and features of farms involved in widespread dissemination of this outbreak. To improve accessibility the modified model has been implemented as the R package ‘BORIS’ for use in future outbreaks.

Abstract

The properties of streamers traveling over the surface of oil-immersed solid dielectrics were experimentally studied under lightning impulse conditions. Streamer polarity and the position of a grounded side electrode significantly affected the relationship between the streamer extension length and the applied voltage. The solid surface charging had also a large effect on the streamer propagation. However, the streamer propagation properties showed a consistent dependence on the potential at solid-liquid interfaces. In addition, the potential drop inside the streamer channel was measured as a function of normalized streamer length. The curve revealed that the potential drop was increased drastically within the region of ~20% from the streamer tip. The streamer appeared to progress with a constant mean velocity.

Abstract

We have been developing polymer nanotechnology including nano-mechanical mapping (NMM) based on atomic force microscopy and transmission electron micro- tomography (3D-TEM) and so on. By using these technologies, we can get information on mechanical properties and higher order structure of complex real rubber materials in nano-scale and design rubber materials for specific purpose like eco-tyre materials with low fuel consumption and high wear resistance or high damping rubber for elastomeric seismic-protection isolators to protect buildings and bridges from earthquakes. 91. Approaching Higher Dimension Imaging Data Using Cluster-Based Hierarchical Modeling in Patients with Heart Failure Preserved Ejection Fraction

Abstract

Heart failure with preserved ejection fraction (HFpEF) is a major cause of morbidity and mortality, accounting for the majority of heart failure (HF) hospitalization. To identify the most complementary predictors of mortality among clinical, laboratory and echocardiographic data, we used cluster based hierarchical modeling. Using Stanford Translational Research Database, we identified patients hospitalized with HFpEF between 2005 and 2016 in whom echocardiogram and NT-proBNP were both available at the time of admission. Comprehensive echocardiographic assessment including left ventricular longitudinal strain (LVLS), right ventricular function and right ventricular systolic pressure (RVSP) was performed. The outcome was defined as all-cause mortality. Among patients identified, 186 patients with complete echocardiographic assessment were included in the analysis. The cohort included 58% female, with a mean age of 78.7 ± 13.5 years, LVLS of -13.3 ± 2.5%, an estimated RVSP of 38 ± 13 mmHg. Unsupervised cluster analyses identified six clusters including ventricular systolic-function cluster, diastolic-hemodynamic cluster, end-organ function cluster, vital-sign cluster, complete blood count and sodium clusters. Using a stepwise hierarchical selection from each cluster, we identified NT-proBNP (standard hazard ratio [95%CI] = 1.56 [1.17-2.08]) and RVSP (1.37 [1.09-1.78]) as independent correlates of outcome. When adding these parameters to the well validated Get with the Guideline Heart Failure risk score, the Chi-square was significantly improved (p = 0.01). In conclusion, NT-proBNP and RVSP were independently predictive in HFpEF among clinical, imaging, and biomarker parameters. Cluster-based hierarchical modeling may help identify the complementally predictive parameters in small cohorts with higher dimensional clinical data.

Abstract

Research aimed at understanding transmission networks, representing a network of “who infected whom” for an infectious disease outbreak, have been actively conducted in recent years. Transmission network models incorporating epidemiological and genetic data are valuable for elucidating disease transmission pathways. In this study, we reconstructed the transmission network of the foot-and-mouth disease (FMD) epidemic in Japan in 2010, and explored farm-level risk factors associated with increased risk of secondary transmission. A published, systematic Bayesian transmission network model was applied to epidemiological data of 292 infected farms and whole genome sequence data of 104 of the infected farms. This model can make inferences for known infected farms even lacking genetic data. After estimating the consensus network, the accuracy of the network was examined by comparison with epidemiological data. Then, risk factors inferred to have been sources of secondary transmission were explored using zero-inflated Poisson regression model. As far as we are aware, this study represents the largest FMD outbreak transmission network to be published by such means combining epidemiological and genetic data. The consensus network reasonably generated the epidemiological links, which were estimated from the actual epidemiological investigation. Among 292 farms, 101 farms (35%) were inferred to have been the sources of secondary transmission, and amongst these farms, the median number of secondary cases was 2 (min:1–max:18) farms. The farm-type (small and large -sized pig farms), the number of days from onset to notification, and the number of susceptible farms within a 1-km radius were significantly associated with secondary transmission. Transmission network modelling enabled inference of the connections between infected farms during the FMD epidemic and identified important factors for controlling the risk of secondary transmission. Abstract

A study was conducted on the antioxidant activities of polyalkylbenzochromanol, polyalkylbenzochromenol and 2,3-dihydro-5-hydroxy-2,2,4-trimethylnaphtho[1,2-b]furan in sodium dodecyl sulfate micelles. The activities were compared to that of the alpha-tocopherol model compound 6-hydroxy-2,2,5,7,8-pentamethylchroman. The chain initiation rate was controlled via either a water-soluble initiator, azobis(2-amidinopropaneHCl), or a lipid-soluble initiator, di-tert-butyl hyponitrite, during polyunsaturated fatty acids peroxidation.

96. Multifocal Gingival Squamous Cell Papilloma: A Case Report and Literature Review

Abstract

Squamous Cell Papilloma (SCP) is a benign, asymptomatic, exophytic cauliflower-like growth of mucosal mass. It is mostly associated with Human Papilloma Virus (HPV) subtypes 6 and 11, and has a very low virulence and infectivity rate. The papillomas are commonly known as warts or verrucas when found on the skin. The typical lesion is a single mass with finger-like extensions, resembling a soft, pedunculated growth supported by a stem or stalk. If keratin, a skin protein, accumulates around the lesion, the projections can be long and pointed or short and rounded. Intraorally, the most common sites of occurrence are the tongue, lips, buccal mucosa, palate, and uvula. Hereby, the author present a case report of 25-year-old male patient with a 4x9 mm exophytic growth exhibiting a pebbled surface in the upper left first premolar region, with no signs of radiographic bone loss. Additionally, smaller pinpoint lesions were observed bilaterally in the interdental region of the first and second maxillary molars. A nevus on the right side of the face was also noted. Histological analysis confirmed the diagnosis of SCP. The presence of multifocal gingival squamous papilloma, along with an extraoral nevus, represents a novel finding that warrants reporting. The gingival lesion in the maxillary left first premolar region was surgically excised, along with 1 mm of healthy surrounding gingiva. This resulted in complete healing, and no recurrence was observed during the 12-month follow-up period.

97. Evaluation of Stability of Implants Placed Simultaneously with Lateral Window Sinus Augmentation using Putty Alloplastic Bone Substitute: A Prospective Interventional Study

Abstract

Introduction: Lateral window sinus augmentation is done to augment the vertical sinus height for implant placement. Putty alloplasts have been used due to their longer resorption time and provide resistance to implant insertion. Although, widely used, the stability and bone loss around implants placed simultaneously following sinus augmentation with putty bone graft has not been evaluated. Aim: To evaluate the effect of putty alloplastic bone substitute on implant stability. Materials and Methods: This prospective interventional study was conducted in the Outpatient Department (OPD) of Oral and Maxillofacial surgery at SGT Dental College and Research Institute, Gurugram, Haryana, India. The duration of the study was two years and 11 months, from December 2014-November 2016. A total of 15 implants were placed simultaneously after lateral window sinus augmentation. Primary implant stability measurements were done using Resonance Frequency Analysis (RFA). Vertical Bone Height (VBH), Maximum Insertion Torque (MIT) and Crestal Bone Loss (CBL) were measured till six months of follow-up. The data was analysed using standard statistical analyses with Shapiro-Wilk-test, Wilcoxon signed-rank test and Spearman’s correlation co-efficient. Results: The mean age of the study participants was 58±3.04 years. A total of 15 implants were placed in 12 patients. Adequate primary stability was achieved with MIT >36 N/cm2 in 9/15 patients whereas, in 6/15 patients the MIT was ≤36 N/cm2 . The implants showed 100% survival rate. Postoperative bone gain obtained was in the range of 7.89 mm to 11.9 mm, with a mean of 9.92 mm. Acceptable levels of implant stability were obtained after six months. Conclusion: Within the limitations of the study, it can be concluded that, putty bone alloplast can serve as an adequate bone substitute in simultaneous implant placement after lateral window sinus augmentation and help in achieving stability.

Abstract

Two-color photon-gated persistent spectral hole burning by electron transfer from a doped donor to an acceptor branched to a host polymer matrix was investigated. The results confirmed that the hole formation mechanism was donor-acceptor electron transfer. Thermal stability of the burnt holes increased when the acceptor was branched into the host polymer, poly(methylmethacrylate).
