Abstract

Objective. The purpose of this investigation was to improve a rankit ordinal model for evaluating and validating dichotomized tests in a prospective Nordic project. Material and methods. The model is based on the assumption that the S-shaped curve of fractions of positive for increasing concentrations can be de-convoluted to a histogram and thereby used to calculate the parameters for a ln-Gaussian distribution. In a Nordic survey, four urine samples with known concentrations of hCG (human chorionic gonadotrophin) and nitrites were distributed to more than 2500 practitioners' offices. Results. The results are presented as parameters (geometric mean and CV) for the components urine-hCG and urine-nitrites, together with fractions of positive for clinical critical values (5 and 40 IU/L for hCG), for which fractions should be below 0.01 and above 0.99, respectively, and 7 µmol/L for nitrites. Furthermore, the concentration intervals of varying fractions of positive from 0.01 to 0.99 are estimated as grey zones. The parameters and grey zones for different kits are compared. No urine-hCG kit fulfilled the low clinical criterion, whereas all fulfilled the high criterion. Seven of the eight nitrites kits had fractions of positive above 0.9 for the company confirmation limit, but varying fractions for the clinically important limit of 7 µmol/L (fractions from 0.06 to 0.83). Conclusions. The present model makes it easy to estimate parameters for the kits, and also to estimate the fractions of measured positives for specified concentrations. 71. Enhancement of Indices of Viral Infection by Simultaneously Administering Helicoverpa zea and Autographa californica Nuclear Polyhedrosis Viruses to Larval Helicoverpa zea (Boddie)

Abstract

Corn earworm, Helicoverpa zeo (Boddie), were simultaneonsly administered Heliothis zea and Authographa californica nuclear polyhedrosis viruses (HzSNPV and AcMNPV, respectively). These viruses are classified within the Baculoviridae (subfamily Eubaculovirinae) and are known to he pathogenic to H. zea. When viruses were simultaneously introduced, significant reductions of median lethal dose (LD50) estimates occurred for second- and fourth-instars. However, statistical analysis to determine the virus ratio which optimized LD50 estimates found that second-instars were most susceptible to treatments containing HzSNPV alone. Analysis of fourth-instars indicated that a viral mixture of 65% HzSNPV and 35% AcMNPV resulted in significantly lower LD50 estimates than either of the viruses as single infections. Similar studies involving median survival time (ST50) analysis indicated that viral mixtures significantly reduced the time required between infection and mortality. Abstract

Identification of sequence variants robustly associated with predisposition to diabetic kidney disease (DKD) has the potential to provide insights into the pathophysiological mechanisms responsible. We conducted a genome-wide association study (GWAS) of DKD in type 2 diabetes (T2D) using eight complementary dichotomous and quantitative DKD phenotypes: the principal dichotomous analysis involved 5,717 T2D subjects, 3,345 with DKD. Promising association signals were evaluated in up to 26,827 subjects with T2D (12,710 with DKD). A combined T1D+T2D GWAS was performed using complementary data available for subjects with T1D, which, with replication samples, involved up to 40,340 subjects with diabetes (18,582 with DKD). Analysis of specific DKD phenotypes identified a novel signal near GABRR1 (rs9942471, P = 4.5 × 10-8) associated with microalbuminuria in European T2D case subjects. However, no replication of this signal was observed in Asian subjects with T2D or in the equivalent T1D analysis. There was only limited support, in this substantially enlarged analysis, for association at previously reported DKD signals, except for those at UMOD and PRKAG2, both associated with estimated glomerular filtration rate. We conclude that, despite challenges in addressing phenotypic heterogeneity, access to increased sample sizes will continue to provide more robust inference regarding risk variant discovery for DKD. 99. Parasites and Pathogens of the Fall Webworm in Illinois

Abstract

Naturally occurring parasites and pathogens of Hyphantria cunea (Drury) were recovered from field-collected larvae from northern, central, and southern Illinois in 1970. The following parasites were reared from these larvae: Sinophorus validus (Cresson), Meteorus hyphantriae Riley, Apanteles hyphantriae Riley, Elachertus hyphantriae Crawford, and Mericia ampelus (Walker). Total parasitism ranged from 34 to 39% from both the orange-headed and black-headed races of H. cunea. Three microsporidia, Nosema necatrix Kramer, Nosema sp., and Pleistophora schubergi hyphantriae Weiser, all new North American records for this host, and 1 nuclear polyhedrosis virus also were found. The most common pathogen was N. necatrix. Of the colonies sampled, 43% were infected with at least 1 of the 4 pathogens mentioned.

100. History, Implementation and Current Use of the IFCC-IUPAC's Nomenclature for Properties and Units (NPU) Terminology in Denmark, Norway and Sweden.

Abstract

Electronic exchange of health care data demands code/terminology systems. In the Scandinavian countries, the IFCC-IUPAC's Nomenclature for Properties and Units (NPU) terminology is used for results in biochemistry, pharmacology, and immunology. Implementation, use and administration of NPU has differed between the countries despite similar health care and lab sectors. In Norway and in one Swedish region NPU - with supplementary SNOMED CT codes is also used for reporting results in microbiology. In Denmark and to some extent in Norway and Sweden NPU is also used for ordering tests. In Norway NPU (as part of NLK) has since 2018 been mandatory in requesting governmental reimbursement for laboratory tests. The numbers of national codes vary considerably (DAN: 303, NOR: 1612, SWE: 415). Furthermore, in Denmark >3500 local codes are used for requisition and to communicate more details with the analytical result than the NPU terminology allows. Also, in Norway the NPU codes are by many lab professionals considered insufficient for communicating all relevant information with results. However, the Norwegian reimbursement system has been a strong motivator for implementing international NPU codes. We find it necessary to add information about "how" a measurement is done to the information about "what" is measured in the laboratory report. Until this is settled otherwise, we suggest an increased pragmatism towards producing national codes including method specific information.
