Abstract

Background: Pancreaticobiliary maljunctions (PBMs) are congenital anomalies of the junction between pancreatic and bile ducts, frequently associated with bile duct cyst (BDC). BDC is congenital biliary tree diseases that are characterized by distinctive dilatation types of the extra- and/or intrahepatic bile ducts. Todani’s types I and IVa, in which dilatation involves principally the main bile duct, are the most frequent. PBM induces pancreatic juice reflux into the biliary tract that is supposed to be one of the main factors of biliary cancer degeneration, although the diagnostic criteria of PBM that can be either morphological and/or functional are not well defined especially in Western series. Objective: The aim of this study was to assess the relative prevalence of PBM in BDC in a large European multicenter study, to analyze the characteristics of PBM and try to propose diagnostic criteria of PBMs based on morphological and/or functional criteria and define the positive, negative predictive values, sensibility and specificity of either criteria. Results: From 1975 to 2012, 263 patients with BDC were analyzed. Among them, 190 (72.2%) were considered to present PBM. Types I and IVa had a similar rate of PBM association. According to the “AFC classification,” 57.2% had a C–P type, 34.5% a P–C type and 8.3% a complex type (“anse-de-seau”). The median length of the common channel in patients with PBM was 15.8±6.8mm (range 5–40mm). The median intrabiliary amylase and lipase levels were 65,249 and 172,104UI/L, respectively. For the diagnostic of PBM, a common channel length of more than 8mm and an intrabiliary amylase level superior to 8000UI/L were associated with a predictive positive value and a specificity of more than 90%. Synchronous biliary cancer had an incidence of 8.7% in all patients with BDC and PBM 11.1% in adults. Compared to type IV, the type I BDC was associated with statistically more cancer patients in the presence of PBM. Conclusions: Characteristics of PBM associated with BDC in Western population are quite close to reported Eastern series. The results suggest considering both the intrabiliary value of amylase >8000UI/L and a length of a common channel >8mm as appropriate values for positive diagnosis of PBM.

Abstract

Diagnosis of benign liver tumors is experiencing exponential growth, mainly owing to the diffusion of more accurate imaging techniques. This monograph examines the epidemiology, histopathology and genetics of these tumors and provides a systematic overview of their evolution and differential diagnosis. Care is taken to document surgical indications accurately, with a view to assisting in the avoidance of useless interventions. Open, laparoscopic and robotic surgical techniques are described and illustrated, emphasizing the optimization of intra- and postoperative management in order to avoid potential complications and degeneration. In these chapters, attention is drawn to the ways in which the surgical management of benign liver tumors differs from that of malignant tumors. The book concludes by examining the role of interventional radiology in patients with benign liver tumors.

70. Local banks and Centre-northern banks in Mezzogiorno: temporal evolution and acomparison with other European emergent areas

Abstract

The paper examines the evolution in the presence of banks, whose headquartes are in the Centre-Northern Italy, in the southern banking system. Analysing market shares in loans, deposits and branches emerges an increase in the incidence of banks not from the Southern area in the relationships with southern clients. This occurred mainly in the second half of the Nineties and in the first years of new millennium. After a first period characterized mainly by mergers; in the second half of the Nineties there were mainly acquisitions of control by Centre-Northern banking groups of southern banks which maintained their formal juridical independence. Also the importance for Centre-Northern banks of their relationships with southern clients increased, moreover, on the other hand, the relevance of Centre-Northern clients for southern banks is not negligible. Lastly, the paper proposes a comparison between the national integration of the southern banking system and that of the banking system of similar emergent areas in Spain and Germany. Previous results highlight that Italian southern banking system is relatively more integrated with the national one than that of Spain and Germany.

Abstract

Objective: To evaluate improvements in operative and long-term results following surgery for hilar cholangiocarcinoma. Design: Retrospective multicenter study including 17 Italian hepatobiliary surgery units. Patients: A total of 440 patients who underwent resection for hilar cholangiocarcinoma from January 1, 1992, through December 31, 2007. Main Outcome Measures: Postoperative mortality, morbidity, overall survival, and disease-free survival. Results: Postoperative mortality and morbidity after liver resection were 10.1% and 47.6%, respectively. At multivariate logistic regression, extent of resection (rightor right extended hepatectomy) and intraoperative blood transfusion were independent predictors of postoperative mortality (P =. 03 and P =. 006, respectively); in patients with jaundice, mortality was also higher without preoperative biliary drainage than with biliary drainage (14.3% vs 10.7%). During the study period, there was an increasingly aggressive approach, with more frequent caudate lobectomies, vascular resections, and resections for advanced tumors (T stage of 3 or greater and tumors with poor differentiation). Despite the aggressive approach, the blood transfusion rate decreased from 81.0% to 53.2%, and mortality slightly decreased from 13.6% to 10.8%. Median overall survival significantly increased from 16 to 30 months (P =. 05). At multivariate analysis, R1 resection, lymph node metastases, and T stage of 3 or greater independently predicted overall and disease-free survival. Conclusions: Surgery for hilar cholangiocarcinoma has improved with decreased operative risk despite a more aggressive surgical policy. Long-term survival after liver resection has also increased, despite the inclusion of cases with more advanced hilar cholangiocarcinoma. Preoperative biliary drainage was a safe strategy before right or right extended hepatectomy in patients with jaundice. Abstract

Guardando alla storia del fallimento emerge una doppia anima dell'istituto che ne mette in risalto, a volte, la funzione punitiva, di procedimento volto a costituire un deterrente nei confronti di condotte spregiudicate e a isolare il debitore insolvente dalla comunità; altre volte, quella di consentire, nel rispetto del principio di parità di trattamento e attraverso un’esecuzione collettiva, la distribuzione del residuo patrimonio del debitore che, suo malgrado, non ha avuto successo nell’esercizio dell’attività di impresa, ha cioè, appunto, “fallito”. Tuttavia, analizzando l’evoluzione delle norme che regolano l’insolvenza degli imprenditori e dei debitori comuni, si può notare come l’accezione negativa da sempre associata al debito si è gradualmente affievolita per lasciare il posto a una visione dello stesso per così dire “fisiologica”, quale elemento naturale sia della fattispecie “attività di impresa” sia, più in generale, della vita umana. L'articolo ripercorre i punti di snodo di tale processo evolutivo, dimostrando come le crisi economiche dell’ultimo decennio abbiano indotto il legislatore a dare vita a un sistema di regole che dovrebbe incoraggiare l’emersione precoce dello stato di crisi aziendale, al fine di ridurne quanto più possibile le conseguenze negative sull’impresa e sui relativi stakeholders

Abstract

Aim To compare clinical presentation, operative management and short- and long-term outcomes of congenital bile duct cysts (BDC) in adults with children. Methods Retrospective multi-institutional Association Francaise de Chirurgie study of Todani types I+IVB and IVA BDC. Results During the 37-year period to 2011, 33 centers included 314 patients (98 children; 216 adults). The adult population included more high-risk patients, with more active, more frequent prior treatment (47.7% vs 11.2%; p < 0.0001), more complicated presentation (50.5% vs 35.7%; p = 0.015), more synchronous biliary cancer (11.6% vs 0%; p = 0.0118) and more major surgery (23.6% vs 2%; p < 0.0001), but this latter feature was only true for type I+IVB BDC. Compared to children, the postoperative morbidity (48.1% vs 20.4%; p < 0.0001), the need for repeat procedures and the status at follow-up were worse in adults (27% vs 8.8%; p = 0.0009). However, severe postoperative morbidity and fair or poor status at follow-up were not statistically different for type IVA BDC, irrespective of patients' age. Synchronous cancer, prior HBP surgery and Todani type IVA BDC were independent predictive factors of poor or fair long-term outcome. Conclusion BDC is a more indolent disease in children compared to adults, except for Todani type IV-A BDC.

Abstract

KRAS testing is relevant for the choice of the most appropriate first-line therapy of metastatic colorectal cancer (CRC). Strategies for preventing unequal access to the test should be implemented, but their relevance in the practice is related to economic sustainability. The study adopted the Delphi technique to reach a consensus on several topics. Issues related to execution of KRAS testing were identified by an expert's board and proposed to 108 Italian oncologists and pathologists through two subsequent questionnaires. The emerging proposal was evaluated by decision analyses models employed by technology assessment agencies in order to assess cost-effectiveness. Alternative therapeutic strategies included most commonly used chemotherapy regimens alone or in combination with cetuximab or bevacizumab. The survey indicated that time interval for obtaining KRAS test should not exceed 15 days, 10 days being an optimal interval. To assure the access to proper treatment, a useful strategy should be to anticipate the test after radical resection in patients at high risk of relapse. Early KRAS testing in high risk CRC patients generates incremental cost-effectiveness ratios between 6,000 and 13,000 Euro per quality adjusted life year (QALY) gained. In extensive sensitivity analyses ICER's were always below 15,000 Euro per QALY gained, far within the threshold of 60,000 Euro/QALY gained accepted by regulatory institutions in Italy. In metastatic CRC a time interval higher than 15 days for result of KRAS testing limits access to therapeutic choices. Anticipating KRAS testing before the onset of metastatic disease in patients at high risk does not affect the sustainability and cost-effectiveness profile of cetuximab in first-line mCRC. Early KRAS testing may prevent this inequality in high-risk patients, whether they develop metastases, and is a cost-effective strategy.
